Maurice Enriquez-Sarano, MD, FACC JACC: Case Reports Associate Editor, is joined by author Emmanuel Daniel, MD, discussing this study from Daniel et al presented at ACC.25 and published in JACC: Case Reports. Large lead-associated vegetations carry a significant risk of pulmonary embolism with hemodynamic instability when transvenous lead extraction is attempted. In this case, a 54-year-old male came to the hospital with fever and dyspnea. He was found to have MSSA bacteremia and a large RV lead vegetation on echo. He had multiple co-morbidities and was a high-risk surgical candidate. He underwent successful vegetation aspiration using the Flowtriever system, followed by endovascular lead extraction.

I presume the lead story in the Herald on Sunday was welcomed by police and those behind our stronger gun laws.   An Auckland pensioner and his daughter have been caught for legally buying 13 guns for the Comancheros motorcycle gang.   It's the latest of dozens of discoveries by police of licensed gun owners buying for gangs who cannot legally purchase guns.   It was validation of good old-fashioned police work. Over the past 4 years the police have analysed more than 350,000 sales records looking for suspicious patterns of spending.    They then correlate the purchases with the records of gun owners, and they discover the gang's straw buyers.   But to me, it also validates the strengthening of the laws back in 2019 after the Christchurch mosque massacre.   That saw the banning of military-style semi-automatics, stricter rules on the “fit and proper” test to hold a license, the establishment of a gun registry, and a set of rules designed to ensure gun clubs and ranges are safe places.   At the time, gun owners made out that the laws criminalised legal gun owners which was a massive over-exaggeration.  It criminalised a type of gun only. A type of gun that non MSSA owners find intimidating and unnecessary. A gun that turns any idiot into a killing machine as long as they can handle the recoil.   And the new rules also told the citizens of New Zealand that ownership of a gun is not a right but a privilege that must be earned.     But they also say that once it's earnt there's no problem at all as long as it's not a military-style automatic.   And slowly it's beginning to make sense to even the law's hardest detractors.   Act campaigned on a full repeal of the legislation and the minister in charge is Act's Nicole Mckee   She is also the former spokesperson for the council of licensed firearms owners.   She was also interviewed in the paper yesterday where she said gun owners hoping for a rollback will be disappointed.   Act campaigned on greater access to MSSAs and scrapping the gun registry, but these didn't make it into coalition agreements.   Instead, the National-Act agreement committed to repealing the regulations around gun clubs and shooting ranges - which Mckee has now backed off from doing completely - a review of the registry and a rewrite of the arms act.   But, at the moment, the laws are being seen to work.  And here's the rub.  If you support getting tough on crime then you must also support getting tough on gun ownership.  LISTEN ABOVE.See omnystudio.com/listener for privacy information.

The online user is in a symbiotic relationship with the virtual world, which is optimal for dependency and cultural scripting. Big Tech is our Momma who bounces us on her knee and tells us how to live. The user is Mindless (passive) and Joyless (split from the body): a state of simple consciousness (i.e., Infant). This dependency contract (Mother-Infant) and the technological form holds us in place (e.g., in front of a desk or clutching a device). We brand and curate our online persona to make an Empire of the Self. The script is to become the Top Gun in order to “win” in the entrepreneurial game. To make oneself into a perfected trophy that goes “viral”. YouTube's slogan is “Broadcast Yourself,” which means track, record, upload, share, and incorporate the self for monetization. The mass shooter is the grandiose reflection of this scripting. Their act is neo-fascistic and imperialistic (i.e., a forced symbiosis for domination). For many of the killers, the Columbine massacre is the model that is remodeled: a performance of exhibitionistic revenge (violence/suicide) intended for online virality. The solution is to meet passivity (violence) with consciously passive behavior (withdrawal, contemplation, meditation). A stay-at-home-day where we connect and love one another. From the MSSA conference material (description and learning objectives): In the U.S., 51 people die from mass shootings each year. However, over 100 people are killed by guns each day. What do we know about American culture, violence, and mass shooters in the U.S.? How does online self-promotion for Likes and Friends relate to these phenomena? This lecture will present data on mass shootings, analyses of Big Tech, and illustrations of American culture to comprehend the truth of the present situation. Through grants, technical assistance, convening, advocacy, and policy initiatives, Angela's work in striving to end chronic homelessness and youth homelessness has resulted in state and local collaboration to meet the needs of the unhoused community.  