    Marijuana Use During Pregnancy and While Breastfeeding: Is It Really A Big Deal?

    To start 2024 out, our first episode focuses on marijuana use, its impact on pregnancy, the growing fetus and the child's future development, as well as the implications of its use during breastfeeding. Who would have thought that in 2024, we would be living in a time where marijuana use is so prominent and that it would be legal in 24 states as of January of 2024?!?But, nonetheless, here we are! Because marijuana has been legalized in so many states, there are now false evolving perceptions on its general safety as well as its implications on the developing fetus and infant who are exposed to it either in utero or through breastmilk. In general, the perception of harm from marijuana is decreasing while the potency of marijuana is scarily increasing. Marijuana is the most widely used illegal drug during pregnancy in the United States. And, its use in gaining speed amongst all adult age groups, in both sexes, and in pregnant women. For our 56th episode, I reviewed the most up-to-date literature and will share the findings from several studies that reveal both short and long-term negative effects on the pregnant mother and developing fetus if used during pregnancy, and its future implications on the growing infant, child, and their transition into the teenage years, and as an adult after prenatal exposure.     The BIG and Positive Impacts of Skin-to-skin Care in the NICU

    November is Prematurity Awareness Month! During the month of November, we reflect on all of the the premature infants born each and everyday, the clinicians who care for them, and the parents. Despite all of the latest technology and advances in healthcare, one in ten children are born prematurely in the United States. Additionally, infants born prematurely are more likely to have long-term academic difficulties, cognitive impairments, learning disabilities, and/or emotional and behavioral issues that will not only impact their life, but they also have a significant toll on the entire family. A practice that has repeatedly shown to improve both neonatal and parental outcomes depsite a preterm delivery, is early and consistent skin-to-skin care. The small action of assisting parents with immediate skin-to-skin care has a very BIG IMPACT! Obviously, every clinical situation is different, but when possible, infants should be skin-to-skin with their parents. The benefits of kangaroo care are endless and not just for the baby! Kangaroo care has shown benefits for homeostasis for preterm infants including maintaining a healthy body temperature and increased cardiorespiratory stabilization. The most substantial benefit of skin-to-skin care has been the increased volume of breast milk production by the mother resulting in a longer duration of breastfeeding. Skin-to-skin care promotes participation of both parents in caring for the infant, it strengthens the family role in caring for a fragile infant, and decreases the common feelings of helplessness which helps parents become more comfortable with their infant and more confident!     Finding the Beauty in Life After the Loss of a Child

    To close out Pregnancy and Infant Loss Awareness Month, I sat down with Tasha Ruholl. Tasha was kind enough to openly share the losses and subsequent grief that she and her husband have endured. She speaks about how it was her grief that drove her to make a difference and help so many other families. Tasha wanted to give the gift of time to any other parents who have to endure the unimaginable path of losing a child by donating cuddle cots to 10 local hospitals. Additionally, she continues to honor their son, Daniel and their other 2 losses by working on the White Lily Project. Along with 3 other mothers who have endured loss, they are raising money to fund an Angel of Hope Statue. The Statue of Hope and healing garden will offer parents/family members who have experienced a pregnancy loss or the loss of an infant/child, a place to honor their loved one and find a sense of community with others who have experienced a similar tragedy.     Supportive Organizations and Resources for Bereaved and NICU Families

    There are some amazing organizations that help to support and provide valuable resources for bereaved and NICU families. Many parents who have had a child in the NICU as well as parents who have endured a pregnancy loss or the loss of an infant have felt led to give back to other families enduring the same experience. Using their personal experience, they have created a platform or organization to pay it forward so they can help other families. After our son William was home from the NICU, I felt driven to help other NICU families and I have been blessed to be able to do that in a variety of ways. For this podcast episode, I spotlight some incredible organizations that provide support and/or resources for families enduring a NICU journey, have a child with complex medical needs, disabilities, or those who will benefit from bereavement support. This is not to say you will want to connect with and utilize all of the resources mentioned today, but find the resources that are in sync with the type of information and support that you are looking for.     Embracing Parental Advocacy and Empowerment in the NICU

