Today we will be discussing an approach to a low TSH. We will be going over 2 review studies from The Journal of Clinical Endocrinology and Metabolism  and then Cleveland Clinic Journal of Medicine. But first today's question: A 50-year-old postmenopausal woman with no other notable history presenting with palpitations, frequent bowel movements, and tremors. She has no family history of thyroid dysfunction. She has mild tachycardia. Her thyroid gland is 20 g and nontender to palpation. Her TSH is < 0.1. What is the best next step in evaluating this patient?A Thyrotropin receptor antibodiesB Check Total T4C Check Free T4 and T3D Thyroid US And the answer is T4 and Total T3. These questions, while seemingly simple, are actually guaranteed on board exams. Initial thyroid function evaluation should start with TSH. In this case the TSH is low and there is clinical suspicion for hyperthyroidism. If TSH is suppressed, the immediate next step is to check T4 and T3 to confirm and further characterize the thyroid dysfunction (overt vs subclinical). This is worth repeating: investigating the cause of the suppressed TSH by checking thyroid hormones is important in determining how to proceed with evaluation and treatment.   In this question, checking the Free T4 is favored over total T4 because Total T4 levels can be affected by alterations in binding proteins.  Checking T4 only is not sufficient because there are cases of isolated T3 thyrotoxicosis. Ordering receptor antibodies or starting treatment are premature at this point. Thyroid US is not a routine part of the diagnostic algorithm to hyperthyroidism. For this case, an iodine uptake scan could be performed to differentiate thyroiditis from true hyperthyroidism, but this would not be the best next step in this question. An Approach to Low TSHIf the T4 and T3 level are normal, repeat TSH, T4, and T3 in 6-8 weeks before giving a diagnosis. When TSH suppression is transient, most times thyroid dysfunction will be resolved in this time. A suppressed TSH that is not normalized in this period requires more investigation. Low TSH can be differentiated by level of TSH suppression such as mild (TSH 0.1 – 0.4 mIU/L) milli-international units per liter and complete TSH suppression TSH < 0.1 mIU/L. It is unclear the incidence of low TSH within the population but in a representative sample of the US without known thyroid condition that 0.7% of patients had suppressed TSH (< 0.1 mU/L) and 1.8% of patients had a TSH level below the reference range (< 0.4 mU/L) It can be helpful to think of the etiologies of low TSH 1) in their relation to the pituitary/hypothalamus or 2) in terms of accuracy of the assay measurement / drug effect Relationship to Pituitary/Hypothalamus·       Category #1: low TSH due to  an appropriate pituitary response to high thyroid hormone, the pituitary is actively attempting to reduce thyroid hormone production because of advanced or early elevated thyroid hormone levels In this category differentiating the source of the excess thyroid hormone can be helpful·       #1 Excess endogenous thyroid hormone production from multinodular goiter, autonomous thyroid nodule, Graves' disease,·       #2 Exogenous thyroid hormone commonly from excess levothyroxine supplementation (iatrogenic or intentional in context of high risk thyroid cancer) or ingestion of natural thyroid preparations (athletic performance and integrative health) – in   these cases exogenous T4 is suppressing TSSupport the show

Increasing the consumption.  Cholesterol is essential for life and is important for cell formation, nutrient absorption, hormone production, and healing. LDL is often labelled as "bad" cholesterol, but it is responsible for carrying out important functions in the body. The level of LDL can vary from person to person and it's important to check whether it is damaged or not. High cholesterol is not a bad thing and can be beneficial for our health. This week I interview longevity queen, Dr Sandra Kaufmann the author of the Kaufmann Protocol and Aging Solutions. Dr Sandra is the head of department of Anesthesiology at  and just happens to be obsessed with longevity science. What started out as her own search to slow and reverse ageing turned into a comprehensive system that allows people to personalize their own regime to their particular set of problems or needs and goals. This ingenious system has revolutionized my approach and makes it so simple to construct a very personalized approach. There are innumerable things a person can read concerning aging, but at the most basic level, organisms age because their component cells age. We are all made of cells or cell products and as they falter, so do we. Because cell aging is multifactorial, the organization of these concepts is important. The Protocol organizes these various theories of cellular aging into seven categories or tenets. The information in this podcast discusses these concepts in the language of science. The following seven tenets encompass all of the presently known modes of cellular aging.  