Dr. Eftekhari, based in Salt Lake City, Utah, is accompanied by two esteemed colleagues, Dr. Cat Burkat (current ASOPRS Fellowship Education Director and AUPO Oculofacial Plastic Surgery Manager) and Dr. Seanna Grob (Young ASOPRS Chair). Given their unique roles and perspectives, together they delve into the diverse and expanding scope of oculoplastic surgery, its unique blend of artistic and medical practices, and the personal journeys that led them to pursue this fascinating specialty. The discussion opens with Dr. Burkat describing the breadth of oculoplastic surgery, highlighting procedures from lacrimal system surgery, pediatric oculoplastics, droopy eyelids, thyroid eye disease, to orbital tumors and aesthetic enhancements. Dr. Grob adds that the field allows practitioners to evolve their interests and specialize as their careers progress. Both emphasize the importance of mentorship and connections within the oculoplastic community, which plays a crucial role in fostering interest and skill development in the field. These relationships can help provide collaborations that can be beneficial for fellowship applications, as well as for long-term career aspirations. Dr. Burkat also emphasizes that the ASOPRS-approved Oculofacial Plastic fellowships have a long-standing reputation of excellent training and are carefully selected to provide the best fellowship education for patient care. The episode transitions into the specifics of academic oculoplastic surgery, discussing the dynamic nature of clinic and OR schedules, the collaboration with other specialties, and the rich opportunities for teaching and research. Key Takeaways: The Scope of Oculoplastic Surgery: Oculoplastic surgery encompasses lacrimal system surgery, orbital tumors, trauma, eyelid surgeries, pediatric conditions, aesthetics, and more, offering a wide variety of challenges and opportunities. Mentorship and Connection: Building relationships with mentors in the field is crucial for personal and professional growth, creating a supportive environment for learning and innovation. Today's discussion came from our archives and was recorded in July of 2023. Our talk is hosted by Ed Dodson, a long-time faculty member here at the Henry George School, who is joined by our guest Mr. Lars Doucet. Mr. Doucet is a consultant, blogger, entrepreneur, game developer, and currently the Director of Outreach for Common Ground USA. As a consultant, Lars has been accredited with developing numerous video games and software packages, such as Defender's Quest and Super Energy Apocalypse. He is also the founder of Geo Land Solutions, which appraises large tracts of land to better calibrate land values and taxes more equitably. Mr. Doucet recently published his first book: Land is a Big Deal, where he explores Georgist ideas, such as rent, natural resource extraction, and collectivism.   As economies move away from being mostly industrial, they grow in complexity. You've probably heard of financialization, the phenomenon of how finance becomes increasingly intertwined with the real economy. But this is only one layer to this growing complexity. Technology has also changed the economy, everything is becoming digitalized as analog machines and processes become obsolete. All of this adds up to a highly complex economy that becomes increasingly hard to understand and comprehend. Older economic models were once capable of understanding the economy in its less complex form. But as technology and the economy become more advanced, these frameworks and analyses becoming increasingly similar to the “vulgar economics” we talked about last week. These models are often linear, can't capture human or societal behavior, and fail to predict crises. Critics of mainstream economics, like Mr. Doucet or, most prominently, Steve Keen, advocate for a new framework for our economic models. One new method embraces “system dynamics”, a computer science and mathematical modeling technique showing how actors within complex systems, say industrial workers in a factory, interact with other fixtures or incentives in the system. Unlike other frameworks, proponents of system dynamics believe these models can better understand how feedback loops are created, how changes in incentives or behaviors lead to changes in the system as a whole, or how dynamic or evolutionary the economy is. As a video game designer, Lars created educational games that utilize system dynamics. When observing the games he designed, Lars often noticed how the economy in the game would lead to recessions or economic downturns. With an understanding of how the system would work, Mr. Doucet would then suggest changes similar to Georgist policy that would then ameliorate the recession. This led him to better understand the importance of Georgist policy in our complex world.   Mr. Doucet earned his bachelor's and master's degrees from Texas A&M University in Visual Sciences.   Together, we discussed how Mr. Doucet came to his Georgist epiphany, how speculation leads to inequality and economic downturns, and how proper appraisal of land values can lead to beneficial downstream effects. Sight is what you see with your eyes, whereas vision is what you see with your mind. Vision is claimed to be the most potent force on earth.  It allows us to see things not yet on earth.  Vision is the ability to see the invisible, make it possible to formulate a clear picture of a preferable future, and choose an option that you may consider worthy of pursuing. Thus, we can all agree that vision drives us to victory. However, there are things that can cloud our vision and prevent us from pursuing our passions.    