In Episode 13, we heard the fascinating career story of Biomedical Scientist in Haematology turned famous comic and actor Dave Spikey. Dave is best known for his stand-up comedy tours, starring in sitcom Phoenix Nights alongside Peter Kay and Paddy McGuinness, presenting Bullseye, Chain Letters and being a team captain on 8 out of 10 Cats. Before he moved over to comedy, Dave led a successful 30-year career in Biomedical Science – rising the ranks to become Chief Biomedical Scientist in Haematology at The Royal Bolton Hospital. Dave recollects his journey into Biomedical Science from school pupil to trainee medical laboratory technician to senior biomedical scientist. He discussed the various roles he held and the innovations in the field he brought about. We then challenged Dave, who is a former Mastermind champion to our very own Haematology quiz. He then took us through his move from Biomedical Science into comedy, writing, film production and television - from a local hospital pantomime performance to a national stand-up tour, starring in iconic sitcoms and hosting television game shows. In the Quick-Fire round, Dave shared his most funny memory from his days back in the lab! Then in LabLife, Dr Martin Khechara is joined by Andrologist Sheryl Homa, who discussed the importance of this marginalised, but vital specialism. In this episode, we're focusing on Male Infertility which is still not understood well enough and it's something Natalie has been passionate about giving a voice too. As it was her and her husband's experience and the pair felt very unsupported when going through treatment. Since its launched The Fertility Podcast has shared numerous episodes about Male Fertility explaining the issues that affect sperm health and also explaining how Men can seek more support and investigation.   Men are often overlooked in conversations with medical professionals and we have had numerous conversations about men feeling ignored at the fertility clinic, the very fact that when it comes to IVF treatment in a heterosexual relationship is it is the woman who has to go through the procedure means men are often left to feel redundant. In the many conversations we've had with Men, they have talked about the shame and guilt they have felt if the issue has been a male factor and how many men struggle in silence.   Kate and Natalie discuss the factors that can affect male fertility, such as diet and lifestyle issues. Smoking and drugs are a real no-no for sperm health. It's really important that alcohol is taken into needs to be taken into consideration and ideally should be limited but not stopped. Factors such as heat and stress can impact. So consider saunas, heated seats, mobile phones in pockets, laptops on laps as important things to think about. Plus wearing tight pants and trousers can also be restrictive for the wrong reasons. Personally, Kev has had two failed attempts with NHS funding - IVF and ICSI and he couldn't find anything online and has learned more from being on Instagram than just googling.   Kev's diagnosis is Non-Obstructive Azoospermia which is defined as no sperm in the ejaculate due to failure of spermatogenesis and is the most severe form of male infertility. He had to undergo a micro-tease ( sperm extraction( which didn't work and Kev was told his options were sperm donor or adoption.   You can imagine how heartbreaking this was for him and he was told the news without any support which was part of his motivation for setting up what he has done. It was an absolute pleasure to chat with Professor Sheryl Homa this week on the podcast. Sheryl Homa is an HCPC registered Clinical Scientist with a special interest in male fertility. She obtained her degrees in Biochemistry at Imperial College of Science and Technology and the Middlesex Hospital Medical School. She spent a large part of her career as an academic research scientist in the field of oocyte maturation, funded by the National Institutes of Health in the USA. For many years she served as Scientific Director of several fertility clinics in the UK working in both the public and private sectors. During this time, she became aware that male fertility is considerably marginalised and as a result, opened Andrology Solutions in 2007, the first and only HFEA licensed clinic of its kind, dedicated to male infertility. She has collaborated on many research projects and is the author of numerous scientific articles, abstracts and book chapters in the fertility field. Her studies include investigating calcium signaling in sperm and more recently, the role of oxidative stress and infection on sperm quality. Sheryl has been responsible for introducing and CE marking novel state of the art testing for male infertility, including a chemiluminescence assay for measuring seminal reactive oxygen species. Currently Sheryl is honorary Professor in Biosciences at University of Kent and consultant clinical lead for Andrology at The Doctors Laboratory. We chatted about how men are equally responsible for creating an embryo that would lead to a child, how half of the genetic material is from men and that the child is a reflection of the genetic material of both partners. That it isn't just about getting pregnant or staying pregnant but ultimately the health of a child.    We talked about how sperm and eggs need to be as fit and healthy as they can be and how if the sperm or egg is damaged in anyway this passes to every cell of the child and this is an area that I talk about a lot with my clients: that we need to widen our vision looking further than just getting pregnant.  This helps us make the changes necessary. Sheryl said something that really made sense to me and she said that she advises her male clients to act as though they are pregnant so in the few months leading up to conception to think about their sperm as that they are carrying pre babies. We know that 11 to 12 weeks is necessary for both the sperm and the egg to develop fully and so a preconception preparation plan is absolutely vital and she compared this to preparing for Wimbledon and saying that if you were going to compete at Wimbledon you wouldn't just rush on and play would you? No, you would prepare with training, health and with the right equipment.  