The challenges of opening a second or third clinic in your PT practice aren't the same challenges that you face when opening the first clinic. As you scale, the need for more robust systems and processes increases. When your marketing goes after everybody, you are actually going after nobody because you attract some people, but they can't look at you and say exactly who you serve. You are not different from anybody else out there! There are a lot of brands that are big, and there are also small brands that are doing the same thing that you do. The difference between what you do and how you do it is the message. Internally, you are never as good as you think you are, and you are never as bad as you think you are. How do you Control your variables? There are so many crafts in the marketplace, and you should have a goal to rise above the crafts with quality content, images, and experience. Everything you have to do has to be about impact! Why in the beginning, stages, when you are just starting your business, you should say "yes" to everything? At what point do you have to learn to say "no"? Focus on what you do best to help the company grow. To be a good business owner, you have to learn to say no to almost everything. Sometimes I think we have a tendency to think we need some rock bottom or very large event to change the trajectory of our life. But I'd argue that's rarely the case. Yes a person can certainly have a near death experience or some kind of epiphany. That's not my personal experience or expectation for you. Today's episode is about both decisions, and also outside scenarios that lead to a large or small change to the path of my adult life. That's the other important piece of this context - I am referring to my adult life. When I feel I mostly had autonomy and control over my life choices. I've organize these in chronological order. And to be fair, there is actually six of these. Because I just thought of a prequel to what I have listed here. 6 experiences that changed the trajectory of my life: After graduating high school with honors, in every after school club, and a three sport varsity athlete I decided I'd take off a semester because I hadn't fully decided on what I'd go to college for, and I didn't see the point of racking up debt until I knew what that debt was going for. I had no desire to leave home, I was coaching competitive cheer locally and was happy with that. I am sure my mother was dying inside. Fast forward - I enroll in a local community college in their fitness training associates program. I was deciding between fashion school, cosmetology or being a trainer. And landed on being a trainer. But wanted to go to school, not just get a cert. In my second to last semester, we had to interview someone who was doing what we wanted to do. So, I knew of a local strength and conditioning coach who worked with a few MMA fighters in the UFC. He was willing to have me interview him and he told me I needed my CSCS, certainly if I wanted to work in college athletics. But that required a 4 year degree. After that interview, within a week, I was shifting my whole life, researching Exercise Science, Kinesiology, or biomechanics programs at local universities. Found Concordia, met with their advisor and changed all classes at my community college to essentially set me up for an easy transition to their exercise and science program in a year. All because of that one interview when I was 19 years old. Thanks to him, I had a new route for my life, which very much so lead to the next 5 trajectory changes. Me declining two dream jobs Fast forward to 2013/2014. I complete my ESS degree, get my CSCS, all to decide I wasn't going into college strength and performance. You'd think that decision alone was the life changing one, but it was the two that came after that really set up the new direction. I'd interned at two places - University of Portland as a strength coach, and a Physical Therapy Clinic that focused on the gap between physical therapy and return to sport. I'd had a lot of injuries and was always fascinated by the PT world. I used to joke with them that I wish I could get a PT education but use it to train normal people. So, did't honestly love my experience at the PT clinic but did love and support the owner. After my internship I was offered the job of my supervisor. Which never sat right with me, because I was fairly sure he was unaware of this - though I was not one of his fans. Side note, another mentor told me while I was interning there that I should take it as an opportunity to learn what I don't like, what I won't do, what doesn't make sense to me. So, I did. The pay was good, it would be an amazing job right out of college, but it was also an hour commute and the hours were shit. And, my gut just said no. I applied for a personal training gym locally but they required you to fill in with group/class coaching and I fucking hated that with every ounce of my being. I got offered that job as well, was told I was overqualified. And I just said no. It didn't feel right. And perhaps that sounds super outrageous for someone who is fresh out of college.

