O câncer do rim é silencioso e representa apenas 3% dos tumores malignos urológicos no Brasil, segundo dados do Ministério da Saúde. Por essa razão, ele não é alvo de uma política pública de rastreamento e o resultado é que muitas vezes os pacientes acabam descobrindo a doença tarde demais. Taíssa Stivanin, da RFI em ParisO paciente que descobre um câncer do rim em estado precoce pode ter 100% de chance de cura, explica o oncologista Cleydson Santos, do hospital Mater Dei em Salvador, que também atua nos hospitais das Obras Sociais Irmã Dulce, na capital baiana. Segundo ele, todos os anos os pacientes deveriam realizar um ultrassom do abdômen para detectar precocemente nódulos suspeitos no rim. Como todos os cânceres, quanto mais tarde é feito o diagnóstico, menores são as chances de sobrevida e de remissão. O rim é um órgão vital, responsável pela filtragem do sangue. “O câncer nos órgãos abdominais não se manifesta de uma forma tão clara quanto um nódulo na mama, por exemplo, ou do intestino, que logo sangra, levando o paciente a buscar ajuda", diz o oncologista. "O diagnóstico dos tumores renais quase sempre acaba acontecendo por acaso. O paciente vai fazer um exame por algum motivo e de repente é surpreendido pelo nódulo”, acrescenta.Os sintomas em geral aparecem quando o câncer já está em estado avançado e, em 25% dos casos, disseminado. De acordo com Cleydson dos Santos, isso acontece “porque a doença é traiçoeira, e não porque o paciente não busca ajuda”. Sintomas mais comunsNão existe um exame de sangue específico para detectar o câncer renal. Quando há sintomas, as queixas mais comuns são a presença de sangue na urina, anemia, fraqueza, dor e cólica na região lombar, além de perda de peso.  Caso a doença já tenha se espalhado, em função do órgão afetado, podem aparecer outros sinais. No hemograma, a função dos rins, explica o oncologista, muitas está normal – e isso pode gerar uma falsa tranquilidade no paciente.  “O câncer renal não vem necessariamente acompanhado da insuficiência renal, que é bem frequente. Então às vezes o paciente tem um tumor, mas a função renal dele e os níveis de ureia e creatinina são normais”, diz.  O oncologista brasileiro também recomenda que, na hora do check-up, o paciente peça ao médico que inclua um ultrassom do abdômen. “Uma checagem anual geral talvez seja um bom caminho para pegar a doença no início.” Homens entre 50 e 70 anosO câncer do rim atinge principalmente homens entre 50 e 70 anos. A incidência é duas vezes maior em relação às mulheres. Pacientes obesos, sedentários, que fumam ou têm histórico de câncer renal na família também têm maior probabilidade de desenvolver a doença. Rara em crianças, a doença pode se manifestar de várias formas nos adultos, em forma de cistos e nódulos. “A maioria dos cistos de rim não é uma doença maligna, não é câncer e não se transformará em câncer”, esclarece.“Mas a gente tem alguns cistos chamados complexos, com divisórias dentro, e nódulos sólidos, que fazem suspeitar da doença. O câncer de rim mais comum é o subtipo conhecido como células claras, que corresponde a 85% dos casos.” Quando a doença se espalha, pode ser bilateral, ou seja, já ter atingido o outro rim. Se o tumor já atingiu outras partes do corpo, em geral os órgãos mais afetados são os pulmões, o fígado, os linfonodos, ou ínguas, os ossos e às vezes até o sistema nervoso.  A boa notícia é que, se o paciente tem poucas metástases, ainda pode ser curado, diz o médico. “Nesse caso a gente investe no paciente e corre atrás, para tentar removê-las por inteiro e garantir uma sobrevida a longo prazo. Em torno de 5% a 15% das pessoas vão ter o câncer controlado a médio ou longo prazo”.  Prognóstico O prognóstico, diz o oncologista, depende do tipo de câncer e do estágio da doença. “Existe uma diferença entre um tumor localizado no rim de um que já cresceu e ultrapassou os limites do órgão e de um câncer que já se espalhou, criando metástases. Isso significa que a doença já se ‘enraizou'”, explica. O tratamento é cirúrgico e nem sempre é necessário retirar todo o órgão. Em alguns casos, é preciso realizar uma imunoterapia depois da operação, para estimular o sistema imunológico do paciente e combater as células residuais.  “Quando a doença já se espalhou, o tratamento é a base de medicamentos orais. O câncer de rim normalmente é resistente às quimioterapias tradicionais”.  Os tratamentos, lembra o oncologista, evoluíram muito, mas infelizmente existe um “abismo” entre os pacientes da rede privada e do SUS (Sistema Único de Saúde), o que torna ainda mais importante o diagnóstico precoce.  “É importante nós lutarmos para que o mesmo tratamento que o paciente tenha no sistema privado, com resultados tão bons, seja aplicado no sistema público. Em muitos lugares, não temos nenhum tratamento disponível quando a doença já está avançada”.  O custo que envolve os novos fármacos, descobertos nos últimos dez anos, é apontado pelo oncologista como uma das principais razões dessa desigualdade.

