Chantal Traub is a prominent New York City birth facilitator, doula, childbirth educator and wellness coach. In her 20-plus years in practice, she has become internationally acclaimed for her expertise on the pelvic floor and her Pushing Power technique. Early on, Traub identified a severe lack of preparation among birthing people for the pushing stage of labor. In today's episode we are chatting with Lynn Schulte. Lynn is a Birth Healing Specialist and Pelvic Health Physical Therapist for over 30 years and founder of the Institute for Birth Healing, an educational platform for practitioners and Center for Birth Healing, her clinical practice. She has been helping moms heal from all the issues women experience after birth and is now teaching courses to bodyworkers to help them do the same. Episode notes: When we have prolonged exposure to stress… It affects our immune response. This triggers an inflammatory cascade due to increased levels of cortisol and surges of insulin. Where else can the inflammation be coming from? 1.stress - decreases our bodies ability to fight inflammation and in addition your body translates inflammation as a physical stressor 2.Food - even if we think we're eating healthy we're not always sure how things show up in our bodies 3.Too much exercise = too much information or not giving our bodies enough time to recover 4.Too little exercise= we don't clear inflammation bec we are not moving our bodies. Our bodies produce cortisol from our adrenal glands to manage inflammation …..if our bodies are in a persistent inflammatory state, we will produce too much cortisol. This will cause fatigue or adrenals. We will end up with a cortisol imbalance. This can affect multiple systems throughout the body. In summary, our liver, which is the major detoxification center in our body, needs to be working optimally to filter toxins out of the body and make sure that we're producing hormones and managing inflammation. Athletes may be particularly susceptible to PFD due to the repetitive strain and impact on their pelvic floor muscles during training and competition Increased intraabdominal pressure with lifting There is already a stigma surrounding the topic of PFD but it is especially evident in the athlete population, therefore, issues will go overlooked and underdiagnosed Relationship between orthopedic injuries and pelvic floor dysfunction EXAMPLE 1: DEEP CORE SYSTEM EXAMPLE 2: HIP AND GROIN STRAINS To simplify, I explain to patients the difference between the stability muscles and the moving muscles, If the stability isn't effective, think of how much harder the movers need to work to create a strong contraction = INJURY Refer to the specialists! There are a few simple ways to incorporate things into a POC but to really provide a comprehensive treatment plan to patients whose primary diagnosis revolves around the pelvic floor, I would always resort to my Pelvic floor specialists When in doubt, refer out! On today's episode we are chatting with Dr. Sarah Bedell. Dr, Bedell currently practices Gynecology with her sisters and mother at New Age Women's Health in Miami FL since 2017.. She does general gynecology but also sees patients with vulvar skin conditions and conditions that can cause pain with intercourse. She went to Medical School at New York University where she met her mentor, Dr. Andrew Goldstein, who introduced her to this subspecialty. Dr. Bedell trained with him after she completed her residency at the University of Texas-Southwestern in Dallas, and worked part-time in his office in New York while primarily practicing in Miami (yes, she would commute to NYC once a month for about 1.5 years). Erica Meloe is a physical therapist in private practice in NYC. In today's episode we talk about taking control of your bladder. Bladder leaking is a “common but not normal” issue that people of all ages struggle with. We will go over basic anatomy and function of the pelvic floor and bladder, how the pelvic floor and bladder interact. We will discuss signals your body may give you to let you know that your bladder is asking for help. And we will discuss tips to optimize bladder health. Episode Notes: WHAT IS CONSIDERED NORMAL Bladder Function? The average bladder can hold about 2 cups of urine before it needs to be emptied. The average range of voiding urine is 6- 8 times during a 24 hour period. As we get older, our bladder capacity can get smaller and we may need to pass urine more frequently but usually not more than every 2 hours. Urine should flow easily without discomfort in a good, steady stream until the bladder is empty. No pushing or straining is necessary to empty the bladder. An urge is a signal that you feel as the bladder stretches to fill with urine. Urges can be felt even if the bladder is not full. Urges are not commands to go to the toilet, merely a signal and can be controlled. *** any amount of leaking is considered incontinence (peesing) *** Can Happen at any age *** You don't have to live with it Common Factors that affect the bladder and leaking Pregnancy Perimenopause/menopause