In 2022, 608,000 people died of malaria, which is about 26.4% lower than neonatal conditions. However, we have a cost-effective, scalable model for preventing malaria-caused death (e.g., with AMF and Malaria Consortium). Unfortunately, there has been no equivalently cost-effective and scalable model for preventing neonatal mortality. In this post, we will introduce Ansh, a 1-year-old Charity Entrepreneurship incubated charity that is working towards building tractable, scalable solutions to neonatal mortality in low- and middle-income countries (LMICs). 81% of neonatal deaths happen in low and Low-Middle SDI countries. The disparities in mortality rates between low and high-resource contexts suggest that most neonatal deaths are preventable. In the sections below, we will first introduce Ansh and its mission statement, share our results thus far, and then introduce some of our plans for how to increase our reach and impact over the next few years. We are very excited to share the work we've done so far with the EA community, and to hear your constructive feedback on how we can make our non-profit even more impactful! I. The Problem and Solution More than half of all neonatal deaths occur within the first three days after birth (Dol J, 2021) and over 75% in the first week of life (WHO, 2024), making it imperative to reach babies as soon after birth as possible. Moreover, low birth weight (LBW)[3] is considered the number one mortality risk factor for children under 5. In fact, according to the Global Burden of Disease, around 89% of all newborn deaths in India (the country where about 22% of all newborn deaths in the world occur) happen to LBW and preterm newborns. Further, 81% of all newborn deaths occur in Low or Low-Middle SDI countries (Global Burden of Disease Collaborative Network, 2019). Hence, the most effective path toward reducing neonatal mortality rates globally lies in developing interventions aimed at helping LBW babies during their first week of life in LMIC contexts. Thankfully, such an intervention exists: Kangaroo Care. Kangaroo Care (KC) needs neither fancy equipment nor expensive technology - the methods of KC are both simple and highly effective, especially for LBW newborns. KC requires early, continuous, and prolonged skin-to-skin contact between the mother (or another caregiver) and the baby for about 8 hours of contact per day-paired with exclusive breastfeeding and close monitoring of the baby. This is often assisted with a cloth binder, between the LBW newborn and caregiver (preferably the mother), to allow for mobility. Premature babies often need a lot of expensive specialised care - but that isn't always available. So, doctors in Colombia are teaching mothers to look after their babies in a similar way that kangaroos look after their own young.It's called "kangaroo mother care" and instead of being in an incubator, babies are wrapped tightly against their mother's skin.The technique was developed in Bogota in the late 1970s as a response to overcrowding in hospital maternity units. There weren't enough incubators and around 70% of premature babies didn't survive.Doctors started using this simple skin-to-skin method. They found it wasn't only saving babies but was also helping them to thrive. Today Kate a nurse practitioner gives us some insight into what she's learned about having babies skin-to-skin on mom during a c-section. Kate (Katherine Chokan, DNP (Doctorate of Nursing), BSN, RN ) has been a maternity nurse (Labor and Postpartum) for 6.5 years. She recently graduated from a Nurse Practitioner program in December where she conducted a doctoral research project. An organization that distributes Oral Rehydration Solution and zinc co-packages to effectively treat life-threatening diarrhea in under five year olds in low-and middle-income countries. A charity that builds healthcare capacity to provide “Kangaroo Care”, an exceptionally simple and cost-effective treatment, to avert hundreds of thousands of newborn deaths each year in low-and middle-income countries. One-Paragraph Summaries 1. Preventing growth of antimicrobial resistance by advocating for better (pull) funding mechanisms to drive R&D and responsible use of new antibiotics Antimicrobial resistance (AMR) is where harmful microbes develop the ability to resist treatment. Recent reports suggest that there are 47 million disability adjusted life years (DALYs) per annum attributed to antibiotic resistance (a key type of AMR). The current financial incentives for new antimicrobial drug development are insufficient, and only a handful of novel antibiotics have been brought to market in the last 30 years. There are well-developed proposals for new market mechanisms using a “subscription” model that would solve this issue. This new charity will advocate for these better market mechanisms, to incentivise the creation and responsible deployment of important new antimicrobial drugs. We estimate that government spending on new market mechanisms for antibiotics could lead to a DALY averted for $100-$150. 