In this episode of The Mind Gut Conversation, I speak with Dr. Rima Kaddurah-Daouk, Professor in the Departments of Medicine and Psychiatry at Duke University Medical School and internationally recognized thought-leader in the pathophysiology of Alzheimer disease. Dr. Kaddurah currently leads and coordinates an international multi-center research consortium, the Alzheimer's Disease Metabolomics Consortium or ADMC which consists of more than 120 scientists from over 30 academic institutions. She has published over 140 peer reviewed scientific publications, leads a large number of active NIH grants and in 2018, she ranked as the highest funded researcher in departments of psychiatry nationally. Dr. Kaddurah has more than 60 patents or patent applications on use of metabolomics for the diagnosis and treatment of brain diseases. Topics we discuss in this podcast include: 1) What are the major scientific breakthroughs in our understanding of AD that have come from the ongoing research of the NIH AD consortium? 2) The scientific approach on which the AD grant is based on is radically different from the classical reductionistic biomedical research strategy and is based on systems biology and the paradigm of interconnectedness. Based on the lack of progress in effective treatments for AD to date, do you believe this new approach has a better chance of success? 3) Are there any new therapeutic strategies emerging from this new research?

Dr David Albert is a physician, inventor and serial entrepreneur. He studied at both Harvard and Duke University Medical School. In 2001, he sold Data Critical to GE in a deal that valued Data Critical at $45M. Perhaps his Magnum Opus: AliveCor has raised over $150M and creates devices which are able to take ECGs or heart traces — from anywhere, at anytime. It's newest innovation: The KardiaMobile Card is a credit card which can take an ECG — all whilst fitting into your wallet. He also has over 75 patents and 100 scientific publications. We speak about his formula and framework for innovating, lessons he's learnt from his buddies such as Eric Topol and Vinod Khosla and whether everything in Medicine should be democratised. Dr. Marion Mass is a practicing pediatrician who is also a physician advocate and healthcare activist. She is driven to take back medicine for patients and the medical profession. The COVID-19 pandemic threw a spotlight on dangerous deficiencies within the US healthcare industry. At the center of these deficiencies are Group Purchasing Organizations (GPOs) and Pharmacy Benefit Managers (PBMs), which are healthcare intermediaries that increase costs and create drug and supply shortages. Although GPOs and PBMs don't produce or distribute a single product or provide direct care to a single patient, they control nearly 90% of the pharmaceutical supply chain. These middlemen amass huge profits because they enjoy government-mandated safe harbor protections at the expense of physician practice needs, sound medical decision-making, and patient choice. Marion Mass, MD, is a pediatrician in the Philadelphia area and co-founder of Practicing Physicians of America. She is a graduate of the Duke University Medical School and was trained in pediatrics at Northwestern University. They seek out inflamed cells that are old, damaged or injured, and are to restore them” said Dr. Eric Weiss. “Multiple studies demonstrate this cord blood can treat certain conditions successfully in children and adults including autism, osteoarthritis, rheumatoid arthritis, traumatic brain injury and more.” “Stem cell therapy has helped my son in so many ways. Children with autism have inflammation in their brains. Stem cells combat inflammation. When we did stem cell therapy, we saw immediate results. Marston asked questions, he followed conversations, and he became acutely aware of his surroundings. Marston talked for conversation and not just for directions. Marston was more focused and his memory was spot on. To this day I still hear him use vocabulary words I never thought he knew and in the correct context. The veil was finally lifted and I saw who Marston really was,” said Chris Weiss. Educating Marston is available for purchase now. The hardcover retail cost is $24.99 and is available for purchase on Amazon, Barnes & Noble Booksellers, Indie Bound, Walmart and Bam! Books-A-Million. Epigenetics expert Dana C. Dolinoy of the University of Michigan School of Public Health illuminates how our nascent understanding of the epigenome is leading to breakthroughs in understanding the causes—and potential treatment—of some public health problems. Co-hosts Anne Chappelle and David Faulkner also talk with Dr. Dolinoy about the collaborative science happening in the field and what the future holds for epigenetics. About the GuestDana C. Dolinoy, PhD, received her PhD in genetics and genomics and integrated toxicology and