    For our 52nd podcast episode, I was joined by Dr. Jess Daigle. She is a board-certified pediatrician and neonatal/pediatric hospitalist. Additionally, she is the proud mother of two premature infants, Liam and Avery. Her son Liam, was born at 31 weeks' gestation and spent 5 weeks in the NICU. After her family's personal experience in the NICU, she founded and is the CEO of Mom & Me, LLC which focuses on helping both mothers and infants thrive during the postpartum journey and beyond. Dr. Jess carries out her mission by offering local, in-home/virtual care services to newborns, with a particular focus on NICU babies. With the goal to support new mothers and ease their transition home, she also offers life coaching for NICU moms. Dr. Jess helps them navigate through their NICU journey so they can become the empowered, confident mom they deserve to be. Jess and I are both very passionate about empowering NICU parents to be present and engaged while their baby is in the NICU. She speaks about how her personal NICU experience changed her professionally and addresses why it is so beneficial for parents to advocate for their infants.     Evidence-Based Recommendations for the Diagnosis, Management, and Treatment of Gastroesophageal Reflux Disease in NICU Infants

    For our 51st podcast episode, I discussed a contensious topic in the neonatal world, Gastroesophageal Reflux (GER) and Gastroesophageal Reflux Disease (GERD). Although it is a common occurrence amongst all infants, the lack of updated, clear, and consistent diagnostic and management recommendations remains, especially for the NICU population. Due to my personal and professional experiences, it is difficult for me to have one solid stance on the topic. As a former NICU nurse, I saw so many infants experience reflux. As a NICU parent, my son William, struggled with reflux once we brought him home and yes, at that time, it was managed with medication. And finally, now as a Neonatal Nurse Practitioner, I am more knowledgeable about what the evidence does and does not show regarding reflux and I understand its complexity.     Necrotizing Enterocolitis (NEC): What causes it and how is it managed in the NICU?

    For the 49th episode on the Empowering NICU Parents' Podcast, we review necrotizing enterocolitis (NEC) which is a life-threatening newborn condition that involves ischemia and inflammation of the bowel. Necrotizing Enterocolitis is a complex disease that despite decades of research, is still not well understood. The evidence continues to support the concurrent presence of several factors that when combined with a trigger, may lead to a disruption of the normal intestinal bacterial flora followed by an altered inflammatory host response. On this episode, you will learn what factors place infants, especially premature infants, at an increased risk to develop NEC. The common clinical symptoms that infants present with once they develop NEC will be reviewed as well as when they are most likely to occur. Additionally, we touch on some of the subtle behavioral changes infants may display at the onset of the disease that are often most obvious to the parents. Next, the diagnostic tests and labs that are typically done once NEC is suspected will be reviewed followed by a description of the Bell staging system which is used to provide a more uniform clinical definition for NEC. The typical management for NEC will also be discussed, followed by a review of some of the common preventative measures that have been put into place in NICUs today to continue to decrease the incidence of NEC. We close out the episode discussing what the prognosis and mortality rate is for infants who have been diagnosed with NEC.     The Composition of Human Breast Milk: How Does Preterm, Term, and Donor Milk Vary?

    Maternal human milk is the absolute best food for newborn nutrition that supports all of their accelerated growth in the first year. And although we have a basic understanding of the composition of human milk, every woman's breast milk has a different complexity when it comes to the nutritional content. But, you may be wondering, what exactly is breast milk composed of? For our 48th podcast episode, I review the general composition of human breast milk and what the variations are between preterm, term, and donor breast milk. I also discuss some of the anti-infective protective properties in maternal breast milk and how a preterm mother's milk has unique and complex properties that are lacking in donor breast milk. I discuss human milk fortification, the different formulations available, and why it is so important in our preterm population. I share why donor breast milk is used in preterm infants over formula and explain its protective mechanisms against necrotizing enterocolitis (NEC). Finally, I discuss how the pasteurization process, although important, impacts the composition of donor breast milk. I share why it is so crucial that mothers feel supported and are educated.     Breaking Down the Milk Expression and Breastfeeding Barriers Common to NICU Mothers

