(Adam@TheWorldview.com) By Adam McManus Pakistani Muslims kill 14-year-old Christian teen Pakistani Muslim gunmen shouting threats against Christians shot a 14-year-old Christian dead on Monday, February 5th, reports Morning Star News. Sunil Masih and other Christians were standing in a market in the Mandiala Warraich area of Punjab Province when six Muslim men, armed with pistols, arrived on motorcycles and opened fire on them, said the slain boy's uncle, Mehboob Gill. He said, “We were talking to each other when suddenly Zaman Butt and his accomplices Anas Yaseen, Adil Rehman, Ashraf Ullah and two unidentified men came there on motorcycles. Adil shouted that no Christian in the area should be left alive, after which Zaman opened fire on Sunil with his pistol, hitting him in the chest.” Yaseen shot at another Christian boy, identified only as Jamshed, with the bullet only grazing him. The uncle said, “We rushed Sunil to a local hospital, but he succumbed to his bullet wound before doctors could begin their treatment,” adding that his nephew was an eighth-grade student whose father, George Masih, works at a local car shop. Pakistan ranks seventh on Open Doors' 2024 World Watch List of the most difficult places to be a Christian. In Acts 22:4, Paul confessed, “I persecuted the followers of this Way to their death, arresting both men and women and throwing them into prison.” If Jesus Christ can convert Saul, please pray that He converts these Pakistani Muslims who are killing Christians. Israel says over a fifth of Hamas hostages are dead Israeli intelligence officers have concluded that more than one-fifth of the 136 individuals still held hostage by Hamas in Gaza are dead, reports The Christian Post. The Israeli military confirmed the deaths of at least 30 of the more than 130 remaining hostages, according to a copy of the intelligence report reviewed by the New York Times. While Hamas, the Muslim terrorist group, released several hostages during a temporary ceasefire in November, dozens remain in captivity. Another Biden gaffe In an appearance in Las Vegas, President Joe Biden confused French President Emmanuel Macron with the late French President François Mitterand who was the president of France between 1981 and 1995 and died in 1996. Listen. BIDEN: “Right, right, right after I was elected, I went to what they call a G7 meeting, all the NATO leaders. It was in, it was in the south of England. And I sat down and I said, ‘America is back.' And Mitterand from Germany, I mean from France, looked at me and said, said, ‘You know, why, how long you back for?'” Fox News reporter Peter Doocy raised the issue of Biden's mental competence in a White House Press briefing with Karine Jean-Pierre. DOOCY: “How is President Biden ever going to convince the three-quarters of voters who are worried about his physical and mental health that he is okay, even though in Las Vegas he told a story about recently talking to a French president who died in 1996?” JEAN-PIERRE: “I'm not even going to go down that rabbit hole with you sir.” DOOCY: “What's the rabbit hole? He said he talked to Mitterand?” JEAN-PIERRE: (referring to another reporter) “Go ahead. Go ahead.” Supreme Court skeptical about keeping Trump off ballot And finally, the Supreme Court sounds broadly skeptical of efforts to kick former President Donald Trump off the 2024 presidential election ballot in any of the 50 states, reports the Associated Press. In arguments ticking past 90 minutes Thursday, both conservative and liberal justices raised questions of whether Trump can be disqualified because of his efforts to undo his loss in the 2020 election, ending with the January 6, 2021 rally at the U.S. Capitol. At the heart of the case is a provision in the 14th Amendment preventing those who “engaged in insurrection” from holding office. Listen to former President Trump's attorney, Jonathan Mitchell. MITCHELL: “The Colorado Supreme Court held that President Donald J. Trump is constitutionally disqualified from serving as president under section three of the 14th Amendment. The Colorado Supreme Court's decision is wrong and should be reversed for numerous independent reasons. The first reason is that President Trump is not covered by section three, because the President is not an officer of the United States as that term is used throughout the Constitution.” Even liberal Supreme Court Justice Elena Kagan sounded skeptical about ruling to disqualify Trump from a state's ballot. Listen. KAGAN: “I think that the question that you have to confront is why a single state should decide who gets to be President of the United States. This question of whether a former president is disqualified for insurrection, to be President again, is just say it, it sounds awfully national to me. So, whatever means there are to enforce it would suggest that they have to be federal, national means. “You know, if you weren't from Colorado, and you were from Wisconsin, or you were from Michigan, what the Michigan Secretary of State did is going to make the difference between whether Candidate A is elected or Candidate B is elected. I mean, that seems quite extraordinary, doesn't it?” Here's the response of Jason Murray, the Denver attorney who successfully persuaded Colorado's top