Also check out the video version on my WeitzChiro YouTube page.]    Podcast Highlights 2:02   Dr. Chopra was born without eight teeth, so she was born to be a dentist.  When she was in high school, she developed a toothache and her dentist could not figure out where it was coming from. She saw several different dentists and it took nine months and she eventually had her tooth pulled, but they pulled the wrong tooth. Finally she got to an endodontist, who gave her the root canal she needed. 5:53  Healthy practices to keep your teeth healthy.  Flossing regularly is really important because a lot of the decay happens from the contact point between our teeth where the toothbrush can't go.  While the waterpik may have some benefits, it does not replace flossing. Dr. Chopra uses two different types of floss--the thin floss and an expanding floss.  You should make a C around the side of each tooth with the floss and then go up and down gently into the gums. 11:22  Biofilm.  While the healthy bacteria in the oral microbiome do form a biofilm, generally you do want to break up most of these biofilms because if this biofilm that we call plaque gets too thick, it will cause you to have gingivitis. 13:12  Toothbrush. Dr. Chopra prefers the Sonicare toothbrush and she feels that the ultrasonic action of the brush will help to break up the bacterial cell wall.  14:05  Toothpaste.  Dr. Chopra feels that some patients benefit from flouride in the toothpaste because it can protect the teeth against cariogenic bacteria.  Her daughter was having a lot of cavities and they stopped after she started using a toothpaste with flouride.  17:45  Root Canals. With a root canal procedure you save a tooth that has a deep infection in the pulp area instead of having it pulled.  According to Dr. Chopra, if a root canal is done in the right manner by properly disinfecting the tooth, it is a regenerative process and some of us can actually regrow our own bone back without a bone graft and without extra calcium.  "There's just so much value to our teeth and we were born with them for a reason. We're not supposed to take them out."  19:52  Root canal myths.  Some people claim that root canals are dangerous to our health because when you do the root canal you kill the nerve and the blood supply to the tooth, so now the tooth is dead.  The second argument why root canals are bad is that there's no way to remove all the bacteria, and the bacteria will fester and lead to various chronic health problems.  Dr. Chopra explains that most of the time the tooth is already dead before the root canal because the bacteria got in there and already killed the nerve.  Some people have been living with a dead tooth for years and that's when you can grow spaces in the bone due to bone reabsorption.  Even after root canal, you still have proprioceptors that are in the periodontal ligament that attach the tooth into the bone, which is swimming in these haversion canals that continue to provide the tooth a blood supply. Therefore, root canals are regenerative and often a better alternative to an extraction. 28:26  Implants.  Some teeth are fractured or split and cannot be saved by a root canal, so in this case, an implant is the last resort.  Implants can sometimes fail and sometimes they don't take.  One potential problem with implants is that you are putting a piece of heavy metal such as titanium in your mouth and your tissues may react and you can have bone loss around the implant.                               Dr. Sonia Chopra, DDS, a board-certified endodontist, TEDx speaker, author, and E-School founder, argues that it is time to debunk such myths. Plus, we are the specialty that saves teeth, and in my opinion, just based on my own experience and being born without eight teeth, saving teeth has really become a very important topic for me." Sonia discusses the  evolution of jury consulting to trial consultants and, finally, to litigation consulting.  Part of that evolution is the constant balancing of the science and the art of human psychology and social psychology.  The science is through training -- Sonia's training is as a Ph.D psychologist with an emphasis in  litigation.  The art is the constant exposure to prospective jurors through focus groups and voir dires.  Unlike trial lawyers who, even the best of them on high exposure trials, get to trial four to ten times a year, Sonia has the capacity to  accomplish the same volume in a month.  As a result, she has interacted with more people than almost any trial lawyer.   That type of real life exposure -- data -- helps to minimize the subjectivity to her methods.An essential aspect of jury selection is identifying jurors with biases that warrant the Court excusing them from presiding over the trial.  Sonia discusses the impediments that lawyers face in overcoming their own biases to be effecting in communicating with and exposing prospective juror's biases.Like any other person, lawyer's biases are innate and potentially translate into their personal lives.  The irony of that is not something that is lost on Rahul and Ben.  We spend our careers trying to eliminate bias, like many trial lawyers, but in order to effectively do so, we need to make sure we have done so in our own lives.Rahul has been working with Sonia for many years and is a firm believer in the benefit of an effective litigating consultant.  Ben has utilized litigation consultants in focus groups but his time with Sonia may have expanded his interest in using litigation consultants more broadly.  About SoniaSonia Chopra, Ph.D. is co-founder and president of Chopra Koonan Litigation Consulting. It’s the dream and goal of many dentists to eventually start and run their private practice. You may have this perception that having your own practice is better than being an associate dentist, as you have more control over your time and what you want to do. While it is true that there are advantages to a private practice, there are also struggles that those who chose this path go through. These are normally what you wouldn’t see portrayed on T.V. or on social media feeds. Endodontist Night for Smiles (https://anightforsmiles.org/) , a nonprofit organization that’s dedicated to make dental care more accessible in Charlotte and North Carolina. On top of all her professional work, she is also a wife and mother to three children. In this episode of White Coat Wellness, Dr. Chopra shares some of the issues she encountered, as her practice was growing. One major problem was trying to find the right work/life balance, which can be a struggle if you’re used to doing everything on your own. It’s important to see where you need support and seek help for it. (08:38) Find the right associate or partner clinicians to help you run the practice is also essential, even if it might take years because not everybody’s going to be a good fit. (12:02) And when you have your staff, learn to let go and delegate tasks. Create systems for accountability too, so you know that things are getting done even when you’re not around. (21:30) When burnout begins, she believes it’s important to do a bit of introspection and see where things need to be different. Change starts from the top down, so leaders have to take the first step. (16:09) She also suggests other strategies to help when things in your practice get exhausting, like meditation and taking breaks and time off for yourself. (17:17) Ultimately, a great private practice is one where priorities are sorted out, collaboration is occurring among the partners, and that work is also done on the practice and not in it. (23:19) This week’s White Coat Wisdom is for transitioning physicians who are considering purchasing a house as they relocate to a new city. While every situation is different, there are a number of factors that you should consider if you’re in the same position. Ask yourself if you’re familiar with the new city and how long do you see yourself staying. Is this practice going to be the right for you? There are also financial considerations, like how much you have in your budget now for housing and if it would be better to rent a property instead. Dr. Sherilyn Sheets and Fern Ingber, founders of America’s Tooth Fairy, National Children’s Oral Health Foundation (https://www.americastoothfairy.org/) is featured in this episode’s White Coat Achievement Segment. These two individuals established the organization, which is a collaborative effort to address the epidemic of pediatric dental disease. Since it was established, America’s Tooth Fairy was able to provide oral healthcare and education to over 7 million children. Their activities include awarding financial grants and educational materials to nonprofit dental clinics and community partners as well, with a focus on children in underserved populations.
