This Clinical Practice Guideline was written by a fellow 18D with input from around the surgical community. It reconciles the differences between wound care done in a role 2 or 3 facility, such as serial debridements, with what is taught in the 18D Special Forces Medical Sergeant Course with regards to delayed primary closure. One way is not "right" while the other wrong, it has more to do with the amount of time and resources available to the medic or other provider. The remainder of the blog post and podcast is meant to be a refresher for those who have already been taught these procedures. It is also meant to be informational for those medical directors who may not be exactly certain of what has been taught as far as wound care and surgery. If you haven't been trained to do these procedures before going ahead with them, it is very likely that you may do more harm to the patient than good. This Clinical Practice Guideline was written by a fellow 18D with input from around the surgical community. It reconciles the differences between wound care done in a role 2 or 3 facility, such as serial debridements, with what is taught in the 18D Special Forces Medical Sergeant Course with regards to delayed primary closure. One way is not "right" while the other wrong, it has more to do with the amount of time and resources available to the medic or other provider. The remainder of the blog post and podcast is meant to be a refresher for those who have already been taught these procedures. It is also meant to be informational for those medical directors who may not be exactly certain of what has been taught as far as wound care and surgery. If you haven't been trained to do these procedures before going ahead with them, it is very likely that you may do more harm to the patient than good. People in the special operations community often find themselves learning to be human swiss army knives. They do intensive courses in anything from second languages to advanced trauma medicine, where their ability to succeed depends on their ability to manage their time, attention, and energy and study and learn effectively. For instance, we did an episode recently on how to be successful in the Special Forces medic course, known as 18D, and a consistent theme was the importance of good study habits. But nobody teaches that. People either figure it out or fail. And lots of them fail. We want to give people a framework that they can use to understand how to establish strong time management and study habits so that they can learn better and faster during challenging courses.To do that, we talked to Dr. Karin Nordin, a Ph.D. in health behavior change, a published researcher on mindset theory, and a curriculum writer for Precision Nutrition. Dr. Nordin is the founder and CEO of her own business, Body Brain Alliance, which provides behavior change coaching for individuals and curriculum design education for business owners. After teaching at the college level for years and then transitioning into the online coaching industry, she quickly realized that the evidence-based practices for both course design and behavior change were not being taught in an industry that desperately needed them. Abstract: Acute activation of innate immune response in the brain, or neuroinflammation, protects this vital organ from a range of external pathogens and promotes healing after traumatic brain injury. However, chronic neuroinflammation leads to the activation of immune cells like microglia and astrocytes causes damage to the nervous tissue, and is causally linked to a range of neurodegenerative diseases such as Alzheimers diseases (AD), Multiple Sclerosis (MS), Parkinsons diseases (PD), and many others. While neuroinflammation is a key target for a range of neuropathological diseases, there is a lack of effective countermeasures to tackle it, and existing experimental therapies require fairly invasive intracerebral and intrathecal delivery due to difficulty associated with the therapeutic crossover between the blood-brain barrier (BBB), making such treatments impractical to treat neuroinflammation long-term. Here, we present the development of an optimal neurotherapeutic using our NanoligomerTM discovery engine, by screening downregulation of several proinflammatory cytokines (e.g., Interleukin-1{beta} or IL-1{beta}, tumor necrosis factor-alpha or TNF-, TNF receptor 1 or TNF-R1, Interleukin 6 or IL-6), inflammasomes (e.g., NLRP1), key transcription factors (e.g., nuclear factor kappa-B or NF-{kappa}{beta}) and their combinations, as upstream regulators and canonical pathway targets, to identify and validate the best-in-class treatment. Using our high-throughput drug discovery, target validation, and lead molecule identification using a bioinformatics and AI-based ranking method to design sequence-specific peptide molecules to up-or down-regulate gene expression of the targeted gene at will, we used our discovery engine to perturb and identify most effective upstream regulators and canonical pathways for therapeutic intervention to reverse neuroinflammation. The lead neurotherapeutic was a combination of NanoligomersTM targeted to NF-{kappa}{beta} (SB.201.17D.8_ NF-{kappa}{beta}1) and TNFR1 (SB.201.18D.6_TNFR1), which were identified using in vitro cell-based screening in donor-derived human astrocytes, and further validated in vivo using a mouse model of