Our guest this time is Dr. Daniel N. Watter. Dan is an existential sex therapist and he has so much to say about appreciating sexuality as we age instead of extolling the virtues of youthful sexuality. He has a fresh approach to the connection between death anxiety and sexuality. Dan talks with us about the existential importance of the penis and the idea that the penis is speaking but sometimes in a whisper. Dr. Watter is the author of more than 30 professional articles and book chapters on topics such as sexual function and dysfunction, and ethics in healthcare practice. He most recently completed the new book, The Existential Importance of the Penis: A Guide to Understanding Male Sexuality and a new article about men, sex, and aging for the Journal of Sexual Medicine. Dan has been a practicing clinical and forensic psychologist and certified sex therapist for more than 35 years. He is licensed as both a psychologist and a marital and family therapist. In addition, he is Board Certified in Sex Therapy by the American Association of Sexuality Educators, Counselors, and Therapists (AASECT), and the American Board of Sexology (ACS), of which he also holds Fellowship status. Dr. Watter is an AASECT certified sex therapy supervisor and has been elected to Fellowship Status in the International Society for the Study of Women's Sexual Health (ISSWSH). In addition to his clinical practice, he is a faculty member at the University of Michigan School of Social Work's Sexual Certification Program and the Modern Sex Therapy Institutes. He has taught at a number of colleges, universities, and medical schools in the past. Natasha Helfer received her bachelor’s degree in Psychology from Brigham Young University and her master’s degree in Marriage and Family Therapy from Friends University. She is also an AASECT Certified Sex Therapist.  Insights: Natasha: "...sexual shame is a public health crisis. It really is a crisis. And living here in Utah, we've got, you know, air crisis. You know, our air is kind of polluted, and we passed the thing about pornography being a crisis. I mean, you know, there's lots of ideas about what is a crisis, but I really, truly believe that sexual shame hits. So deep and can affect so many things, physical things, like vaginismus, like erections, like stomach issues, you know, like indigestion, like immune immunity systems. It can affect our emotional health. You know, if we have sexual shame, it can lead to depression, anxiety, it can lead to scrupulosity and OCD." Liz: "Those who have chosen to not have sex before marriage, or at least not in this relationship, they wanted to say that and to be really instructive of them to someone right, a professional or a parent or a friend an auntie, to say, let's talk about what you can expect. Let's talk about really giving yourself some time on that even, I think you said three to six months, maybe Natasha right to really get your bodies adjusted and how this is going to be, and ways to converse about your sexuality and your preferences. Don't expect that just to take off on that first night. There's a lot of disappointment, a lot of shame. I think that comes from that not being what you're what you think your partner wanted." Dave: "We talked a little bit about parenting and the approach the parents take, they have to be proactive and intentional, thoughtful about the messages. Because, Natasha, you talked about you can take, you know, one direction and induce more shame, or it can be a positive, uplifting experience for parents that they've got to be willing and you have to talk about this topic. So I think, yes, first, I think it starts with parents and when children are young, and then keeping it it's not a one and done conversation, keeping that conversation going. And yes, absolutely. Couples before marriage, got to be able to talk and then keep the conversation going afterwards. It can't be something that all sudden. It's just silent and we don't talk about afterwards, or that was uncomfortable, or what happened there, and it's now I'm embarrassed and the shame cycle, wow, we've got to be able to, just as a broader society be able to talk and share and be open and honest in our conversation." If couples only engaged in sex when both partners were spontaneously in the mood, they might seldom have sex. Love the concepts of reactive and proactive desire, and engaging physically can lead to a physiological response where the body releases dopamine and serotonin, aligning physical readiness with emotional desire. In this interview, I speak with Diane about her expertise in sex therapy, and her American Association of Sexual Educators, Counselors, and Therapists (AASECT) certification. She discusses sex therapy, couples therapy with sex issues, and she discusses the PLISSIT model, which describes various levels of intervention, which include Permission, Limited Information, Specific Suggestions and Intensive Therapy. She discusses her eclectic approach, which is grounded in a Humanistic perspective, helping clients accept themselves and address shame. She explains that sexual relationships are complex involving how each partner feels about each other, the challenges people experience in their