Based on this organizational scheme, Dr Kaufman then developed a strategy where aging is curtailed in every category.   The 7 Tenets are as follows: DNA alterations (telomere attrition, epigenetics) Mitochondrial failure Pathways (Sirtuins, AMP Kinase, mTOR) Quality control (DNA & Protein repair mechanisms and Autophagy) Immune and inflammatory systems Individual cell needs (Senescent, Stem cells) Waste management (Glycation, lipofuscin) Dr Kaufman explains her rating system for various longevity molecules and how to utilize these in your protocols. We also discuss many of the supplements and adjuvants that Dr Kaufmann recommends for each of these tenets and we look at other cutting edge regenerative modalities like Exosomes.  Stem cells are fantastic in terms of longevity, however there are limitations. Exogenous stem cells are currently not available in this country. Meanwhile, autologous stem cells are only as healthy as the person they which they come. Exosomes, on the other hand, constitute up to 80% of the benefits seen with stem cells and are available through Dr Kaufmann in the United States at least,  in addition to being less expensive. I highly recommend Dr Kaufman's books, these are major reference book that will serve those looking to optimise their health, performance and longevity. Key Dietary Recommendations: Intermittent Fasting (IF): IF for even 13 hours has shown to reduce the recurrence of breast cancer. It activates autophagy (cell cleaning) and has other metabolic benefits. Diet Cleanliness: Remove junk and chemicals from diet. Plant-based Spectrum: High in polyphenols, fiber, dense nutrients, and low in simple carbs. Raw food diets can quickly get the body's attention. Ketogenic Spectrum: Shifts body metabolism and may be beneficial against certain types of cancers. It's crucial to balance protein intake since excess protein can be converted into sugar. Modified Mediterranean Diet: High plant-based with clean protein and fat sources, minimal grains. Diet Adaptability: Initial diet is about getting the body's attention; it might differ from the long-term diet. Both ketogenic and raw food diets have shown positive effects on patients with various cancer types. Adapt the diet based on what a person can do and tolerate. Long-term success requires consistency in dietary habits. Observational Cases: Noted a patient surviving stage 3/4 breast cancer for over 15 years primarily using a ketogenic diet. Other cases showed success with a raw food diet transitioned into a modified vegetarian diet. Regular Monitoring: Emphasizes tracking certain blood work indicators (e.g., LDH, alkaline phosphatase, GGT) regularly. These markers can provide insights into inflammation, oxygen levels, and overall body health. Monitoring helps catch subtle shifts in the body's health, guiding adjustments in treatments and dietary habits. Cancer Stem Cells (CSCs) Insights: CSCs are smarter than regular tumor cells (daughter cells). While treatments like chemotherapy or radiation can reduce tumor size, they might make one prone to relapses because CSCs hide and wait. Many people have dormant CSCs in their bodies. The challenge is to keep these cells inactive. CSCs can recruit normal cells to become cancerous. CSC Triggers & Management: One major trigger for CSCs is inflammation. Inflammation sources include lack of exercise, poor diet, toxins, infections. Healthier lifestyles, such as better diets and regular exercise, produce anti-cancer metabolic triggers. Intermittent fasting and periodic longer fasts can slow CSC activity. Diet, detox, and addressing specific toxic exposures can help maintain an environment less favorable for CSC growth. NAD Therapy Insights: NAD is essential for energy production in mitochondria. High doses of NAD can be problematic for those with cancer because some cancers can utilize excess NAD for growth. For those with a cancer history, lower dose NAD therapy can be considered safe. In active cancer cases, high-dose NAD therapy should be avoided. His father passed away from a stroke, and his brother passed away from a heart attack. Growing up, Dr. Mansor was overweight. During his undergrad, Dr. Mansor learned about physiology and metabolism. Plus, during college, Dr. Mansor started to exercise and actually enjoyed exercise for the first time in his life. With all this knowledge, Dr. Mansor realized he could be healthy without worrying about the diseases his family members suffered from.  [10:35] Why Are 88% of American Adults Metabolically Inflexible?  