Taylor King joins us to share his basketball story! Taylor King starred at Mater Dei High School where he had a legendary high school career finishing as Orange County's all-time leading scorer, 2nd all-time in rebounds, 2nd all-time in 3 pointers made, and led his team to three consecutive state titles. He was a McDonald's All-American and went on to play his college hoops at Duke, Villanova and Concordia before continuing his career overseas and in the G-League. Tim Cates sits down with longtime high school basketball coach and current Mater Dei HS Athletic Director Kevin Kiernan. Over his coaching career he has won 900 games, 12 CIF-SS titles, 6 CIF-State championshps and 2 National Championships in girls basketball. Kevin helped build the girls basketball program at Troy HS and has spent the last 17 years at Mater Dei where in 2024 he was named the WBCA National GBB HS Coach of the Year. Kevin has had many players go on to play college basketball and has seen several former players make it to the WNBA. Since 2021, Kevin has been serving as Athletic Director at Mater Dei, overseeing one of the biggest brands in high school sports.

That has been fixed in the files at littleoffice.brandt.id.au/#booklets Refreshing the files at Gumroad and Udemy will take a little longer.Looking back at last month's update, I did manage to accomplish one of the three goals, that of running through the explanation of rubrics in the Benziger Brothers 1914 Little Office book. It took two videos. Unfortunately they are recorded in fairly low resolution as my usual video device was being used elsewhere, so I used an old iPad which I had previously set to low resolution video, as sometimes a small person fills up the hard drive… anyway here they are:And the second installment looking more into the ceremonial aspects:The other two goals were a bit more ambitious - to explain how to sing psalms and make a course on singing Roman Compline.The real answer to the question “how does one psing psalms?” is that one can psing them in any way one desires. Of course, coming from a background in singing psalms in the western chant tradition in Latin, that's going to be the way I will gravitate towards, but there are many different ways to sing. I've been singing with many different people with many different styles and realising that singing in itself is a valuable skill to cultivate. Our culture seems to push people away from singing into being passive consumers of mass produced noise. Any sort of singing is better than nothing.The photo above captures my current list of ideas for Youtube videos. I'm always open to suggestions. Someone asked me how I would recite the Chaplet of the Holy Face in English and I started drawing diagrams describing my interpretation of the description in the 1789 Holy Face Manual, but co-ordinating all the elements of making videos seems to be more difficult than I remember.Also, this particular substack is devoted to the Little Office of Our Lady and sometimes I go outside that area. I have another podcast called the Latin Mass Homeschool Family, which is hosted with Spotify. Unfortunately Spotify has a policy of only hosting podcast episodes which are predominantly spoken. They recently deleted 32 episodes from my Latin Mass Family podcast without warning. I tried contacting them about getting back up copies, but turned out their system was not set up to cater for that service. The episodes were all from the 100 days of Chant challenge which I set myself a few years ago. Fortunately they are all still on Youtube. The recordings I was most concerned about were the family recordings of singing Compline, which would have been very difficult to recreate, and are all linked into a little blog at compline.brandt.id.au. Fortunately all those recordings passed Spotify's test for being mostly spoken.That gives me yet another goal, to migrate the Latin Mass Homeschool Podcast to a more music-friendly host. I'll put that job alongside updating the wowchemy themes on my webpages to hugo blocks.But all this pales in comparison with what seem to be our Holy Father the Pope's last days on earth. Coach Kevin Kiernan is the Athletic Director at Mater Dei High School in Santa Ana, CAH Kevin Kiernan, the all-time winningest girls' basketball coach in California history, will be stepping down as head coach at Mater Dei at the conclusion of the regional playoffs, He will continue as athletic director. He's finishing his 17th season at Mater Dei. He's won three state titles at Mater Dei, six Southern Section titles, six regional titles and 17 straight Trinity League titles. Johnny and I entered the Mount together in 2003. He was an excellent lacrosse goalie and one of the more laid back guys I went to school with. It was an absolutely honor and pleasure to interview JRod. We took a stroll down memory lane during our time at The Mount, his journey through college lacrosse and hockey, and his prolific career as a Major League Lacrosse goalie in Atlanta. JRod opens up about his decade long stint as Mater Dei's head lacrosse coach and winding down coaching after his ALS diagnosis last fall that he continues to battle mightily every single day.

The Divine Praises (Laudes Divinae) is an 18th-century prayer.  It is traditionally recited during Benediction - but can be used at other times, either in a group or individually. The prayer was written in Italian by Jesuit priest Luigi Felici SJ (1736–1818) in 1797. It is intended for making reparation after saying or hearing sacrilege or blasphemy / mis-use of the name of Jesus. It was expanded by Pope Pius VII in 1801, and at some point was translated into English, Latin (and likely various other languages) - the translator(s) are unknown.   The English version is sometimes just called "The Blessed be God". The prayer is traditionally said by having a leader say each line and for the congregation to repeat it - and in this manner,.no visual aids or printed text is necessary (except for the leader).  However other format can be used: the Divine Praises can by prayed individually, or used as the basis of group meditation supported by images and other aids - or in any other way.   DIVINE PRAISES Blessed be God. Blessed be His Holy Name. Blessed be Jesus Christ, true God and true Man. Blessed be the Name of Jesus. Blessed be His Most Sacred Heart. Blessed be His Most Precious Blood.