    For our 47th podcast episode, I review some of the general benefits of maternal breast milk and why it is so important for all infants, but especially those who are critically ill and in the NICU, to receive maternal milk. Human milk, often referred to as liquid gold, is considered the gold standard feeding plan for all infants. Maternal human milk provides many nutritional advantages for infants, but it is also advantageous in non nutritional aspects as well. Unfortunately, NICU mothers face additional barriers that make it more difficult for them to effectively express their milk and ultimately breastfeed. Data has shown that for all NICU infants, whether preterm and/or critically ill, receive less than 20% of their own mother's milk at 6 months of age, most commonly due to insufficient maternal milk production. Mothers whose infants are admitted to the NICU are faced with both physical, emotional, mental, and social inhibiting circumstances that make it much more difficult for them to not only establish their milk supply, but also maintain it. There are also significant racial disparities that exist and human milk provision is lowest among non-Hispanic Black and American Indian/Alaska Native populations. It is the standard of care for most NICUs to utilize Donor Human Milk (DHM) as an alternative or to help bridge the gap for infants when the mother's breast milk is either not available or there is insufficient volume. Although donor breast milk is a great option that is beneficial, it does not have the same overall protective mechanisms and bioactivity as the mother's milk. In the episode, we review what some of the common barriers are for NICU mothers in their physical ability and desire to provide breast milk for their infant. I share recommendations on what we as NICU clinicians can do to educate, support, and guide all mothers in a culturally sensitive and supportive manner. By recognizing the common and individualized barriers for NICU mothers, we can share our knowledge in an empathetic and holistic manner.     A Review of the Infant-Driven Feeding Program in the NICU

    Infants in the NICU, especially those born prematurely, encounter the challenge of effective bottle feeding and/or nursing due to their maturational development. Recently, to support their development, there has been a shift in the paradigm to move away from volume-driven feedings in the NICU to a cue-based approach. The practice of cue-based feedings, specifically the The Infant-Driven Feeding™ Program (IDF™), begins with a neurodevelopmental foundation and promotes positive feeding experiences from the first day of life. The IDF™ program is a developmentally supportive, individualized, cue-based feeding program offered by Dr. Brown's Medical. For our most recent podcast, I sat down with a former colleague of mine, Lisa Kleinz. Lisa is a Speech Pathologist and a Developmental Care Specialist who is currently the Director of Education for Dr. Brown's Medical. She is an expert on the The Infant-Driven Feeding™ Program and explains how the program supports the infant's development, promotes consistent feeding practices, encourages parental involvement, and has resulted in improved breastfeeding rates. The evidence-based research supports the IDF™ program and has shown a reduction in time to full oral feedings, a reduction in the length of hospital stay, reduced hospital costs, improved breastfeeding rates, and increased parental involvement including kangaroo care. The IDF™ program is beneficial for clinicians, parents and the infant. Once staff is educated, more consistent feeding methods are practiced which results in a positive feeding experience for the infant and caregiver.     A Review of Cue-Based Feeding in the NICU

