Today, our guest is Shella Keilholz. Dr. Keilholz received her Bachelor's in Physics from Missouri University of Science and Technology in 1997, and her PhD in Engineering Physics from the University of Virginia in 2004. She went on to a do a post-doc at NIH in Dr. Alan Koretsky's lab and in 2004 joined the department of Biomedical Engineering at Emory University as a faculty member. She is now a full professor at Emory University and also closely affiliated with Georgia Tech. Since around 2008 she has been uncovering new spatial and temporal patterns in resting state fMRI. She was the first to identify spatial propagation of wave-like behavior in the resting state time series and in 2011 coined the term quasi-periodic patterns (QPP) to describe whole brain networks that dominate much of the resting state signal. Recently, she and others have described three such patterns that account for most of the variance. Here we not only talk about her career but also delve into how these quasi-periodic patterns mesh with the current landscape of diverse features of resting state fMRI that continue to be found. She has shown that these patterns may have a central driver and may relate to such things as vasomotion, global signal, arousal state, and attention. In general, there is much more information from basic neurophysiology to cognition that remains to be derived from resting state fMRI, and Shella is among those leading the effort. Abstract: Infraslow ( less than 0.1Hz) electric activity, involved in attention and arousal fluctuations, serve as the unique neurophysiological basis for infraslow BOLD fluctuations. Particularly, the dynamics of infraslow electrical activity in both resting and task-performing human brains exhibit a quasi-periodic pattern (QPP) of consistent, phase-locked spatiotemporal BOLD signal changes. However, it is unclear how environmental stimuli affect the infraslow dynamic QPP or how the ongoing infraslow dynamics might modify the response to the environment in awake humans. To investigate this, we presented checkerboard sequences flashing at 8Hz to subjects. This is a salient visual stimulus that is known to produce a strong response in visual processing regions. Three central questions were investigated: 1) how does visual stimulation affect the QPPs compared to rest, 2) how does variability in visual stimulus onset affect the QPPs, and 3) how does the BOLD response to visual stimuli depend on the phases of the QPPs. Two different visual stimulation sequences were employed, a systematic stimulation sequence in which the visual stimulus appeared every 20.3 secs and a random stimulation sequence in which the visual stimulus occurred randomly every 14~62.3 secs. Three central observations emerged. First, the systematic condition produced a different spatiotemporal pattern of infraslow dynamics compared to both the resting-state and the random stimulation conditions. Second, the QPP was more frequent in the systematic condition with significantly shorter intervals between consecutive QPPs compared to the random condition. Third, the BOLD signal response to the visual stimulus across both conditions was swamped by the QPP at the stimulus onset. Multiple Regions of Nonthermal Quasi-Periodic Pulsations during the Impulsive Phase of a Solar Flare by Yingjie Luo et al. on Thursday 13 October Flare-associated quasi-periodic pulsations (QPPs) in radio and X-ray wavelengths, particularly those related to nonthermal electrons, contain important information about the energy release and transport processes during flares. However, the paucity of spatially resolved observations of such QPPs with a fast time cadence has been an obstacle for us to further understand their physical nature. Here, we report observations of such a QPP event occurred during the impulsive phase of a C1.8-class eruptive solar flare using radio imaging spectroscopy data from the Karl G. Jansky Very Large Array (VLA) and complementary X-ray imaging and spectroscopy data. The radio QPPs, observed by the VLA in the 1--2 GHz with a sub-second cadence, are shown as three spatially distinct sources with different physical characteristics. Two radio sources are located near the conjugate footpoints of the erupting magnetic flux rope with opposite senses of polarization. One of the sources displays a QPP behavior with a ~5-s period. The third radio source, located at the top of the post-flare arcade, coincides with the location of an X-ray source and shares a similar period of ~25--45 s. We show that the two oppositely polarized radio sources are likely due to coherent electron cyclotron maser (ECM) emission. On the other hand, the looptop QPP source, observed in both radio and X-rays, is consistent with incoherent gyrosynchrotron and bremsstrahlung emission, respectively. Flare Quasi-Periodic Pulsation Associated with Recurrent Jets by Dong Li et al. on Thursday 22 September Quasi-periodic pulsations (QPPs), which carry time