One Brand, Two Directions Drawing on four documented historical parallels from the history of American medicine — the AMA and managed care, osteopathic medicine's identity divide, family medicine's emergence as a separate specialty, and the micropractice movement — the episode makes the case that two communities with genuinely different economic interests and regulatory priorities currently sharing a brand name may, consistent with historical precedent, find their own distinct professional homes over time. Stamford Chidge & Jonathan Kydd are joined by Marco Worrall to look back at a great Chelsea performance against Man City.In part one we applaud a great Chelsea performance, redolent of Chelsea v Barcelona from days gone by; tactically disciplined, counter attacking brilliantly and defending like demons. Did this young team come of age on Saturday? Massive shout outs to the defence - Disasi a rock; Colwill assured; Gusto superb; Chilly back to his best; should this be the defence going forward? Can or should Gusto keep James out when he's fit? The midfield were superb; Moises was tenacious and did well not to get a 2nd yellow; Enzo was here, there and every fucking where; Gallagher was imperious and snuffed out Rodri. In attack Palmer was ice cold as per; fair play to Sterling with a great goal and Jackson with a great assist and good work all match.In part two we ask are we better as a counterattack team, getting the ball forward quickly, mixed with long balls makes us more threatening but it doesn't work against low block teams? But we still struggle to finish and need a striker. Nkunku was poor when he came on, struggling to adjust to the pace of the game, perhaps not match sharp but he didn't help with the defence. Chelsea fought like a team and it looks like a bond and understanding is growing. The fight and spirit was good to see and is something to build on. But did Poch do the right thing - should we have gone for the win? 5 at the back made sense as City were getting round the back. Jackson and Palmer on yellow cards was perhaps an issue, but was it negative or sensible?When all is said and done, Poch got the tactics and selection right while Pep is salty and still bald. Does this performance give us hope if not expectation against Liverpool next weekend? Most thought we'd get hammered v City so this performance should give us confidence. The injury curse strikes again as Colwill withdraws but his replacement Gilchrist did well. Chelsea were dominant in the first half but, wash, rinse, repeat - can't score, although Martinez is a top keeper and kept them in it. Chelsea were on the front foot, pressing and were up for it and the atmosphere was rocking as a result. In the second half we tired, and Petrovic kept us in it.We ask was the Ref Taylor in disguise? And discuss Mudryk and Broja being truly hopeless; it was like playing with 9 men when they came on. But it was a very good performance, especially the pressing, just a frustrating result. We potentially have Leeds at home next if we can beat Villa in the replay – but can we?In part two we preview the Liverpool v Chelsea match on Wednesday night, kicking off with Chidge's team selection. Stamford Chidge & Jonathan Kydd are joined by Marco Worrall, Chelsea author and Gate 17 publisher to look back at Chelsea's 1-0 win over Fulham.In part one we discuss Chelsea's laborious first half AGAIN with too many touches and often poor ones; too slow in the build up and of course no idea how to hit the back of the net. Until referee Anthony Taylor shocked the entire world by awarding Chelsea a penalty at the end of the half, which Cole Palmer put away for his 9th goal of the season.The second half was much better with more energy, pressing and more direct with both Sterling and Gallagher hitting the post. But Chelsea couldn't kill the game off and the last 10 minutes was a shambles, aided and abetted by Taylor and Keystone Kops defending, with stupid free kicks given away, running into defenders and wrong passes. At least we cheered the hoofs up field as Chelsea eventually saw the game out, but it was stressful!In part two we praise Petrovic who was quietly impressive again. Should he be Chelsea's No.1 keeper? Enzo was more dynamic and Caicedo did good terrier like work. Colwill had a decent game albeit not suited to the left back role. Just as well Chilwell came on with 23 minutes to go, but replacing Sterling as winger not Colwill at left back! A win is a win albeit a marginally ugly one but are there genuinely signs of encouragement and improvement? The stats don't lie. Stamford Chidge & Jonathan Kydd are joined by Martin Wickham to look back on muggy 3-1 defeat away to West Ham.In part one we discuss another poor start, a great goal by Chukwuemeka and a poor penalty from Enzo, which arguably turned the match. Why didn't Jackson take it? And then a comedy of errors from Disasi and Colwill led to an Antonio scorcher which turned it in West Ham's favour.In part two we discuss whether Mudryk is being weighed down by expectations. Sterling gets the plaudits for a decent performance. Jackson impressed again but was out muscled by two giants, on his own and crowded out. Caicedo probably shouldn't have come on and unsurprisingly looked at sea and off the pace - not his fault. Pochettino needs to sort his system out and play the formation that best suits the players he has available. West Ham away was a reality check and it's going to take time to gel this team. Nick is joined by Jéssica Frota (@Je_Frota) as Chelsea's premier league season got under way with a 1-1 draw vs Liverpool thanks to Axel Disasi's debut goal.  It was an encouraging performance as Chelsea came from a goal down to earn a point in Poch's first game. It was a shaky start but after that lots of players grew into it with Colwill, Enzo, Gallagher & Jackson singled out for praise.  With Nkunku's injury worse than initially feared it's a blow & we discuss the impact that will have as well as the opportunity it opened up for Carney Chukwuemeka who slowly grew into the game.  Robert Sanchez's debut is assessed as well as Kepa's seemingly imminent loan to Madrid with Jéss giving her verdict on who she'd like Chelsea to sign.  Reece James & Ben Chilwell we're announced as captain & vice captains & we give our thoughts on that. The Moisés Caicedo saga is coming to an end, Jéss shares her excitement on his imminent arrival. A deal for Tyler Adams fell through which Nick isn't sorry about & finally Romeo Lavia could also join. That'd be nice.
