We're doing something a little different this week on The Backstory. We're two-thirds of the way through this series and has talked to 13 incredible leaders. So, we wanted to take a moment for host Anne Hancock Toomey to reflect on one of the questions that she asks each guest. That is, "What are the three characteristics of the most effective leaders?" Every one of the leaders interviewed has a unique story. At the same time, they have certain things in common. They are successful because of who they are and how they lead. And so the hope in asking this question, which always comes near the end of the conversations, is to get at those ingredients to their secret sauce. The question isn't "What makes you successful?" The black box revealed a variety of misaligned incentives in our current healthcare system. Heroism is Counter-productive. Dr. Nash pushes back on the habit of calling doctors “heroes.” It reinforces the mythology that doctors can fix and control everything in healthcare. While individual doctors did their best, in reality it was the American health system that failed during the pandemic. This type of model incentivizes prevention, wellness, and upstream care. Kaiser Permanente and Humana are models of this. Meet Tom Balcezak, M.D., MPH:Tom Balcezak, M.D., MPH is the Chief Medical Officer of Yale New Haven Hospital. Previously, he was the Senior Vice President and Chief Quality Officer for Yale New Haven Hospital. He has been with the organization for over 30 years. Dr. Balcezak is an internist and received his medical degree from the University of Connecticut, and his MPH from Yale University. Key Insights:Dr. Balcezak provides insight into the position of Chief Clinical Officer, and the role it plays in the c-suite, strategy, and decision making.The Role: Chief Medical Officer. CMOs have the responsibilities of what historically has been a Chief Physician Executive. His direct reports include Chief Medical Officers and Chief Nurses, and he plays a role in quality, safety, accreditation, policy regulations, as well as pharmacy and supply chain. From a strategic perspective, Dr. Balcezak centers his decisions around the clinical needs of the patients and broader community. (6:50)Integration at Scale. Yale New Haven Hospital was able manage the large influx of patients during the pandemic due to the large size of the organization, combined with a high level on integration. The institution could pivot quickly due to system-wide standardizations related to EMRs, practice groups, and care protocols. (12:43)Staffing Shortages. America's aging population combined with pandemic burn out is causing staffing shortages in healthcare. Additionally, Dr. Balcezak notes wage competition is occurring from parts of the economy that healthcare doesn't normally compete with. Staffing will be one of the biggest challenges ahead given the person-intensive nature of healthcare. Friends,There are numerous critical healthcare reframes in this week's episode. First, what Dr. Silverstein makes abundantly clear is that the senior leadership team at ChristianaCare is fully comitted to accelerating the shift to value based payment. Second, they are committed to greatly expanding the use of virtual care as a  primary mode of care. Third, they are focused on health outcomes vs healthcare delivery. Underlying these three patient-centered goals is another profound reframe, the notion that in order to achieve the quadruple aim, provider organizations must form vertical integrations and create complementary collaborations with other types of stakeholders in healthcare - such as payers and retailers.Our guest today, Ken Silverstein, is the Chief Physician Executive at ChristianaCare. He previously served as chair of ChristianaCare's Department of Anesthesiology, Medical Director of Perioperative Services and as Chief Medical Officer. He received his undergraduate degree from Brown University, his medical degree from New York University School of Medicine and his MBA from the University of Delaware. Dr. Silverstein was a resident and a fellow in anesthesiology at the Brigham and Women's Hospital in Boston. He completed his clinical fellowship in anesthesiology at Harvard Medical School. In this episode, we'll learn about:The 10 year value-based collaborative agreement that ChristianaCare and Highmark have launched together. The 5 major “vehicles” that ChristianaCare & Highmark are planning to deploy within their collaboration. The guiding “partnership principles” that are being used to create a relationship-centered approach to this synergistic collaboration. The fascinating independent spin-off virtual primary care practice that ChristianaCare is building out with Highmark, that greatly improves the efficiency and effectiveness of care. At the beginning of our conversation, Ken states, “we created a new company and the intent is to completely transform healthcare.”  That bold remark speaks volumes about the visionary leadership at ChristianaCare. Another statement which gripped me was the fact that the ChristianaCare leadership team is discussing Clayton Christenen's ‘dilemma zone'', which the folks at ChristianaCare have relabelled as the “commitment zone”. The point of the concept is that in order to transition to a value paradigm and realize a long-term gain, leadership must accept a short-term loss. It's amazing to see hospital leadership thinking and acting in this way. But even more amazing is the fact that their Chief Financial Officer was the one who introduced this concept to the leadership team. In our correspondence prior to the interview, Ken wrote that American healthcare is “facing an apocalyptic forecast…”.  