The BIO in Biopsychosocial – Medications for Sexual Health In this episode, Dr. Mona Gupta and Dr. Melissa Dahir join Dr. Kelly Casperson to discuss the importance of sexual health and the misconceptions surrounding it. They emphasize that sexual health is a bio-psycho-social issue and should be treated as such. The panel also addresses the black box warnings and misconceptions associated with sexual desire drugs, highlighting the benefits and considerations of these medications. They debunk the myth that alcohol is unsafe to consume while taking these drugs and discuss the positive effects of these medications on sleep. The panel also explores the sexual side effects of antidepressants and provides strategies for managing them. They delve into the use of testosterone replacement therapy and its impact on sexual health. Finally, they offer advice on navigating medications and seeking help for sexual health issues. Takeaways Sexual health is a bio-psycho-social issue that should be treated holistically. Addyi – how it works, its influence on sleep, and it's safety profile, and it works in pre and post menopausal women. Black box warnings and misconceptions surrounding sexual desire drugs should be reevaluated, as these medications can have significant benefits for individuals. Alcohol consumption is not necessarily unsafe while taking sexual desire drugs, and the effects of these medications on sleep can be positive. The sexual side effects with SSRI's and common psychiatric medications. Antidepressants can have sexual side effects, but there are strategies for managing these issues, such as adjusting the dosage or adding complementary medications. Testosterone replacement therapy can be an effective treatment for sexual health issues, particularly when used in conjunction with other therapies. Dr. Cigna is also the head of the Female Sexual Medicine Fellowship at George Washington University. And guess what? Our guest this time is Dr. Pebble Kranz from the Rochester Center for Sexual Wellness. She is a Family Medicine doctor with a background in theater who now specializes in sexual medicine. Mentored by a beloved sex therapist, the late Gina Ogden, she has been trained in sexual medicine through the European Society of Sexual Medicine and became a Fellow of the European Committee on Sexual Medicine (FECSM) in 2018. Because one training program is never enough, she also has trained with the International Society for the Study of Women's Sexual Health (ISSWSH), the American Association of Sexuality Counselors Educators and Therapists (AASECT), and the World Professional Association on Transgender Health (WPATH) where she is pursuing certification in gender-affirming medical care. Dr. Kranz founded the Rochester Center for Sexual Wellness with her husband Dan Rosen, a sex therapist and supervisor in 2017. They see patients and clients of all genders and also do training for therapists interested in becoming sex therapists and sexuality counselors. Since 2021, she has been honored to serve on the board of the Scientific Network on Female Sexual Health and Cancer. Pebble talks with Sabitha and Jane about seeing her patients as whole people, exploring their life stories, asking what sex means to them in their own bodies, and enjoys teaching about so many areas of sexual health and wellness. I wasn't sure if I'd publish this as a podcast episode considering I'm exploring the new territory of just collecting information from health care providers that can support my mission to help people navigate the stigma that comes with not only a herpes, but any STI diagnosis. As I learn more, I learn more. While I filter and curate this information from them to you, maybe you'll have my same experience and growth while I'm learning out loud here through these conversations. So I decided to get their consent to share these recordings that aren't quite podcast episodes, but conversations with providers on their challenges and perspectives as they treat patients. Dr. Lourdes here is the first interview in that series. I look forward to sharing with you here. Lourdes Mantecón-Garza obtained her medical degree in 1982, at the medical school of the Autonomous University of Nuevo León, in Monterrey, Mexico. She continued with specialty studies in Internal Medicine at the SSA Metropolitan Hospital in the same city in 1988. She completed a master's degree in business administration at the University of Texas (MBA, UT-PanAmerican) in 2005. She continue with studies on human sexuality at the Institute for Advanced Studies of Human Sexuality (IASHS) and at the Buehler Institute, both in California USA. Dr. Mantecón is a certified clinical sexologist by the American College of Sexologists International and is also a certified sexuality counselor by the American Association of Sexuality, Educators, Counselors and Therapists (AASECT) also in the US. Dr. Mantecón -Garza has fellow studies in women's sexuality by the International Society for the Study of Women's Sexual Health (ISSWSH). She is a visiting professor for human sexuality readings at different educational institutions in Mexico (UANL) and the United States (UTRGV school of medicine and family residency). She has presented research papers at national congresses in Mexico and the United States and world ISSM (International Society for Sexual Medicine) 2018, WAS (World Association for Sexual health) 2019 and ISSSWSH 2020. In addition to her practice, Dr. Mantecón Garza decided to write reference books that could be available to the population without the difficulty of medical technicalities. His first work, “The diet and the diabetic”, was published by Editorial Trillas in 1996, the second is “Healthy Sexuality” in the process of printing with the same publisher. She is dedicated to work as sexuality counselor at her office in McAllen Texas, she also offers services through telemedicine in English and Spanish.

For too long, women's health problems have been a mystery in the medical field. Often they are under-researched or just brushed off as part of the normal female experience. But with the help and expertise of our wonderful guest today, Dr. Andrew Goldstein, we are gaining massive insight and making large strides towards solving many of the issues that affect women's bodies. Dr. Goldstein is the Director of the Centers for Vulvovaginal Disorders and is currently a Clinical Professor at the George Washington University School of Medicine. On the show today, Dr. Goldstein chats with us about the plethora of research and clinical work he has done over the years on vulvar and vaginal pain. Dealing with sexual problems involves desire, arousal, orgasm, and pain and entails varying treatments. Sometimes a vaginal estrogen prescription will help postpartum for better and faster tissue healing. Physical therapy can also help with muscle healing postpartum. The secret to remaining sexually active with a sexual partner is having both individuals gain joy and pleasure - it is never one-sided. The genitals are capable of bringing so much joy and life forms through them. Therefore, it should not be seen as shameful or disgusting. How are the different treatment options administered? And what do you need to monitor and watch out for when receiving testosterone replacement?
