This episode continues a theme of bringing on guest's who crossed paths with Alex in the Army and helped shape his perspective on human performance. SGM Jaime Espinoza's career started with troubles involving weight gain, mental health, alcohol abuse, and more. But thanks to help from several people along the way (some human performance professionals, but also just fellow soldiers) he managed to overcome those hurdles and excel professionally. He joined us to talk about how, and how lessons he learned in the process can help improve outcomes for people experiencing the same challenges. Sergeant Major Jaime Espinoza is a native of Azusa, CA. Upon enlisting in the Army in August 2006, he attended basic training at Fort Leonard Wood, Missouri, followed by advanced individual training at Fort Huachuca, Arizona. After completing advanced individual training, he was awarded the MOS 35F Intelligence Analyst. Home hemodialysis (HHD) is hemodialysis that is performed at home entirely by a patient or with assistance from a care partner. Solo HHD is when a person performs their own hemodialysis at home without a care partner present. Solo HHD can be challenging for patients and requires the whole care team coming together with the patient to develop an individualized approach for each patient in order to increase their likelihood of success. This episode will discuss common safety concerns and other barriers to solo HHD encountered by patients and offer some potential strategies for overcoming these.
Interviewing Marcelo Pena The expression “making up for lost time” is used in many different situations. Buckets lists. To-do lists. Rekindled friendships and long-lost loves. But what if you spent decades of your life – from childhood in adulthood – feeling sick, and unable to live a fulfilling life? And what if, after living that way all those years, you suddenly found something that could help you feel better, be more independent, and enjoy the things you'd been missing? This is a quick glimpse into the life of Marcelo*, a 35-year-old home hemodialysis patient from North Carolina doing everything in his power to make up for the time he lost to end stage renal disease. Childhood Interrupted Marcelo started his life like most kids. He was happy and healthy, enjoyed playing with his cousins, and attended grade school in his home city of Chicago. At 10-years old he got sick and went to his doctor to figure out what was going on. After some testing, they found blood and protein in his urine and immediately ordered a biopsy. Then, a short time later, on his eleventh birthday, he was diagnosed with Focal Segmental glomerulosclerosis (FSGS). FSGS is a disease that causes scar tissue build-up on the kidneys and can permanently impair their function. When Marcelo was 13 it was determined that his kidney function had declined and was now at a point where a decision had to be made, receive a transplant or start dialysis. That's when Marcelo's mother offered her own kidney. “My mother was my hero,” says Marcelo. “She gave me life twice. She said, ‘I'll give him one of my kidneys and hopefully this will just all be a memory.'” Sadly the kidney Marcelo received only lasted around 6 months, once again due to complications from FSGS and he had to start dialysis. First it was peritoneal dialysis (PD), which was effective but left Marcelo with what felt like almost no time for himself. “Back then the machines were different, and I had to dialyze up to 12 hours straight,” Marcelo explains. “That was my life for three years. I felt like no one was going through what I was going through. I felt alone for a very long time.” In his late teens, Marcelo learned he could not continue PD and doctors prescribed in-center hemodialysis (ICHD) as the only alternative. That began a 16-year journey as an in-center patient. During that time, Marcelo experienced other health problems that almost turned deadly. And even though dialysis therapy kept him alive, he saw it as taking away from his quality of life. “It was almost like I was sentenced to dialysis,” Marcelo says. “For 16 years it was a Monday therapy session, and then I'd feel tired all day and into Tuesday. When I felt somewhat OK, I'd go out for a walk