What is your favorite game that has had one of these treatments? Third Strongest Mole — 6/13/26, 8:47 AM I'll try to give my condensed thoughts here since I wont' make it onto next week's show. The only remakes that I consider justified are games that truly had potential they failed to live up to. Games that are perfectly good as they are don't need remakes - just ports. While I appreciate keeping classic games available for people who don't have original hardware, ports address that problem in a much more cost effective way - and the "need" for remasters is selling a solution to a problem created by the planned obsolescence of the idea of console generations. We don't actually need new hardware (at least not to play old games)- console makers just need new hardware and software to sell. By all means keep the classics available through reprints and officially licensed emulation. But those aren't new releases and we shouldn't pretend they are. If you really "can't wait to play Ocarina of time again" - plug in the N64, or boot up the NSO app. Community Couch Savannah the Hyena Queen — 6/12/26, 6:59 PM I like remakes, for two reasons. One, they allow a new generation of players to experience an old game with the conveniences of the modern era. And two, they give fans a brand new way to experience their favorite games. Pokémon Omega Ruby and Alpha Sapphire was the first point for me, and Brilliant Diamond and Shining Pearl was the second. That being said, I think Nintendo has been doing too many remakes and updates to old games recently and not enough new ones. But maybe that's just cause I'm not super interested in Star Fox or Ocarina of Time Drex1981 [ARC], — 6/12/26, 7:04 PM Remakes are tricky things. One of the side they can fix issues that at the time couldn't be addressed. They can expand on storylines and tell the story with more detail. They can introduce new players to these games and create new fans. On the other side they can be souless cash grabs and change what makes these games great. MooseGooseNinja [SEAF], — 6/12/26, 7:18 PM I like remakes as long as they do the original game justice (looking at RE3R), but otherwise I do like the idea of remakes for the same reasons Savannah mentions. I generally try to judge remakes on a case-by-case basis though, and my personal favorite part of remakes is playing both versions (if I can get my hands on the original) and comparing and contrasting the two. I also try not to decide which is better or anything, but just objectively finding differences. It was the most fun for RE4 because I LOVE those games. My favorite game that got a remake was probably Dead Space. DoubleD — 6/12/26, 8:57 PM I've been mulling this over for a bit now and I think I like remasters the most. Making older games accessible to a new audience while improving the graphics and QoL features will get me every time (Don't ask me how many versions of the various Final Fantasy games I've bought through the years). Final Fantasy Tactics: The Ivalice Chronicles is probably my favorite remaster though. It kept one of my favorite games mostly the same but added amazing voice acting that let me experience one of my favorite video game stories in a completely different way! They are more interesting to analyze, and they provide more of a reason to play them if you have access to the original. Solo [nDAD], — 6/13/26, 8:37 AM I welcome a remaster of the game. The trend now in console camps is to remaster games to bring them into the contemporary level of graphics and/or new art design to make them more appealing to new generation of gamers. OoT is a cherished entry into the Zelda series. Off of this one entry we get the branches into the Downfall causing BoTW/ToTK, Child and Adult timelines of Zelda lore. Would be interesting if in the remaster then made methods to connect the game to others; for example the lore we see in BoTW/ToTK that many have played (even those that are not deep dive Zelda fanboys and just played off IP recognition alone). Hoping myself the remaster is not a lockstep redo of the original since that version is already out there. I find it more exciting if they were to add new story beat elements into a new artwork scheme. adam — 6/13/26, 9:25 AM i find the discourse around remakes and remasters to be kind of pointless, because if you don't like them, just don't play them. is there a conversation to be had about companies rereleasing games instead of taking risks on new ips? yeah absolutely. but i think that's different then should these remakes/remasters exist. emulation and ports are important in the preservation of media, but remakes/remasters update games to be more accessible on modern hardware. remakes/remasters recontextualize their stories into the modern broader canon. i think people are excited to play oot remake/remaster because of the first two reasons. i don't think anyone would pretend that the z targeting and camera controls in the original are good. (innovative for the time but obviously not great in comparison to today) so updated qol features, coupled with excitement to see how they will recontextualize oot to better fit with the current branch of botw/totk, and hopefully give us a glimpse of what's to come next (In response to adam) Your