    For infants in the NICU, especially those born prematurely, breast or bottle feeding effectively can oftentimes be a challenge. Maturational and developmental issues impact a premature infant's ability to adequately coordinate their sucking, swallowing, and breathing. Yet, according to the American Academy of Pediatrics (AAP), attainment of adequate oral feedings that fully support growth without physiologic instability is a skill that needs to be fully established prior to an infant's discharge from the NICU. But, what is the best way to achieve full oral feedings in a developmentally supportive manner? For this podcast, we review cue-based feeding programs and why there has been a recent shift in the paradigm to move away from volume-driven feedings in the NICU. We discuss what places NICU infants, especially those born prematurely, at an increased risk for oral aversion and how we as caregivers and parents can minimize the risk. We discuss the benefits of cue-based feedings including how it promotes a more positive feeding experience for the infant and caregiver, its ability to support an infant's neurodevelopment, as well as findings from some recent research studies that have focused on cue-based feeding.     For our most recent episode, I continued to summarize the updated recommendations for care of the preterm and low birthweight infant from the World Health Organization (WHO) that were just released on November 15, 2022. The World Health Organization reviews the most up-to-date research findings and they develop global guidelines that are meant to help end-users or clinicians make informed decisions on whether, when, and how to undertake specific actions such as clinical interventions, diagnostic tests, or public health measures with the aim to achieve the best possible individual or collective health outcomes. Their goal is that everyone, everywhere can attain the highest level of health. The updated recommendations are relevant to national policy makers, NICU supervisors, managers, Neonatologist, NNPs, and NICU nurses. I think it's extremely important for anyone who works with the preterm and low birthweight population to be aware of the updated recommendations by the World Health Organization. I also believe that it is important that parents of preterm or low birthweight infants have some idea of what the recommendations entail and how it may impact the care given to their infant.     Do you often wonder what helps to guide care and policies in healthcare? Guidelines for medical care are driven by research and what the evidence has shown, otherwise known as evidence-based practice. And there are organizations, like the World Health Organization, who review the most up-to-date research findings and develop global guidelines that are meant to help end-users or clinicians make informed decisions on whether, when, and how to undertake specific actions such as clinical interventions, diagnostic tests, or public health measures with the aim to achieve the best possible individual or collective health outcomes. Their goal is that everyone, everywhere can attain the highest level of health. On November 15th, 2022 the World Health Organization (WHO) released updated recommendations for care of the preterm and low birthweight infant. The recommendations were derived from a group of diverse experts and stakeholders. After they examined the most up-to-date evidence from low- to -high income countries, they developed the updated guidance about the care of preterm or low birthweight infants. The recommendations are directed at a large target audience and are beneficial for national policy makers, supervisors, managers, NICU clinicians plus more! Not only is the information pertinent to all of the aforementioned, but it is also important for NICU Parents to know what the general recommendations are for care of the preterm or low birthweight infant. So I wanted to provide you with the cliff notes version spotlighting the most pertinent points out of the 137 page document.  The recommendations include guidelines for preventive and promotive care, care of complications, family involvement and support and a good practice statement.     A Review of the WHO Recommendations for Care of the Preterm or Low-Birth-Weight Infant

    On November 15, 2022, the World Health Organization (WHO) released some updated recommendations for care of the preterm or low birthweight infant. New up-to-date evidence has emerged regarding the most effective clinical interventions in caring for preterm or low birthweight infants so a group of diverse experts and stakeholders from six different regions were selected and began working on these guidelines back in 2020. The guidelines developed by the WHO are meant to help end-users or clinicians make informed decisions on whether, when, and how to undertake specific actions such as clinical interventions, diagnostic tests, or public health measures with the aim to achieve the best possible individual or collective health outcomes. All of the recommendations are evidence-based and they have been reviewed by several committees through several stages to ensure the validity of each one. Some of the recommendations are brand new, like the “hot topic” recommendation of immediate kangaroo care, whereas other recommendations are updated. As I was reviewing the updated recommendations, I realized that it would be beneficial for all NICU clinicians and parents to learn more about them. Since the entire document is around 137 pages, we summarize and provide you the cliff notes version on this podcast episode.     Keeping an Eye on Your Angel in the NICU

    As a NICU parent, one of the most difficult pieces of the journey is the physical separation that parents have to endure initially and each day that follows. Every time parents walk out of those NICU doors to go home without their baby is devastating. Parents are left sitting at home wondering if their baby is awake, or are they restless, did they gain weight, did they take their full bottle, or were they able to keep their ng tube out?     A Review of Venous and Arterial Lines Commonly Used in the NICU

    There are many terms and acronyms commonly used in neonatology that most people are not familiar with. Even medical clinicians who are not familiar with the NICU, are unlikely to know about UACs or UVCs. And most NICU parents probably did not know that you can access a newborn's venous and/or arterial system through their umbilicus until the providers explained the procedure to them.  For our 40th podcast episode, we reviewed some of those common NICU acronyms, but specifically the different venous and arterial line options commonly used in the NICU. These terms may be casually discussed during conversations between NICU clinicians, but it is important that as a parent, you understand the different peripheral and central lines infants in the NICU commonly need. Not only should you understand what they are, but you should also know when they are used or why one option is chosen over another.     RSV in Infants and Children: Symptoms, Prevention, and Synagis Criteria