environmental health from Duke University in 2007 and conducted her postdoctoral fellowship in radiation oncology at the Duke University Medical School. She is currently the NSF International Chair of Environmental Health Sciences and Professor of Environmental Health Sciences and Nutritional Sciences at the University of Michigan School of Public Health, as well as Faculty Director of the Epigenomics Core at Michigan Medicine. Through these roles, Dr. Dolinoy provides a rigorous environment for training future scientists, instilling in them the need for critical experimentation and evaluation and interpretation of data.Dr. Dolinoy has been involved in evaluating the effect of environmental exposures on the pathogenesis of diseases for over 15 years. An internationally recognized leader in the field of environmental epigenetics, Dr. Dolinoy leads innovative research evaluating how the health effects of environmental exposures can be mediated through changes in the epigenome. Within the past five years, Dr. Dolinoy has conducted numerous studies, both in her laboratory and in collaboration with others, to demonstrate how exposure to a variety of environmental chemicals—including bisphenols, phthalates, and metals—can cause specific alterations in the epigenome.Dr. Dolinoy's research also is pushing the boundaries of understanding the epigenome as well as tools to evaluate this critical system in gene regulation. Most recently, she was able to demonstrate the tissue-specific expression of piwi-interacting RNAs (piRNA) in most mouse somatic tissue in the first study to do so. These small RNAs were previously assumed to be expressed exclusively in germ line tissues, where they are known to be involved in transposon silencing by DNA methylation. Thus, this study not only suggests a possible role of the Piwi proteins/piRNA in regulating the epigenome in the soma, but also offers the promise of a tool for epigenome editing to improve human health.Between 2015 and 2019, Dr. Dolinoy's publications received a total of 4,386 citations, resulting in a five-year h-index of 35, easily placing her in the top 2% of scientists in her field. Recognition of her work also is evident from numerous national and international invitations to present her research, as well as service as an Associate Editor of Toxicological Sciences and Environmental Health Perspectives and on the editorial boards of the Journal of Nutritional Biochemistry, Epigenetics, and Environmental Epigenetics.Dr. Dolinoy has been an active SOT member since 2005. Heart disease is the biggest cause of death in women, but until recently it wasn't the most widely known or discussed risk factor. Our heart health has been underappreciated, dismissed, misdiagnosed and under-treated for too long, and now it's time for it to become a top priority and concern in our personal Me-Care. By building our awareness around heart disease and taking the necessary preventive measures, we can rewrite the future of women's health in a huge way. What are the leading contributors to heart disease and why does age play a significant role? How does technology empower us as patients and improve our outcomes? What are some of the ways women can be empowered to become active participants in our own heart health? In this episode, I'm joined by one of health technology's most notable pioneers, Dr. Dave Albert. As a physician, inventor, serial entrepreneur and developer of life-saving technologies, he is playing a huge role in the realm of women's cardiology. We discuss why women's heart health is an urgent matter, and the heart health innovation that's changing and saving lives. 3 Things We Learned From This Episode The underlying cause of heart attacks and strokes A heart attack happens when a blood clot stops the blood flow that carries oxygen to the heart. A stroke is a blood clot that stops the delivery of oxygen to the brain. Our bodies need oxygen to function and even seconds of deprivation can cause a catastrophic cardiological event. Heart health is vitally important for women, especially women of color Even with huge improvements in the diagnosis and prevention of heart disease, there are still disparities in the level of healthcare available to women. Socio-economic factors contribute greatly to the level of care received, with women of color far more likely to suffer with poorer access, inferior service and higher death rates. Why technology is so pivotal in improving the patient outcome and experience When people have valuable health data and technology at their fingertips, it empowers us by making us active participants in our own health and care. It takes us from just following orders to an important part of the observation and decision making process. Guest Bio David E. Albert, MD, an Oklahoma native, is a physician, inventor and serial entrepreneur who has developed life-saving technologies and