Welcome to Season 3's first episode of onePERspective. As a reminder, this is a tri-weekly segment in which patient partner Kathy Smith shares a synopsis and key reflections from the previous week's episode of asPERusual -- a podcast for practical patient engagement. My name is Anna Chudyk and I am asPERusual's host. Before I turn things over to Kathy to provide a recap of Season 3 Episode 1, I want to do a huge shout out to her. Kathy, you faced huge tech related barriers that would have made most other people peace out on making this episode a reality. But you kept trying all week and persevering and here we finally are. Something you kept saying was, I don't want to let you down. But the thing is, I don't think you could ever let me down if you tried. Our long-standing bond grew out of and transcends or working relationship, and that's what patient engagement is all about. So thank you so much, Kathy, for loving this labor of love of a podcast as much as I do. And now let me turn it over to you for your onePERspective.Kathy Smith:Thank you Anna and hello everyone. First off, I would like to gratefully acknowledge the ancestral keepers of the beautiful and bountiful land upon which my city of Thunder Bay was built about a quarter of a century ago. We are at the epicenter of Canada, at the head of the greatest, deepest and cleanest of the Great Lakes, Lake Superior, the mighty Gitche Gumee. This area is the traditional territory and homeland of the Anishinaabe People, which includes the Ojibwa/Chippewa of Fort William First Nation, signatories to the Robinson Superior Treaty of 1850. My father's family migrated from Sweden to Turtle Island (Canada) in the late 1800s. They all settled in Minnesota, but only my grandfather preferred to live and work in Canada as a lumberjack. My mother's family also first migrated to America way back in the early 1700s. A family joke is that they told the Mayflower where to land. They moved first up to New Brunswick, as United Empire Loyalists, with some settling here to work in the forest building and serving the railway. Both families chose to live and thrive here in pristine northwestern Ontario, and they gratefully spoke often of the huge debt we owed to its original land keepers. Whether as a child in the family car on a Sunday drive to nowhere, or as a mom camping everywhere with my own four children, or now just meandering with my hubby along the majestic woodland trails, I am in awe of the breathtaking, balance, and restoring beauty of our natural surroundings. I respect the ancestral stewards for their profound spiritual connection to Mother Earth and all the living things that guided them to practice reverence, humility and reciprocity with her gifts of pure air, clean water, mineral rich soil, and abundant flora and fauna. I am very grateful also for the significant contributions made by the Metis Peoples, both past and present. Miigwech!Thank you to our Building Capacity with SPOR IMAGINE guests Aida Fernandes (executive director), along with Dr. Deborah Marshall and Sandra Zelinsky, co-leads of the Patient Engagement Subcommittee. You all wear different hats with your lived and work experience backgrounds, but you are all laser focused on partnering to better manage inflammatory bowel disease (IBD) and irritable bowel syndrome (IBS). Your IMAGINE network is shedding some much needed light on these puzzling and troubling chronic disorders. I have seen a close friend's partner struggle and stumble with Crohn's disease, one of the inflammatory bowel diseases. Needing to be hyper vigilant with what he eats and ever mindful of maintaining a work life balance is difficult. And he pretty much had to learn by trial and error. He recently passed away from pancreatic cancer, but I know he would be extremely delighted to see that the Mind And Gut Interactions Cohort (MAGIC) Study focuses on more proactive and preventative strategies than on reactive treatment regimes. WOWZA! Almost 8000 subjects were recruited into the pan-Canadian MAGIC study by the IMAGINE network. Participants were retained in the study in high numbers even though they had to submit quarterly blood, stool, and urine samples. How did IMAGINE accomplish this incredible feat? With the aid of a very strong, very engaged, very well trained and supported patient partner group. 