Wallet's Defense Attorney — 7:56 AM People get annoyed about remakes because of the opportunity cost. Time you take developing a remake/remaster is time you could have spent developing something new. So there are some stakes to the discussion. Also, as a community based around gaming, we like talking about games. So we wanna talk about whether remakes/remasters are interesting to us, and listen to how other like minded people feel about them. The discussion itself is often the point. (Adam thinks the opportunity cost argument doesn't hold water because plenty of studios are able to output both) (Flightsy says there is STILL an opportunity cost. And resources were spent) (Adam responds) adam — 12:16 PM I can't recall a situation where a new game or a game that's in development was sidelined by a remake/remaster. if anything the remakes and remasters make studios more money to put into new titles. … but like i said i think really the bigger issue in the industry is the live service model/microtransactions being pushed by suits in c-suites. it's why naughty dog hasn't dropped anything, it's why xbox is in such a bad spot financially. *and then to fill the void of canceled games companies just rerelease or remaster something … i think it's an interesting topic, and i think that the recent uptick in remakes/remasters is a barometer for the health of the current industry. and if it's good or bad is up to interpretation, or more likely somewhere in the middle Xomber — 1:35 PM Have mixed feelings on remakes. Some games are just classics and it doesn't really do much to put a fresh coat of paint on a classic. I will still play them, however, hence the mixed feelings. Like when Last of Us 2 was remade on the PS5, it seemed, pointless to me, but since I hadn't played it up until that point that'd be the option I would go with. I still like good games being remade to a certain extent, but are we going to do that every 5-10 years or every 2-3 generations? Xomber — 1:39 PM Another thing I wonder, would a remake make some bad games good? I'm curious about that. Could a story be fleshed out more, or an annoying game mechanic be improved to where it's not cumbersome? That's what actually interests me. Flightsy: I was definitely disappointed by the OOT remake announcement, and in general by the large amount of old content on the Switch 2. But in the end, that's not actually what I'm disappointed about. I'm disappointed that a year in, I still don't really feel compelled to buy the Switch 2. I'm annoyed about the remasters because it reminds me of that core issue, but it isn't actually the issue itself. Is Mixtape a “real game”? Honestly, who cares; we love this coming-of-age story set in the 90s from Beethoven and Dinosaur, through Annapurna Interactive. It's quick, it's robust, and it's memorable, and that's what we're after from narrative-focused experiences like this. Snackaroo is advanced Snap by way of Aardman, for up to 5 players, and while it's a children's game for sure, it's got enough meat in the bun for the parents of those kids too. Now in her 49th year in nursing—beginning in oncology and entering hospice as it emerged in the United States—Dr. Ferrell reflects on the remarkable growth of the field and where we stand today. At the heart of this conversation is the idea of a “professional home.” Drawing on the foundational work of pioneers like Florence Wald and Cicely Saunders, she highlights the enduring importance of interprofessional, whole-person care—and the need to stay grounded in those values as the field evolves. She is a Fellow of the American Academy of Nursing and she has over 500 publications in peer-reviewed journals and texts. She is Principal Investigator of the “End-of-Life Nursing Education Consortium (ELNEC)” project. She directs several other funded projects related to palliative care in cancer centers and QOL issues. Dr. Ferrell was Co-Chairperson of the National Consensus Project for Quality Palliative Care. Dr. Ferrell completed a Masters degree in Theology, Ethics and Culture from Claremont Graduate University in 2007. She has authored 12 books including the Oxford Textbook of Palliative Nursing (5th Edition, 2019) published by Oxford University Press. She is co-author of the text, The Nature of Suffering and the Goals of Nursing published by Oxford University Press (2nd Ed, 2023) and Making Health Care Whole: Integrating Spirituality into Patient Care (Templeton Press, 2010). In 2013 Dr. Ferrell was named one of the 30 Visionaries in the field by the American Academy of Hospice and Palliative Medicine. In 2019 she was elected a member of the National Academy of Medicine. In 2021 Dr. Ferrell received the Oncology Nursing Society Lifetime Achievement Award and she was inducted as a “Living Legend” by the American Academy of Nursing Brett Snodgrass, DNP, FNP-C, ACHPN®, FAANP Dr. Brett Snodgrass has been a registered nurse for 28 years and a Family Nurse Practitioner for 18 years, practicing in multiple settings, including family practice, urgent care, emergency departments, administration, chronic pain and palliative medicine. She is currently the Operations Director for Palliative Medicine at Baptist Health Systems in Memphis, TN. She is board certified with