or something, but the next day I had to start all over again. It felt like dialysis, dialysis, dialysis, nonstop. I was so depressed and on all kinds of medications and antidepressants, just trying to cope.” It wasn't until his thirties that Marcelo learned of another alternative to ICHD. A nurse at his clinic began telling him about more frequent home hemodialysis. Marcelo was in a dark place, and he needed a change, so he decided to try it. After his training was complete, he was able to do his therapy at home for the first time in decades. In addition to the physical changes Marcelo has seen and felt, he's found several ways to make up for all those lost years. Since starting on HHD he has taken his NxStage machine to Disney World, Nashville, North Carolina, and Pennsylvania. He hosts a kidney health-based radio show on iHeart Radio and has an established presence on social media. He's learning to play ukulele, writing a book, and is certified in yoga and meditation instruction. He also has a new best friend – a Boston Terrier named Benjamin! SEEYA NeXt time! Hekate as a Triple Goddess is one of her most iconic images. Usually including either three heads or three bodies, there are many ancient and contemporary depictions of her like this. Circe, Medea, and Hekate comprise a triplicity of Goddesses of Witchcraft, and are the spirits of my book, Entering Hekate's Garden. She partners with companies across industries to deliver enterprise-wide value in deal transactions, including being a trusted advisor and project leader for executives. Overview: Katy Poole '11 HHD is a Senior Athletic Trainer at the University of Kentucky, where she works with the national championship women's volleyball team and serves as a leader in their athletic department overseeing training for the gymnastics and golf teams. Katy graduated from State College Area High School, and shares her experiences coming to Penn State with a focus on becoming an athletic trainer by earning her Bachelor of Science in Kinesiology with Honors. She provides an in-depth look at this healthcare field from undergraduate through professional. High Holy Days As days turn into weeks, weeks to months, and months to years; life evolves and times change. A new normal takes shape, but the desire to create music has never wavered for High Holy Days. After enduring 22 years of musical existence filled with trials, tribulations and victories; come hell or high water, HHD are in it for the long haul. Their blueprint reads like a blue-collar guide of straight-ahead lyrics, heartening vocals, strong melodies, and crunchy guitars. High Holy Days create guitar-driven meat and potatoes rock, heavily influenced by the likes of The Band, The Cult, Supertramp, and Soundgarden. Formed in the fall of 2000 by founding members vocalist Marc Arcand and guitarist Billy MacGregor, High Holy Days steadily rose from playing local bars around their home base of North Bay, Ontario, to dominating clubs and festivals across the country. 2002 saw High Holy Days sign a recording contract with RoadRunner Records/Universal Music and in 2003 they released their debut album ‘All My Real Friends' to both Canadian and US markets. Their Top 10 Canadian radio singles ‘All My Real Friends' and ‘The Getaway' rotated on mainstream rock radio and the nation's music video station, Much Music. The latter even found its way onto British and American airwaves. To support the album, HHD hit the road, touring across Canada with fellow rock acts Three Days Grace, Theory of a Deadman, The Trews, The Headstones and Default, while sharing the stage with the likes of Nickelback, Our Lady Peace and Sam Roberts. After two years of touring Canada and the UK, and a couple of line-up shuffles, the band hit the studio to begin recording a sophomore release. HHD travelled to Wigan, UK to begin tracking before returning to Canada to finish tracking in Wasaga Beach with renowned Juno Award nominated engineer/mixer Richard Chycki (Rush, Aerosmith, Dream Theater). Further tracking and mixing ensued at Blue Sound in Toronto with engineer/producer Russ Mackay (Styx, Gowan, Kim Mitchell). Unfortunately due to some major life changes and line-up shifts, the music was never