40 patient peer-to-peer mentors conducted face-to-face and/or online virtual contacts with the participants in the study to encourage and support them. The patients engaged as partners in the research had flexibility in how and when they could contribute to the study. The IMAGINE network met patient partners where they were at and what they were comfortable with contributing. Anna compared this to Roger Stoddard's choosing patient engagement tasks from a menu analogy, because appetites are all different when it comes to the amount of engagement one can or will want to do. Patient engagement was kept strong by relying on this flexibility, plus good support, plus capacity building and first and foremost, once again, respectful relationship building. The unique but equally valuable voices and perspectives of the 4 Ls that Linxi Mytkolli told us about in asPERusual's Diabetes Action Canada podcast delivers the lived, the lovers, the learners (so the academic leads) and the laborers (all the clinicians) blended together to make this one strong, patient centric study. Patients and providers also had access to tools necessary to support their preferred level and type of engagement. Anna has posted a really good list of these resources recommended by today's guests. In particular, they highlighted the free online patient and community engagement research (PaCER) program, a one year certificated course from the University of Calgary. It teaches patient research partners how to create, conduct, and even lead a research project. In fact, that's the requirement for the certificate. Another way patient engagement was supported reached beyond taking part in the research study process. IMAGINE empowered patients to share their lived experience stories. Having just completed this task myself, I sure wish I had access to that training with the help from patient mentors like Sandra. Storytelling is harder than it looks. All in all, this is a very strong study with a very strong network of networks and a very strong method of patient engagement. I wish the team much success. Miigwech. PERsonally SpeakingMy three big takeaways will sound pretty familiar. The secret sauce to any successful patient engagement in research platform is to overcommunicate at the start. Make friends. Take all the time at the start to develop the bidirectional relationships needed to build a strong team of friends. Teamwork makes the dream work. Know the end game goal. List the tasks needed to get there, and then collaborate as a team to “match the hat to the task.” Impressed IMAGINE seems to have no power differential barrier at play. Regular feedback and ongoing evaluation can help maintain a productive and respectful partnership. If we focus research funding on lived experience patient priorities like MAGIC's mind-gut biome connection study, it not only enriches the research process, but also ensures that the outcomes are more relevant and beneficial to those who need them the most. Patients are the alpha and the omega of health care. Thank you to SPOR Strategy for Patient-Oriented Research for all your resource allocation for studies that bridge the gap between science and experience. Working together, maybe we will get more proactive strategies to better self-manage many chronic diseases. I'm very optimistic MAGIC will make some magic happen for patients with IBD and IBS.Anna:Thanks so much Kathy for sharing your onePERspective. I think this is my favorite one yet. Something that really also stood out for me about IMAGINE was the pivotal role that patient research partners play in shaping research within the network and throughout the research cycle. The MAGIC study is such an impressive undertaking, having enrolled almost 8000 patients in a Pan-Canadian longitudinal venture. To see meaningful and active engagement interwoven into it is beyond heartening and definitely something we can all stand to learn from. Filming this videocast was a true honor because I had the pleasure of interviewing someone I respected and admire. Her organization helped me improve my health and vitality after a traumatic birth experience AND regain some sanity during the events of 2020 and beyond. Please meet Sally Fallon Morrell, founding president of the Weston A. Price Foundation (WAPF), an organization inspired by the work of a 20th century dentist and author who traveled the world for a decade studying traditional cultures and diets. She is the author of numerous books including my favorite, Nourishing Traditions: The Cookbook that Challenges Politically Correct Nutrition and the Diet Dictocrats, a farmer, activist and so much more. As a member of the foundation, I look forward to her quarterly WAPF journals and especially her “Caustic Commentary” section where she dishes up what she really thinks about current events and topics related to health; in other words, the straight talk I value.Today, the heart of her message is all about real, whole, traditionally sourced and prepared foods and supporting local farmers. This includes foods such as real raw milk (find it here near you!), organs, and meat with fat and skin intact as they all contain the animal fats that are “absolutely essential to our health.” WAPF both teaches the fundamentals of traditional eating as well as helps its members find nutrient-rich foods. (Spoiler alert — this is a more broad approach to eating which can include starchy carbs prepared properly, so in my opinion more realistic for the general population.)Many of the situations and conditions to which we've become accustomed to as the norm — chronic disease, jaws that don't fit our teeth, medication lists including injectables for weight loss, infertility, poor sleep, mental illness and issues around food, traditional