In this Part 2 of the Alloutcoach podcast episode focused on phage therapy to fight infections and antimicrobial resistance, I spoke to the Chief Physician at the globally leading center of excellence of an alternative, effective treatment approach to antibiotics, Dr. Dea Nizharadze at the Eliava Phage Therapy Center (EPTC) based in Tbilisi, Georgia. She explains the basic principles of phage treatments, its unique qualities, advantages and real-world clinical examples of methods and types of patients and infections this personalized therapy specifically targets. 0:00 Episode Introduction 1:42 Why the Republic of Georgia is a global leader in phage therapy 2:39 How phage therapy works in bacterial infections?Eliava Phage Therapy Center (EPTC) was founded in 1923 by Professor George Eliava who had collaborated with Felix D'Herelle, who had discovered phages, and while most other countries turned to antibiotics only, it is the only institution in the world that has continued to research bacteriophages without a pause of even one day since its inception and therefore collected the greatest research and real-world clinical experience with phages. 5:34 How do phages work to eradicate or stop further growth of bacteria? Phages are biological "weapons" against bacteria, namely viruses that target specific microbes only, without destroying the "good" bacteria or microbiota. They are naturally occurring substances and have no safety concerns. In some patients with antimicrobial resistance phage therapy enables improvement in sensitivity. 10:42 Synergistic effect of phage combination therapy with antibiotics 11:58 Phage Therapy Doisng and Success Factors 15:14 Length of phage treatmentIn chronic infections phage therapy may divided into 3 stages and last 15-20 days, and varies by nature and severity of disease. Patient treatment includes days off therapy, or holidays, however, bacterial analysis is continued and patient condition is monitored throughout the entire course of treatment. Success of therapy is indicated when bacterial titers and virulence decrease or bacteria are eradicated. Patient's objective and subjective signs and symptoms often vary throughout response to therapy. 16:42 Phage Product Variability and Market Availability How do we ensure educating patients to access the correct, most appropriate therapy for their infections 17:35 Multi-disciplinary personalized team care and telemedicine 19:53 Steps for patients outside of Georgia to access phage therapy via telemedicine? 24:04 Affordability and access to phage treatments 25:23 How to contact Eliava Phage Therapy Center

In this special episode I spoke with an international guest from my native country, Republic of Georgia, and global ambassador of a critical alternative solution against tough chronic infections and antimicrobial resistance on behalf of a global center of excellence in bacteriophage therapy, discovered over 100 years ago. My guest, Davit Sturua, Doctor of Business Administration, Public Health and Medical Tourism expert, is the director of the Eliava Phage Therapy Center, a global leader in real-world, clinical, research, and commercial experience with naturally occurring bacteriophage treatments. He discusses the Georgian center's 100-year old history and evolution, its extensive experience treating patients across 84 countries, explains the current global public health problem, and provides an update along with rationale on the recent growth in clinical studies investigating phage therapy across the globe in this part 1 of 2 episodes on this topic. Watch this episode with English subtitles to learn about how you, your friends, family, healthcare payers and experts, healthcare providers, patients and their loved ones can consider alternative options via telemedicine or in person to treat chronic, tough bacterial infections or prevent antimicrobial resistance. Stay tuned for the next episode (part 2) on this topic with my guest Dr. Dea Nizharadze, Chief Physician at the Eliava Phage Therapy Center, who delves deeper into the typical course of treatment, patient journey, and various scientific attributes of phage treatments. 0:00 Introduction 0:08 Introduction in English 2:19 Special Guest Mr. Davit Sturua, Director of Eliava Phage Therapy Center Introduction in English 4:34 Episode Introduction in Georgian (English subtitles) 4:53 Overview of Part 1: Overview of 100-year history of Eliava Phage Therapy Center, its advances, growth, current goals and Part 2: Scientific Rationale for phage treatment, appropriate patients, typical course of treatment, and access to care for patients worldwide 5:54 Davit's personal inspiration and history at the Eliava Center 10:27 How is EPTC raising awareness about phage therapy outside of Georgia? The center has treated patients from 84 different countries in 2023 alone. The majority of patients internationally who obtain care from EPTC suffer from various antibiotic-resistant or chronic infections. 12:20 5 million patients die from antimicrobial resistantce every year - underestimation, with tens of millions of deaths projected by 2050 13:04 Future Scientific and Commercial goals of Eliava Center to expand globally, standardize phage production to meet GMP standards, conduct randomized clinical studies and secure access for patients worldwide 15:23 Challenges in scaling phage production and quality standardization. Georgia is currently a global leader in commercial production of bacterial phage treatments. While only a few years ago there were only a handful of clinical studies conducted globally, currently there are over 50 clinical studies in 2023-2024 alone over the past year being conducted in the United States alone. Similarly, other European countries, particularly Great Britain are also actively pursuing and advancing clinical research in bacteriophage therapies. Government and research funding in phage treatment is therefore continuing to grow rapidly. 18:33 Final Greetings - Conclusion of Part 1. Podcast Description: In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Drs. Christhardt Köhler and Achim Schneider to discuss radical vaginal trachelectomy. Prof. Christhardt Köhler is a highly accomplished gynecologist and obstetrician specializing in oncological gynecology. He is the Chief Physician of the Department of Gynecology at Asklepios Hospital Altona Hamburg and has won the Briker Award and the Robert B. Hunt Endowed Award. Dr. Kohler is a member of several professional associations, including the German Society for Gynecology and Obstetrics. His expertise is recognized worldwide, and he is listed in the FOCUS top list of physicians for gynecological operations and tumors. He has 1381 scientific publications focusing on spinal muscular atrophy, cervical cancer, and innovative gynecological surgery approaches. Achim Schneider, born on September 17, 1950, in Augsburg, is a distinguished German gynecologist and obstetrician. He earned his doctorate in 1975 and has since made significant contributions to the field, including pioneering new laparoscopic surgical techniques in gynecological oncology in Germany. Schneider held prestigious professorships at the Friedrich Schiller University in Jena and the Charité in Berlin. He has focused on the diagnosis and treatment of genital neoplasms, HPV-associated ano-genital neoplasms, and fertility-preserving surgery for cervical cancer patients. Currently, he heads the dysplasia consultation at the Medical Care Center in Berlin, continuing his impactful work in minimally invasive procedures and cancer prevention. Highlights: Radical vaginal trachelectomy is a fertility-preserving treatment for early cervical cancer, but large studies on its outcomes are lacking. In a study of 471 patients (median age 33), 62% sought pregnancy with a 73% success rate, and 46% experienced pre-term delivery; recurrences occurred in 3.4% of cases, with a 2.1% mortality rate. The study confirms the oncologic safety of the procedure and suggests that high pre-term delivery rates may be due to cervical volume loss, providing a benchmark for future surgical modifications

