In this episode, we interview Morgan Anderson, PharmD, BCIDP, a graduate of the RFUMS College of Pharmacy, about her career path from a pharmacy resident, emergency medicine specialist, infectious diseases specialist, and now a medical sciences liaison. Key Concepts Having a wide breadth of skills and making yourself marketable is important when transitioning between jobs or career paths. Skills like communication and teamwork can be improved and are applicable to a wide variety of careers within pharmacy. Medical Science Liaisons (MSLs) are a common role for pharmacists in the pharmaceutical industry. MSLs are field-based roles within the medical affairs department of the company. MSLs are medical and scientific experts who build collaborative relationships with key thought leaders, facilitate exchange of scientific information and insights, and serve as a conduit between these thought leaders and other areas of the company. Two common career paths to pharmacists becoming an MSL are via a fellowship program or after years in clinical practice. A fellowship program provides a more structured approach, including mentoring and networking, with access to a variety of areas of the company outside of medical affairs. A pathway after clinical practice is more self-directed with less structure, but provides pharmacists with a strong clinical background that can be helpful in an MSL role. Being a scientific communicator, possessing strong emotional intelligence, and being adaptable are critical soft skills that are essential for success in an MSL role. The Academy Exchange: HIV Today & Tomorrow

Dr. Milena Murray joins Bruce to discuss the implications of aging and HIV. Since nearly half of Americans living with HIV are over the age of 55, it is important to recognize their unique care needs as well as the risk of HIV in people over 55 generally. Bruce and Dr. Murray talk about the use of PrEP and how people can approach sexual health discussions with their providers. They also talk about what to expect with the use of antiretroviral therapies in conjunction with other medications people commonly use when they age. In this episode, we discuss six newer antibiotics that target multidrug resistant gram negative bacteria with Dr. Christie Bertram, PharmD, BCIDP. We review common resistance mechanisms, particularly to carbapenems, and highlight the current role in therapy for the following antibiotics: ceftolozane/tazobactam (Zerbaxa®), ceftazidime/avibactam (Avycaz®), meropenem/vaborbactam (Vabomere®), imipenem/cilastatin/relebactam (Recarbrio®), cefiderocol (Fetroja®), and eravacycline (Xerava®). Key Concepts Ceftolozane/tazobactam (Zerbaxa®) is primarily used for multidrug resistant Pseudomonas; it does not cover carbapenemase-producing organisms and (despite the tazobactam) needs metronidazole for intra-abdominal anaerobic coverage. Ceftazidime/avibactam (Avycaz®) is primarily used to cover CRE (Carbapenem-resistant Enterobacterales) but also has activity for many other gram negatives except Acinetobacter. Meropenem/vaborbactam (Vabomere®) has similar coverage to Avycaz® but may provide coverage for certain KPCs (Klebsiella pneumoniae carbapenemase). Vaborbactam does not restore activity for meropenem-resistant Pseudomonas. Imipenem/cilastatin/relebactam (Recarbrio®) has similar coverage to Avycaz® and Vabomere®; true niche in therapy is not yet well defined. Cefiderocol (Fetroja®) uses a unique mechanism to enter gram negative bacteria and has a broad spectrum of activity against carbapenemase-producing bacteria and many other multidrug resistant gram negatives. It has no gram positive activity. Eravacycline (Xerava®) is a tigecycline-like tetracycline with a broad spectrum of activity against carbapenemase-producing gram negative, gram positive, an anaerobic bacteria EXCEPT it lacks coverage for Pseudomonas. Outpatient Antiviral Treatment for COVID-19Whether you're a patient or a provider there are many considerations to know before taking or prescribing the new oral antiviral medications, Paxlovid, and molnupiravir. Indicated for the treatment of COVID-19, Paxlovid and molnupiravir are approved under the Emergency Use Authorization (EUA). Join me for a conversation with Drs. Tyler Melton and Brandon Hawkins, Pharm D, and assistant professors in the College of Pharmacy at the University of Tennessee Health Science Center as we discuss what you need to know about these medications. Paxlovid and molnupiravir, protease inhibitors, are highly effective in the treatment of COVID-19 in the outpatient setting but have very specific instructions for use. Drs. Melton and Hawkins discuss the drug-drug interactions with both medications, how long males and females must consider alternative methods of birth control during this drug administration and after, as well as both medications being limited to select pharmacy chains. More about Dr. Melton:Tyler Melton is an Assistant Professor in the Department of Clinical Pharmacy and Translational Science at the University of Tennessee Health Science Center (UTHSC), College of Pharmacy. He has ten years of pharmacy practice experience including inpatient and outpatient pharmacy in rural environments. Tyler joined UTHSC in September of 2020, after completing a postdoctoral research fellowship in Community Pharmacy Practice and Prescription Drug Abuse and earning his Master of Public Health from East Tennessee State University. Overview: Antimicrobial Stewardship is well established in many inpatient settings; however, outpatient programs are not as common despite high volume of inappropriate prescribing. Literature is lacking to help guide the best interventional strategies. Additionally, patient volumes are high and time to decision making is fast in the ambulatory and ED settings. Thus, strategies that are successful inpatient may not be feasible, successful, or sustainable in the outpatient setting. Finally, hospitals fund stewardship programs through pharmacy department resources or perhaps infection prevention or quality, but what about in the outpatient setting what are sources of funding? In our first episode of SHEA's Challenges and Innovations in Antimicrobial Stewardship podcast series, Dr. Rebekah Wrenn sits down with Dr. Larissa May of University of California Davis and Dr. Jeff Linder of Northwestern University Feinberg School of Medicine to address these topics and more.