lipopolysaccharide (LPS)-induced neuroinflammation. The combination treatment SB_NI_111 was delivered without any special formulation using a simple intraperitoneal injection of low-dose (5mg/kg) and was found to significantly suppress the expression of LPS-induced neuroinflammation in mouse hippocampus. These results point to the broader applicability of this approach towards the development of therapies for chronic neuroinflammation-linked neurodegenerative diseases, sleep countermeasures, and others, and the potential for further investigation of the lead neurotherapeutic molecule as reversible gene therapy. That being said… The following are recommendations made by the International Committee of the Red Cross (ICRC) concerning the surgical management of war wounds in austere conditions and with limited resources. This is when the provider has some or all of the following considerations which would prohibit him from performing serial (follow-on) debridement with associated post-operative care. Dirty environment Limited supplies Limited manpower Limited time (mission dictated) Wounds greater than 24 hours The considerations above, accompanied with the position that the provider will be managing that patient for more than a couple days or become the definitive provider, should warrant ICRC recommendations for surgical management. Jason Mitchler works full time as the Director of Operations at the William J. Hybl Sport Medicine and Performance Center. He also works as an injury prevention specialist for various fire departments throughout the country and is an Instructor at the University of Colorado Colorado Springs. Jason retired from active duty in 2019 and served the majority of his Army career in operational units. His enlisted career began as an infantryman and later as a Special Forces Medic with the 3rd Special Forces Group (Airborne) while completing two tours to Afghanistan. After being commissioned as a physical therapist he was the lead during the implementation of the USASOC THOR3 program at 10th Special Forces Group (Airborne). During this time he oversaw the development of a comprehensive performance and rehabilitation program at Ft. Carson, CO. Following this assignment he was selected to be the Chief of the Physical Therapy at the Pentagon. His most recent deployment was as the sole physical therapist for the Special Operations Task Force- Afghanistan (SOTF-A) serving both American and coalition forces. He is a graduate of the University of Northern Colorado with a BA in Kinesiology and the Army-Baylor program with a Doctorate in Physical Therapy. He is an Orthopaedic Certified Specialist (OCS) and Certified Strength and Conditioning Specialist (CSCS). Jason lives in Colorado Springs, CO. He is an assistant high school wrestling coach and holds a purple belt in Brazilian Jiu Jitsu. In his free time he is an active cyclist, runner, and skier. The last 30 minutes we dug deep into what John calls the "Hidden Wounds" that he suffered through, TBI and PTS and the struggle that goes with these combat injuries that you cannot see. Normally it is continued exposure or proximity. Look at something or practice a habit long enough we don't see it.·         For example, think about the keyhole in your door. Which direction do the key groves point when you unlock the door? Up or down?·         The lock in your car door – do you turn the key to the right or left to unlock it?·         Oh, I forget. We have remotes now. Okay, where is the button to lock the door, is it on top of the unlock button or below it.·         Not fair to look if you're driving!·         If the whole key/lock thing isn't doing anything for you, how about a denominational bill? On this week’s episode of the Ones Ready podcast we sit down with RJ Casey who is basically like the guy you only read about...an all American hero. RJ has a resume that would make Arnold jealous! After attending the New Mexico Military Institute he commissioned into the Army as an 11A, then went to Ranger school and the Special Forces Selection Course to become an 18A. RJ wanted to get his hands dirty even more so he resigned his commission as an officer and became an enlisted 18D. Somewhere in here he also went to PA school after being an 18D to where he was deployed in the early days of Iraq. He left Iraq and spent 10 days chilling and relaxing before then going through INDOC as an E-7. RJ then became a PJ to where he missed the officer side of the house and decided to go CRO. Now as a CRO he is assigned to the Cocoa Beach PJ team, helps with the NASA Space mission, and advises on movies in the US and overseas. Please enjoy the episode and give us your feedback. If Saturday's crossword was a main course, this was definitely dessert, somewhere between flan and merengue pie, that ought to SATE those looking for a COMFIER puzzle. It had a lot of fun cluing, like 55A, Square things (ATONE) and 53D, Rest of the afternoon? (SIESTA). Jean cruised through it as usual, Mike assumed that, on the basis of NA, that 18D, Mission-driven org., would be NATO; alas, no.There were some cute touches. For example, the puzzle was titled "PI R Squared", and AREA made an appearance; and both MANUAL and ONAUTO popped up. And yesterday OLMANRIVER made an appearance, and today the subject of that song, MISSISSIPPIRIVER, flowed in. All in all, a nice puzzle, 5 on the JAMCR scale.