Weight LBP Injury,Trauma, surgery,scar tissue How do we Get control of our bladder? See a pelvic floor PT to assess Strength,tone,timing coordination in all layers of the PFM , 80% effective at CURING incontinence ( cochrane) So much more than kegels, sometimes kegels would make it worse Learn how to perform a pelvic floor contraction properly Core component coordination- Our core needs to be coordinated and use that coordination during dynamic movement Making sure that you pelvic floor muscles are reflexive and anticipatory Using breath to maintain continence- Exhale with exertion Alignment Stay relaxed on the toilet, don't “push” out the flow of urine Avoid peeing “just in case” Avoid camel bladder Eliminate foods that can irritate your bladder Identify Urge trigger ( learn urge suppression strategies) Hydrate Avoid Constipation Practice makes Permanent…. Take Action….. Do something about it now!! The Bladder is a retrainable organ…. Don't let your bladder run your life, Become the boss of your bladder!! In today's episode we are speaking with Becky Rhew. Becky has been a practicing attorney for the last 20 years. Becky has a genetic condition called G6PD Deficiency. G6PD deficiency is an inherited condition. It is when the body doesn't have enough of an enzyme called G6PD (glucose-6-phosphate dehydrogenase). This enzyme helps red blood cells work properly. A lack of this enzyme can cause hemolytic anemia. This is when the red blood cells break down faster than they are made. This caused a variety of digestive issues for her. Becky talks about her journey in discovering her diagnosis of G6PD and her path to healing. Trista Shares: Hypopressives use postures and poses with an apnea. It is a whole body exercise and can offer a deep myofascial release from deep within. Hypopressives retrains the reflexivity and automaticity of the deep core system It is a breath practice that uses rest breaths and apneas to decrease pressure. Hypopressives activate your muscles reflexively as opposed to active recruitment of Kegels. It is a practice and a flow that can be restorative and a great maintenance exercise. Hypopressives help with neuromuscular retraining and proprioception. It can help those who do not feel connected to their pelvic floor and have a difficult time actively recruiting. Words Matter- We need to use caution when describing what is going on in a client's body. We need to be honest but not pathologizing. Hypopressives can help with constipation, digestion, fascial release, nervous system regulation, pelvic organ prolapse, postpartum recovery. Avoid trying to learn the technique from online videos, it is so important to be sure that your technique is correct to get the benefit of it but also to be sure that you are performing it in a way that decreases pressure and not increasing intra-abdominal pressure Trista's Takeaways: Be patient with yourself. Infertility Facts: Stats: 1 in 8 or 12%- 15% of couples struggle with infertility About 7 million American women IVF Success Rates: depend on many factors, such as age and the reasons for infertility. Overall, first-time IVF success rates often fall between 25-30% for most intended parents. However, this probability tends to increase after multiple IVF cycles. Types of infertility: Primary infertility is when a pregnancy has never been achieved by a person. Secondary infertility is when at least one prior pregnancy has been achieved. Mechanical Infertility - Scarring from surgery. Pelvic surgery can damage and scar the fallopian tubes, which link the ovaries to the womb. … Some other causes of infertility? Hypothalamic amenorrhea -Women can experience hypothalamic amenorrhea when poor nutrition or stress interferes with the brain's regulation of the menstrual cycle. Fibroids PCOS Endometriosis. ... Pelvic inflammatory disease. ... Medicines and drugs/ toxins Premature Ovarian Insufficiency Women with POI do not ovulate (release an egg) every month. This makes it difficult to get pregnant. Having the right mindset will help you navigate the journey and help you make the right decisions and feel good about them Everyone needs different things throughout their journey Mindset can help open your mind to take on changes that you may not have been open to before such as dietary changes. Positive Support is so important, be mindful to avoid catastrophizing chat rooms. A mindset coach, pelvic floor physical therapist , acupuncturist are all potential resources for building a fertility journey village Exercise is important during the process What role did seeing a pelvic floor physical therapist play in your fertility experience? 1. Pelvic health was one of the most important things for me to look at when I was trying to conceive, and, unfortunately, it took me a long time to realize it because it wasn't on my doctors' typical list of things to investigate. So, my expertise comes from a few areas, one of them is personal experience. I know the power of working on pelvic health because, aside from mindset, working with you was THE lynchpin in my pregnancy. 