baby formula, etc. are NOT AT ALL normal. I admire WAPF for boldly shining the light on these problems as well as proposing solutions, including cost effective approaches, that work, as well as combatting those who wish to silence them.Sally and I discuss that improved health happens one small change at a time — she offers two quick and easy tips to “make a huge difference in your health.” I sincerely hope you enjoy this episode, even if you find items or opinions with which you don't agree. If you want a deep dive into Weston Price's work, his book Nutrition and Physical Degeneration (first published in 1939) is a commitment that is worth it.FINE RECIPE-One of Sally's two easy tips for improving your health is to avoid store-bought dressings which contain inflammatory fats and often preservatives and not so natural “flavors.” The good news is that making your own dressing at home is both easy and inexpensive. Her cookbook has plenty of dressing recipes and below I share an edited version of the most basic:* add 1 tsp of Dijon mustard to a small bowl;* add 2 TB plus 1tsp of raw wine vinegar and mix;*add 1/2 cup EVOO (extra virgin olive oil) in a thin stream mixing well.The above makes a 3/4 cup serving. Her recipe recommends a little expeller pressed flax oil which I don't tend to have so I usually make it without it and add quality sea salt and any finely chopped fresh herbs I have. Welcome to the Sell with Authority Podcast – I'm Stephen Woessner, CEO of Predictive ROI. If you've been listening to the show for a while now – you know that about every 4-6 weeks, I record an episode where it's just you and me — so we can explore a topic with some real depth. But you may have noticed that within the last couple of months — I've recorded more solocasts than my normal cadence. I've done that for a couple of reasons. First — there have been a lot of biz dev situations, questions, etc., that we've helped agency owners in our community work through — and some are helpful examples that turn into specific teachable moments to share. Second — my Predictive team and I have been running at a breakneck pace with new experiments that we're putting through our Lab and then sharing the results during our Intensives. If you joined us for our July Intensive — you saw the results of an experiment that included 121,000 repetitions in the data set. Wowza. We're kicking off a brand new experiment in the Lab tomorrow that will include at least 50,000 repetitions, and we'll share the data set in full transparency during our next Intensive on November 6th & 7th. And Praise God — we've had the blessing of onboarding some new clients into the fold here at Predictive. All of that is to say — my schedule for interviewing guests has been challenging. But thankfully — that is changing. In fact — during our quarterly leadership team meeting — my fellow leaders…Erik, Megan, and Hannah — said, “Okay, Stephen — what needs to come off your plate so you can stay in the biz dev lane.” Staying in my lane will give me the time and space I need to interview the long list of guests that I have been wanting to invite to the podcast…so stay tuned. But for today — I'm going to start us off by looping back to the new experiment and the 50,000 repetitions that I mentioned a minute ago. For years — we've taught a nurture and lead gen cadence that we've called “The 6:1 Ratio.” It was published back in my second book and focused primarily on social media and how many personal or professional posts ought to be made — 6 — to every 1 “ask” post to create the optimal opportunity for conversion. Why did I decide to encore this episode where Cora Opsahl from 32BJ spends 29 minutes talking about the importance of getting your data if you are an employer or a union health fund? Let me quote Jeff Hogan with some light edits here. Jeff wrote about the “outsized role” that employer data and intentional analytics can and will play. This is emerging and a must-have. The show with Andreas Mang (EP419) from three weeks ago, the show with Dan Mendelson (EP385), the one with Mark Cuban and Ferrin Williams (EP418) … everything that has been talked about in all of these shows and more is gonna be hard to do without having the data so you know what's going on. But I will let Cora Opsahl explain far more succinctly than I can here. One more note before we dive in here: After you listen to this show, you might want to go back and listen to episode 373 with Cora—and that one is entitled “How to Kick a Big Hospital Out of Your Network”—because this is one of the things that 32BJ did when it got its data. 32BJ realized that if it kicked out the really expensive hospital from their network, it would (and did!) save $35 million. Kicking this one hospital out of its network enabled the union to get its biggest wage hike in however many years, and also the employers employing union members got a premium holiday and did not have to pay into the health fund for a few months. Imagine if they didn't have this data and realized the millions and millions of dollars