life, and their different stages in their relationships. She discusses her blog article, “To the Wife Upset About Her Husband's Porn Viewing: An Open Letter From a Sex Therapist”, and how she addresses how porn is symbolic and is a “fantasy” and what we can learn from this about our partner. She explains how she often sees men who come in by themselves, because they're partner is upset with them about their pornography use, and how this often suggests an Identified Patient perspective, and missing the couple dynamics and how the couple can heal and improve their relationship together. We also discuss the conflict between being respectful of women, but then also, for heterosexual men particularly, to also be enjoying pornography and objectification of women. She discussed perpetrator fear and how it become more pronounced after the #MeToo movement, and how a number of clients came to work with her to discuss their past sexual encounters. She discusses consent and what that means and looks like in sexual encounters and relational sexual relationships. Diane Gleim, LMFT is a Licensed Marriage and Family Therapist and AASECT Certified Sex Therapist and Supervisor practicing online throughout California and in-person in Santa Rosa, California. As the first Certified Sex Therapist in Sonoma County, Diane treats the many diverse issues related to sexual identity, sexual expression, sexual behavior, and sexual relationships. Her clients include everyone with a sexual concern: individuals and couples ages 18-85, and the LGBTQ+, kinky, and poly populations. When a person is told they have breast cancer, there are many emotions that come from that diagnosis, including grief and fear. Lisa Chism is very aware of these emotions and the way they continually show up throughout the journey of battling breast cancer. This is why she looks at the whole picture with those affected by this diagnosis – from their sex lives, care and well-being, and how menopause or perimenopause is affecting them. In this episode, Kara and Lisa discuss the many things a person can experience with breast cancer and how to advocate to get the best care they deserve.  Lisa Chism holds a Doctorate of Nursing Practice (DNP) and is the clinical director of Oakland Macomb Center for Breast Health. She holds three specialty certifications which include certification as a Menopause Practitioner through the Menopause Society, certification as sexuality counselor through the American Association of Sexuality Therapists, Counselors and Therapists (AASECT), and after 15 years caring for breast cancer survivors and patients at high risk for breast cancer, she became certified in breast care through the Oncology Nursing Society. Lisa has established a dedicated menopause and sexual health clinic caring for the menopausal and sexual health needs of women who have a history of breast cancer or are at elevated risk for breast cancer.  Lisa has authored numerous publications related to women's healthcare including serving as lead author of the 2023 Menopause Society's Non hormonal position statement. She guest lectures at universities across the country and her textbook The Doctor of Nursing Practice: A Guidebook for Role Development and Professional Issues is in its fifth edition. She is a Fellow of the American Association of Nurse Practitioners and was selected as Menopause Practitioner of the year 2011. She formally served on the Board of Directors at the North American Menopause Society as well as a federal advisory committee with the CDC regarding breast cancer in young women. In 2022, she self-published her debut memoir, "Phreak Like Me: Becoming The Real Hot Girl Doc," revealing her profound personal healing. Dr. Francis also introduced an apparel line embodying the movement's principles: bodily autonomy, sexual agency, and pleasure. Presently, she contributes to the Diversity, Equity, and Inclusion committee at the American Association of Sexuality Educators, Counselors, and Therapists (AASECT) and the Leadership Collective of the Women of Color Sexual Health Network (WoCSHN). The old saying "to get to, you must go through" is real. Therapy is a tool that many use to make sense of their behavior patterns to help discover the pieces that contribute to life's blocked path. In many cases, this process includes confronting elements of your life that you've avoided by being in "autopilot" mode. An area impacted most by autopilot is our relationships. During this episode, K.S. lewis and I discuss the importance of communication when it comes to intimacy, and how the health of your relationship with others is directly related to the relationship you have with yourself. Kay Lewis is a certified relationship coach, and psychotherapy intern scheduled to graduate in 2024 with a Master's degree in Marriage and Family Therapy from Capella University. Her clinical focus is sex therapy, sexual functioning/issues, anxiety, life transitions, exploration, PTSD, trauma, sexual mindfulness, somatic practices, and culinary therapy. Kay holds memberships in both the American Association of Sexuality Educators, Counselors, and Therapists (AASECT), and the American Association for Marriage and Family Therapy (AAMFT).