the American Academy of Nurse Practitioners. She is also a Fellow of the American Association of Nurse Practitioners and an Advanced Certified Hospice and Palliative Nurse. AbstractThis article explores the often-overlooked tragedy of promising happiness through overcoming disability. It draws on qualitative interviews and focus groups with 36 adults with cerebral palsy to explore how medical discourse shapes the ways in which individuals are encouraged to pursue a good life, leading to unintended consequences. Sara Ahmed's theory of happiness is used to understand the dialectics of pursuing a good life through overcoming disability, revealing how medical interventions and discourse during childhood inadvertently contribute to feelings of inferiority and social alienation. The article highlights the need to reconsider how individuals with disabilities are encouraged to pursue a good life, emphasizing the paradox of disabling effects arising from attempts to minimize and overcome disability.https://www.sciencedirect.com/science/article/pii/S027795362500098X

In episode 110 of the Medical Nursing Podcast we're sitting down and talking about something we're often afraid to mention - and that's quality of life. Not end of life, not euthanasia, but quality of life. An often misunderstood term, and one we need to normalise. Because the truth is, quality of life is the ultimate goal every single one of us have for every single one of our patients, every day. We make a bigger difference to it than you realise, and the evidence to back that up is VERY clear. So we are going to get into the evidence today. We're going to look at what the research really says about which factors most influence quality of life in our patients with chronic conditions, whether that's diabetes, CKD, osteoarthritis, chronic enteropathy, or other long-term diseases. Is it possible? Did I meet Rob Straughan 13 years ago? Rob is the GM of radio station 96.7FM... it goes by the call sign CKLU. This station has hosted the original "Eat This! Drink That!" which morphed in the current program. QOL now deals with all the many things that fall under the umbrella of Quality of Life. In this episode Rob and I reminisce on the early days of the radio station; its origins as a smaller closed-circuit system for the university campus, followed by applications for a broadcast signal. We will be seeing major changes to event and scheduling structures, a move away from event tickets to GO Passes, and some fun QOL updates like the ability to catch evolved form Shinies in the wild. Memories In Motion will be the potential dawn of a new era, with all of the announcements kicking off with the start of the new season. ITL Kalos Tour gave us a preview of the new Mega System. Link Charges have caused a stir online, however we feel strongly that they are a massive win for Trainers as a no cost option to Raid. The March Content Update has been met with mixed reviews. Niantic's new strategy seems to give us even less news than before, which was already a sore spot. With the top secret debut of Mega Dragonite, it does feel good to be pleasantly surprised.This all feels very calculated, and we will keep a close eye on it over the Season. Our final discussion takes a macro look at the upcoming changes. For better or worse, the Scopely Effect is beginning to take shape. Is the Community ready to embrace the change, or will they form a resistance? Time will tell, and we are exceptionally here for it all. This week’s Pulm PEEPs Pearls episode is a focused discussion between Furf and Monty about non-pharmacologic techniques for airway clearance in the non-Cystic Fibrosis bronchiectasis population. This is a focused, high-yield discussion of the key points about airway clearance, including practical tips and a discussion of the evidence. This episode was prepared in conjunction with George Doumat MD. Goerge is an internal medicine resident at UT Southwestern and joined us for a Pulm PEEPs – BMJ Thorax journal club episode. He is now acting as a Pulm PEEPs Editor for the Pulm PEEPs Pearls series. Key Learning Points 1) Why airway clearance matters in non-CF bronchiectasis Non-CF bronchiectasis is defined by irreversible bronchial dilation with impaired mucociliary clearance, leading to mucus retention. Retained sputum drives the classic vicious cycle: mucus → infection → neutrophilic inflammation → airway damage → worse clearance. Airway clearance techniques (ACTs) are meant to interrupt this cycle, primarily by improving mucus mobilization and symptom control. 2) What ACTs are trying to achieve clinically Main benefits are: More effective sputum clearance Reduced cough/dyspnea burden Improved activity tolerance and quality of life Effects on spirometry are usually small. Exacerbation reduction is possible, but evidence is mixed—some longer-term data suggest benefit for specific techniques. 