2. Advocacy for restricting potentially harmful “dual-use” research Dual-use research of concern (DURC) is well-intended research which can lead to harm, either by accident or through deliberate misapplication of the technology. DURC could be a key risk factor for the occurrence of a particularly deadly pandemic during the next 50 years. This risk should be mitigatable with the development and adoption of good practice. For this podcast episode, we review some recent research studies that not only show the importance of frequent skin-to-skin care, but how it correlates with a more synchronous relationship between the infant and parents, promotes parental attachment, involvement, and breast milk production! You will hear many of the ways skin-to-skin care positively affects the infant and the parents. The practice of kangaroo care between an infant and their parent results in this beautiful, virtuous, positive cycle of unending benefits for everyone involved! With both the physical, mental, and neurobiological benefits, kangaroo care is one of the most effective non-pharmacological nursing interventions used in the NICU for the care of infants and their family. Skin-to-skin care has been shown to minimize stress in infants and mothers, promote parental engagement, positively influence the infant's microbiome, stabilize an infant's physiologic functioning, improve an infant's neuromaturation, decrease the risk of postpartum depression, and improve maternal-child attachment. Once parents begin to bond with their infant, they are more likely to remain involved which allows them to learn their infant's behavioral cues. Yusuf mencium semua saudaranya itu dengan mesra dan ia menangis sambil memeluk mereka. Sesudah itu barulah saudara-saudaranya bercakap-cakap dengan dia." (Kejadian 45:14-15) Renungan:   Jamie Ogg dan Emily Ogg adalah sepasang bayi kembar dari pasangan suami istri yang bernama David Ogg dan Kare Ogg. Jamie dilahirkan dalam kondisi fisik yang kurang normal, yaitu dengan berat badan yang hanya 900 gram, sedangkan Emily lahir dalam kondisi normal. Setelah tidak ada tanda-tanda kehidupan selama 20 menit, Jamie dinyatakan meninggal. Padahal dokter telah berjuang untuk menyelamatkan Jamie. Kemudian dokter menyelimuti Jamie yang sudah tidak bernyawa lalu memberikan kepada ibunya, Kate.  Hati Kate dan David sangat hancur melihat salah satu anak kembarnya meninggal. Dengan hati hancur, Kate memeluk Jamie di dadanya selama 2 jam, sambil menceritakan tentang keluarga dan apa yang akan dilakukannya jika Jamie masih ada. "Aku menaruh dia di dadaku dengan kepalanya di lenganku dan terus memeluknya sambil berbicara. Dia tidak bergerak sama sekali," kata Kate dalam sebuah wawancara.      Pelukan seorang ibu itu ternyata membawa sebuah keajaiban. Tiba-tiba hal yang mengejutkan terjadi, Jamie bergerak seperti terkaget dan nafasnya mulai teratur. Setelah itu, Jamie mengulurkan tangannya dan meraih jari ibunya, serta menggerakkan kepalanya. Melihat keajaiban tersebut, dokter yang bertugas saat itu pun terkejut dan mengatakan bahwa ini adalah keajaiban. Dokter menduga bahwa tubuh hangat Kate bertindak seperti inkubator yang merangsang tubuh bayi agar tetap hangat. Metode seperti ini disebut Kangaroo Care, yaitu metode perawatan bayi prematur dengan cara memberikan sentuhan kulit ke kulit, antara si ibu dengan bayi. Pelukan sang ibu dengan metode Kangaroo Care lebih menguntungkan dibandingkan dengan perawatan dalam inkubator. Yang menarik dari metode ini adalah, bayi tidak harus masuk dalam pemanas buatan, namun dengan metode ini bayi akan mendapatkan pemanas alami. Kisah Kate dan Jamie di atas mengajarkan kepada kita, bahwa pelukan itu memiliki kekuatan karena besar manfaatnya. Terlebih lagi bila pelukan itu diberikan kepada keluarga kita, seperti suami istri, anak dan saudara.  Saat Yusuf mengenalkan dirinya kepada saudara-saudaranya, mereka tidak langsung dapat menerima Yusuf, bahkan mereka takut dan gemetar terhadap Yusuf. Namun, keadaan berbalik saat Yusuf mulai memeluk Benyamin dan saudara-saudaranya yang lain. Hubungan persaudaraan yang retak pun menjadi pulih.    Kisah Jamie dan Yusuf membuktikan bahwa pelukan memiliki kekuatan yang luar biasa, hanya saja kita seringkali menyepelekan tindakan nyata itu. Saat ini mungkin ada seseorang yang membutuhkan pelukan dan kasih dari kita, mereka membutuhkan kekuatan dari pelukan tersebut. Untuk itu, nyatakanlah kasih kita dengan sebuah pelukan dan lakukanlah hal ini sekarang juga terhadap keluarga kita yang mungkin saja sedang duduk di samping kita. Tuhan Yesus memberkati.   Doa: Tuhan Yesus, ajarilah aku untuk dapat menyediakan waktu, sehingga aku dapat memberikan pelukan penuh kasih kepada keluargaku. Amin. (Dod).

