Glioblastoma. It's the most aggressive and least treatable type of brain cancer. Research into treatment has been slow, mostly because of the difficulty in accessing and treating a tumor that exists behind the blood-brain barrier.
 A new two year fellowship into Glioblastoma Multiforme, or GBM, aims to better drug therapies to fight the cancer while developing better tests to determine which drugs are working on an individual patient's tumor and which are not. 
 The fellowship, established by Shawn Lane and the Lori Lane Foundation together with Chris and Florence Spyrow, honors Lori Lane and Andrew Spyrow, whose lives were cut short by GBM.

This episode features an interview with Sandra Hillburn, who was diagnosed with glioblastoma in 2006, and Dr. Deborah Doroshow, an oncologist at the Tisch Cancer Institute, Icahn School of Medicine at Mount Sinai. Doroshow, who is also a historian of medicine, is a member of the editorial board of the Cancer History Project. When Hillburn was first diagnosed, she was given two to three months to live. Then, she sought treatment at Duke Cancer Institute, where she underwent surgery to remove the tumor. Afterward, she began receiving an experimental CMV vaccine treatment. The treatment worked. Dr. Mariela Roca is a mother, veteran, and dedicated civil servant. Raised by a strong and independent single mother, Mariela knows all too well how hard work, sacrifice, and dedication are key to living a meaningful and successful life. Mariela was born in San Juan, Puerto Rico and at an early age moved to Queens, NY with her mother and older brother. She obtained a public school education, which continued when her family moved back to Puerto Rico at the age of 13. There, she finished middle and high school and attended the University of Puerto Rico for her first year of college. Inspired by her older brother's military service within the United States Marine Corps, she decided to herself enlist in the United States Air Force. During her time in the Air Force, Mariela served as a Medical Logistician providing lifesaving Medical Logistics support to military warfighters across all services both stateside, and while deployed overseas. While on her second deployment, tragedy struck when her mother, Lydia, was unexpectedly diagnosed with Stage-4 Glioblastoma Multiforme, a fast-growing and aggressive brain tumor. Mariela quickly became her primary caregiver and knows firsthand how the stress and burden of caring for a sick loved one can take on someone. In addition, Mariela understands the importance of making prescription drugs affordable, especially for life-saving drugs. After her military service, Mariela relocated to the Frederick area and continued her Medical Logistics career as a Federal Employee working for the United States Army at Fort Detrick. Mariela holds a Master of Business Administration from Mount Saint Mary's University in Emmitsburg, MD and a Doctor of Business Administration from the University of Maryland Global Campus. Senator McCain has a malignant brain tumor called a Glioblastoma Multiforme, orglioblastoma or GBM for short. He’s not the first person politically connected to get one of these things. Glioblastoma multiforme (GBM) is the most common malignant primary brain tumour in adults with a median survival, despite of multimodal aggressive therapy, of only 15 months. Relapse occurs inevitably because of the infiltrative nature of GBM. To date, only the MGMT promoter methylation is a reliable marker for therapy sensitivity. Survival-associated prognostic factors as well as novel therapy targets are urgently needed. Multiple genetic and metabolic pathway alterations contribute to tumour progression and therapy resistance. Recently proposed CSC markers for solid cancers include the aldehyde dehydrogenase (ALDH) superfamily. This cytoplasmic enzyme family consists of 19 different isoforms. The ALDH enzymes act upon oxidative stress and participate in proliferation, differentiation, and cell cycle arrest. The evolutionary conservation of the protein family enables comparative considerations of different species. In the presented study, isotype specific expression of ALDH is analysed in human GBM tumours and in two commonly used mouse GBM models. Expression of ALDH in the mouse models is then compared with the human GBM to find a suitable model for further research. The presented results indicate that there is, though challenging, a necessity for isotype specific analysis of ALDH expression. ALDH1A1 immunohistochemical expression in human PGBM was found primarily in the tumour adjacent region, whereas ALDH1A3 positive cells were more frequently found among tumour cells. Prognostic relevance for PGBM patients’ outcome was found for the ALDH1 immunohistochemical expression. Moreover, female PGBM patients were shown to have prolonged survival if neither ALDH1A1 nor ALDH1A3 expression is present. For male PGBM patients, ALDH1A1 and ALDH1A3 immunohistochemical expression could not be correlated to the medium overall survival. The reasons for this gender difference remain yet undetermined. Both the murine and the human GBM cells analysed in this thesis did not show ALDH1A1 immunohistochemical expression in cell culture or after implantation. The expression of ALDH1A3 is inhomogeneous in the analysed groups, raising further questions, which will be investigated in the future.The investigative approach of this thesis showed that the analysed canine GBM samples express ALDH1A3 but not ALDH1A1. This difference to human PGBM tumours in ALDH expression can help to understand more about the metabolic processes in tumour cells and the reactions to the tumour cells in the surrounding tissue. Finally, there are two particularly promising research subjects for future investigations: the gender specific prognostic power of ALDH expression in PGBM patients and the reason for the change in ALDH1A3 expression between in vitro and in vivo conditions. This knowledge can contribute to finding new targets for PGBM therapy and to prolonging patient survival.

Die Behandlung und auch die Prognose hängen stark von der Operabilität, Lokalisation und anderen Faktoren, wie z.B. MGMT-Status, ab. Andere konventionelle bildgebende Verfahren, wie MRT oder CT, konnten bislang, insbesondere nach Therapiebeginn aufgrund von unspezifischen Entzündungsreaktionen, keine sichere Aussage bezüglich Tumoraktivität und Therapieansprechen treffen. Das vorrangige Ziel bestand daher unter anderem darin, durch nicht-invasive Maßnahmen den Krankheitsverlauf des Patienten im Voraus einschätzen zu können, um hierdurch eine Therapie speziell an die Bedürfnisse des Patienten anzupassen, aber auch darin, eine bereits begonnene Therapie auf ihren Erfolg überprüfen zu können. Anhand der Daten durch FET-PET-Untersuchungen zu verschiedenen Zeitpunkten innerhalb eines Therapieprotokolls wurden 79 Patienten auf eine Korrelation zwischen Volumen, SUVmax/BG bzw. Kinetik und Gesamtüberlebenszeit bzw. progressionsfreier Zeit untersucht. Es stellte sich heraus, dass alle drei untersuchten Marker (SUVmax/BG, Volumen, Kinetik) in der Eingangsuntersuchung geeignete Marker sind, um sowohl die progressionsfreie Zeit als auch die Gesamtüberlebenszeit bei einem Patienten einschätzen zu können. Hier erscheint insbesondere das BTV vor Radio-Chemotherapie interessant, da es mit PFS und GÜZ korreliert. Ebenso konnte gezeigt werden, dass nach Therapiebeginn die Untersuchung der Kinetik ein hervorragender Marker ist, um prognostische Aussagen bezüglich des weiteren Krankheitsverlaufs treffen zu können. Ein Therapieansprechen und, im Falle einer Operation, ein Resttumor können erkannt werden. Zusammenfassend lässt sich sagen, dass die additive FET-PET einen festen Stellenwert in der Diagnostik und der Beurteilung des Therapieansprechens bei Glioblastoma multiforme haben könnte, da mit den hier untersuchten Markern bessere Ergebnisse erzielt werden konnten als mit den konventionellen Standardverfahren.