Angela has worked closely on an initiative called A Place 4 Me, which brings together diverse community partners to collectively prevent and end youth homelessness, especially among youth who have aged out of the foster care system. Working closely with the program director of A Place 4 Me, Angela co-facilitated a community planning process to develop the Cuyahoga County Strategic Plan to Prevent and End Youth Homelessness. She also works on behalf of the foundation to garner private and public investment to support implementation of the plan.Angela has a Bachelor of Science in psychology and sociology from Kent State University and a Master of Science in social administration (MSSA) from Case Western Reserve University in Cleveland, OH.Links:1. New dipole instabilities in spherical stellar systems by Martin D. Weinberg. on Thursday 15 September Spherical stellar systems have weakly-damped response modes. The dipole modes are seiche modes. The quadrupole are zero pattern-speed prolate modes, the stable precursors to the radial-orbit instability (ROI). We demonstrate that small deviations in the distribution function (DF) can destabilise the dipole modes and describe the newly identified instabilities in NFW-like dark-matter (DM) halos and power-law spherical systems. The modes were identified in N-body simulations using multivariate singular spectrum analysis (MSSA) and corroborated using linear-response theory. The new mode peaks inside the half-mass radius but has a pattern speed typical of an outer-halo orbit. As it grows, the radial angle of the eccentric orbits that make up the mode correlate and lose angular momentum by a resonant couple to outer-halo orbits. This leads to an unsteady pattern with a density enhancement that swings from one side of the halo to another along a diameter, like the orbits that comprise the instability. In this way, the dipole mode is similar to the ROI. Since the DF found in Nature is unlikely to be smooth and isotropic with $df(E)/dE

One of my favorite things about this podcast is that it has allowed me to meet fantastic people. Today's guest I met at The Hang a few months ago, and we instantly hit it off. His name is John Sandford and even though he lives 15 miles east of Oxford, and I live outside of Atlanta, our passion for water treatment has made us lifelong friends.   He has a wealth of experience and a genuine passion for improving the standards of the industry he works in. John is going to give you some insight into how our colleagues across the pond test and treat Legionella bacteria and what their government requires of them.    To give you some background, John Sandford CMIOSH is the Director of Water Hygiene Specialists at SMS Environmental. He has four offices across the U.K., and over 140 employees, over 30 years of practical and research experience in the field of water treatment and Legionella and has expertise in the application of Health and Safety in this area.    John is on the council of the Water Management Society (WMSoc), one of the founders and Health and Safety Director at SMS Environmental, and he is a UKAS accredited Legionella Risk Assessment provider which is and a national supplier of water hygiene services to over 1000 organizations across the U.K.    As many of you know, the U.S. is one of the slowest countries to create legislation around Legionella testing and mitigation. It's only a matter of time for those changes to be written; just ask any water treater living in New York. Recently I moderated a panel with Cleveland Area Healthcare leaders and Ohio Congresswoman Marcy Kaptur, representing Ohio's 9th Congressional District, to discuss constricted access to social services. The virtual event was hosted by CareAdvisors, a healthcare technology firm.  They had recently done a survey of Greater Cleveland social workers to determine what needs had changed because of the pandemic, and more importantly, what are their clients saying are their biggest concerns.  The responses were eye opening.   