    It is likely that you've heard about a child who has had RSV, or Respiratory Syncytial Virus from a friend, a family member, or perhaps you've even had your own personal experience with it. But, you may not realize how common it actually is! Did you know that almost all children get RSV at least once before they are two years old. For healthy children, it is likely to present just like a typical cold or virus, but for some children, they become very ill with RSV. Due to certain infants being more at risk of developing the severe symptoms of RSV, they may receive a series of Synagis injections during the peak RSV season. Infants born prematurely, infants diagnosed with bronchopulmonary dysplasia (BPD), or children with hemodynamically significant congenital heart disease are considered high-risk patients who may be eligible to receive Synagis.October is RSV Awareness Month, which is appropriate because the typical RSV season is from November to April. Since we are approaching the peak RSV season, we are going to review RSV, how it gets transmitted, what the typical symptoms are, and when parents should contact their provider. We will also take a closer look at Synagis, what exactly it is, the current inclusion criteria, how often it should be given, the length of treatment, and signs and symptoms parents should watch for that may be indicative of an allergic reaction.  Since RSV is so common and can cause infants and children to require hospitalization, it is important for parents to know preventive measures that will minimize the risk as well as how to identify concerning symptoms.     The Wolff Family's Post NICU Journey Through Home Trach Care, Decannulation, and Unexpected Surprises Too

    For this podcast, I continued my conversation with Jessica Wolff. We discussed what their life was like once they brought Lily home. She explains how they adjusted to caring for Lily with her tracheostomy along with the necessary life-saving equipment and how all of that completely changed once COVID hit. We discussed how the plan for Lily's decannulation went completely sideways, how they coped with it, and how they learned that Lily actually needed a full laryngotracheal reconstructive surgery. Jessica proudly shares how Lily is doing today, medically, developmentally and emotionally and how they are navigating everything as a family, especially in preparation for sweet Lily to head to Preschool! We conclude the episode speaking about Nolan, Jessica and Pat's son who they welcomed earlier this year who also spent some time in the NICU. Jessica shares some great information and advice pertinent to other families who are either currently in the NICU and weighing the heavy decision of a tracheostomy for their child, or for the families who have a child at home with a tracheostomy, as well as families who have suffered or may suffer a loss. Listeners will be surprised to hear some of the unexpected emotions that Lily and the Wolff family has had to work through once Lily had her trach removed.     The Wolff Family's Journey Through Loss, A Prolonged NICU Stay and What They Have Learned Along the Way

    For our most recent podcast episode, I sat down with Jessica Wolff, a mother who has endured loss and 2 very different NICU journeys. For part one of this podcast, she shares personal details on her very complicated twin pregnancy and the difficult choices they had to make along the way. She reflects back on regrets both she and her husband, Pat have experienced as well as what helped them cope through their daughter, Lily's 256-day NICU stay. Jessica explains what helped them make the decision whether or not Lily should have a tracheostomy placed and she speaks candidly about what their expectations were after Lily's tracheostomy placement, versus their reality. She offers frank advice for NICU families, especially those with similar situations who have children with feeding issues, ostomies, bronchopulmonary dysplasia or BPD, pain medication withdrawal, tracheostomies, and what going home looked like for their family with a ventilator-dependent child. You will be amazed by what this family has been through, but even more so by their strength and resiliency.     Why are Term and Preterm Infants at an Increased Risk for Skin Injury?

    Our skin is an incredibly important, multifunctional organ. It has many different functions including helping us maintain our body temperature, it assists with the excretion of water and toxins, and most importantly, protective mechanism of acting as a barrier. But in newborns, especially preterm infants, there are developmental differences in the anatomy and physiology of their skin that place them at an increased risk for skin injury. Not only is the immaturity of an infant's skin problematic, but infants in the NICU are predisposed to skin trauma due to the life-saving monitors and equipment necessary for their care. Furthermore, infants with compromised skin integrity are at an increased risk for infection, excess water loss, absorption of toxic substances, and additional sequelae. On this episode, we review the basic anatomy of our skin and all of its important functions. We discuss how the skin of a term and preterm infants differs from adults and why the structural variations place infants at an increased risk for skin trauma and additional complications. The episode will be beneficial for parents of infants, especially preterm infants or those in the NICU to understand why their infant is at an increased risk for compromised skin integrity.     Elevated Bilirubin Levels in Infants: Screening and Management

