She began her Islamic studies over 20 years ago at Zaytuna Institute in the Bay Area California where for several years she served as the lead female organizer and studied aqeeda, seerah, Hanafi fiqh, tazkiyyah an-nafs, tajweed, hadith, Arabic, and other sacred subjects with several resident and visiting scholars. She offers talks throughout the year locally and internationally on a range of topics including spirituality, self-development, seerah, women's issues, family/marriage, youth issues, social media literacy/safety, and mental health advocacy. She teaches weekly and monthly spiritual development classes (tazkiyyah) for adults and youth for multiple local and international organizations and via social media. He has worked for Solano County Library system full-time for the last 9 years. I have a great interview with Kivanc and Danielle of See Bee Sailing Kivanc and Danielle Aslaner from the Bay Area California, who just bought their dream boat in Croatia, quit their jobs and are about to live full-time on their boat and start sailing the world. Kivanc is originally from Istanbul and had an interest in sailing and the nautical lifestyle from a young age. While he was growing up in Turkey, sailing was a sport mostly for those well off and seemed inaccessible. His time and attention were also focused on other things such as finishing his education. Fast forward 10 years later to California where he discovered the benefits of belonging to a sailing club. OCSC (Olympic Circle Sailing Club) in the Bay Area opened up the world of sailing in an affordable way, while offering courses taught by top-notch sailing instructors. After Kivanc received a number of sailing certifications, Danielle decided to take a crew class so she could be of more help to Kivanc on the sailboats they chartered in the club and understand the sailing nomenclature better. After the crew class, things started to click with Danielle around understanding basic sailing principles and how things functioned on the boat. Within one year, Danielle jumped all in and completed nearly all the sailing courses offered by the club, including Coastal Passage Navigation. She even started a woman’s only sail group with another sailing pal from OCSC. Over the years as Kivanc and Danielle grew into their sailing hobby their priorities shifted. Both had established careers in high-tech but they craved something more out of life. After doing a number of destination charters around the world, they developed a taste of what it would be like to liveaboard a sailboat. The decision was made 6 years ago to start saving for a sailboat and take a 2-3 years sabbatical from work to experience living aboard and exploring part of the world while sailing. The plan was to start in early 2020 but then the Covid pandemic hit and they were stuck at home an extra year. Danielle even quit her job right beforehand. Instead of being down about this unexpected delay, a lot of soul searching happened and they made the decision to go “all in” with their sailing adventure when they could eventually depart from America. Instead of the original 2-3 year plan, they now have no plans to return to America and will keep sailing as long as it’s enjoyable and as long as the money lasts. Kivanc and Danielle bought their dream boat a 45’ Elan Impression in Croatia at the beginning of 2020, right before the pandemic. They named her Sea Bee a combination of their two shared hobbies – beekeeping and sailing. She has been patiently waiting in a marina for their return – which is about to happen at the end of April, 2021. Kivanc, Danielle and I talked about their adventures in buying a boat in Croatia to go cruising with. The American Society for Colposcopy and Cervical Pathology (ASCCP) recommended HPV vaccination for clinicians routinely exposed to HPV. This recommendation encompasses the complete provider team, including physicians, nurse practitioners, nurses, residents, and fellows, and others in the fields of OB/GYN, family practice, gyn-onc, and dermatology. While there is limited data on occupational HPV exposure, ASCCP, recommends that members actively protect themselves against the risks(1).  This recommendation on HPV vaccination for health care workers was published on February 19, 2020. She told me she enjoyed a lot working with Dr Beare, our street medicine doctor, whom I hope can be our guest in this podcast one day. Welcome, Dr Asefi. As you know we will ask you 5 questions. Let’s start with our first question number 1. 1. Who are you?I’m Dr Golriz Asefi, I’m a PGY1 here at Rio Bravo family medicine residency program. I grew up in the Bay Area (California). I went to UC Berkeley and then to Ross University School of Medicine. I picked our residency program because I was very interested in community medicine and helping the underserved. On my “spare time” I like to take long walks by the beach and go hiking with my friends. I also have a newly found love for yoga and barr method. 