Panelists included; Terrance “Terry” Byrne, MBA, MJ, FACHE, vice president of Health Center Operations, at Neighborhood Family Practice Community Health Centers; Dr. Patricia Lyons, PhD, LISW-S, owner of Lyons Counseling and Consultation Services, president of the Columbus Association of Black Social Workers, and director of the Molina Healthcare Social Determinants of Health Innovation Center; Dr. Michelle Medina, MD, FAAP, associate chief of population health at Cleveland Clinic; Margaret Mitchell, president and cEO, YWCA Greater Cleveland; Dr. Adam T. Perzynski, PhD, associate professor of medicine and sociology, MetroHealth and Case Western Reserve University; and Lisa S. Sign up for the full show at hippoed.com/PEDSRAPPOD. Pediatric ID specialist Michael Neely, MD, and Michael Cosimini, MD discuss how cephalosporins work and which bugs they do and do not kill. Pearls: In general, cephalosporins do not cover anaerobes, enterococcus, listeria and MRSA. Oral cephalosporins are generally not first line in pediatrics.  Some exceptions include the treatment of UTIs and some skin and soft tissue infections; group A strep and sinopulmonary infections in penicillin allergic patients are other common indications. 1st generation cephalosporins do have gram positive coverage, but do not work well against strep pneumo, MRSA, enterococcus. They do cover some enteric gram negative bacteria, the “PECK” organisms.   What are cephalosporin antibiotics and how do they compare to penicillins?  Chemically, both penicillins and cephalosporins are beta-lactam antibiotics, with the beta-lactam portion responsible for bacterial killing.  The chemical portions off the beta-lactam ring make the antibiotics different. Beta-lactam antibiotics work by binding to the penicillin-binding protein on the bacterial cell wall.  These proteins have structural functions that maintain the integrity of the bacterial cell wall and therefore, when these antibiotics bind, the penicillin-binding protein is disrupted, the cell wall falls apart and the bacterial dies.       How do you keep a straight spectrum of activity for antibiotics?  To help with this, think of bacteria into big categories: gram-positive, gram-negative and “other”. Gram-positive bacteria include: Staph aureus (MSSA, MRSA), Streptococcus (Group A Strep, Group B Strep, Strep pyogenes, Strep viridans), Enterococcus, Pneumococcus, Listeria Gram-negative bacteria are a much bigger group and can be divided into: Respiratory gram-negatives include Moraxella, Haemophilus, Meningococcus Enteric gram-negatives include the “PECK” bacteria: Proteus, E.coli, Klebsiella What bacteria do cephalosporins not cover?  In general, cephalosporins do not cover anaerobic bacteria, enterococcus, listeria and MRSA.  There are a few exceptions to this rule. Cefoxitin (a second generation cephalosporin), for example, does have anaerobic coverage.  It is commonly used in the treatment of PID as it covers enteric anaerobes and Neisseria gonorrhea. There is a 5th generation cephalosporin that does cover MRSA (discussed later). Are cephalosporins well absorbed?  Generally speaking, cephalosporins in oral formulations are not as well absorbed as penicillins and are more difficult to get where they need to go outside the urinary tract. Also, generally speaking, no beta-lactam really gets into the spinal fluid in very high concentrations; all of them do have better penetration when there is inflammation.  Practically, remember that the penetration into the CSF between ampicillin and ceftriaxone is negligible. What bacteria do first generation cephalosporins cover?  Although the classic teaching is that cephalosporins are good for gram-positive coverage (staph and strep), this is not a hard and fast rule.  As stated, enterococcus is not covered by any cephalosporin and MRSA is not covered by most cephalosporins.  First generation cephalosporins are also good for coverage of the “PECK” enteric gram negative bacteria, but not good for coverage of other gram negative bacteria.  These organisms tend to cause UTIs and therefore, first generation cephalosporins (for example, cephalexin) are frequently used for UTI treatment. Of course, resistance can occur.    Even though strep pneumo is a gram-positive organism, when it comes to first generation cephalosporins, it acts like a gram-negative organism and therefore, first generation cephalosporins do not work well against strep pneumo.   What about bacteria that develop resistance?  A patient with an E. Coli UTI, for example, may have a microbiology laboratory report stating that the E. Coli is resistant to a first generation cephalosporin but the patient is still getting better.  This may have to do with the type of infection the patient had; for example, a healthy patient with a simple cystitis may have been able to stay well hydrated and the normal immune system was able to clear the E. Coli. This question can also be answered in the context of breakpoints, that is when the bacteria become susceptible or resistant to the antibiotic depending on the site of infection.  