I’m a huge fan of an analogy myself, I’ve often used the example of how you wouldn’t ‘just run’ a marathon. We looked at how it's important to consider quality and not quantity and how this is important to maximise your chances of both natural and assisted fertility and we need the best quality egg and sperm. We talked about the sperm analysis and how this is a crucial first step and provides great information but it only tells you if there are potentially enough and we know the parameters are low, too low, and if sufficient are moving. However, it doesn't tell us whether the sperm is capable of getting to the egg or if it is capable of entering the egg or if it has the right triggers to ensure embryo development. Resistance infertility circles was something that we discussed as well and this letters onto the resistance amongst the fertility clinics to undertake two very specific tests that give it so much information about the sperm in terms of fertility but also importantly in terms of miscarriage and how this really should be offered to all couples experiencing recurrent miscarriage and those tests are the DNA fragmentation test and the oxidative stress test on the sperm. Something that comes up for me with my clients and with something that we discussed is how some Doctors and clinics will say that even if the test showed DNA fragmentation or oxidative stress there isn't anything that can be done about it, but Sheryl explained how there might be quite a bit you can do about it. We discussed also how a condition called Variocele maybe a huge factor in terms of sperm quality, how a full clinical history should be taken for male partners, how women have an ultrasound but that men aren’t examined at all. How underlying infections which are not looked at are an underlying cause of conception and potentially miscarriage as well and how a microbiology test on the sperm is a crucial  test. Of course we discussed the recent documentary by Rhod Gilbert ‘stand up for infertility’ Which led us to chat about physical issues and emotional issues. Not only emotional issues in terms of how they may affect sperm physically but the lack of support for men who are going through a fertility journey. She mentioned a Facebook group the men's fertility support run by Gareth Down which I would urge you to point your male partners in the direction of   We chatted about some basic general advice at the start of a couples fertility journey and we discussed as you've heard me talk about before why trying to conceive at or just before ovulation may in fact be too late and how important a regular ejaculate is because stored sperm is in effect rubbish sperm, and that again it's not just about numbers it's about freshness and quality She will also explained how we were missing a vital piece in the journey and that your GP referring you to the fertility clinic straight away which is ultimately for assisted fertility is like going to your doctor with heart palpitations and the doctor saying let's get you a heart transplant. She explains that IVF is circumvention, how it is not managing fertility and that after initial GP tests, further investigation should take place and treatments suggested to correct possible fertility factors.  Further investigation can be undertaken by an Andrologist. Something of interest that was pointed out in Rhod Gilbert's documentary is that there are 8000 registered Gynaecologists in the UK and only 200 registered Andrologist. Infertility is far more common than we think. It can feel like a very lonely journey but we never know who around us is having a similar experience. On in six couples struggle to conceive. Over 50% of infertility cases involve male infertility either solely or in-conjunction with female infertility. So why is it there is so little support specifically for men? In this episode comedian, TV and radio host Rhod Gilbert shares his experience of being diagnosed with fertility problems and how it felt to be suddenly immersed into the world of fertility. Rhod has created an awesome TV documentary called Stand Up to Infertility is on BBC Wales on  Monday 25th January, 9pm and then on iPlayer and also being broadcast BBC2 in the UK on 30th January, 9.40pm. Based on my and my wife's 10 year fertility journey and the work of my clients I believe too many couples have IVF unnecessarily and that the success rate of IVF is unnecessarily low. We can see how much our mind and emotions played a part in our fertility journey and when we found a place of peace without giving up that's when the magic happened - we conceived naturally against all odds. I help me clients do the same - remove psychological blocks to getting pregnant naturally or getting the successful IVF outcome they so want.    When I ask women what having a baby will give them they usually say a sense of completeness, love and purpose. It's our fear of not having these things, a fear of not being enough, worthy that tends to be behind our fear of not having a baby. These limiting beliefs have usually been around since an early age and infertility shines a spotlight on them. Many of my clients have strived for success in their life whether career or other things. That approach doesn't work for fertility. In fact it puts tension in the body which decreases their chances of success.   The podcast is short does of insight, giving women and couples new perspectives that allows them to see their fertility from the ‘inside-out'. To see help them let go of their insecure thoughts and feelings and connect to their innate wellbeing that knows they are enough and OK whatever happens. This moves having a baby from being a 'need' to to a desire. It takes the pressure of the process and their mind and body relax into it's natural flow state. Stress can impact the delicate balance of hormones required for a successful pregnancy.