products over the last 30 years, turning a number of those innovations into tech startups. Today, he is a founder of three technology companies, InnovAlarm, Lifetone Technology, and AliveCor. His previous startups include Corazonix Corp (sold to Arrhythmia Research Technology) and Data Critical (sold to GE). Dr. Albert left GE in 2004 as Chief Scientist of GE Cardiology to disrupt several new markets. His latest invention, AliveCor's KardiaMobile personal ECG technology, became a global sensation via a YouTube Video in January 2011 around the Consumer Electronics Show and was featured on ABC, CBS, CNN and Fox News among many other media outlets. Dr. Albert has 32 issued US patents, a large number pending and several new “secret inventions” in development. He has authored or co-authored over 50 scientific abstracts and publications, principally in the cardiology literature. Dr. Albert has lectured at the Entrepreneurship programs at the MIT Sloan School and the University of Oklahoma. COVID-19: Commonsense Conversations on the Coronavirus Pandemic
The interview in this episode was recorded on March 30, 2020. In the episode, Dr. Ted O'Connell and Dr. Sallie Permar discuss various aspects of COVID-19 vaccine development for traditionally vulnerable groups such as pregnant women, infants, and children. Dr. Permar provides insight into what coronavirus vaccine development looks like now and what it may become in the future. Some questions covered include:Are the differences in rates of infection in infants and children related to how the virus attaches in their bodies, differences in their immune systems, or other factors? How does work developing vaccines against neonatal viral pathogens such as HIV and CMV relate to the development of a COVID vaccine? How does this challenge relate to the challenge of predicting which strains of influenza to include in the vaccine each year? Are there concerns about COVID-19 mutating or developing other strains?Sallie Permar is a physician scientist focusing on the prevention and treatment of neonatal viral infections. She leads a research laboratory investigating immune protection against vertical transmission of neonatal viral pathogens, has made important contributions to the development of vaccines for prevention of vertical HIV transmission, and is leading the development of HIV vaccine strategies in preclinical maternal/infant nonhuman primate models and translation of this work for clinical vaccine trials in infants. Dr. Permar has a PhD in Microbiology/Immunology from Johns Hopkins Bloomberg School of Public Health in Baltimore, an M.D. from Harvard Medical School and completed her clinical training in pediatric infectious diseases at Children’s Hospital in Boston. Dr. David Albert is a visionary physician and innovator in the field of digital health. As an entrepreneur he has developed medical products and nurtured new technologies over the last 30 years. A graduate of Harvard College and Duke University Medical School, Dr. Albert has founded three tech companies, InnovAlarm, Lifetone Technology and AliveCor his most recent endeavor, which has developed the mobile ECG or electrocardiogram sensor. The Kardia mobile device, which is aimed at everyone, detects and analyzes heart rhythm with a simple sensor and mobile app. The ECG - sometimes referred to as EKG - is the electrical activity of the heartbeat and the data could help to detect signs of heart disease. In this in-depth interview, recorded at the USC Center for Body Computing Conference, Dr Albert explains his vision for future medical technology, which he says has the potential to impact disease, aging and enhance human longevity. SHOW NOTES David Albert (@DrDave01) is the founder and Chief Medical Officer at AliveCor (@AliveCor ). LLAMA host Peter Bowes met David at the 2017 USC Center for Body Computing Conference, where he was presented, by Dr Leslie Saxon, with the inaugural USC CBC Digital Health Innovator Award. Watch videos from the conference here. “Peace of mind is often times as important as a piece of medical information. Anxiety and stress impact our lives, impact our longevity.” "I shame my children in their work out habits and that's had a positive impact on them. I think we can all have an impact on our fellow man and whether or not I live a year longer or not, the quality of my life will be better and I think that's key. You know it's not just longevity it's quality." “You can have a direct impact with your lifestyle on your longevity and on your functional longevity, that is your quality of life, at an older age." “Affluence impacts longevity anywhere in the world so if you are more affluent you will live longer. Now that's the kind of discrimination that I think we have to work to fight." "Knowing facts is not nearly as important as being able to adapt to a changing environment and being able to continuously learn."