Some labs will actually label a bacteria resistant or susceptible depending on whether the infection is in the urine, spinal fluid or blood. Therefore, if a lab that reports site-specific breakpoints suggests that an E. Coli is resistant to a first generation cephalosporin in the urine, the lab has already taken into account the higher concentration of drug in the urine.  Similarly, a pneumococcal isolate that may be resistant to ceftriaxone in the CNS may be susceptible to ceftriaxone as a pneumonia because there are much higher concentrations of ceftriaxone in the lungs than in the spinal fluid. When should a skin or soft tissue infection be treated with a first-generation cephalosporin?  When should MRSA be suspected? This can be a tricky question as an outpatient, but there are some clues to gauge whether or not the infection may be caused by MRSA.   One, it is helpful to know the community prevalence of MRSA.  In some communities, community acquired staph aureus infections are up to 80-90% MRSA.  In these communities, MRSA coverage should of course be given. If the prevalence is much lower, using a first generation cephalosporin, such as cephalexin may be reasonable.   Other clues can be more specific to the patient the their families.  Is the patient or family known to be colonized with MRSA? Have they had an MRSA infection in the past? If MRSA coverage is needed, either trimethoprim-sulfa or clindamycin generally is a good approach.  A randomized controlled trial published in the NEJM showed no difference in outcomes when treating a known MRSA infection. Miller LG et al.  Clindamycin versus trimethoprim-sulfamethoxazole for uncomplicated skin infections.  N Engl J Med. 2015 Mar 19;372(12): 1093-103. https://www.ncbi.nlm.nih.gov/pubmed/25785967 In general, the quality of the infection does not help point to MRSA or not.  There is some suggestion that if there is a soft-tissue abscess this is more likely staph aureus and if there is just pure erythema, tenderness and warmth without abscess, this is more likely to be Group A strep.   Remember, no matter what antibiotic is started for cellulitis (or any infection, for that matter), follow up is essential.  If you are on the right antibiotics, cellulitis should be improving within 24 hours.

Para ello invitamos a las historias del arte Caroll Yasky y Pamela Navarro, encargadas de las colecciones del Museo de la Solidaridad Salvador Allende y del Museo de Arte Contemporáneo de la Universidad de Chile respectivamente. Como responsables del acervo público que ambos museos ponen a disposición de la comunidad y de las futuras generaciones, las especialistas abordan las implicancias de la obsolescencia tecnológica, la necesidad de pensar en nuevos paradigmas de conservación, así como los límites de la preservación y el vértigo de pensar el futuro de las colecciones de arte contemporáneo. Formadas en la Facultad de Artes de la Universidad de Chile, en el Departamento de Teoría e Historia del Arte, Caroll Yasky realizó un magíster en administración de museos en la City Universidad de Londres (UK) y Pamela Navarro se especializó en gestión cultural en su alma mater. Durante varios años trabajaron juntas en la Unidad de Conservación y Documentación del MAC, donde Caroll fue coordinadora antes de emigrar al MSSA como responsable de la colección de arte contemporáneo internacional más diversa a nivel nacional. Consider past infections, bug susceptibilities, healthcare acquired vs. community acquired infection, foreign travel, and comorbidities.  Always check local antibiogram and prior culture results. Septic shock - Start broad spectrum antibiotics within 1 hour. Stable patient - find the source! UA, CXR, and a good skin exam are fast and can help guide antibiotic choice. Remember some patients need surgical management!  Antibiotic choice in septic shock. 1st agent - Piperacillin/Tazobactam (covers GP/GN + Pseudo)  2nd agent - Choose based on patient characteristics Ceftriaxone - simple community acquired infections. Vancomycin - covers MRSA. Meropenem - use for patients with a hx of ESBL. Flagyl + cefepime/meropenem - Use for intra-abdominal infections. More patients are being admitted to the hospital with skin problems, and specialized dermatologists are needed to provide effective treatment. Dr. Vincent DeLeo talks with Dr. Michi M. Shinohara about the evolving role of the dermatology hospitalist in the inpatient setting. Dr. Shinohara highlights some key takeaways about job satisfaction and barriers to care from a recent survey of members of the Society for Dermatology Hospitalists. We also bring you the latest dermatology news and research: 1. Cephalosporins remain empiric therapy for skin infections in pediatric atopic dermatitis “When a patient with AD walks into your office and looks like they have an infection of their eczema, your go-to antibiotic is going to be one that targets MSSA [methicillin‐sensitive Staphylococcus aureus].” 