officially released. The band decided to go on an unofficial hiatus, playing only occasional live shows for dedicated fans in the northern Ontario region. A studio was built in Arcand's house, allowing the band to write and record new music on their own terms, without the distraction of outside opinions, influences, or pressures. Fast forward to 2022, and not ones to rest on their laurels, High Holy Days are on their high horse and back in the saddle again. For HHD the summer of 2022 is all about playing live shows once again, and digitally releasing their EP ‘It's Not OK' from the previously unreleased music from their second album recording sessions on all music download and streaming platforms. Nate also is the CEO of NJF Consulting, LLC, which provides acquisition and business consulting services. From 2010 - 2017, Nate was President, CEO and co-owner of Home Health Depot, Inc. (HHD) and its subsidiary, Genesis Healthcare Services, LLC, providers of complex rehabilitation mobility equipment, hospice, respiratory and home medical equipment. During his tenure, HHD grew from one location in Indianapolis, to 20 locations in 6 states, and from 30 employees to 240 employees, while quadrupling revenue. Nate was also a partner with the law firms Faegre Baker & Daniels and Ice Miller, in Indianapolis, and Altheimer & Gray in Chicago, where his practice focused on representing public and private companies with their merger and acquisition activities, as well as advising local governments on public-private partnerships and economic development strategies. Before his U.S. legal experience, Nate spent nearly four years in Moscow, Russia (1994-1998) with the international law firms Baker & McKenzie and Steptoe & Johnson, where he represented companies seeking to do business in Russia. Appointed by Governor Mitch Daniels, Nate served as Secretary of Commerce for the State of Indiana and CEO of the Indiana Economic Development Corporation from 2006-2008. Ray Anthony Hurd, 71, passed away Oct. 28, 2021. He was born on July 15, 1950 in Mobile, to the union of Lee Dennis (Pat) and Christine Peoples (Nee) Hurd. He spent his early years in New York City (Harlem). From NYC Ray enlisted in the U.S. Army and served his country from 1971-1974. Upon completion of Basic Training, he was assigned to HHD, 96th Military Police Battalion as a 94B, Food Service Specialist. His work ethics and dedication earned him promotion to Specialist. Vaka-1: 40 yaş erkek hasta, sol kolda ağrı şikayeti ile acil servise başvuruyor. Bilinen hastalık öyküsü olmadığını söylüyor. Ağrısının keskin, hareketle artan tarzda olduğunu ve ağrı kesici kullanımı sonrası geçtiğini tarifliyor. Eşlik eden bulantı, soğuk terleme, eforla kötüleşme tariflemeyen hastanın fizik muayenesinde sol göğüs kasında ve sol omuzunda palpasyonla hassasiyet tespit ediyorsunuz. Çekilen EKG normal sinüs ritminde, ve iki troponin sonucu negatif çıkıyor. Diğer laboratuvar sonuçlarında dikkat çeken sadece plazma glukoz değerinin (190mg/dL) yüksekliği oluyor. Vaka-2: Bilinen hipertansiyon, hiperlipidemi öyküsü olan aile hekiminde rutin muayenesinde vermiş olduğu kan tahlili sonuçlarında plazma glukoz düzeyi yüksek tespit edilen hasta acil servise yönlendiriliyor. Sorgunuzda hastanın son zamanlarda idrara sık çıktığını ve normalden fazla su içtiğini öğreniyorsunuz. Hafif kilolu hastanızın benzer şikayetlerinin önceden de olduğunu, aile hekimi doktorunun yaşam tarzı değişik önerisine uyduktan sonra şikayetlerinin gerilediğini öğreniyorsunuz. Hastanın aktif başka şikayeti yok (görme bulanıklığı, göğüs ağrısı terleme vb.), mukozaları kuru, vitalleri stabil. Parmak ucu kan şekeri 270mg/dL, idrarda keton yok, kan gazında asidemisi yok, osmolaritesini de normal tespit ediyorusunuz. Eminim hepimizin bu iki tip hasta ile karşılaşma öykümüz vardır. Acil servis doktorları diyabet açısından yüksek riskli hastaları tespit edip, uygun tedaviye başlayabilecek bilgi ve donanıma sahiptirler.Acil serviste hastanın hiperglisemik acil durumlar açısından değerlendirilmesi tamamlandıktan sonra, Tip 1 ve Tip 2 diyabet arasındaki ayırıma, enfeksiyon ve dehidratasyon varlığına, hiperglisemiye neden olabilecek malignite, gebelik vb diğer faktörlerin araştırılmasına