court to declare Trump ineligible to run in that state due to the Constitution's insurrection clause. MURRAY: “No, Your Honor, because ultimately, it's this court that's going to decide that question of federal constitutional eligibility and settle the issue for the nation. And certainly it's not unusual that questions of national importance come up.” KAGAN: “Well, I suppose this court would be saying something along the lines of that a state has the power to do it. But I guess I was asking you to go a little bit further and say, ‘Why should that be the right rule? Why should a single state have the ability to make this determination, not only for their own citizens, but for the rest of the nation?'” MURRAY: “Because Article Two gives them the power to appoint their own electors as they see fit. But if they're going to use a federal constitutional qualification as a ballot access determinant, then it's creating a federal constitutional question that then this Court decides. If this court affirms the decision below, determining that President Trump is ineligible to be president, other states would still have to determine what effect that would have on their own state's law and state procedure.” Eight of the nine justices suggested that they were open to at least some of the arguments made by Jonathan Mitchell, Trump's attorney at the Supreme Court. Trump could win his case if the court finds just one of those arguments persuasive. Only Justice Sonia Sotomayor sounded like she might vote to uphold the Colorado Supreme Court ruling that found that Trump “engaged in insurrection” and is ineligible to be president. The state court ruled Trump should not be on the ballot for the state's Republican primary on March 5th. Healthcare in the U.K. is state-funded and led by way of a political manifest. The issues of governance in the National Health Service are complicated, and it is a 'wicked challenge.' The hierarchical structure, multi-layered bureaucracy, and under-investment in the health system are coupled with crippled capital restrictions and a mountain of workforce issues. In this podcast, Dr Sabine Dembkowski, Founder and Managing Partner of Better Boards, discusses the challenges of boards in the NHS and healthcare with Nabil Jamshed. Nabil is recognised for his outstanding governance work in the largest Trust in the National Health Service in the U.K. He is Head of Corporate Governance at Guy's and St Thomas' NHS Foundation Trust for the Integrated Specialist Medicine Clinical Group. "A wicked problem doesn't equate to a wicked answer"Nabil explains that health care as a service provided to customers, clients, and patients has grown significantly in complexity over the years. Governance within that is what he describes as a really wicked problem, and he thinks solving this is the biggest challenge in the health sector. "If you don't take a holistic approach, and take everybody with you on the journey, the governance in the traditional way will fail"Nabil explains there are many different approaches to governance, and there is still an absolute need for the mechanical aspect of governance - meeting the minimum compliance regime, standards to ensure a safe environment, producing a high-quality product, etc. However, governance also needs both a vision and strategy. He feels this is where traditional models fail because they do not emphasise the softer aspects of governance sufficiently and focus too much on compliance. "When you have your non-executives involved versus your executives, they have two different lenses" Nabil relates that executives tend to focus on operational delivery. In contrast, the Non-Executives focus on the assurance that everything is happening as it should happen, with no loopholes, and that this does not expose the organisation to significant risk. In his Trust, they modelled the governance structure on assurance committees and fixated on those capitals. Every committee led on one, two, or a combination of capitals, with responsibility against that. "Any single report that comes to the board is aligned to those six capitals"All information produced for board packs is now aligned to the six capitals, as is any single report. He is proud of the Triple-A model that has been introduced. Within each report, the three A questions are 1. What do we want the committee or the board to be Alerted to? 2. What do we want to provide the board and the committee Assurance with?3. What Actions are we taking to address going forward?Every single report that comes to the board covers those three elements. But he notes the real change is in the discussions held at committees in local feeder groups, which is an absolute bottom-up approach. The three top takeaways from our conversation are:1. Have a clear-cut, long-term strategy; stick to it regardless of changes and believe in the process it will deliver. 2. Build agility in governance and link it to your strategy and the risks you identify through that process, not getting hung up on one compliance only. 3. Complex problems do not mean a complex solution. They mean a simple solution. But that simple solution doesn't mean the complexities are avoided - you understand and address them and build your governance to simplify those complexities.