2. Should you market your aesthetic services to the ‘Me Me Me Generation’? By 2020, spending by millennials will account for $1.4 trillion in U.S. retail sales. Things you will learn in this episode: Inpatient care is getting increasingly complex, but dermatology has become more outpatient-centric overall: “There has really been a shift over time from dermatologists acting as the primary admitting service to more of a consulting service,” Dr. Shinohara explains. As a result, inpatient dermatology has become more specialized, leading to the development of the dermatology hospitalist. The Society for Dermatology Hospitalists was created in 2009 by a group of medical dermatologists to develop the highest standards of clinical care in hospitalized patients with skin disease. Most requests for inpatient dermatology consultations come from medical services for conditions commonly seen in an outpatient clinic. However, the hematology/oncology service is a common source of dermatology consultations, requiring a separate knowledge base. Dermatology hospitalists typically dedicate 25%-50% of their time on inpatient consultations. Time that dermatology hospitalists spend in the hospital is fundamentally different than time spent in clinic: “You have a lot more time to think about your patients and to teach about them to your trainees,” Dr. Shinohara notes. “It’s really one of the few places that I find you still have the opportunity to work as a team together.” Personal fulfillment is high among dermatology hospitalists, which can help combat burnout. A key challenge that dermatology hospitalists face is that most don’t generate the same revenue doing consultations as they do in clinic. Financial support from medical institutions and recognition of the value of the work is crucial to the longevity of dermatology hospitalists, who tend to be a younger workforce. Acacia was on her honeymoon when she became symptomatic. She shares the stories of her struggles in dealing with the disease. She also shares how she encourages herself using skills that she teaches her clients in session, going to therapy, drawing strength from her faith and the support of her family and friends.

Danny Chung is an positive example of a Veteran that found a way to give back to the transitioning active-duty community.  Danny’s team is trying to do is fill a gap in the civilian workforce. At any given time, there are over 500,000 open jobs in the computer industry. The current education system is producing only 49,000 computer science majors a year. Danny’s goal is to fill the gap with a portion of the 250,000 service-members that leave active duty every year. One solution is the Microsoft Software and Systems Academy (MSSA). It is an 18-week course that active duty can take on base towards the end of their service. They offer training in high demand fields like cloud development, cloud administration, cybersecurity, databases and business intelligence administration. In addition, graduates are guaranteed an interview with Microsoft or an industry partner. There are currently 15 military bases that offer the MSSA. Currently, Danny’s biggest drive is to help military spouses find gainful employment in the technology industry. The Military Spouse Technology Academy is an effort by Danny’s team to combat the military spouse 18% unemployment and 53% underemployment rates. In addition, we talk about his service, difficulties transitioning during the Great Recession, and his feelings on artificial intelligence taking over the planet. Lymph node chains draining nasopharynx, sinuses, middle ear, etc. run through it. Epidemiology & Microbiology: most common kids 2-4, (neonates too). Polymicrobial (GAS, MSSA, MRSA, respiratory anaerobes). Signs and symptoms can include pharyngitis, dysphagia, odynophagia, drooling, torticollis, muffled voice, respiratory distress, stridor, neck swelling, and trismus. Exam may show drooling, posterior pharyngeal swelling, anterior cervical LAD, or a neck mass. Imaging: Get CT neck w/IV contrast! DDx: epiglottis, croup, bacterial tracheitis, peritonsillar abscess, trauma, foreign body, angioedema, cystic hygroma, meningitis, osteomyelitis, tetanus toxin. Tx: Unasyn, if not responding add Vancomycin or Linezolid; surgical drainage if airway is compromised. Tara C. Smith discusses her work uncovering ties between agriculture and methicillin-resistant Staphylococcus aureus (MRSA). Her studies have found MRSA on and around pig farms, on animal handlers, and even in packaged meat in the grocery store. She