features and plasma characteristics of flare emissions, are frequently observed in light curves of solar/stellar flares. In this paper, we investigated non-stationary QPPs associated with recurrent jets during an M1.2 flare on 2022 July 14. A quasi-period of about 45$pm$10 s, determined by the wavelet transform technique, is simultaneously identified at wavelengths of soft/hard X-ray and microwave emissions, which are recorded by the Gravitational wave high-energy Electromagnetic Counterpart All-sky Monitor, Fermi, and the Nobeyama Radio Polarimeters, respectively. A group of recurrent jets with an intermittent cadence of about 45$pm$10 s are found in Atmospheric Imaging Assembly (AIA) image series at 304 {AA}, but they are 180-s earlier than the flare QPP. All observational facts suggest that the flare QPP could be excited by recurrent jets, and they should be associated with nonthermal electrons that are periodically accelerated by a repeated energy release process, like repetitive magnetic reconnection. Moreover, the same quasi-period is discovered at double footpoints connected by a hot flare loop in AIA 94 {AA}, and the phase speed is measured to 1420 km/s. Based on the differential emission measure, the average temperatures, number densities, and magnetic field strengths at the loop top and footpoint are estimated to 7.7/6.7 MK, 7.5/3.6*10^{10} cm ^{-3}, and 143/99 G, respectively. The CPP came into effect on January 1, 1966 and applied to all provinces and territories except Quebec, where the separate but similar QPP was established in the same year. The contribution rates for QPP are higher than those for CPP. Although the year's maximum pensionable earnings ($61,600 for 2021) and annual basic exemption ($3,500) for both plans are the same, an employee paying into the QPP will pay contributions at a higher rate (5.90% for 2021) compared to the rate for an employee who pays into the CPP (5.45% for 2021). In terms of value, CPP Investments holds a very diverse $550.4 billion global investment portfolio. Its most recent results showed it produced a 10-year annualized net return of 11.6%. Abstract: Alzheimer's disease (AD), a neurodegenerative disorder marked by accumulation of extracellular amyloid-beta plaques leads to progressive loss of memory and cognitive function. Resting state fMRI (RS-fMRI) studies have provided links between these two observations in terms of disruption of default mode and task positive resting state networks (RSNs). Important insights underlying these disruptions were recently obtained by investigating dynamic fluctuations in RS-fMRI signals in old TG2576 mice (mouse model of amyloidosis) using a set of quasi-periodic patterns (QPP). QPPs represent repeating spatiotemporal patterns of neural activity of predefined temporal length. In this article, we used an alternative methodology of co-activation patterns (CAPs) that represent instantaneous and transient brain configurations that are likely contributors to the emergence of commonly observed resting state networks (RSNs) and QPPs. We followed a recently published approach for obtaining CAPs that divided all time frames, instead of those corresponding to supra-threshold activations of a seed region as done traditionally, to extract CAPs from RS-fMRI recordings in 10 TG2576 female mice and 8 wild type littermates at 18 months of age. Subsequently, we matched the CAPs from the two groups using the Hungarian method and compared the temporal (duration, occurrence rate) and the spatial (lateralization of significantly activated voxels) properties of matched CAPs. We found robust differences in the spatial components of matched CAPs. Finally, we used supervised learning to train a classifier using either the temporal or the spatial component of CAPs to distinguish the transgenic mice from the WT. We found that while duration and occurrence rates of all CAPs performed the classification with significantly higher accuracy than the chance-level, blood oxygen level dependent (BOLD) signals of significantly activated voxels from individual CAPs turned out to be a significantly better predictive feature demonstrating a near perfect classification accuracy. Our results demonstrate resting-state co-activation patterns are a promising candidate for a diagnostic, and potentially, prognostic biomarker of Alzheimer's disease. This is ASCO's podcast series where we explore policy and practice issues that have an impact on oncologists, the entire cancer care delivery team, and most importantly, the individuals we care for-- people with cancer. My name is Clifford Hudis, and I'm the CEO of ASCO, as well as the host of the ASCO in Action podcast series. For today's podcast, I'm going to give our listeners a quick update on an important announcement from the Centers for Medicare and Medicaid Services. In an August podcast, I outlined the proposed Medicare Physician Fee Schedule and the Quality Payment Program Rule for 2019. This is commonly