2. Pelvic health matters for fertility for two main reasons, one is physical. For example, women who have endometriosis, like I did, or weak pelvic floors or tight pelvic floors, (of which I had both) can see their fertility negatively impacted. 3. My area of expertise is less on the physical side, though, and more on the emotional side. Emotional blocks can surface in the body, and for many women who are struggling to conceive releasing those blocks in both a physical and emotional/spiritual way is critical. While I am not a therapist, I do help women come to realize what is or may be in their way and they become inspired to seek specialists who can help them get to the root cause and become the moms they are meant to be, but before they are in the right mindset, they tend to be stuck, frozen or just confused and hopeless about moving forward. Takeaways for listeners? 1. If you are struggling to conceive, what is blocking you may not be just physical. Working to reset your mindset can create a clear path for you to make decisions, navigate the myriad competing recommendations, deal with the emotional ups and downs of the fertility journey and actually live it from a place of empowerment and joy. 2. When you get in the right mindset of truly believing you are meant to be a mom, the universe responds and miracles do occur. 3. If you like science, know that women who work with other women on the emotional side of the fertility journey are 65% more likely to become pregnant. That number includes women in support groups and those who seek ongoing support from other women who get it. Episode Notes: Many times people have pain points in several different areas of the body, they get treated but often it's only temporarily relieved. This is because the area that was symptomatic was treated NOT the root cause. Healing any pelvic health issue always requires a whole body approach. We look at the big picture and listen to people's story and connect the dots into a cohesive plan for healing. The core is meant to be automatic and reflexive. Sometimes we lose this automaticity or the speed of recruitment is delayed and compensatory movement patterns are integrated. We need to retrain the core and generate support for our bodies from the center. It is always recommended that Pregnant women initiate pelvic floor physical therapy during pregnancy. This helps them maintain optimal and efficient movement strategies and alignment. It will facilitate a faster postpartum recovery and may prevent pregnancy. In today's episode we talked with Heather Koerner owner/founder of East Boca Acupuncture. Heather is an Acupuncturist/Doctor of Chinese Medicine. Heather was open and spoke about her personal healing pelvic health journey. We also dive into a great conversation about 5 elements in nature that we all embody. Chinese Medicine asserts that each of us has a guardian element that shapes how we move through the world and how we react to stressors in the world. When the proverbial “stuff” hits the fan this element takes over and gives us a strategy to deal with the stress. When we are in acute or chronic stress of pain we often get locked in some habituated physiological patterns. Once Heather began pelvic PT she was able to switch her mindset from being injured to returning to activities that bring you joy without worry because she was finally given tools and resources for healing and finally felt empowered instead of frustrated and powerless. Consistency with her healing program was the most challenging part for her. As a client with Pelvicore Rehab Heather felt confident she was going to have a Safe, supportive and empowering experience. Pelvic Floor Physical therapy gave her real hope for long term healing. What would Heather have done differently knowing what she knows now??? She says she would've gotten PT and explored alternative routes earlier on. 5 Elements in Nature : There are many patterns because we can have either an exaggeration or a collapse of the elements but generally speaking we tend to see these strategies with patients: Fire: Seeks fun and novelty as a distraction Wood: Dig Deeper & Push Harder Earth: Give more & do more for others Metal: Seek perfection & Isolate Water: Freeze & Go inward (literally feeling like they're underwater) Once we figure out our element we can “nurture our nature” and we can reduce the impacts of the discomfort and spend more time feeling strong in our element. Basically, working smarter not harder. At the end of the day we are really trying to optimize and harmonize. Optimizing is just a self-awareness of what your needs for safety are and how to meet them. This allows you to better regulate in the midst of adversity or healing. Harmonizing is finding a way to deal with the experience with more ease. We talk about how disease is actually two root words “dis” and “ease” so “without ease”. When we can feel resistance and ask ourselves open-ended questions like “what's of value here?” We can create space to integrate the experience with a little less resistance which ultimately