Today we're diving into insulin resistance—a condition that affects millions but often goes unnoticed until it leads to more serious health issues like type 2 diabetes and metabolic syndrome.  Understanding what it is, how it develops, and how to manage it is important for maintaining overall health. What is insulin? Let's start with the basics. Insulin is a hormone produced by the pancreas that helps regulate blood sugar levels by facilitating the entry of glucose into cells for energy. When someone has insulin resistance, their body's cells don't respond properly to insulin, leading to elevated blood sugar levels. This means the pancreas has to produce more insulin to achieve the same effect, resulting in higher insulin levels in the blood—a condition known as hyperinsulinemia. Over time, this can lead to type 2 diabetes if not addressed. What causes insulin resistance? So, what causes insulin resistance? Several factors contribute, including: Obesity: Excess fat, especially around the abdomen, can interfere with insulin's action.​ Physical Inactivity: A sedentary lifestyle reduces the body's sensitivity to insulin.​ Poor Diet: Diets high in processed foods and sugars can contribute to insulin resistance.​ Genetics: A family history of insulin resistance or diabetes or belonging to Black, Asian, and/or Hispanic ethnic groups can increase the risk. It's also important to note that certain health conditions, like high blood pressure, high cholesterol, and gestational diabetes (diabetes in pregnancy) can be associated with insulin resistance. Chronic stress and steroid medication use can also increase a person's risk of insulin resistance. What are the signs and symptoms of insulin resistance? Now, let's talk about the signs and symptoms. Insulin resistance often develops gradually and may not present noticeable symptoms initially. However, some indicators include: Irregular menstrual cycle Hirsutism (excessive body hair) Dark Patches of Skin (Acanthosis nigricans): Areas like the neck or armpits may develop dark, velvety patches in the skin folds or creases. Weight gain: As glucose levels increase in the blood, the body tries to remove it by storing the excess in the liver and muscles. The excess glucose gets stored as fat. Let's talk about this process a little more in depth. When you consume food, your body breaks it down into glucose, which is the primary source of energy for your cells. After eating, glucose enters your bloodstream, raising your blood sugar levels. To help regulate this, your pancreas releases insulin, a hormone that signals your cells to absorb glucose for energy or storage. Now, here's how excess glucose gets stored as fat: Energy Needs Met: Your body uses glucose to fuel immediate energy needs, like physical activity or basic bodily functions. But when there's more glucose than the body can use right away, it needs to store the excess. Liver and Muscle Storage: The first place the body stores extra glucose is in the liver and muscles. This glucose is stored as glycogen, which is a form of glucose that can be quickly accessed for energy when needed. Conversion to Fat: However, there's a limit to how much glycogen the liver and muscles can store. Once these storage spaces are full, the body starts converting the remaining excess glucose into fat. This process is called lipogenesis. The glucose molecules are turned into fatty acids, which are then stored in fat cells (adipocytes) throughout your body. Insulin's Role in Fat Storage: High levels of insulin, which can occur when you're eating a lot of sugar or processed carbs, promote fat storage. Insulin doesn't just help glucose enter cells—it also signals fat cells to store fat, preventing the body from breaking down stored fat for energy. Over time, when you consistently consume more glucose than your body can burn, this leads to an accumulation of fat, particularly in areas like the abdomen. This is why excessive carbohydrate consumption, especially refined sugars and processed foods, can contribute to weight gain and fat storage in the body. Essentially, the body is designed to store extra energy for times when food isn't as readily available—but when this process happens too often (like in the case of chronic overeating or poor dietary habits), it can lead to fat buildup and weight gain. What are the complications of insulin resistance? Metabolic Syndrome: Metabolic syndrome is a cluster of symptoms that includes high blood pressure, high blood sugar, excess abdominal fat, and abnormal cholesterol levels, which increase the risk of heart disease, stroke, and type 2 diabetes. Prediabetes: Elevated blood sugar levels that aren't yet high enough to be classified as diabetes.​ Fasting blood sugar: Between 100 and 125 mg/dL (5.6 to 6.9 mmol/L). Oral glucose tolerance test (OGTT): A 2-hour blood sugar level between 140 and 199 mg/dL (7.8 to 11.0 mmol/L) after drinking a glucose solution. Hemoglobin A1C: Between 5.7% and 6.4%. Type 2 Diabetes: A chronic condition where the body becomes resistant to insulin, leading to high blood sugar levels.