    For our 33rd podcast episode, I review the recommendations for screening and managing hyperbilirubinemia. The American Academy of Pediatrics recommends universal screening of bilirubin levels in newborns, so I review how that can be done, when it should occur, and when it should be repeated. We discuss which infants are more at risk for elevated bilirubin levels and how those risk factors contribute to guidelines for the treatment plan. I discuss the nomograms commonly used and recommended by the American Academy of Pediatrics that guide the care of identifying infants at risk as well as when they should be treated. I review phototherapy, the primary treatment plan for hyperbilirubinemia, including how it effectively lowers bilirubin levels as well as the clinical considerations that should be applied during treatment. Whether you are a neonatal clinician or a parent, this review on how to effectively screen for and manage hyperbilirubinemia will be beneficial. Hyperbilirubinemia is incredibly common in neonates. Clinicians who care for infants whether they are a term newborn in the nursery or an infant in the NICU, will manage infants with elevated bilirubin levels. And parents of either term, healthy infants, or those critically ill in the NICU, it is essential that you understand the risks of hyperbilirubinemia and the screening and management process for it.     Elevated Bilirubin Levels in Infants: Why Does it Occur?

    It is likely that you have heard the terms jaundiced and/or bilirubin. And although it is very common for infants to develop elevated bilirubin levels otherwise known as hyperbilirubinemia, do you actually know or understand why? As a NICU provider, I think it is essential that parents know the condition their infant is facing, but it is also important to understand the why behind its occurrence. If you do not understand the why, the treatment plan will also not make very much sense to you. I believe parents should be actively involved and partners in their baby's care. But, it is nearly impossible to be actively engaged in the decision-making process if you do not understand the why behind the condition.  For our 32nd podcast episode, I break down how our bodies process bilirubin, how we eliminate it, what causes the skin color to become jaundiced, why we as neonatal clinicians monitor bilirubin levels so closely, and I also review some of the common conditions that increase your infant's risk of developing hyperbilirubinemia. Some of the pathophysiology of hyperbilirubinemia can be confusing, but I review it in a way that will make sense to you so you can learn why elevated bilirubin levels occur, and even more importantly, understand your baby's treatment plan. The review will also be very beneficial for novice NICU clinicians or those that need a refresher on the pathophysiology of hyperbilirubinemia in term and preterm infants.     Trauma-Informed Care in the NICU with Mary Coughlin

    For this podcast episode, I was very honored to be joined by Ms. Mary Coughlin. Mary is a Neonatal Nurse Practitioner, but also a published author of several pieces of work. She is well-known in the neonatal world for her development of the core measures for developmentally supportive care, but most recently with her published work and creation of the trauma-informed professional certification program as well as additional programs, workshops and masterclasses through Caring Essentials Collaborative. I was so honored to be able to sit down and discuss Mary's work experience that led her to the transformational work she is doing today. Just listening to Mary is transformational! As NICU clinicians, Mary reminds us of the importance of our role and why it is so crucial for us to embrace our vulnerability, take our armor down and truly connect with each other on a human level. Trauma-informed care is not about doing more as a clinician, it is about being more of your loving, compassionate self so you can make a difference by fully showing up for your patient and their family. We also discussed the trauma that is so common for NICU parents throughout their NICU journey and in the years to follow. So many NICU parents feel alone and isolated in the process, yet it is common to so many. We discussed the word trauma and why it is often downplayed or not spoken openly about it NICUs like it should be. And most importantly, we share the importance of working through and moving through your trauma, which will be difficult but is essential for the well-being of the entire family unit.Trauma-informed care is a concept that has been applied to several areas of medicine and patient care, but its application to the NICU is still emerging. Mary learned about trauma-informed care and knew that she had to introduce it into the NICU world. Not only does trauma-informed care in the NICU directly affect the baby, but it also addresses the family as well as the clinicians who are caring for the infant and family. Mary explains why the personal journeys we all go through and each of our stories play into an intricate web that involves each and every one of us.     The Positive Effects of Kangaroo Care: A Literature Review