Men don't talk about fertility and society largely views fertility as a woman's issue despite the fact that men are said to be a factor in around half of all fertility issues between couples. That's the view of the stand up comedian Rhod Gilbert who is raising awareness of the issue and wants men to open up and be part of the conversation. He's made a BBC documentary 'Stand up to Infertility' where he shares his own struggles to conceive with his wife Sian. Stand Up to Fertiliy is streaming now on BBC iPlayer and on BBC Two on Sunday 31st January at 10.00pm. A new study indicates teenage girls experience a sharper decline in well-being and self-esteem compared with boys due to “heavy social media use". It has also suggested that the pandemic might be making it worse. The report - by the think tank, the Education Policy Institute, and the Prince's Trust - calls on the government to act quickly to improve mental health support for schools in England. Emma is joined by Whitney Crenna-Jennings , the author of the report, and Matt Haig - the author and mental health campaigner who decided to leave twitter earlier this week - where he had nearly half a million followers. Today is International Holocaust Memorial Day where we remember the millions of people murdered by the Nazi regime. For decades, survivors have spoken about their experiences in the hope that nothing of its kind will ever be repeated. But when the last survivors are gone, who will tell their stories? And how do we talk to children about such harrowing events? Children’s author, Liz Kessler has written fictional a novel called When The World Was Ours that was inspired by the true story of her father’s escape from Czechoslovakia. Noemie Lopian is the daughter of Holocaust survivors Ernst Israel and Renee Bornstein. Ernest wrote The Long Night about his time in 7 different concentration camps and several death marches. They join Emma to discuss why it’s so important to talk to children about the Holocaust and the parallels between now and then.

Why is it that you're supposed to want to have children, what does it mean if you're really not sure that you do, and why it is that so many people feel they have the right to tell her she's wrong to feel the way she does? Sian Harries has written comedy for The Now Show, Greg Davies' 'Man Down' and she's worked closely with her husband, the comedian Rhod Gilbert. But despite her success, she explains how bad it made her feel when someone walked up to her at a party and asked her when she was going to have children. She wonders why people feel they have the right to ask this question, and why it only seems to be of women but never men. The Royal couple are having a baby…and a radio station in New Zealand played a prank on the hospital. This has now escalated as the poor nurse who took the call has killed herself. Who is really to blame for this poor nurse's death? …. the advent of modern technology is turning the UK into a nation of morons….we don’t speak anymore because we are always on our ipads etc…There is a “helicopter” you can buy from Amazon which is radio controlled and has a small camera on board so you can fly it and take movies from the air which you can send directly to your phone…. There has been a national disaster….the DANDY is no more!!....the last copy has gone on sale!!....I think they are going to go on line.They tried the comic out on young kids who were not impressed…they said it wasn’t exciting enough….……however the annuals sell well according to bookshops………nowadays if you don’t run with the pack you are a gonner. I watched The Royal Command Performance … a little too much opera perhaps….…I like Katherine Jenkins …but for a completely different reason…in fact when she sings ballads in her lower range she’s great….I thought Diversity and the young dancing acrobats were really good and even the comedy wasn’t bad with a few exceptions (Rhod Gilbert and poor old Des)…but otherwise good stuff for our royal ruler to enjoy.