odaklanılmalıdır. Acil Serviste Hiperglisemik Hasta Yönetimi Genellikle hiperglisemik hastalar, poliüri, polidipsi, bulanık görme, geç yara iyileşmesi gibi şikayetlerle başvururlar.1 Bu hasta grubunda detaylı fizik muayene yapıldıktan sonra 3 önemli nokta değerlendirilmelidir. Hiperglisemik acil durum varlığı,Hiperglisemiye neden olabilecek sekonder sebepler,Ailede diyabet varlığı, yine ailede ve/veya hastada otoimmün hastalık öyküsü. Öykü ve fizik muayenede dikkat etmemiz gereken noktalar: Yakın zamanda belirgin kilo kaybı ve diğer şiddetli semptomların varlığı,Yeni gelişen bilinç değişiklikleri,Ekstremite distallerinde duyu kayıpları,Geçmeyen yaralar (özellikle ayaklar), Cilt tugorunda azalma bulguları, kuru mukoza varlığı,Yukarıda da bahsettiğimiz sık idrara çıkma, bol su içme durumu. Bu tür şikayet ve fizik muayene bulgularıyla acil servise başvuran hastalarda: Rastgele bakılan glukoz > 200 mg/dL ve klasik hiperglisemi veya hiperglisemik kriz semptomları varlığı ya da aşağıdaki kriterlerden ikisinin olması:Açlık plazma glukozu > 126 mg/dL,Oral glukoz tolerans testi sırasında 2 saatlik plazma glukozu > 200 mg/dL, HbA1c> %6.5 ise bize yeni tanı DM teşhisinde, Amerikan Diyabet Derneği (ADA) kriterleri2 yardımcı olacaktır. Stephen L. Williams joined the City of Houston as the director of the Houston Health Department in June of 2004. He serves on several boards of community, professional and civil rights organizations and is a past president of the Texas Association of Local Health Officials. Under his leadership, the HHD launched community outreach programs such as Project Saving Smiles, See to Succeed, Assessment, Intervention, and Mobilization (AIM) efforts and in 2006, Williams and his incredible team initiated the Hip Hop for HIV Awareness intervention project and collaborated with Enroll Gulf Coast to aid in providing access to health insurance through the Affordable Care Act of 2010 to the region’s estimated 1.4 million uninsured while also gaining national attention and White House support. His most recent endeavor includes pioneering the recovery efforts for the COVID-19 pandemic.

Your knuckles getting crushed in an overenthusiastic handshake by hands the size of watermelons isn't a fun experience. Do these knuckle-crushers know they're squeezing that hard, or do they just regularly snap pencils while taking notes, and wonder why pens and pencils are so fragile nowadays? How much grip strength do you actually need, even if you're not planning on crushing any knuckles the next time you meet someone? How much grip strength do your patients need when recovering from a hand, wrist or upper limb injury? Testing and building grip strength is a really important part of helping your hand, wrist, elbow pain and injury patients get back to work and day to day life. Gripping also pre-activates the rotator cuff, so you can use gripping as part of your patients shoulder rehab exercises. Grip strength tests using handheld dynamometers (HHD)* test your "Power Grip", but this test doesn't assess thumb or pinch grip strength. There are two other grip strength tests that are pretty easy to perform, that are going to be better suited to some of your patients. What are they, and how can you test the different types of grip strength in your patients? Why your patient can have a painfree grip and still be painful with weightbearing on the hand What exercises, weights and reps should you use following upper limb injury? How can you accurately measure wrist range of movement? How are the proximal radio-ulnar joint (PRUJ) and radio-humeral joint (RHJ) involved in hand and wrist injuries, and how can you assess these? Check out this video that highlights the hard working #militarypoliceofficers of the 101st Airborne Division (Air Assault), 101st Sustainment Brigade "Lifeliners", 716th Military Police Battalion Battalion as they conducted the second annual "Peace Keeper Challenge"! The competition was fierce as each Soldier pushed through sweltering heat and pain. When the final data was tallied, HHD came out on top of both the individual and team events. Congratulations to MAJ Barrett (Individual winner) and 1LT Griest, 2LT Burrell, and SPC Baxley (team winner) for coming out on top.