3) The main ACT “families” and when to use them Breathing-based techniques (device-free, flexible) ACBT (Active Cycle of Breathing Technique): breath control → deep breaths with holds → huffing. Pros: portable, adaptable, good first-line option. Key requirement: teaching/coaching to get technique right. Autogenic drainage: controlled breathing at different lung volumes to move mucus from peripheral → central airways. Pros: no device, can work well once learned. Cons: more technically demanding, needs training and practice. PEP / Oscillatory PEP (stents airways + “vibrates” mucus loose) PEP: back-pressure helps prevent small airway collapse during exhalation; often paired with huff/cough. Oscillatory PEP (Flutter/Acapella/Aerobika): adds oscillation that many patients find easy and satisfying to use. Good fit for: people who benefit from airway stenting, want something portable, and prefer a device. Mechanical/manual techniques (help when patient can't self-clear well) HFCWO (“the vest”): external chest wall oscillation; helpful for high sputum volumes, dexterity limits, or difficulty coordinating breathing maneuvers. Postural drainage/percussion/vibration: caregiver/therapist-assisted options; still useful but consider: GERD/reflux risk with certain positions Hemoptysis risk with vigorous techniques 4) How to choose the “right” technique (the practical framework) There is no one-size-fits-all. Match the tool to the patient: Sputum burden (volume/viscosity) Strength, coordination, cognition, dexterity Comorbidities (GERD, hemoptysis history, severe obstruction/airway collapse) Lifestyle + portability (what they'll actually do) Cost/access and availability of respiratory therapy/physio support A key mindset from the script: this is not a lifetime contract—reassess and adjust over time with shared decision-making. 5) Evidence takeaways (what improves, what doesn't) ACTs reliably improve sputum expectoration and often symptoms/QoL. QoL/cough scores (e.g., SGRQ, LCQ) tend to improve modestly, particularly with oscillatory PEP and some vest studies. Lung function: typically minimal change; occasional short-term FEV₁ benefit is reported in some vest trials. Exacerbations: mixed overall; the script highlights a longer-term RCT of ELTGOL showing fewer exacerbations at 12 months vs placebo exercises. Safety: generally excellent; main cautions are hemoptysis and reflux (depending on technique/positioning). 6) Special population pearls Hemoptysis / fragile airways: start with gentle breathing-based ACTs (ACBT, controlled huffing); avoid overly vigorous oscillatory/manual methods if concerned. Severe obstruction or early airway collapse: PEP/oscillatory PEP can help by keeping small airways open on exhalation. Mobility/coordination barriers: consider HFCWO vest or simple oscillatory PEP devices to enable daily adherence. During exacerbations: keep it simple—1–2 reliable techniques, prioritize daily consistency, and re-check technique. 7) The “real” bottom line Start with simple, self-manageable options (often ACBT ± PEP). The “best” ACT is the one the patient will do consistently. Reassess technique and fit over time; education and demonstration are part of the therapy. Sinai in New York City, and is currently a first year pulmonary and critical care fellow at Johns Hopkins. Key Learning Points Clinical context Prednisone remains the traditional first-line treatment for pulmonary sarcoidosis when treatment is indicated, with evidence for short-term improvements in symptoms, radiographic findings, and pulmonary function—but with substantial, familiar steroid toxicities (weight gain, insomnia, HTN/DM, infection risk, etc.). Despite widespread use, glucocorticoids haven't been robustly tested head-to-head against many alternatives as initial therapy, and evidence for preventing long-term decline (especially in severe disease) is limited. Immunosuppressants (like methotrexate) are often used as steroid-sparing agents, but guideline recommendations are generally conditional/low-quality evidence, and practice varies. Why PREDMETH matters It addresses a real-world question: Can methotrexate be an initial alternative to prednisone in pulmonary sarcoidosis, rather than being reserved only for steroid-sparing later? It also probes a common clinical belief: MTX has slower onset than prednisone (often assumed, not well-proven). Trial design (what to know) Open-label, randomized, noninferiority trial across 17 hospitals in the Netherlands. Included patients with pulmonary sarcoidosis who had a clear pulmonary indication to start systemic therapy (moderate/severe symptoms plus objective risk features like reduced FVC/DLCO or documented decline, plus parenchymal abnormalities). Excluded: non–treatment-naïve patients and those whose primary indication was extrapulmonary disease. Treat-to-tolerability with escalation: both drugs started low and were slowly increased; switch/add-on allowed for inadequate efficacy or unacceptable side effects. Primary endpoint: change in FVC (with the usual caveat that FVC is “objective-ish,” but effort-dependent and not always patient-centered). Noninferiority margin: 5% FVC, justified as within biologic/measurement variation and “not clinically relevant.” Outcomes assessed at weeks 4, 16, 24; powered for ~110 patients to detect the NI margin. Patient population (who this applies to) Mostly middle-aged (~40s) with mild-to-moderate physiologic impairment on average (FVC ~77% predicted; DLCO ~70% predicted). Netherlands-based cohort with limited Black representation (~7%), which matters for generalizability. Would have been helpful to