    For this podcast episode, we review some recent research studies that not only show the importance of frequent skin-to-skin care, but how it correlates with a more synchronous relationship between the infant and parents, promotes parental attachment, involvement, and breast milk production! You will hear many of the ways skin-to-skin care positively affects the infant and the parents. The practice of kangaroo care between an infant and their parent results in this beautiful, virtuous, positive cycle of unending benefits for everyone involved! With both the physical, mental, and neurobiological benefits, kangaroo care is one of the most effective non-pharmacological nursing interventions used in the NICU for the care of infants and their family. Skin-to-skin care has been shown to minimize stress in infants and mothers, promote parental engagement, positively influence the infant's microbiome, stabilize an infant's physiologic functioning, improve an infant's neuromaturation, decrease the risk of postpartum depression, and improve maternal-child attachment. Once parents begin to bond with their infant, they are more likely to remain involved which allows them to learn their infant's behavioral cues.     Life Beyond HIE: A Mother's Journey of Growth and Giving Back

    For our 29th Podcast episode and to close out HIE Awareness month, I sat down with Brigid Janousek. She bravely shares the story of her son William's traumatic delivery and his subsequent treatment with therapeutic hypothermia due to his diagnosis of Hypoxic Ischemic Encephalopathy or HIE. Infants with HIE have experienced a lack of oxygen to the brain either prior to birth, during the birth process, after birth, or during childhood. Infants with suspected HIE are treated in the NICU with therapeutic hypothermia or cooling that needs to be initiated within the first 6 hours of life. The cooling therapy reduces the rate of brain cell death and the risk of reperfusion injury which commonly occurs due to the release of toxins from the injured brain cells. The cooling process occurs for 72 hours post-delivery and has been shown to minimize the extent of death and disability in children. Brigid shares a more in depth look into William's time in the NICU, and how they first learned about his diagnosis. She explains why they felt like they were outcasts in the NICU due to William being a term infant. Brigid shares the struggles they endured once they were discharged home and how she unknowingly was suffering from postpartum depression, perinatal mood disorder, and PTSD. We discuss ways it was helpful for her to work through her trauma and why she is so passionate about bringing awareness to the misconception that all NICU babies are preterm as well as the importance of focusing on maternal mental health. We thank Brigid for sharing her family's very personal story.     Parental Stress and Trauma in the NICU

    If you are a NICU Parent who either currently has a baby in the NICU, or a NICU graduate, this episode is for you! I recently presented at a NICU Leadership conference and spoke on the parental stressors common to NICU Parents and the trauma and PTSD often associated with it. I decided to share some portions of my speech with you due to its' relevance to all NICU families. I want all NICU Parents to know that you are not alone and your feelings are common. By minimizing your experience or underplaying the trauma associated with it, it will prevent you from not only addressing it, but also being able to fully bond with and nurture your baby. Any parent who has a baby in the NICU will experience stress and trauma regardless of the baby's diagnosis or the the length of time they spend in the NICU. Parents may be stressed and anxious about the size of their infant, all of the life-saving equipment surrounding them, due to the forced, unnatural separation from their baby, the guilt they experience, or due to all of the feelings of powerlessness and heartbreak they endure watching their fragile infant suffer. And sadly, it is very common for parents to not feel like parents once their baby is admitted to the NICU, which can negatively impact a parent's ability to bond with and nurture their baby. If parental distress is not addressed and their involvement is not encouraged, it can create a negative compounding cascade of events. Tune in to hear how many of the feelings common to NICU parents can impact their mental health and ultimately affect their baby and their outcomes if not addressed. Parental engagement in the NICU has a plethora of benefits and not just for your baby, but for you as parents as well! Although as a NICU parent, you may feel completely powerless, you need to remember that you play a major role in the success of your baby by being present and simply loving on them! In the NICU, we cannot have positive outcomes for our infants without empowering and involving the parents!     The Benefits of Reading to Your Baby in the NICU

