Send me a msg by clicking here :)In episode 51, Jess from the Northern Territory shares the births of Matilda (hospital), Edith (freebirth) and Angus (freebirth > hospital transfer). After years of trying, Jess finally became pregnant through IVF with her 1st baby Matilda, and gave birth in 2020 with little insight into the medicalisation of birth. For her second baby, Edith however, she chose to birth at home, however was unable to access homebirth midwifery care where she lives in the Northern Territory, and so had little other options, other than to freebirth. Empowered by this experience, for her third baby Angus, she chose to lean deeper into that experience and have an anxiety free wild pregnancy and planned to have another freebirth. Whilst we talk about all 3 births in this episode, it is worth highlighting in this intro that during her labour with Angus, she felt that something wasn't quite right, and so made the decision to transfer to hospital for medical support. Whilst Jess has and will continue to work through the common emotions that this decision evokes, I hope sharing this story with you today, the listener, demonstrates how amazing jess is - how she trusted and listened to her body and in the end, she listened to her intuition and sought help to birth all 4.9kg of baby Angus. In 1958, James et al. first described umbilical cord blood gas analysis to evaluate the fetus's metabolic condition at the time of delivery. It's recommended by both the British and American Colleges of Obstetrics and Gynecology for all high-risk deliveries. Cord blood gas provides the most accurate and objective evidence of fetal acidosis at the time of birth. Still, there is no universal agreement on when to perform umbilical artery gas analysis, or cord gas, for babies. Some suggest that all babies should have their umbilical artery and vein sampled immediately after birth, while others recommend only sending a sample if you think you need one, or in high-risk pregnancies. Should we collect umbilical cord gases universally- in all deliveries? And what about umbilical cord lactate levels? Is that helpful in predicting adverse newborn outcomes? Listen in for details.

My name is Sarah Latane. I live in the Shenandoah Valley of VA. I am a board certified behavior analyst and a stay at home parent. Brief overview of pregnancy; surprise pregnancy, early symptoms, initial MD care (on accident), early scare, practice transfer to midwife care, later symptoms (including GBS + and hypothyroid). Day/night of birth; prodromal labor and early labor at home, then hospital, never met before midwife, trauma response to cervical exam, decels, epidural, continued decels, cord prolapse diagnosed and emergency c-section. Some birth trauma, definitely the baby blues, then slowly progressing PPD turned postpartum rage, but mostly better now. 3 Key takeaways from the podcast that listeners will learn today: Point 1: birth is extremely unpredictable; definitely have strong visions for your birth plan, but research other alternatives just in case (I NEVER considered that I would end up having a cesarean and had not researched or planned for this at all and I think it made coming to terms with it harder). Point 2 : maybe prepare for cervical exam if you have experienced sexual assault before and have a first exam pre-labor? I think this really would have helped me. Point 3 : if possible, try to envision and plan for what after your birth will look like, e.g., what are your boundaries? And who can you enlist to advocate for you? The umbilical cord is a vital part of pregnancy, connecting the fetus to the placenta and providing essential nutrients and oxygen to support its growth and development. This cord is formed early in pregnancy and typically measures around 1 feet in length at birth. During pregnancy, the umbilical cord acts as a lifeline between the fetus and the mother. It contains two arteries and one vein that transport blood, nutrients, and oxygen from the mother to the baby, while carrying away waste products and carbon dioxide from the baby back to the mother's bloodstream. This exchange of vital substances is essential for the healthy growth and development of the fetus. The umbilical cord also plays a critical role in the delivery process. When the baby is born, the umbilical cord is clamped and cut, separating the baby from the placenta and the mother's bloodstream. The cord is then discarded, and the baby begins to breathe on its own. In some cases, because the umbilical cord is so vital it can have some kinks or variations that can become more concerning. We all know about the cord can get wrapped around the baby's neck, causing complications during delivery. However, this is rare and can often be detected through routine prenatal care. It can also have differenes in shape, length or characteristics that can be crucial in a developing baby. The umbilical cord is a crucial part of pregnancy, providing essential nutrients and oxygen to support the healthy growth and development of the fetus. Delayed cord clamping (DCC) provides vital placental transfusion to newborns and is endorsed by ACOG, SMFM, March of Dimes, and the ACNM. However, DCC in nonvigorous newborns may not be provided owing to a perceived need for immediate resuscitation. Umbilical cord milking, in late-term and full-term neonates, is an alternative in these cases. In December 2020, the ACOG's Committee Opinion stated a lack of outcomes data for umbilical cord milking in nonvigorous newborns. This Level 1 data has now arrived (February 2023). In this episode, we will summarize the key findings from this soon-to-be released publication from the AJOG on umbilical cord milking in nonvigorous newborns born at >/= 35 weeks.

I remember being so furious with my dad because I felt like I was the only 16-year-old in my friend group that paid for their own car insurance. Looking back now, I really appreciate him making me handle my financial responsibilities early on. Don't get me wrong I still take advantage of my parents picking up the tab at dinner or paying for the hotel on a family vacation but I pay my own bills and it makes me feel so empowered being financially independent. This was the first step toward my parents seeing me as financially mature. Maybe you're someone who's been taking care of yourself since you were a teenager or you might feel guilty that your parents are still financially supporting you. I'm here to tell you there is nothing wrong with being a young adult and getting support from your parents. Life is expensive and I don't think it's a smart idea to rack up credit card debt to try to prove that you can pay for your own things. Although the financial support might be nice, I know you want to be seen as mature enough to make your own money and handle it appropriately. That's what this conversation is about today. How do I move towards this goal without making a huge mistake? How do I gain trust from my parents? My friend Clifton Corbin is going to share some advice as he's gone through this journey himself. After making a mess of his finances in his early 20s by racking up debt and destroying his credit, his parents lost faith in him.