also talks about using zombies as an allegory for infectious disease outbreak preparedness. Links for this episode Tara C. Julie’s biggest takeaways: MRSA transitioned from primarily hospital-acquired to community-acquired infections in the 1990s. In the early 2000s, MRSA strains associated with livestock farming emerged in Europe. Smith’s group was the first to identify agriculture-associated MRSA strains in the United States. Tara found MRSA on the very first farm in which she and her colleagues looked for MRSA. The MRSA strain ST398 appears to have originated in people as MSSA then moved to livestock, where the strain acquired some antibiotic resistance related genes. This is because zoonotic diseases are a two-way street and microbes can pass from people to animals, as well as passed from animals to people. Many factors may contribute to MRSA contamination of consumer meat products: for one, MRSA in farms is aerosolized and the same may be true in meat processing facilities. People can also be colonized and spread from workers to products. It’s likely a mixture of strains from farms and strains from people working in the packing plants. Farms that raise animals without antibiotics were not positive for MRSA. Processing these animals at plants where conventional animals are raised creates potential for cross-contamination, however. Prophylactic and treatment applications of antibiotics are still allowed for livestock, but antibiotics used for growth promotion purposes were phased out in January 2017. June's podcast features outstanding students in the graduating class of 2016. We spoke to Alexis Attinoto, who is an undergraduate nursing major. She is also earning a global health certificate and did her capstone in Galway, Ireland. She received the Intersections Award (first place) in the SOURCE Symposium for her project. We also spoke with Rachel Broderick, who is graduating with her Masters of Nursing. She is the nursing Co-Director of the Student Run Free Clinic 2015-2016. We spoke with Felipe Gómez del Campo, who is an undergraduate Aerospace and Mechanical Engineering student and the CEO of FGC Plasma Solutions, LLC. We spoke with Anton Spencer who is an undergraduate music and biomedical engineering major. In addition, we spoke with Sarah Eadon who is an MSSA candidate who did her field placement at the Cleveland VA and will be going into the military after graduation. She is also co-director of the Student-Run Free Clinic 2014-2016 and is a Mandel Scholar. She has done presentations at national conferences for the Society for Teachers of Family Medicine and the Society for Student Run Free Clinics.

Natasha Trenev, who is known as The Mother of Probiotics, joins Mark Alyn to talk about, you guessed it–Probiotics! Probiotics means for life, where Antibiotics means against life. Over 100 trillion Probiotic microbes are in our gut and they control your health. Live Yogurt has probiotics, these do not include the sugary yogurts you find in the local grocery store. The problem with these sugary yogurts is that they feed the bad bacteria in your gut. Why should you take a Probiotic? Natasha explains how the environment has changed over the last 40 year. These changes have introduced multiple new microbes that have become resistant in our communities. These include MRSA or Methicillin-resistant Staphylococcus aureus (MRSA) a bacterium responsible for several difficult to treat infections in humans. It is also called oxacillin-resistant Staphylococcus aureus (ORSA). MRSA is any strain of Staphylococcus aureus that has developed, through the process of natural selection, resistance to beta-lactam antibiotics, which includes the penicillins (methicillin, dicloxacillin, nafcillin, oxacillin, etc.) and the cephalosporins. These strains are unable to resist these antibiotics and are classified as methicillin-sensitive; Staphylococcus aureus, or MSSA. The evolution of such resistant bacteria does not cause the organism to be more intrinsically virulent than strains of S. aureus that have no antibiotic resistance, but resistance does make MRSA infection more difficult to treat with standard types of antibiotics and thus more dangerous. Natasha talks about the benefits of Kombucha. Kombucha is a tea-based, fermented drink beverage that is often taken for its health benefits or medicinal purposes. Kombucha is a living organism drink made by fermenting tea and with the Kombucha culture. The result can taste like something between sparkling apple cider and champagne, depending on what flavor tea you use. The first recorded use of kombucha comes from China in 221 BC during the Tsin Dynasty. It was known as “The Tea of Immortality”. One of the benefits of Kombucha is that it helps the body absorb vitamins and minerals.