Our guest this time is Dr. Pebble Kranz from the Rochester Center for Sexual Wellness. She is a Family Medicine doctor with a background in theater who now specializes in sexual medicine. Mentored by a beloved sex therapist, the late Gina Ogden, she has been trained in sexual medicine through the European Society of Sexual Medicine and became a Fellow of the European Committee on Sexual Medicine (FECSM) in 2018. Because one training program is never enough, she also has trained with the International Society for the Study of Women's Sexual Health (ISSWSH), the American Association of Sexuality Counselors Educators and Therapists (AASECT), and the World Professional Association on Transgender Health (WPATH) where she is pursuing certification in gender-affirming medical care. Dr. Kranz founded the Rochester Center for Sexual Wellness with her husband Dan Rosen, a sex therapist and supervisor in 2017. They see patients and clients of all genders and also do training for therapists interested in becoming sex therapists and sexuality counselors. Since 2021, she has been honored to serve on the board of the Scientific Network on Female Sexual Health and Cancer. Pebble talks with Sabitha and Jane about seeing her patients as whole people, exploring their life stories, asking what sex means to them in their own bodies, and enjoys teaching about so many areas of sexual health and wellness. I wasn't sure if I'd publish this as a podcast episode considering I'm exploring the new territory of just collecting information from health care providers that can support my mission to help people navigate the stigma that comes with not only a herpes, but any STI diagnosis. As I learn more, I learn more. While I filter and curate this information from them to you, maybe you'll have my same experience and growth while I'm learning out loud here through these conversations. So I decided to get their consent to share these recordings that aren't quite podcast episodes, but conversations with providers on their challenges and perspectives as they treat patients. Dr. Lourdes here is the first interview in that series. I look forward to sharing with you here. Lourdes Mantecón-Garza obtained her medical degree in 1982, at the medical school of the Autonomous University of Nuevo León, in Monterrey, Mexico. She continued with specialty studies in Internal Medicine at the SSA Metropolitan Hospital in the same city in 1988. She completed a master's degree in business administration at the University of Texas (MBA, UT-PanAmerican) in 2005. She continue with studies on human sexuality at the Institute for Advanced Studies of Human Sexuality (IASHS) and at the Buehler Institute, both in California USA. Dr. Mantecón is a certified clinical sexologist by the American College of Sexologists International and is also a certified sexuality counselor by the American Association of Sexuality, Educators, Counselors and Therapists (AASECT) also in the US. Dr. Mantecón -Garza has fellow studies in women's sexuality by the International Society for the Study of Women's Sexual Health (ISSWSH). She is a visiting professor for human sexuality readings at different educational institutions in Mexico (UANL) and the United States (UTRGV school of medicine and family residency). She has presented research papers at national congresses in Mexico and the United States and world ISSM (International Society for Sexual Medicine) 2018, WAS (World Association for Sexual health) 2019 and ISSSWSH 2020. In addition to her practice, Dr. Mantecón Garza decided to write reference books that could be available to the population without the difficulty of medical technicalities. His first work, “The diet and the diabetic”, was published by Editorial Trillas in 1996, the second is “Healthy Sexuality” in the process of printing with the same publisher. She is dedicated to work as sexuality counselor at her office in McAllen Texas, she also offers services through telemedicine in English and Spanish.