Your nerves have a higher risk of getting damaged when there is a constant increase in elevated glucose in the blood. Your body knows something is wrong with increased glucose, which triggers chronic inflammation. Metabolism is all about balance and moderation.  Every enzyme has a specific temperature range at which it's optimal.  When you activate inflammation constantly, there are bound to be adverse side effects.  [15:35] Life Is All About Challenges and Growth - Use Trial and Error To Find Your Sweet Spot  Life is all about giving yourself a challenge, adapting, learning, and growing. Next time you face the same challenge, you can tackle it in a much more efficient way.  It can be difficult to find the sweet spot when you are challenging yourself. Dr. Mansor uses a trial and error approach when it comes to knowing his sweet spot while exercising.  Your body will adapt. People want instant change; however, you need to give your body time to change.  [22:40] You Need To Train Your Body To Respond To Certain Conditions Effectively and Optimally  Metabolic inflexibility is your body's inability to switch from burning glucose to burning fat. When there is dysfunction, there is bound to be a deficiency, and there is bound to be wasted energy. Everyone thinks cortisol is the bad guy; however, you need cortisol to function.  The goal is not to shut down whatever is related to chronic disease. Instead, you want to train your body to respond to specific conditions effectively and optimally.  [30:20] Exogenous Ketones Are a Source of Energy and Can Help Lower Your Hunger Hormones Ketones are fuel for you to create energy.  There are still calories with exogenous ketones.  You won't lose weight if you have too many calories and are not exercising.  Ghrelin is the hunger hormone. Ketones can help lower that hunger hormone. [36:05] Exogenous Ketones Are Useful For Getting Back Into Ketosis  If you are following a ketogenic diet, you may not need exogenous ketones. However, do you have a hard time getting back into ketosis? If you fall out of ketosis, exogenous ketones can be a helpful way for you to get back into ketosis. Exogenous ketones are also a great tool to use before exercising.  [43:20] The Brain Doesn't Metabolize Glucose As Efficiently After A Traumatic Brain Injury  Dr. Mansor researched how ketones can be a therapeutic intervention to treat traumatic brain injury. Our attention has become a commodity and is getting hijacked by technology. Dr. Gloria Mark (@Dr.GloriaMark) is the author of a groundbreaking book called Attention Span. Attention Span is a must-read as it covers Dr. Mark's decades of research into how our minds have been affected in the digital age. Gloria received her Ph.D. from Columbia University in psychology. She studies the impact of digital media on people's lives. Her work is a deep dive into examining multitasking, interruptions, mood, and productivity due to digital devices becoming so intricate in our lives. She has published over 200 articles and has presented her work at multiple venues.   Author and Creative Dr. Gloria Mark(@Dr.GloriaMark) is a Chancellor's professor of Informatics at the University of California Irvine. And you can think of it as your attentional capacity. And it's limited, these resources are limited, and they're very precious.” - Dr. Gloria Mark   Key Takeaways:   3 Types of Attention: Intentional attention is when you use your mental focus to do an activity such as writing, lifting weights, or scuba diving. Exogenous attention, or the default mode network, is when we respond to things outside of us. Kinetic attention is shifting from an iPhone to a computer or from one screen to another. Recent studies show that most people average 40 seconds per screen.   Technology as and Extension of Self: Devices like cellular phones and computers have become an extension of ourselves. Our lives have become intimately intertwined with their devices in this modern age. However, where is the line, and how can we draw it? Learning to be sovereign from our devices and drawing a healthy line for our identity is important.   Switch Cost: Every time you switch your attention, you incur an extra cost in time. Imagine you have an internal whiteboard in your mind and are writing everything you need for a specific task; if you switch in the middle of that list, you are erasing the board for the new task you have changed to. Therefore, when you return to the first task, it takes longer to reorient yourself. Multitasking or switching focus takes time and creates exhaustion and a lack of clarity.   Saying No in Service to Your Yes: Monotasking, a good night's sleep, and taking breaks throughout your day allows you to have balanced energy levels and replenish them if needed.