being siphoned out of the plan by this one hospital charging way too much. It's just crazy how many employers or unions wind up becoming imprudent fiduciaries because they just don't have the data to know better. But I'll tell you who is realizing it: class action attorneys. In this healthcare podcast, I am speaking with Cora Opsahl, who directs the 32BJ Health Fund. Important to know about Cora's background is this: In previous roles, she's worked deep in the inner workings of the healthcare industry. So, she came to 32BJ armed with a BS meter that is finely tuned, which is, unfortunately, an essential skill for anyone trying to help the patients and members relying on them to successfully navigate the healthcare industry. This conversation gets into everything that the 32BJ Health Fund does with their data. They have lots of data. They demand it. So, besides kicking out overly expensive health systems from their network, here's other things that 32BJ is currently doing with their data and which other employers and unions may get a few ideas from. If you have the data, you (like 32BJ) can use it to: Make smart benefit decisions that are validated, not just guesses. Before you decide to do something (add a wellness program etc), be able to model it accurately for how much it will actually cost you—which, spoiler alert, is most of the time not what the vendor will estimate. You have way more data than the vendor does, so you can certainly use it to great effect in this way. Make sure that the right members are being communicated with so that benefit designs are successful. As Ashleigh Gunter said in episode 368, success when changing benefit designs has a lot more to do with communication than many realize. Create dashboards for leadership that may show trend lines, for example, which could be very helpful to ensure that the fund doesn't run out of money etc … little things like that. Figure out how much the fund is spending on various procedures and where. There's all this talk right now about the crazy variability of prices for the same exact service in the same local market. At one hospital, a colonoscopy could literally cost $10,000; and in another hospital—same quality, same basically everything—that same colonoscopy will be $2000 or $3000. I mean, there's a 500% delta or something in some of these cases. Ensure that if a vendor said they were going to do something, that they are actually doing it. This is especially meaningful for point solutions because of the whole squeezing the balloon thing. I can save money in a silo, and you won't realize that those dollars are getting transferred elsewhere unless you are doing your own math. This is a big deal if you start thinking about how pharmacy benefits are typically siloed from medical benefits. So, if I'm a pharmacy benefit manager, I can talk about how much I'm saving by denying patients drugs without consideration of the medical downstream implications of that. Ensure you're not paying a bill and writing a check for more than the bill was for, which is weirdly common. There's a whole show with Dawn Cornelis (EP285) about this. 32BJ has an engineering team that is creating an app to help members navigate to great doctors with fair prices. All of these things roll into basically three categories: 1.    Cutting wasteful spending and finding fraud 2.    Making smart benefit decisions 3.    Being able to see trends and forecast the future, which is really helpful for financial solvency etc As Cora Opsahl says, “I think we [all can] recognize [that] you [cannot] make smart … decisions and be a fiduciary of [a] fund without having [data].” This whole conversation has been really a big bright spot for me and will provide hope, I think, for any employer/union who is seeking ways to protect their members and patients, the ones on their plans and therefore under their aegis and whom they have a fiduciary responsibility to look out for. 32BJ represents about 200,000 members. They are mostly in residential and commercial real estate—so, for example, your doormen, your maintenance workers, your security, your cleaners, amongst others. Members are in about 11 states, but a lot of them are in the New York City metro area. These union members who are in the fund work for over 5000 different employers. The 32BJ Health Fund has zero-dollar premiums. Wowza on that point—that's a huge benefit. As director of the Health Fund, Cora has implemented multiple benefit changes that saved more than $35 million: removing NewYork-Presbyterian Hospital System and physicians from the network, transitioning to a new pharmacy vendor and pharmacy group purchasing coalition, and establishing an expanded Centers of Excellence program administered by Mount Sinai Solutions. Currently, she is leading a comprehensive medical RFP. Prior to joining the 32BJ Health Fund, Cora spent 12 years at Express Scripts, a pharmacy benefit manager, where she held a variety of roles, including with Medicare Part D, strategy and acquisitions, operations, and account management.