First, Dr. Winter explains her role as a urologist in providing hybrid sexual health consulting for men and women. Then, the doctors discuss basic sexual history intake questions for women. Although there are many screening questionnaires, Dr. Winter prefers to use the Female Sexual Function Index because it evaluates sexual desire, arousal, pain and orgasm ability. She also makes sure to ask about issues indirectly related to sex, such as lower urinary tract symptoms, recurrent UTIs, and pelvic pain. Performing a complete physical exam is important in patients presenting with sexual dysfunction. Dr. Winter explains her usual genital exam, paying close attention to any abnormalities regarding the clitoris, labia majora/minora, and vestibule. She also performs a pelvic floor exam. She does not usually order a hormone panel, but may check TSH and HbA1c to rule out diabetic neuropathy. Next, Dr. Bagrodia and Dr. Winter delve into the evaluation and management of specific sexual dysfunctions. Low libido, or hypoactive sexual desire, is a common issue for women. It can be a result of menopause, oral contraceptive use, cystectomy, postpartum concerns, vulvar disorders, selective serotonin inhibitors, history of breast/cervical cancer, or history of abuse/trauma. For patients in the last category, Dr. Winter encourages collaboration with social workers and therapists. She mentions that the American Association of Sexual Educators, Counselors, and Therapists (AASECT) is a great resource for finding these professionals. For peri/post-menopausal women, she recommends prescribing a testosterone gel. Additionally, non-hormonal treatments for low libido include flibanserin and bremelanotide. For issues regarding sexual arousal, Dr. Winter emphasizes proper education and screening for diabetes first. If arousal is inhibited because of a lack of lubrication, then correction of estrogen levels may be necessary. Estrogen supplementation, or topical/vaginal estrogen, can be used to treat problems with arousal, as well as recurrent UTIs and genitourinary syndrome of menopause (GSM). Dr. Winter notes that these low doses of estrogen preparations are unlikely to actually raise blood estrogen levels and cause systemic side effects. Finally, in patients who complain of pain with sex, Dr. Winter recommends education about lubricants and pelvic floor physical therapy. --- RESOURCES AASECT: https://www.aasect.org/

Colonization and Its Impact on Modern Sexuality We tend to think of colonization as something that happened and is over and is done, without realizing that it set up processes and expectations, beliefs, and systems of thought that we are still living with in this current day. This has created historical trauma that remains today. There is a legacy of shame and of limitations that came with settlers in North America. Anne Mauro has been studying this, and we talk about this whole concept of the sexuality that the settlers brought in and what this has come to mean for all of us. We discuss the ways in which it could be manifesting and limiting us, and how it is certainly impacting how people of color, women of color are treated still in this culture. What is settler sexuality? We know that when the settlers arrived in North America, some were coming for a better life and to avoid persecution for their own religious beliefs. And when they arrived, they had their own ideas of what sexuality was, and a lot of that was a belief that it was solely for procreation. But with indigenous people, they saw two spirit people, or a matrilineal model, instead of a patriarchal model. They saw homosexuality, and they saw indigenous people engaging in sexual play outside of marriage. They were completely appalled by this. Their idea of sexuality was no sex before marriage, you are property of your father until you're married, and then you are property of your husband. “We don't want you masturbating, or talking about menstruation, that is bad. You're not supposed to be nude. You're not supposed to have inordinate affection, or too much desire or affection.” Also, women are supposed to dress a certain way. They're supposed to be homemakers and don't work outside of the house. You're supposed to stay a virgin, not just for the religious reasons, but there was economic value in virginity if you were seen as pure. The shame came across with the settlers If you didn't fall within the settler sexuality model, you could be publicly shamed, whipped, or tortured. People were burned for masturbation and for homosexuality. They were shamed for anything that was falling outside of this model. If you got caught, you could get in trouble. When the settlers came, they brought with them their own historical sexual trauma. Still impacting today Anne believes that with the sexual script that's inherited in American sexuality, there is a maltreatment especially of women of color, but of people of color in general. This legacy of the settler sexuality construct has dramatic impacts today, leaving people feeling like there is something wrong with themselves if they don't fit into it, and very few people do. Biography: Anne Mauro is a Licensed Couples and Family therapist, AASECT Certified Sex Therapist, and AASECT Certified Sexuality Educator. She has earned her M.A from Antioch University Seattle (AUS) and a Doctorate of Human Sexuality from the Institute for the Advanced Study of Human Sexuality. Her private practice, Mending Connections, in Tacoma, Washington, specializes in couples counseling and sex therapy. Anne serves as adjunct faculty at AUS, where she created and taught a course titled Colonization and Sex for the Sexuality Certificate Program. Additionally, Anne works in the Couples and Family Therapy program providing clinical supervision to graduate student interns. In partnership with a colleague, Anne is an American Association of Sexuality Educators, Counselors and Therapists (AASECT) continuing education (CEU) provider. Through this venture, Anne co-created the Beyond Settler Sex Sexually Attitude Reassessment (SAR). In this episode we see how communication can affect a person's sexuality and intimacy and how we can work towards that with our partner. Communication, vulnerability and how to have conversations with our partners Kristen correlates communication with vulnerability because we often don't talk about sex, it makes us uncomfortable. Her book starts with communication, vulnerability and normalizing the two. It's overwhelming to read hundreds of pages of a book to be aware of a single topic. Kristen says she wanted some cut to the chase, something practical to read and to apply it in real life. So, she added exercises at the end of every chapter that includes a lot of conversation starters for couples. There are several variables that come into play. Our upbringing has a huge impact on how we communicate in our relations. The way families communicate with each other reflects in how they communicate in their relationships. It's about acknowledging that. Problems with ineffective communication and what's the answer? Kristen describes defensiveness and avoidance as the biggest pitfalls people have with communication. We're so engrossed in defending ourselves that we forget to listen. We also avoid difficult conversations especially about sex because it makes us uncomfortable but Kristen urges us to be uncomfortable. She emphasizes that it's better to be uncomfortable than to avoid the conversation. How does vulnerability play a role in communication? Vulnerability isn't necessarily shown in the matters of sex, even sharing our emotions could be vulnerable. Kristen explains it by sharing a heart-warming example from her personal life. When we share our emotions, and they aren't reciprocated or validated immediately, it makes us feel exposed and thus leaving us vulnerable. But as the conversation goes, Kristen poses a question of whether it's better to have a conversation rather than keep assuming that someone loves you. She says, “There's validation in acknowledging it”. What do you think prevents couples from being vulnerable? Kristen believes fear of judgement from your partner and presuming a partner's reaction keeps us from opening up. We rather let the problems pile up than have a conversation that leaves us with shame and guilt. But If you want to do something differently, give them the opportunity to surprise you, to say something differently even though you know what they're going to say. Kristen talks about how important it is to “just listen” and she emphasizes that you don't have to get judgmental or commit to something but just express gratitude for sharing and then revisit it later. It makes the partner feel safe to have an open and honest conversation with you. Exercises for couples around these ideas: Kristen suggests practicing “Pancake talk”. It involves processing an experience and talking about it at a later time in a neutral territory. It gives you an opportunity to not be reactive and get away from high intensity emotions. She refers to ‘four horsemen of John Gottman' while talking about being mindful with your responses and listening without getting defensive. She suggests repeating it back just like in ‘Imago therapy' and mirroring exercises which compels you to slow down and listen to what the other person is saying. She also talks about risky conversations about a kink or a fetish or wanting to talk about being polyamory doesn't have to be difficult. Your partner might understand what you're communicating with them and acknowledge that but it doesn't necessarily mean they're agreeing with you. Kristen explains that with an example, you can be mad at your partner about something and have consensual sex and still not forgive them. Kristen encourages people to look at their childhood and connect the dots with who they are now. It can be done by questioning how you got your sex education. She talks about unwinding the messages of religious upbringing and false information. Sensate Focus Exercises Sensate focus is a touching technique for couples or individuals that stimulates the primal part of the brain to enjoy a fully immersive experience with your partner. The technique starts with focusing on temperature, texture and pressure and uses a tactile sensation to move away from distraction. Linda uses the technique for almost everything ranging from low/ no desire, sexual dysfunction, trauma, body image issues to rekindling connection between couples. One Size Doesn't Fit All With each individual experiencing sensation differently, Linda highlights building up sexual tension with touch. From Linda's experience, there is often a need to manage each level as it is approached. She removes conflict and pressure from couples relationships, by taking on the instruction/ control role. Her sessions help couples identify the difference between vulnerability and rejection. How Do You Start With Sensate Focus? With no hard and fast rule, couples can choose certain factors in their environment to ensure that they are at ease eg clothing (or no clothing). Linda's rules include no kissing and no talking. She points out that talking uses the front of the brain and therefore brings individuals back to logic. Linda mentions that the toucher is supposed to touch for their pleasure while the person being touched, needs to experience the touch and provide feedback if something is not comfortable. Obstacles Linda shares that avoidance is one of the main issues that couples encounter. She reveals how she handles this delay tactic fairly. For clients that don't like the technique, she reminds them about the basic three areas of focus temperature, texture and pressure. Partner pressure is an obvious obstacle that Linda notices with her clients. She uses a great analogy to help us accept our differences and move couples through the basic steps at their own pace.