2. What did you learn this week?This week I learned about suboxone. Suboxone is a combination of buprenorphine (a partial opioid agonist) and naloxone (an opioid antagonist). It is used in the treatment of opioid use disorder along with counseling and other behavioral therapy. Suboxone is a class III controlled substance in the form of sublingual pill, sublingual film or buccal film.Comment: Suboxone is part of the Medication-Assisted Treatment (MAT) of opioids. It is the use of medications, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders. These medications operate to normalize brain chemistry, block the euphoric effects of opioids, relieve physiological cravings, and normalize body functions without the negative effects of the abused drug.Suboxone is a combined medication. Why do we have both an agonist and antagonist in the same dose you might ask? Well it’s to prevent abuse, you see naloxone when taken sublingually as directed is minimally absorbed, whereas when injected, it is a strong blocker of the opioid receptor. So, you can get the antagonistic effect of naloxone. When taken sublingually naloxone is poorly absorbed, therefore patients experience less withdrawal symptoms. The “Ceiling effect”Buprenorphine exhibits what’s called a “ceiling effect”, which occurs because suboxone partially stimulates opioid receptors even when saturated. Even exponentially increasing the dose only achieves limited additional effect– similar to approaching an asymptote - remember those hyperbolas and parabola in high school geometry? Basically, never reach full effect.  Therefore, it has a lower chance of abuse and accidental overdose. Suboxone vs MethadoneMethadone is full opioid agonist. It is a PO liquid administered through methadone clinics to which patients must go to everyday, and get the medication. Take-home privileges can be eventually earned for methadone.  Suboxone has lower chances of overdose because it is a partial agonist, however, dropout rates are higher. Suboxone can be prescribed to be taken at home, so it is more convenient(2).Candidates for suboxone Opiate users who are motivated for treatment and are willing to adhere to scheduled visits and treatment.  They should understand the indications, risks, benefits, and alternatives. Ideally, they should not be on other CNS depressants, such as alcohol and benzos, however, suboxone is preferable over methadone for people who use CNS depressants or are at higher risk of respiratory failure. Suboxone can also be used during pregnancy.Contraindications to suboxone:-Severe liver impairment.-Conditions that already increases risk of respiratory depression such as head injuries or taking CNS depressants such as benzodiazepines or alcohol. Mixing large amounts of other medications with buprenorphine can lead to overdose or death(2).Required Training to Prescribe SuboxonePrescription is regulated by the DATA (Drug Addiction Treatment Act of 2000), you need to complete a MAT (medication assisted treatment) training which is currently available free of cost. To qualify as an MD, you need to have an active medical license and DEA license. The training takes about 8 hrs for MDs, 24hrs for NPs, CNMs, and PAs. You can take the training at anytime, even as a resident, and save the certificate until you receive your MD license and DEA license. The training is 100% online, you can print your certificate and apply for an X-license to prescribe suboxone. The information is found online at PCSSNOW.ORG (Providers Clinical Support System)(3)3. Why is this knowledge important for you and your patients?Suboxone is underutilized by primary care providers. Opioid use disorder leads to a lot of socioeconomic problems, such as increased rates of suicide, accidental overdose, HIV, HepC, marital problems, and unemployment. About 46,000 people died of opiate related deaths in 2018, steadily rising from 21,000 in 2010. Of that, 18,000 was due to prescription opioids(4). Luckily, treatment of the disorder decreases all of the above. MAT has shown to improve mortality due to opioid overdose, increase retention in treatment, decrease illicit opiate use and other criminal activity, increase patient’s ability to gain and maintain employment, improve birth outcomes in pregnant women with substance use disorder, lower risk of contracting HIV and HepC. You can see, Dr Arreaza, you can make a big impact in your patients if you get this training, you will change lives and whole communities by providing this life-changing treatment. Comment: This is something I learned recently. Having a substance use disorder reprograms your brain, it is like being thirsty and not finding water to drink. So when we tell our patients, “Just stop taking Norco”, it is similar to tell them, “Just stop drinking water.” We have to develop empathy to some of the most neglected patients in our society. Not many people are helping these patients! And many of them also have mental illness, as many as 30-60%, depending on the source you read. So, it’s a good idea to get trained to help this vulnerable population.4. How did you get this knowledge?I was working with Dr. Beare during my community medicine rotation. I really enjoyed learning about suboxone and seeing how rewarding this experience was between him and his patients. This made me want to do more research and present this topic to my colleagues. As Dr. Beare likes to say: “Treating addiction is like treating any other complex chronic disease. There is no such thing as quick fix.” We as physicians need to listen to our patients, to care and to treat this difficult condition.5. Where did this knowledge come from?