In 1996 First Sergeant Collins joined the U.S. Army as an Intelligence Analyst (96B) and completed basic training in Fort Leonard Wood and AIT at Fort Huachuca,AZ. His initial duty station was with the 8th Army in Pyong Taek, South Korea. Upon returning stateside in 1997, he was stationed at 4th ID (Mechanized) at Fort Carson, Colorado. In 1999 First Sergeant Collins was nominated for and assigned to Royal Air Force Base Molesworth in the UK. In 2001 he returned stateside and was assigned to 1/325 Airborne Infantry Regiment/82nd Airborne; attending Jump school over 9/11. First Sergeant Collins was tasked to support US CENTCOM in April - September, 2002 for the early stages of the war on terror. He attended Special Forces Assessment and Selections (SFAS) in October- November 2002. He completed the Special Forces Qualification Course in 2004 and was assigned as an 18D, Special Forces Medical Sergeant, to the 7th Special Forces Group (Airborne), where he served as the Senior Medical Sergeant for Special Forces Operational Detachments – Alpha’s (SFOD-A) 732, and 734. While assigned to 7th Special Forces Group (Airborne), he participated in Operation ENDURING FREEDOM as well as multiple deployments to the SOUTHCOM Areas of Operation. In 2008, First Sergeant Collins was assigned to the Special Warfare Medical Center (SWMG) as an 18D instructor teaching Combat Trauma Management (CTM), Advanced Combat Life Support (ACLS), Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Triage. In 2009 he was assigned to A Special Missions Unit on Fort Bragg, NC; serving as a Senior Medic and Team Sergeant. While assigned there, he participated in in multiple deployments to Operation IRAQI FREEDOM and OPERATION NEW DAWN. In January 2014 First Sergeant Collins was assigned to Walter Reed National Medical Center to rehabilitate from a left below the knee Amputation. In July 2014, First Sergeant Collins was assigned to 1st Special Forces Command’s (A) Office of Special Warfare (OSW) developing the Non- Standard Medical Team, SOF Austere Care Course (SOFACC), and Regional Support Medic (RSM) Courses.These courses were designed to augment tactical and/or austere environments with a webbed network of echeloned medical capability. RECOGNITION In April of 2017 First Sergeant Collins Assumed Duties as HHC First Sergeant for SWMG. In April 2018 First Sergeant Collins was awarded the prestigious Major General Singlaub award. This award is given to one soldier per year within the entire U.S. ARMY Special Operations Command‘s 26,000 people.The Major General Singlaub Award is presented to deserving recipients who exemplify the Army Special Operations Forces Next traits, the Army Special Operations Forces promise, and demonstrate an unwavering commitment to bettering ARSOF. In 2018 First Sergeant Collins also medaled for Team U.S.A. at the 2018 Invictus Games in Sydney, Australia and represented and medaled forTeam United States Special Operations Command at the 2018 DOD Warrior Games.