There will always be something that may make us question our dreams, resulting in  a more doubtful pursuit of our goal. People around us will have diverse opinions that can be beneficial or detrimental to us. Therefore, when plagued with voices incapable of inspiring us, we must learn how to silence them and focus on what God is calling us to do. We must always chase our passion and count on words that aim to help us prosper and do better. Moreover, we must never take "no" for an answer but persevere in adversity. Remember that God gives us the vision to accomplish a greater purpose which might seem incomprehensible to others but is always clear to your heart.   Donavon Dahmer is a brilliant and empowered man who grew up in a small town in Mississippi and studied at the Whiddon College of Medicine at the University of South Alabama. Donavon is a graduating senior who received a 2019 Phi Kappa Phi National Fellowship for graduate study, making him the fifth USA student to receive a national PKP honor in recent years. He is the descendant of Vernon Dahmer, a Civil Rights activist who gave his life to provide equality and the right to vote for Mississippians. He was also selected by the University of Iowa Department of Ophthalmology and Visual Sciences for the 2023 class of resident ophthalmologists.   In this special episode with Donavon Dahmer, we'll learn about the humbling achievements of a graduating medical student who had experienced the unique learning method amidst the pandemic. Be enthralled by how he learned life lessons that helped him thrive for greatness and what compels him to pursue a profession in ophthalmology.   "Sometimes we get so caught up in listening to what others are doing and saying. This episode is sponsored by Bio-Techne. While advances in stem cell science have led to an increasing number of stem cell-based therapies entering clinical trials, the field is still relatively immature. Thus, these first-in-human trials are using pioneering approaches unique unto themselves, leaving scientists, physicians, and regulators to assess the best approaches for a specific therapy and/or disease.  In this episode of the podcast, Martin Pera is joined by physician-scientists Roger Barker and Rajesh Rao who have and are leading stem cell-based trials to discuss some of the critical aspects of the process and reflect on the “lessons learnt” during their translational journey.  Roger Barker, a pioneer in developing cell therapies for Parkinson's disease, is a Professor of Clinical Neuroscience at the University of Cambridge and Consultant Neurologist at the Addenbrooke's Hospital Cambridge. With her BA in Biological Science and her PhD in Physiology and Cell Biology, both from University of California-Santa Barbara, as well as having spent time as a Postdoctoral Fellow at University of Utah Health Sciences Center, a core focus of Dr. Merriman's laboratory research over the years has been the development of a captive breeding colony of the 13-lined ground squirrels. This unique, one-of-a-kind captive breeding program, due to this species very unique cone-dominant, diurnal visual system, as well as their impressive physiological ability to survive in hibernation for over six months without food or water, has served investigators with animals and custom-dissected tissues from the US, Asia, and Europe for decades, as well as been core to Dr Merriman's own work on vision, including cone cell biology and retinal function during the metabolic state transitions associated with hibernation. Over the years, Dr. Merriman expanded her research horizon well outside of vision, into neuroscience, and in recent years she has collaborated on studies of muscle physiology, viral genomics, molecular biology of transposable elements, and comparative genetics of the control of coat patterning. Dr. Merriman's lab research has been funded by the National Institutes of Health, the National Science Foundation, and the Foundation Fighting Blindness, and the University of Wisconsin System Comprehensive Campuses tech transfer organization, WiSys.

Previously, he was the Founder, CEO, and Executive Chairman of IDx. Dr. Abramoff received his M.D. from the University of Amsterdam and Ph.D. in Medical Imaging from Utrecht University. Key Insights:Dr. Abramoff's company Digital Diagnostics developed the first FDA approved autonomous AI. Closing the Care Gap. The primary care physicians and endocrinologists most involved in diabetes care lack the expertise to diagnose diabetic retinopathy. Digital Diagnostics' autonomous AI can make that diagnosis in-office, rather than referring the patient out to another specialist. (8:29)Earning Reimbursement. To get buy in from clinicians and the FDA, Digital Diagnostics had to show that their product was safe, effective, and built on diverse data to prevent bias. Additionally, early on they claimed that as creators of the AI, they are liable for its performance. These features eventually led to a CPT code, creating proof of concept and a path for autonomous AI in healthcare. (12:58)Ethics in AI. When hiring engineers and developers, Dr. Abramoff emphasized the importance of ethics. Single characters in their code could make a difference for a patient. Their work at the aggregate needed to represent individual patients and account for their individual experiences.