know more about comorbidities (e.g., diabetes), which can strongly influence prednisone risk. Main findings (what happened) Methotrexate was noninferior to prednisone at week 24 for FVC: Between-group difference in least-squares mean change at week 24: −1.17 percentage points (favoring prednisone) with CI −4.27 to +1.93, staying within the 5% NI margin. Timing mattered: Prednisone showed earlier benefit (notably by week 4) in FVC and across quality-of-life measures. By week 24, those early differences largely washed out—possibly because MTX “catches up,” and/or because crossover increased over time. In their reporting, MTX didn't meet noninferiority for FVC until week 24, supporting the practical message that prednisone works faster. Crossover and analysis nuance (important for interpretation) Crossover was fairly high, which complicates noninferiority interpretation: MTX arm: some switched to prednisone for adverse events and others had prednisone added for disease progression/persistent symptoms. Prednisone arm: some had MTX added. In noninferiority trials, heavy crossover can bias intention-to-treat analyses toward finding “no difference” (making noninferiority easier to claim). Per-protocol analyses avoid some of that but introduce other biases. They reported both. Safety signals (what to remember clinically) Adverse events were very common in both arms (almost everyone), mostly mild. Side-effect patterns fit expectations: Prednisone: more insomnia (and classic steroid issues). MTX: more headache/cough/rash, and notably liver enzyme elevations (about 1 in 4), with a small number discontinuing. Serious adverse events were rare; numbers were too small to confidently separate “signal vs noise,” but overall known risk profiles apply. Limitations (why you shouldn't over-read it) Open-label design, and FVC—while objective-ish—is still effort-dependent and can be influenced by expectation/behavior. Small trial, limiting subgroup conclusions (e.g., severity strata, different phenotypes). Generalizability issues (Netherlands demographics; US populations have higher rates of obesity/metabolic syndrome, which may tilt the steroid risk-benefit equation). Crossover reduces precision and interpretability of between-group differences over time. Practice implications (the “so what”) For many patients with pulmonary sarcoidosis needing systemic therapy, MTX is a reasonable initial alternative to prednisone when thinking long-term tolerability and steroid avoidance. Prednisone likely provides faster symptom/QoL relief in the first weeks—so it may be preferable when rapid improvement is important. The trial strengthens the case for a patient-centered discussion: short-term relief vs side-effect tradeoffs, and the possibility of early combination therapy in more severe cases (suggested, not proven).

The company shared in an SEC filing that its Capillary Biomedical (CapBio) subsidiary won clearance for its SteadiSet Infusion Set. SteadiSet features an integrated inserter with a hidden needle designed for one-handed insertion. It received clearance to deliver insulin for up to three days of use. However, Tandem plans to submit a separate request to extend the indicated use time to up to seven days. It doesn't expect to begin commercial activities for SteadiSet until after it receives clearance for the extended use time. Many of us have been following this since Tandem acquired CapBio back in 2022.. it's been pretty hush hush – CapBio has never accepted my requests for interviews, so I'm excited to see this approval, mostly because my son really really needs it! https://www.drugdeliverybusiness.com/tandem-capbio-fda-clearance-insulin-infusion-set/ XX Insulet just announced via an email to health care providers that Dexcom G7 compatibility with the Omnipod 5 iPhone app will be released IN THE COMING WEEKS! Great news for people with #t1d and XX Sernova announces positive interim data from its ongoing Phase 1/2 clinical trial in patients with T1D. this is their Cell Pouch Bio-hybrid Organ as a functional cure for type 1 diabetes (T1D), today Interim data from 12 patients with transplanted human donor islet cells show patients achieving insulin independence, islet cell engraftment in Cell Pouch, islet function, islet survival, improved glycemic control, improved patient reported quality of life (QOL) and improved awareness of hypoglycemia and increased sensitivity to severe hypoglycemic symptoms. Improvement in patient outcomes was correlated with a cumulative increase in the quantity of transplanted islets. Based on these findings, the study is on track to meet its primary and secondary endpoints, and the confirmatory Cohort C is expected to initiate in H2 2025. Clinical trials with iPSC islet-like clusters from Sernova's partner, Evotec, are anticipated to begin in 2026 after completion of Cohort C of the ongoing trial. These interim findings, with 8 of 12 patients achieving insulin independence so far, support the thesis that Sernova's high volume ten channel Cell Pouch, used in Cohort B, plus an optimised immune suppression regimen, has the potential to achieve insulin independence, without portal vein transplant, in our planned clinical trial with Evotec's high quality iPSC islet-like clusters.