I couldn’t even listen to it. Green Berets are a family, and this past week we lost a brother, Joel Mitchell. D & I went through the 18D course with Joel and he is someone we both respected and loved.  I know our line of work is dangerous, and we’ve both lost brothers before, but for some reason, this one hit home a lot harder.  Our thoughts and prayers are with Joel’s family and the Regiment. All we ask is that if you are in a place where you need someone to talk to, to hear you out, or to help you out of a dark place – reach out. We are here. And if we are not near you we know someone where you are. We are a brotherhood for life, we walk shoulder to shoulder in training, in combat, in success, and in struggles – that doesn’t end when we retire or move on.  God Speed Joel Mitchell. So let's talk about Russian interference in our election – because that's what they did, they got inside our election process.  They executed a masterstroke UW campaign against the US Presidential election and are still reaping the benefits of their efforts today. But what was their goal? I don’t think it was to get Trump elected, as some claim.  How did they do it? Pretty simple really, they used us against ourselves, with identity politics being the main weapon. In this episode, We start from a grand jury indictment and walk through how the chess masters played their latest game. But it's not just the Russians using these tools, the Alt-Right and white nationalist groups use the same techniques, so does the far left – making us even more susceptible meddling and influence. Originally Published February 28, 2018 In this episode we have Doc “ASH” joining us to discuss 18D training, career, the scope of practice, and mindset, as well as some more in-depth discussion on advanced treatments available to preserve life in the war-zone.  Our conversation provides more background on who we are and the mindset with which we approach life.  This is not a comprehensive exploration of the Special Forces Medic, but rather highlights some of the unique capabilities and the critical role of the medic in the survivability of an SFOD-A and our partner forces. One of the key things we focus on in this episode is the breadth of support an 18D provides to the special forces mission.  Different than combat medics, 18D do not only treat the wounded, our scope of practice includes clinical treatment of illnesses, preventative medicine, hygiene, and sanitation, as well as surgical capabilities, and delivering babies, yes we have done it.  The gold standard for an 18D is to sustain a wounded patient (or critical patient) for 72 hours, enough time to evacuate the patient to definitive care. But this isn’t done in a full hospital or clinical setting but in an austere or even ad-hoc field clinic or non-military environment. However, our job would not be possible without doctors like ASH who invest the time and effort to train us, keep us current, and learn alongside of us from our experiences to bring the highest level of care to the men in the field.  As you’ll hear, his expertise and passion for the work enhance and motivates us to perform at the level we do. ASH brings some details about the medical field into the discussion, so you have to listen close, and hopefully, we captured all the notes so you don’t miss anything. The last part of the discussion shifts back to our primary mission on Inside the G-Base:  teaching you, our G’s, how to think like Green Berets.  We ask a hard question and want to challenge all of you to explore the possibility that it is time our society begins thinking differently about the threats and challenges we face.  No, we don’t discuss gun control – that is for a different debate.  But while that is being debated, why aren’t cities, schools, and “soft targets” preparing for the worst? All three of us recommend changing the civilian first responder mindset closer to that of a tactical medic.  Stage lifesaving medical treatments as close to the point of possible injuries as possible.  That means staging aid bags with tourniquets and clotting bandages in classrooms, theaters, malls.  We all know where to find an AED if someone has a heart attack, why isn’t there and Aid bag there with the capability to preserve life?  Of course, people will need training, but if J’s kids can apply these treatments, surely teachers can.  This is another call for our G’s to think outside the box, it may be time to accept that at least for now, our society and culture have changed and we need to do what we can to preserve life when we can.  That's what an 18D would do, that's what we want our G’s to do. IN AUG 2018 US Forces and Afghan partner forces were ordered to retake Ghazni from the Taliban. The details are described in an AUG 23, 2018 in TIME Magazine in an article entitled "Inside the US Fight to save Ghazni From the Taliban". Nate is an 18D and Gavin is a PJ who supported ODA 1333. They discuss details of the mission and the medicine. There were over 40 casualties over the duration of the fighting with several MCIs. This is the first of several episodes to discuss the prolonged operation.