Desperate scenes of Afghans trying to flee the Taliban takeover of their homeland have shocked the world. But refugees have been trickling into India even during the past twenty years. They say they are free in India in a way that they never could be back home. However, Afghan refugees have been protesting outside the United Nations High Commissioner for Refugees office in Delhi for several days, demanding better opportunities. On this episode of The Suno India Show, our reporter Suryatapa Mukherjee spoke to refugees living in Delhi, Jamshed, Zahra Mohammadi and Valy Ahadi. They have all fled to India from the Taliban's violence over the last decade. As they worry for terrified relatives back home, they feel grateful to have made it to India. But they say being a refugee here is like being stuck in time, unable to move forward. It is a struggle simply to sustain themselves. It means: restrain oneself from doing or enjoying something; to choose not to vote; stay away from some work. In this English vocabulary lesson you will learn how to use the word abstain. The word abstain means restrain oneself from doing or enjoying something or to choose not to vote. For example, after the heart surgery, doctors advised the patient to abstain from smoking and eating junk food. We may also say that when the voting took place for electing the society secretary, three committee members abstained. Listen carefully how we can use the word abstain in 8 different situations in 8 different sentences. Example number 1 of 8: The popular internet search engine appealed to users to respond to a survey regarding some cultural beliefs. He suggested a joint meeting with the school counselor to resolve any complaints about the students. Example number 7 of 8: The tourists taking a boat ride along the lake would feed snacks to the birds flying over the lake. These food articles are harmful for the birds. Hence, the local corporation put up boards asking the tourists to abstain from feeding the birds. Example number 8 of 8: The members of a social media group were having heated arguments over their political preferences. Finally, the group admin intervened and requested the members to abstain from posting politically biased messages. What is SCAPE? For this podcast, we're discussing the acute pulmonary edema presentation. This patient is hypertensive (SBP >140mmHg), severely dyspneic, with diffuse rales and clearly anxious. The "no-shitter, drowning-before-your-very-eyes" type of pulmonary edema. This is the SCAPE patient. SCAPE = Sympathetic Crashing Acute Pulmonary Edema. Patho Quick Hits The core causative factor in the SCAPE patient is an acute increase in left ventricular filling pressure. There are a myriad of causes for a sudden increase in LV pressure, but the end result is a redistribution of fluid into the lungs. 1) Acute increase in LV filling pressure. 2) Fluid redistribution into the lungs and alveolar space. 3) Hypoxia ensues. 4) Catecholamine production and increase in SVR. 5) Activation of the RAAS. It's important to remember that the majority of these patients are not volume overloaded. This is a fluid distribution problem due to increased LV pressure. As the RV continues to pump fluid into the pulmonary circulation, the LV cannot move that fluid forward because of the increased afterload. This creates a pressure gradient that transmits that pressure back into the pulmonary capillaries. 5 Major Causes of SCAPE - Exacerbation of chronic LV failure - Acute myocardial ischemia or infarction involving 25% or more of the myocardial mass - Severe systemic hypertension - Left sided valvular disorders - Acute tachydysrhythmias and bradysrhythmias Treatment In the out of hospital realm, the core treatments are Non Invasive Positive Pressure Ventilation (NIPPV) via CPAP or BiPAP, coupled with nitroglycerine as a first-line medication. For the "regular guy" toolbox, the treatment pathway looks a little like this: 1) Treating the underlying cause if evident. 2) NIPPV 3) NTG 4) More NTG 5) More NTG 6) More NTG Do not delay NIPPV to see if other therapies (like a NRB) will work first. In the awake patient maintaining their own airway presenting with SCAPE, have a low threshold to apply your NIPPV mode of choice. These patients need PEEP: they generally have an oxygenation problem, and not a ventilation problem. To that point, most prehospital disposable CPAP systems do not deliver 100% FiO2. The O_two and Pulmodyne O2-MAX systems we generally use are either fixed FiO2 or provide a titration of FiO2 based on oxygen flow. The O_two system will provide between 59% and 77% FiO2 at oxygen flow rates between 8L/min and 25 L/min respectively. The Pulmodyne O2-MAX system provides 30% FiO2 regardless of PEEP, or with an additional adapter may provide 30%, 60%, or 90% FiO2 independent of the set PEEP. Nitrogylcerin If sublingual NTG is all you have, give it. Often, too. Lifting up the CPAP mask for 20 seconds is highly unlikely to cause clinically relevant harm. If you have the option of IV NTG, that should be your go-to. Standard dosing strategies for IV NTG of 5-40mcg/min are likely ineffective, and there is literature to support higher dosing strategies. Consider that we bolus 400mcg of SL NTG, and that the bioequivalence of SL NTG is comparable to around an IV NTG dose of 60-80mcg/min, so rapid titration of IV NTG even up to 100mcg/min is not entirely unreasonable and largely supported by current literature.