Sustainability in Radiology: Reducing the carbon footprint

The carbon footprint of healthcare has been estimated to be 4 to 5% of the worldwide total. To put this number into perspective, if “healthcare” was a country, it would be the fifth largest emitter in the world, coming in just behind China, the United States, India, and Russia. Today we face pressing global issues like climate change and an energy crisis, that impact our health and wellbeing. It becomes crucial then, that the healthcare industry not only takes responsibility for health, but also actively works towards reducing the impact of the environment on people's health – especially by reducing emissions and consumption of resources. In this episode, Shikha Pillai, Global Head of Sustainability at Siemens Healthineers, is joined by Dr. Elmar Merkle, Head of the Department of Radiology and Chief Physician for Radiology and Nuclear Medicine at the University Hospital Basel in Switzerland; as well as Dr. Christopher Hess, Chair of the Department of Radiology and Biomedical Imaging at the University of California San Francisco; and Dr. Clemens Juettner, Chief Sustainability Officer at Sana Kliniken in Munich. These experts in radiology and sustainability are talking about what the sector is doing to implement consumption-reducing measures that can lead to not only operational efficiency, but also contribute significantly to our sustainable future. What You'll Learn in This Episode: • For hospitals around the globe, the perpetual “idling” state of imagining machines contributes to the huge carbon footprint of radiology departments. • On a global average, one hospital bed consumes the same amount of energy as four family homes annually. • Major strides have been made toward more energy efficient imaging machines, but the most pressing issue is encouraging their use by the medical community. • The production and delivery of imaging equipment can have massive environmental repercussions. • It's not just carbon footprint that can be reduced by greener radiology practices: cost is also a crucial factor.Connect with Shikha Pillai· LinkedInConnect with Clemens Juettner· LinkedInConnect with Christopher Hess· LinkedInConnect with Elmar Merkle· LinkedIn Hosted on Acast. Play Episode Listen Later Oct 11, 2023 18:13