allows for less contraction in the body.You learn which people feel nurturing and promote growth and which people you're going to have to take a deep breath and access your resource for patience. The cool thing about us working together is that we are very much in alignment with the proper empowerment and strategies that people can overcome things the never thought possible. Symptoms that the core is not working : 4 P's Peeing- any kind of urinary incontinence (leaking),dribbling, urinary urgency, urinary frequency Pooping - fecal incontinence, fecal smearing, inability to hold back gas, constipation, incomplete evacuation Pain- Including pelvic pain, back pain, bra line pain, groin pain, hip pain pain with sex (Is NEVER normal!) Pressure- perineal pressure, rectal pressure, a feeling of dragging or organ dropping “To Kegel or NOT to Kegel That is the Question”- There is no one size fits all, just like any other exercise, kegels must be individualize Must be sure that you are performing them correctly and through all three layers of the pelvic floor muscles We use a FIIT: Frequency, Intensity, Interval, Timing prescription to individualize plan Some people get WORSE when doing kegels if they are holding tension in the pelvic floor , Kegels are NOT for everyone ! But if we remain proactive using an incorporating an Ayurvedic approach, we can remain vibrant throughout our life span. The Dosha - everyone has their own unique individual mind-body constitution. They are divided into three groups. Once you have the self awareness to understand what your individual Dosha is, you can live your most vibrant life. Having this information about yourself can improve your overall health. Pelvic Health and Ayurveda complement each other beautifully, in that each person has their own unique constitution. There is the belief that all systems must be working well for the body to stay on track, stay balanced, and re-calibrate itself if there's an injury, illness or interruption. 50% of our population is diagnosed with leaky gut syndrome, and other autoimmune diseases and chronic inflammation. In today's episode I talk about why I am so passionate about pelvic health and wellness, so much so that I gave up my concierge outpatient rehab business seven years ago to launch and become the owner/founder of Pelvicore Rehab and pursue my dream of providing an amazing healing experience for people with Pelvic Health issues. Episode Notes: My journey to specializing in Pelvic Health began with helping men going through Prostate Cancer. I developed a “Prehab” Program for men to jump start their healing before their procedure. I also helped them recover after prostate cancer procedures and for those struggling with urinary leaking, constipation, sexual/erectile dysfunction and pelvic pain. I really enjoyed this work. There is a lack of awareness that pelvic floor issues affect men, they are really underserved in the field of pelvic floor physical therapy and often suffer for too long before finding their way to a pelvic floor specialist. I immersed myself into all things pelvic health and quickly developed a passion, determination and drive to help those struggling with pelvic health issues and expanded Pelvicore Rehab To help EVERYONE struggling and searching for pelvic healing. Pelvicore Rehab helps men, women and children throughout their lifespan with issues ranging from bladder leakage to constipation to painful sex. We support women prenatally, for pregnancy wellness, for postnatal care and for menopause health. Pelvicore helps men overcome all pelvic floor related issues including but not limited to urinary, prostate, and sexual dysfunction. Pelvicore Rehab is unique because we use a holistic approach in order to create a comprehensive, cohesive, and individualized plan of care to meet each client's unique needs and goals. We look at the big picture of healing from a total body perspective. We listen to people's stories and gather the information we need to put the pieces of their pelvic health puzzle together in order to help them achieve long lasting relief. We love being pain detectives!! What also sets us apart is that we use a Biopsychosocial Approach to healing because we understand that there is ALWAYS a physical AND Emotional component to healing and BOTH must be addressed for long term healing. Many times, people with pelvic health issues are bounced from one specialist to another, leaving them with varying opinions, unclear diagnosis, and without a cohesive plan of action. This can be frustrating, confusing and overwhelming for people. Often, patients are treated with medications that provide only temporary relief because they don't address the underlying/ root cause of the problem, oftentimes this is pelvic floor dysfunction. My mission is to help people reclaim their lives and return to activities that bring them joy. I am committed to educating, spreading awareness and providing the highest level of care to optimize pelvic health and healing, ultimately elevating the overall quality of life for people that we help.