​ To diagnose diabetes using fasting blood sugar or A1C, two separate readings that meet the diagnostic criteria are required.  A fasting blood sugar level of 126 mg/dL (7.0 mmol/L) or higher on two separate occasions. An A1C level of 6.5% or higher on two separate occasions. As you can see, It's crucial to address insulin resistance early to prevent these complications. While there's no direct test for insulin resistance, healthcare providers often assess risk factors and may use blood tests to evaluate blood sugar levels and insulin sensitivity. The good news is that insulin resistance can often be managed or even reversed through lifestyle changes. Key strategies include: Regular Physical Activity: Engaging in consistent exercise helps improve insulin sensitivity.​ Healthy Diet: Focusing on a balanced diet rich in vegetables, lean proteins, and whole grains can aid in managing insulin resistance. ​ Weight Management: Losing excess weight, especially abdominal fat, can significantly improve insulin sensitivity.​ Ideally, aim for 30 minutes of physical activity on most days of the week. Stress Management: Chronic stress can worsen insulin resistance, so incorporating stress-reducing activities is beneficial. Formally a sexual abuse therapist and child abuse social worker, Alison brings her training and experience to her writing and activism. She founded a ground-breaking and award-winning non-profit for adolescent girls that published Teen Voices magazine. She authored Unwanted Hair and Hirsutism, a Women's Book. She has served on the boards of many nonprofits that seek to enhance the quality of life for women and girls, ranging from reproductive rights to media and democracy. Check out today's questions:    Tara: Hello, My name is Tara and I've had stomach issues my whole life. Everyone said it was just a stress stomach. Within the last 6 weeks I was introduce to a natural path doctor who has brought this stomach issue of mine to a head. She believes I have Biofilm. I have tried researching this and cannot find many people speaking about it. My doctor and I have started a plan on how to tackle it but curious if Dr Cabral has dived into this as well? I have been eating great and taking top of the line suppliments and test I am still deficient in all B's, D, C , folate, etc. I resonate with so many of these podcasts and yet I have not heard anything on this issue of biofilm. I am just curious if Dr Cabral has any insight on this topic. Thank you   Meagan: Hello Dr. CABRAL! I think I may have heard you mention palate expanders on one of your episodes discussing mouth breathing. Can you expand on that a bit? I'm debating getting one for my 7 year old daughter but want to weigh the pros and cons. Thank you!!   Bailey: I have a 12 month old who has a dairy allergy. I completely avoided dairy & gluten when I was breastfeeding her due to colic and digestive issues. I started her on raw goats milk after nursing, she wasn't a fan.. so she drinks raw cows milk. I plan to switch to nut milk / only water once she is getting more calories and fat from food. One day we ran out of milk, so I gave her some store bought whole milk (temporarily), she developed digestive pain, and hives shortly after. The hives have lasted almost a week now, I know dairy takes a long time to leave the system. I feel like I did everything right with her. She was born vaginally, is unvaccinated, she has never been on antibiotics, she took a probiotic with her breastmilk the whole year she breastfed, dairy wasn't introduced to her till 12 months, we live in the country without a ton of environmental toxins, yet she has this allergy. I myself have always been sensitive to dairy, so my guess is genetics. My question is, what is the bottom line.. what is the ROOT cause of so many childhood food allergies? & Why do we "out-grow" them? This is becoming such a norm.   Jessica: Hi Doctor Cabral! Thank you for being a source of well-studied information on all things health and wellness. I have been looking into the iodine controversy and whether or not iodine is beneficial or harmful for thyroid patients, specifically Hashimoto's. There is research on how going on a low-iodine food diet along with no supplemental iodine in any form with support the thyroid in healing from Hashimoto's. I have heard strong views from both sides, and I am hoping to get your view on the topic and how you would approach this subject in the helping of a family member.   