BIO: Sarah Coutts - Independent Consultant, Kangaroo Care ProjectSarah is a lactation consultant, neonatal nurse and the mother of 4 kids. She has a passion for Kangaroo Care (continuous skin-to-skin contact) with preterm infants and their parents while in the Neonatal Intensive Care Unit. She believes Kangaroo Care can lead to improved outcomes and better experiences for parents of preterm infants. Kangaroo Care helps maintain parents as the primary caregivers and ensures they can be with their baby during their stay in the NICU – promoting zero separation is essential.Her most recent position at Perinatal Services BC in the Kangaroo Care research and implementation project team has been a highlight for her and hopefully leads to supporting BC NICUs to fully embrace Kangaroo Care as a model of care.Sarah is hoping to continue to promote and research Kangaroo Care locally and internationally and reduce separation between mothers and their infants after birth and while in the hospital.Alix Woldring - Project Coordinator, Clinical Systems and Quality Improvement, Perinatal Services BCI currently coordinate the Kangaroo Care program at Perinatal Services BC. In 2016, I completed my Masters in International Development from the University of Sydney in Australia and found myself working in the agriculture and natural resources sector on projects related to sustainability and climate change adaptation in the Asia Pacific region. My background in sociology meant that while surrounded by environmental scientists, agriculture specialists and engineers, I was always curious about the social, cultural and political conditions that contributed to the disparities that were being addressed by these projects.Once I started at Perinatal Services BC, the Kangaroo Care program immediately caught my attention. There was a simple, low cost, low tech intervention, started in Colombia, that was hugely backed by evidence for both mother and infant. More than that, the Kangaroo Care project was about not only about the context of care in the NICU, it’s also about the context of people's lives. Involving families in the care of the infant as part of the care team involves a fundamental shift in the role of families, how they are seen by healthcare providers and how families see their own role. Episode 9 features Yamile Jackson, expert parent, Founder and CEO of Nurtured by Design, and award-winning inventor/designer of The Zaky and the Kangaroo Zak. During the episode, Yamile provides a comprehensive overview of Kangaroo Care and ergonomics as related to premature infants and the NICU environment. Yamile also shares her son Zachary’s story of prematurity and survival including a near fatal encounter with a natural disaster during his NICU stay, and how their experience led her to found Nurtured by Design—the first in its class, global leader in neonatal ergonomics. She discusses: The premature birth of her son Zachary at 28 weeks, How Zachary had to be kept alive by hand for 9 hours after Tropical Storm Allison shut down their hospital, How hers and her family’s experience with prematurity led her to invent The Zaky and the Kangaroo Zak, The clinical evidence showing a 50% reduction in apnea and bradycardia events with the use of The Zaky, no events with the use of a maternally-scented Zaky, and 30 times more effective self-regulation with the use of a maternally-scented Zaky, The importance of developmental care and family-centered care in the NICU.