Tyr Symank (pronounced Tier Psy-mack) is a Green Beret, occasional medic, failed entrepreneur, struggling writer, and committed father. He served 25 years and counting in the active Army and National Guard, achieving the rank of Sergeant Major. Tyr has years of deployed time both as a soldier and a civilian to Iraq, Afghanistan, the Philippines, Asia, Africa, and various countries of interest. Tyr dropped out of University of Oregon’s School of Journalism in 1994 to be a lift operator at Mt. Hood. When the snow melted, he enlisted as a 19K, Abrams Tank Crewman for two years and some college money. Upon arriving at Ft Hood, TX, he tried out for the 1st Cavalry Division’s Horse Cavalry Detachment. Two kids and re-enlistments later, he attended Special Forces Assessment, and was selected to be an 18D, Special Forces Medic. Tyr completed the paramedic portion of his training with FDNY in early September 2001. He deployed to Afghanistan almost immediately after earning his Special Forces tab and Green Beret. His various duties and positions for government entities since have included medic, diplomatic security specialist, intelligence and atmospherics collector, analyst, instructor, operations sergeant, and sergeant major. As an instructor, he has taught at every level from Sub-Saharan tribesmen to university students to federal agents to ambassadors. He is accredited by the Federal Law Enforcement Training Center.Tyr has several divorces and three wonderful children. He is currently a program manager for Black Rifle Coffee Company and a Special Forces company sergeant major in 19th Special Forces Group (Airborne). On 6 April 2008, ODA 3336 was on a mission to capture or kill high-value targets of the Hezeb Islami al Gulbadin in Shok Valley, Nuristan Province, Afghanistan. Dagens text:Han bar själv sitt kors ut till den plats som kallas Skallen, på hebreiska Golgota. 18Där korsfäste de honom tillsammans med två andra, en på var sida med Jesus i mitten. 19Pilatus hade också låtit skriva ett anslag som sattes upp på korset, och där stod: Jesus från Nasaret, judarnas konung. 20Detta lästes av många judar, eftersom platsen där Jesus korsfästes låg strax utanför staden, och texten var på hebreiska, latin och grekiska. 21Men judarnas överstepräster sade till Pilatus: ”Skriv inte: Judarnas konung, utan vad han själv har sagt: Jag är judarnas konung.” 22Pilatus svarade: ”Vad jag har skrivit, det har jag skrivit.”23Soldaterna som hade korsfäst Jesus tog hans kläder och delade upp dem i fyra delar, en på varje soldat. De tog också långskjortan, men den hade inga sömmar utan var vävd i ett enda stycke. 24De sade därför till varandra: ”Vi skär inte sönder den utan kastar lott om vem som skall ha den.” Ty skriftordet skulle uppfyllas: De delade upp mina kläder mellan sig och kastade lott om min klädnad.Det var vad soldaterna gjorde. 25Men vid Jesu kors stod hans mor och hennes syster, Maria som var gift med Klopas och Maria från Magdala. 26När Jesus såg sin mor och bredvid henne den lärjunge som han älskade sade han till sin mor: ”Kvinna, där är din son.” 27Sedan sade han till lärjungen: ”Där är din mor.” Från den stunden hade hon sitt hem hos lärjungen. 28Jesus visste att nu var allt fullbordat, och för att skriftordet skulle uppfyllas sade han: ”Jag är törstig.” 29Där stod ett kärl som var fyllt med surt vin. De satte därför en svamp som doppats i det sura vinet på en isopstjälk och förde den till hans mun. 30När Jesus hade fått det sura vinet sade han: ”Det är fullbordat.” Och han böjde ner huvudet och överlämnade sin ande.31Eftersom det var förberedelsedag och kropparna inte fick hänga kvar på korset under sabbaten – det var en stor sabbat – bad judarna Pilatus att de korsfästas benpipor skulle krossas och kropparna tas bort. 32Soldaterna kom därför och krossade benen på dem som var korsfästa tillsammans med Jesus, först på den ene och sedan på den andre. 33Men när de kom till Jesus och såg att han redan var död krossade de inte hans ben, 34utan en av soldaterna stack upp sidan på honom med sin lans, och då kom det ut blod och vatten. 35Den som såg det har vittnat om det för att också ni skall tro; hans vittnesbörd är sant, och han vet att han talar sanning. 36Detta skedde för att skriftordet skulle uppfyllas: Inget ben skall krossas på honom. 37Och på ett annat ställe heter det: De skall se på honom som de har genomborrat.Joh 19:17-37 (Bibel 2000)Marie Ek Lipanovskahttp://www.marieeklipanovska.se

Mike joined the Army and became what is known as an 18D or combat medic within the Green Beret community. Over the course of his nine deployments, he has been blown up, hurt or otherwise injured countless times which ultimately led to his medical retirement from the Army. Now he fills his time in service to the Global War on Terrorism Memorial Foundation as a Board Member as well as an Advisor on the George Bush Presidential Center's Military Service Initiative Council. Never one to be deterred, he also manages to spend time in the forge, creating one-of-a-kind pieces of functional art, some even forged from relics recovered from each site of the 9/11 attacks.