-SAMHSA – substance abuse and medical health service administration, The National Institutes of Health, an Up to Date article, and other articles. See details below.______________________Speaking Medicalby Lisa Manzanares The medical word of the day is leukorrhea.  It is the flow of whitish, yellowish, or greenish discharge from the vagina. Leukorrhea can be normal, or it can be a sign of infection. Leukorrhea commonly occurs during pregnancy, and is normal if the discharge is thin, white, and odorless. Physiologic leukorrhea is a normal condition occurring within several months to 1 year of onset of menses in adolescent girls.  Leukorrhea that is not normal can be caused by bacteria: bacterial vaginosis, chlamydia, gonorrhea, or postpartum endometritis. Fungal causes include candida species. Rodilla is a large joint in our body that may wear off over time and gets injured easily. Yes, you guessed it, rodilla means knee. This word comes from the Latin root “rota” or “rueda” which means round or wheel. Patients may come to you with the complaint: “Doctor, me duelen las rodillas” or “Doctor, tengo la rodilla hinchada” which means: “Doctor, my knees hurt” or “Doctor, my knee is swollen”. I gotta be honest, I am not a website article reader that much, I stick with podcasts. My most favorite is when Daily has full crew with 2 of my faves Chris and HT!!   This genius new segment "Life Advice with Chris" is amazing, funny and maybe even helpful, so far it's a big Maybe :):):) Ok got with that out of the way... Here is my question. I'm an immigrant from a country with a much more relaxed age rating system and I grew up watching hard R even NC-17 rated films such as "shocking asia" at age 12. Hollywood R rated movies were acceptable in our household with no problems.  After coming to US I have encountered this new to me system of age restriction and parental control. To me, at the time it didn't make much sense and I saw it as a dumb thing. However, time went by and I am a father now, although my child is not yet old enough to watch movies. I have drastically changed my views towards parental control and movie age ratings....  I am definitely not going to show my son a Tinto Brass film at age 10, but I also don't want to be so strict that not watching some R rated films at a young age can turn him into fragile scaredy cat. Where does Chris stand on this subject and how should I proceed with my dilemma? Should I censor the media for my kid and let him decide later in life if he likes that type of entertainment? Or should I take my own experience as an example and allow him to watch whatever interests him (within moral boundaries) of course.   Thank you in advance. Keep up good work. Al Lagura is a Brazilian Jiu Jitsu purple belt under Alan "Gumby" Marques and trains out of Hereos Martial Arts in San Jose, CA. He's a long time car enthusiast and his early work as a videographer is well known throughout the BJJ scene. This led to the creation of Want vs Need which is one of the most highly sought after brands on the market . Al's creativity and passion are a huge asset to the Jiu Jitsu community in the Bay Area California and beyond. It's a pleasure to finally sit down with him on the podcast.  I also open up about some personal stuff I usually don't dive into on the show. I took a few weeks off and I offer a little bit of explanation as to why about 2/3 of the way through the show. I may make a separate video for this and throw it on YouTube at some point. Fairly early on in my research for my history of ideas series on Margaret Sanger in New York City, I discover that she delivered one of her more famous addresses here in Oakland. Practically just around the corner from where I live, in fact! So I've long had the idea of doing a follow-up Sanger project here in the San Francisco Bay Area, certain I'd find she's been here more than once. That turns out to be the case. And not only do I discover that she visited here several times, I find that the excellent library at the University of California at Berkeley has some great resources for filling in some gaps in my information about Sanger in NYC. So, this story will have two parts: the first part is about sites associated with Sanger here, and the second part is about some discoveries I make in U.C. Berkeley's archives following up on two NYC sites...

Workers from the bay area rapid transit (BART) system went on strike this week causing massive disruptions and major media attention.  Strangely, news reports have had no discussion of how, and to what extent, the strike is affecting people with disabilities and seniors. Today we begin to rectify that problem by opening our phones. At what point have transit officials dropped the ball for people with disabilities around this strike?   Have the unions done anything to protect and serve us during this time?  How is the inter-community support?  Are friends and family helping with rides?  Did disabled folks feel  annoyed by the strike because BART employees make so more money than most have from SS/SSI?   Or, are strike sacrifices worth it for decent salaries and benefits for these workers?