This week, we continue our Nashville Health Care Council Sessions series with an episode focused on the industry's stepwise progression to value-based care (VBC). The shift to value is something that everyone across the healthcare continuum supports in principle, but achieving it is a massive economic and technological undertaking. Ingrained structural challenges have made the transition slower than hoped. Onstage at Sessions, speakers talked about how the unsustainability of our current healthcare spend is forcing the issue. That, combined with creative new models from entrepreneurs and national organizations, are helping to finally make an accelerated transition to value feasible for providers, payers, patients and every stakeholder in between.    We spoke with Dr. Wyatt Decker, Executive Vice President and Chief Physician for Value Based Care and Innovation at UnitedHealth Group, and Rob Allen, CEO of Intermountain Health, about the innovations that will be necessary to streamline VBC, as well as their respective philosophies about long-term health in a volume-based world.   Key Points:   VBC is only possible at scale if everyone works together. The most formidable roadblock to value is its operational complexity, requiring every stakeholder to rethink how they administer and measure the success of their services.   The shift to value poses incredible business opportunities for entrepreneurs. From the need for better clinical data tracking to changes in risk sharing to disjointed communication between payers and providers, there is a lot of space to fill for specialized start-ups. Per Dr. Decker: “We work with entrepreneurs of all sizes in terms of their business who have solutions.”  Achieving VBC hinges on better systems for understanding patients' needs. Large providers and payers will rely more heavily on personal relationships with individuals to ensure the clinical understanding necessary to set and achieve health metrics.   Data is top down and bottom up. While VBC is tailored to the individual, it can only be achieved through a clear understanding of community and population-level data. Data collection and analysis have finally reached a point where the resulting information can be used effectively to drive population health.  Learn more about your ad choices. Transcript:Hello! This is Pastor Don of Christ Redeemer Church. Welcome to The Kingdom Perspective! Though ancient Christians practiced the strictest sexual ethic of their day, they were anything but judgmental towards those ravaged by the effects of sexual sin. The followers of Jesus were distinguished by the grace they extended to those exploited by the sexual confusion and callousness of Roman culture. The church was seen as a place of healing. Not a trophy case for saints, but a hospital for sinners! Now, why was this the case? Well, classical Christianity teaches us that sin is not just bad things we do. Rather, it is a deep dysfunction that causes destruction in both soul and society. When we participate in sexual sin, it tears at the very fabric of human nature. When someone commits sin against us, it's as if they are pulling on a thread in our soul's fabric, causing us to unravel. Anyone who has been the victim of such sin can testify to its lasting negative impact. Now, arguably, the ancient world was not much different than our own, rife with both perpetrators and victims of sexual sin—people whose lives were unraveling. Into that world came the radically restorative message of Jesus. Jesus's main mission was not to condemn, but to mend and heal that which sin had destroyed—to make us whole. It is for this reason that the church became a place of healing. This was true not only because of the healthy sexual ethic it practiced (truth), but also because it so welcomed the victims of a bad sexual ethic (grace). It didn't require wholeness; it created it. The church was a hospital and like its Chief Physician (the Lord Jesus), it was full of both grace and truth. Something to think about from The Kingdom Perspective. …but Jesus went to the Mount of Olives. Early in the morning he came again to the temple. All the people came to him, and he sat down and taught them. The scribes and the Pharisees brought a woman who had been caught in adultery, and placing her in the midst they said to him, “Teacher, this woman has been caught in the act of adultery. Now in the Law, Moses commanded us to stone such women. So what do you say?” This they said to test him, that they might have some charge to bring against him. Jesus bent down and wrote with his finger on the ground. And as they continued to ask him, he stood up and said to them, “Let him who is without sin among you be the first to throw a stone at her.” And once more he bent down and wrote on the ground. But when they heard it, they went away one by one, beginning with the older ones, and Jesus was left alone with the woman standing before him. Jesus stood up and said to her, “Woman, where are they? Has no one condemned you?” She said, “No one, Lord.” And Jesus said, “Neither do I condemn you; go, and from now on sin no more.”~ John 8:1-11 (ESV) Some sources to consider: The Rise of Christianity: A Sociologist Reconsiders History by Rodney Stark (Princeton University Press 2020). The Patient Ferment of the Early Church: The Improbably Rise of Christianity in the Roman Empire by Alan Kreider (Baker Academic 2016).

A deep, inspiring, and often challenging study of the Lord Jesus Christ's miracles and parables. Men who were led by the hand or groped their way along the wall to reach Jesus were touched by his finger and went home without a guide, rejoicing that Jesus Christ had opened their eyes. Jesus is still able to perform such miracles. And, with the power of the Holy Spirit, his Word will be expounded and we'll watch for the signs to follow, expecting to see them at once. Why shouldn't those who read this be blessed with the light of heaven? This is my heart's inmost desire. I can't put fine words together. I've never studied speech. In fact, my heart loathes the very thought of intentionally speaking with fine words when souls are in danger of eternal separation from God. No, I work to speak straight to your hearts and consciences, and if there is anyone with faith to receive, God will bless them with fresh revelation. - Charles H. Spurgeon In this fourth volume, Charles H. Spurgeon expounds in marvelous detail on the following miracles: • Jesus tells Simon to cast his net on the other side of the boat and he obeys. (Luke 5:4-5) • Jesus heals the centurion's servant and marveled because of the centurion's great faith. (Matthew 8:7-10; Luke 7:4-8) • Jesus casts an unclean spirit out of a man, and all are amazed at His authority. (Mark 1:21-28) • Jesus heals the persistent Gentile woman's daughter and compliments her remarkable faith. (Matthew 15:24-28)