    Did you know that not only is it okay to read to your NICU baby, but that it is strongly recommended? In our 25th Podcast episode, I review all of the benefits of reading to your baby while they are in the NICU as well as tips for success! In the first few months of an infant's life, their language and speech areas grow exponentially in their brain. But, for our babies in the NICU, many of them not only miss out on months of intrauterine language exposure that promotes normal development, but they also spend those formative months amongst the stressful NICU environment. So how can we expose our NICU infants to oral language that positively promotes neurodevelopmental outcomes? We need to read to them out loud on a daily basis! NICU infants who have been read to out loud to on a daily basis have shown enhanced language and literacy outcomes at 2 years of age. Plus, for us mothers, our babies know our voice the very best, and research has shown that babies become more alert and exhibit a positive response when they hear their mother talking, singing, or reading out loud.     For NICU Parents, the day they are able to bring their baby home is unforgettable. Many parents have spent weeks and months traveling to the NICU to see their baby and have endured a large amount of stress, worry, and concern each day as they watched their baby fight against so many obstacles. So when parents hear that they are able to take their baby home, they are so thrilled that many underestimate the challenges they will face at home. For many NICU babies, their complex medical journey does not end once they exit the NICU doors. Sadly, not only do their medical needs continue, but it may actually feel more overwhelming for the parents at home due to new equipment and less resources being available. Despite all of the coaching and empowerment provided by the NICU, the time at home with a NICU graduate may be more challenging than parents anticipated. And unfortunately, there really is not a proper way to fully prepare NICU parents for the days, months, and sometimes years that follow their discharge, especially for children with complex medical needs. For this podcast episode, I sat down with a former NICU Mama, Alissa MacDonald who shares her family's trials and triumphs as they have navigated through their post NICU days with their son, Tegan, a former 24-weeker. She shares what she has learned along the way and she wants to emphasize to parents that it's okay to not be okay. Despite both Alissa and her husband Matt being medically trained and prepared for a foster child with medical needs, they were not aware of the monumental impact Tegan would have on their lives as they previously knew it. Alissa speaks frankly on the challenges they have faced and how important mental health, support from family and friends, and making their marriage a priority has been. Alissa reflects on how even as a NICU nurse, their days at home with Tegan were very different from what she had envisioned. She now admits that they were not at all prepared for the mental and physical toll it would have on her and their entire family. We discuss how it is not unusual for former NICU parents to feel overwhelmed, unprepared, angry, and alone in their post-NICU journey.     Questions Parents Should Ask Their Baby's NICU Care Team Each Day

    Our NICU Journal called Our NICU Roadmap has officially been published! My vision for this journal stemmed from the lack of a comprehensive NICU journal available when our son was in the NICU. I wanted to develop a journal that not only was a great resource for parents, but also served as a place for you as parents to document your baby's progress each day while in the NICU. As a Neonatal Nurse Practitioner, I had the medical knowledge and experience on my side during our son's NICU journey, so I knew the questions to ask our son's NICU care team, but I felt there was not an appropriate place to document it all. Our NICU Roadmap equips and guides NICU parents with resources, information and questions to ask so you can stay actively involved in your baby's daily care. In today's podcast we spotlight my favorite part of the journal, the daily log. As any NICU parent knows, there are new changes that occur daily in the NICU.     NICU Trauma - How Well Are You Coping?

    Are you suffering from trauma after your baby or babies spent time in the NICU? Have you brushed off symptoms of trauma or PTSD? How could you suffer from PTSD when your baby only spent a few days in the NICU? Trauma is the result from an upsetting experience. That is it. It results from any experience that is disturbing and overwhelms your ability to cope. There is not a definitive scope that defines trauma. I used to minimize my trauma after my stressful pregnancy, our son's emergent delivery, and his time in the NICU because despite being severely premature, we were able to bring him home and he was healthy. How could that be traumatic when so many other NICU parents were unable to bring their babies home or their children had such complex medical needs. Why was I struggling on certain days at home or professionally when I had such an amazing blessing and miracle right in front of me? But, no one can define my trauma, just as I cannot define yours. If your experience was less than ideal, that can affect you for years to come if it is not dealt with appropriately. Jessica, a mother of 33 weeks and I discuss how although her boys did not have any major setbacks while they were in the NICU, the experience was difficult for her. The anticipated delivery of her boys, the immediate time thereafter, and not bringing them home right away has continued to affect her. After listening, you will realize that trauma may look different for everyone. The ability to identify and cope through NICU trauma is an evolving and personal process.     Advice for Current NICU Parents From Mothers of Multiples

    If you are a NICU parent and in the midst of your baby's NICU journey, you must listen to this episode. This episode features great advice, tips, suggestions, and encouragement from former NICU Moms that you must hear! Although the tips are mostly from mothers of multiples, any NICU parent will benefit from what they have to share! Whether you are a seasoned NICU parent or brand new in your journey, you will want to listen to what those that have gone before you have to share! I believe we can all learn a great deal from those who have previously endured a similar experience prior to us.
