For the podcast this week we spoke to Dr Abid Ali Khan, a GP trainee in East London currently on a Clinical Fellowship in Leadership (National Medical Directors Clinical Fellow). We focused on his experiences as an International Medical Graduate to gain an understanding of the challenges he faced coming to the UK. We got his advice for other International Medical Graduates and those supporting IMGs. But doctors moving to a new country are not a homogenous group, and those welcoming them need to understand them as individuals with specific skills and experiences. There needs to be a mindset shift to seeing new starters (whether international or not) not as liabilities, but as an asset bringing a range of new experiences and ideas with them. Surfing the MASH Tsunami continues its 2023 wrap-up conversations with Novo Nordisk International Medical Vice President Michelle Long, along with co-hosts Louise Campbell and Roger Green. The conversation focuses on advances in research, collaboration and insight creation the entire MASLD research community has made in 2023 and what this new perspective portends for the future of MASLD research.Michelle starts by using the word "momentum" to characterize the MASLD research community in 2023. She says the pace of change makes it impossible to summarize or describe the year easily, For example, the adoption of new nomenclature came six months ago at EASL, but it feels like it was far longer than that. Roger compares the optimism in the field to 2019 while awaiting what was the anticipated approval of OCA. Today, he notes,  the entire community understands so much more about MASLD and has so much more confidence in resmetirom than in OCA that we see optimism about future drug developments along with a greater appreciation of the potential scale of the global crisis around MASLD. Michelle comments that in the aftermath of OCA, the scientific community has been "willing to adapt and eat humble pie." Today, we see proposals for future studies that demonstrate how fast researchers are adopting new knowledge and using it in new study designs. In particular, she notes that the year's FDA workshop showcased a shift in drug development strategies and a deeper understanding of surrogate endpoints, reflecting a desire within the field to embrace new insights for advancement.Louise adds to the dialogue by discussing the swift adoption of new terminology and the integration of noninvasive diagnostic technologies, emphasizing the patient's perspective and the importance of comprehensible communication about their health condition. She advocates for broader inclusivity in future Delphi processes, specifically seeking greater involvement for nurses and allied health professionals, groups that provide line communication with patient communities. Roger notes a newfound humility within the academic medical community that promotes collaboration and ongoing improvement. Michelle agrees, stressing the need to dismantle barriers such as pride and the critical role of cross-sector collaboration in addressing the field's unmet needs and the lack of approved therapies. In this episode, we sit down with Dr. Iván Cuesta, MD. Dr. Cuesta was born and raised in the Dominican Republic (DR) where he attended and graduated from the Instituto Tecnológico de Santo Domingo. He completed his residency in Neurology at Temple University in Philadelphia and his fellowship in Neurologic Critical Care at Baylor College of Medicine in Houston, Texas. Currently, Dr. Cuesta serves as a Neurocritical Care Attending at Advocate Lutheran General Hospital here in the greater Chicago area as well as on faculty at the Rosalind Franklin University Chicago Medical School. We spoke extensively with Dr. Cuesta about the process he took to match into a US residency. Toward the end of medical school in the DR, his interest in pursuing a US residency was sparked after a rotation in Miami, Florida. We talk about the different hurdles that International Medical Graduates (IMGs) have to overcome to match into a US residency, including obtaining a visa and learning medicine in a new language. Then, he explains what IMG applicants can do to strengthen their applications before applying. He emphasizes the importance of making connections to the American programs through doing research work. These positions can be obtained through cold calling and emails, but he notes that there are also tenures and society fellowships available. Unfortunately, many of these positions are usually unpaid. Dr. Isaiah recommends that international medical students pursue postgraduate training in their home country and spend 5-6 years in the United States doing research before applying to US residency programs. There is a less common, alternative way to match into residency by first completing an unaccredited US fellowship program and then applying for residency. Dr. Shah and Dr. Isaiah also discuss board exams for IMGs, which include Step 1, Step 2, and Educational Commission for Foreign Medical Graduates (ECFMG) certifications. They agree that letters of recommendation and personal statements are important. Dr. Isaiah recommends that IMG applicants read US applicant essays and have their mentors look over their essays for language and grammar revisions. Finally, the doctors discuss the different types of visas that are needed for IMG residents and attendings (i.e.- H-1 visa, J-1 visa) and differences between visas, sponsorships, and green cards. Dr. Cochran describes how working in Puerto Rico and Mexico with limited resources made her a better physician who was able to critically think in situations and gives advice about applying to residences and the cost associated with it. She describes that not many people are aware at the amount it costs to go through the full journey.