Kasia: Hi Dr. Cabral, Hope this email finds you well. I watched your video about Serrapeptase enzyme. Thank you so much for the detailed informative and useful information. I have silicone breast implants. I have purchased the Serrapeptase product for my chronic nose sinus, but I am concerned it might deteriorate the capsule that naturally form around the a breast implants. I've read on line it eats non-living living tissues; including scar tissue and maybe capsule. Others note, it should be fine to use for someone with breast implants. Do you know if taking the enzyme Serrapeptase, can be taken by someone with breast implants? Do you know of women with breast implants, who have been taking it? What's their feedback? For the past 3-4 weeks, I have been taking a daily therapautic dose of about 120,000SPU. I have noticed my breast fat has slightly reduced. I don't know if it's because of the Serrapeptase, or the intermittend fasting I have started at the same time. I appreciate your input, and advise.   Anonymous: Hi Dr Cabral, I was wondering what can be the cause (and possible solution) for hirsutism, particularly if blood sugar levels are well managed, my periods are regular and I am not overweight. In the past I used the birth control pill for 7 years. I got off, had my two kids, had a chin hair here and there, but now, 2.5 years later after having my second child, I have a full-blown highly undesired situation happening on my chin and around a little the nipples. It's been at least a year and it's only getting worse. I've used a CGM for several months for the sake of research and investigation, and my blood sugar levels appear very well managed. I don't drink alcohol or coffee, nor do I smoke. I have no known health conditions. But I'm stressed and don't sleep super well. When I tested a cycle mapping of my hormones (Dutch test), my testosterone, estrogen and progesterone levels were fine BUT my adrenal glands might just hate me. Both free cortisol and cortisone were VERY low (possible due to a prolonged period of stress) while Etiocholanolone & Androsterone were both super high. I have no cysts on my ovaries and my periods are quite regular so I don't feel like it would be PCOS either. Please please please… help a sister out! The first area that I want to address is weight loss, and I have to say in the first place that I don’t even agree with that term!   During these seven episodes you will learn how to put on muscle and reconfigure your body composition, it is not about the number that the scale shows, but instead we should make our choices based on the composition of our body. I am also going to be giving you deeper information about your thyroid, estrogen, progesterone, androgens, DHEA and testosterone, aspects that could be contributing to conditions such as polycystic ovarian syndrome and Hirsutism (excess of hair growth). In this series, we will also dive deep into PMS and PMDD (Premenstrual dysphoric disorder). The whole purpose of this series is to bring you back to school, offering valuable information to promote a deeper connection between you and your body.   These are the main topics of the first episode of an amazing series:   I share my experience during my modelling years and how that opened my eyes in regards to body composition. Alopecia - hair falls out in the same area all at one time.  Kind of like crop circles. 3 Stages of the Hair Follicle life cycle - Anagen phase - active phase, cells are multiplying, hair is growing - Catagen phase - resting phase, "cat nap" - Telogen phase - shedding phase Hair thickness is determined by 2 things: the density of hair follicles (how many follicles in one area) and how large a single hair follicle is (thick vs fine hair). male patterned baldness - receding hairline from front to back, and the loss of the hair on the crown of the head. Female patterned baldness - the overall thinning of the hair. Hirsutism - females get male-patterned balding due to testosterone imbalance Why does all your hair fall out after you have a baby?? Telogen effluvium - a mass exodus of hair follicles This can also happen when the body goes through extreme shock or trauma. There is a condition called post-partum alopecia - this is temporary, and hair grows back afterwards. Itchy Scalp Dandruff is caused by the inflammation of the scalp.  Inflammation causes skin to be dry and itchy.  The topmost skin layer releases in larger amounts, thus the pieces are visible, opposed to when the skin naturally sheds, it's microscopic so you don't usually see the dead skin cells you lose. Tinea capitis - ringworm on your scalp, aka cradle cap.  Usually requires an antifungal to get rid of it. Tidbits Hair can actively grow for up to 6 years. Since hair is dead cells, hair hangs onto the DNA and it can be used to identify people (missing persons) or for paternity tests (who's your daddy)? Dying and perming affect the proteins that give it color or keep it connected. If someone has had prolonged exposure to toxin (carbon monoxide, heavy metals, smoking), the hair shows markers of that damage from the toxins.  It can also hang on to markers from being exposed to illicit drugs.  It's like a timeline of what you've been exposed to.