referred to as the Physician Fee Schedule. Today, I'm going to provide an update on where we are with this for next year. I have to say in passing, it's probably a good day for me not to have a guest, because I'm here with a terrible cold. So what is the 2019 Medicare Physician Fee Schedule? This is a fee schedule which consists of a complete listing of all of the fees that Medicare uses to pay doctors or other providers and suppliers. It's a comprehensive listing of the maximum fees. And it's updated each year and then used to provide reimbursement to physicians and other providers working on a fee-for-service basis. Now at ASCO, we, every year, review this rule very closely. And we try to determine and predict the impact that it will have on our members, and of course, on our patients. There are three provisions in particular that we want to highlight today. The first of these is related to care provided in calendar year 2019. And CMS estimates that there will be, overall, a 1% reimbursement cut for hematology and oncology, as well as radiation oncology specialties. It is important to note, however, that the actual impact on any individual physician or physician practice will depend on their mix of services-- that is, what it is they exactly provide and bill. Now the administration has publicly stated its aim to reduce the growing administrative burden that we've all been noting and complaining about for the last few years. And the second item we want to point out is there is some evidence of their sensitivity to this issue in the 2019 fee schedule. They intend to reduce the documentation required for evaluation and management services, frequently referred to as E/M. What CMS did is finalize provisions that consolidate E/M payments. And ASCO had expressed concerns about this previously, which the agency acknowledged, along with other stakeholders, by revising the proposal. And, if fully implemented, they believe that the impact will be delayed-- that is, it will not impact providers until 2021. But by that time, CMS plans to consolidate what has historically been Levels 2, 3, and 4 into a single billing level, and then to pay for Level 5 E/M services separately. So overall, this represents a simplification. And it fulfills one of their stated aims, again, of reducing some of the administrative burden that practitioners face. Finally, the third area that I want to highlight is a new rule starting in 2019 that refers to the amount of reimbursement you will receive for new Medicare Part B drugs. Currently, those drugs in Part B are reimbursed at wholesale acquisition cost plus 6%. They will, going forward, be reimbursed at wholesale acquisition cost plus 3%. It's critically important to emphasize that this relates only to those new drugs that are introduced into the supply chain this year. This new provision will also apply to drugs that have not yet reported an average sales price. But the point is it will not apply to drugs that have already been in use. So it only applies to new drugs, meaning that its reach is going to be relatively limited. However, what you can imagine going forward with each new year and new drugs being introduced is that the percentage over wholesale acquisition cost will translate into more and more absolute dollars. And therefore, this may be a growing concern for practices. I want to switch our attention and talk about the Quality Payment Program, or QPP. In the final rule, there is an update to QPP for 2019. The final 2019 payment adjustment for Merit-based Incentive Payment System, or MIPS, practices and providers will become plus or minus 7%. And it will have adjustments to maintain budget neutrality, as well as to reward exceptional performance. Other noteworthy changes will include an increase in the MIPS performance threshold from 15 points, which is where we were in 2018, up to 30 points for 2019. CMS also finalized two new optional opioid-related measures that MIPS providers can use to report on under the Promoting Interoperability category. These measures will give providers an opportunity to earn bonus points and therefore potentially boost their overall MIPS score. These are the two measures specifically. One allows for checking a prescription drug monitoring program, or PDMP, prior to submitting an electronic opioid prescription for any individual patient. And the second is an attempt to verify an existing opioid treatment agreement with the patient receiving the prescription. So I hope that this summary of the updates to the Physician Fee Schedule for 2019 is helpful to our listeners. Ultimately, our goal is to make sure that oncologists can provide the right treatment to the right patient at the right time. And we aim to help CMS implement policies that will advance that goal. ASCO will continue to work closely with the administration to ensure that CMS understands the needs of the oncology community and the full impact that the rule is likely to have.