It's a disconcerting pronouncement from a seasoned physician executive.  But, what was concerning for me was that I had heard this very same comment from two other physician executives in the prior week. During our interview, I asked Ken what he meant by this statement. He went on to discuss issues such as the worsening unaffordability of healthcare, fragmentation of care, and inappropriate overutilization. He also passionately talked about the unacceptable inequities and disparities in healthcare, using Delaware as an example.  In Wilmington, the difference in life expectancy - what Ken referred to as the “death gap” - between white communities and black communities only 2 miles apart is 16 years! He punctuated this statistic by asking how that is acceptable to anyone.The situation may seem dire but I have to say that I have never been more hopeful, more inspired and more encouraged about the future of healthcare than I am today. The reason stems from listening to leaders like those at ChristianaCare who are not just speaking the rhetoric of reframing, but are taking the very concrete steps necessary to create a new and better healthcare. They are making the painful trade-offs required, and are investing in a better future. They are, as Simon Sinek would put it, playing the generative "long game". ChristianaCare is making a demonstrable commitment to creating a value-based future. They are aligning their resources with their rhetoric. They are doing this not because it's easy or safe. Prior to joining VHS, Dr. Brenner served as Chief Medical Officer (CMO) of Summit Medical Group for nine years and, subsequently, as CMO of Summit Health Management. In those roles, he was responsible for strategic expansion; enhancement of organizational performance, patient safety and patient experience through the creation of robust population health, risk management and advocacy departments; clinical program development and advancement of clinical information technology. Prior to that position, he was a Senior Executive at Mountainside Hospital/Atlantic Health System and served as the Chairman of the Family Medicine Department. Dr. Brenner has also spent 15 years in graduate medical education, the last seven of which he served as the Family Medicine Residency Director at Mountainside Hospital/Atlantic Health System. ------ Simon Samaha, MD, MBA, is a veteran healthcare executive and entrepreneur. He is currently a Senior Advisor at Oliver Wyman and a member of its Health and Life Sciences Team. He is a recognized thought leader in provider with deep expertise in Clinical Services and Physician Enterprises. His career started as an executive (including serving as CEO at Summit Medical Group) on the provider side in both not and for profit, later joining a top global consulting firm initially as their healthcare center cluster leader (serving Europe Africa, India and Middle East) and later leading the Clinical Transformation practice. Dr. Samaha's focus is on strategic clinical transformation, assisting organization in optimizing their clinical assets and engaging their physicians in these transformation journeys. He has deep expertise in all aspects of physician organizations including M&A, VBC, network development, operational and financial optimization, and service line design and implementation. Dr. Samaha also helps lead Kentan Staffing Solutions, the healthcare division of ICS. He plays a key role in defining the client needs and is personally involved in reviewing every clinical SME.

The federal government is sending hundreds of ventilators to Florida to help deal with rising COVID-19 cases. The Biden Administration said it sent the ventilators per the request of Florida hospitals that have been overrun with COVID patients . Flagler Hospital will receive ventilators as well. Gov. Dr. Meehan is a board-certified Emergency Medicine physician, and was the driving force behind the creation of the New Hampshire Physician Leadership Development Program, a joint effort between the New Hampshire Medical Society, the New Hampshire Hospital Association, and the University of New Hampshire. In this podcast we discuss Dr. Meehan's own journey from living in a music studio above a peanut butter factory through medical school, residency, and his own development as a physician leader. We discuss in detail what the medical school and residency experiences are like because the New Hampshire Physician Leadership Development Program has a unique flavor based on Dr. Meehan's own experience of medical training, as well as his observations about how physician training creates some specific challenges for physicians who want to transition from lead clinician to clinician-leader. I have been part of the Physician Leadership program since its inception, so it was fun to talk with Neil and capture some of the stories and insight I have heard him share over the last few years. While some Americans can afford the luxury of working from home, other workers put their lives in peril every single day, just doing their jobs. Non-Hispanic American Indian or Alaska Native people had an age-adjusted COVID-19 hospitalization rate about 5.3 times that of non-Hispanic white people, according to data from The Centers for Disease Control and Prevention (CDC). Black people and Hispanic or Latino population were both about 4.7 times the rate of non-Hispanic white people. After controlling for age, sex, and mortality rates over time as to the likelihood to die, Asians were 53% more likely, Black Americans were 37% more likely to die than Whites, Native Americans and Alaskan Natives, 26%more likely, and Hispanics, 16% more likely to die.