There is no shortage of individuals who deal with aches and pains. In fact, according to the CDC, an estimated 20.4% of the American public deals with chronic pain. Many more individuals deal with subclinical levels of pain and injuries every day. This limits functional capacity and increases fearfulness around physical activity and exercise. However, when exercise is properly performed, it can reduce pain, improve function, and most importantly increase confidence in physical abilities. Proper performance, that is proper prescription and progression, are critical components of helping people reduce chronic pain, prevent injuries, and live the lives they want to live. The nuts and bolts of how to go about prescribing exercise for this population will be the topic of conversation in episode 61, with Charlie Rowe.Charlie has over 20 years in the field working as a fitness professional and currently works for Physicians Fitness, an organization that specializes in working with individuals who want to become more active through exercise, but are concerned doing so might exacerbate their underlying orthopedic issues and/or pain. If you may not know, when finally deciding to work in the healthcare field, there are many different avenues in which to embark upon your journey. Whether it's social work, speech pathology, nursing, or even a neurologist, all can be very rewarding but involve various levels of skill and perseverance in order to achieve your desired outcome of being a provider in the healthcare space. When considering each career path, our ultimate decision can be summed but not limited to a few factors for our chosen profession. First and foremost, when choosing which field to go into, our first consideration is how hard the classes are for the desired profession. For some, the idea of overcoming organic chemistry and calculus 2 to become a physician is just a small notch in the belt of a long career in learning. However, to some, the simple thought of those same classes is enough for some to choose other options in the healthcare field. Second, our next consideration is often compensation in terms of how much I will make and how much am I going to need in order to pay off my loans if I get this job. Of course, this varies greatly depending on your position in healthcare and responsibilities you take on. And lastly, ironically enough, the third factor we consider is will this job be fulfilling in the sense of, do I feel like I am doing the greatest amount of good or am I helping the greatest amount of people with the career I am going to partake in. None of these factors are in any particular order but for our next guest, it was ultimately the third factor mentioned was the deciding factor that lead him into the field of becoming a doctor of physical therapyOur next guest is a close friend of mine who I had the pleasure of meeting in PT school. Originally from Upstate NY, our next guest shares his unique story into the healthcare space and how he has advanced not only as a clinician, but as an adult trying to navigate the perils of determining your profession in healthcare. As we hear more about his story, we will also get to hear how he has successfully started to run his own clinic in Nashville Tennessee as part of a larger organization just a short 2 years after graduating PT school. The number of Iowans hospitalized with COVID-19 increased overnight as the state added 667 new positive cases and seven additional deaths, including three more in Woodbury County reported on Tuesday night, for a total of 67 people. More than 87,500 Iowans have tested positive for the disease, with 77% recovered. Thirteen counties are above a 14-day positivity rate of 15%. Lyon is top in the state with almost 29% with Sioux falling to second with almost 28%. Woodbury County is down to 15%. Any county with the 15% rate or high does meet the threshold to apply for virtual learning only with the state. Two test Iowa clinics are opening up in Orange City and Sioux City to expand testing in Sioux County. Information from the IDPH: Starting today (Tuesday), testing will be available at the Orange City Area Health System Occupational Health and Physical Therapy Clinic, located at 1000 Lincoln Circle SE. The Paul Gough Audio Experience: Business Lessons for Physical Therapists
Every successful Physical Therapy Clinic has one thing in common - they are able to command DRASTICALLY higher prices than their competition. They are not only able to charge higher prices than most - they're able to do it without the fear of loosing any patients. At my own clinic - the Paul Gough Physio Rooms - we are charging the equivalent of “$150” for a visit. When most of the private physio clinic owners in my area are charing “$60” equivalent (at best)… So how can we charge 150% more than them for providing the “same service” - and without fear of losing all our patients? Well, that's something that I talk about in-depth on this latest Episode of the Podcast - and comes in response to talking to a PT from Orlando, who told me that she's only “able” to charge “$80” for a one hour session. My response?: “What the hell is going wrong…”? Because if you don't feel as though you can charge higher fees - for fear of loosing your patients - then something is going severely wrong in your the way you run your business… On this episode I show you why charging “low” is not only hurting your profits on a day to day basis - it's possible that by being so low, people wont actually ever come and see you. On the flip-side of that… There's providers out there who patients are choosing simply because they are DRASTICALLY more expensive than anyone else. When it comes to health - people really do not want to see the “cheapest” but if you're in the pack - charging close to what everyone else is and the difference is only $5 or $10 per visit, then they will always go with the lowest… If you are $100 more expensive - they start to ask why you're so different. Who do you want to be? In the pack? Or, In a league of your own - with people asking you how and why you're able to command so much more than the others…? I know which one is more healthy and more profitable and I'm never going back into to the pack! In this episode I interview Isaac Osborne, Advanced Practitioner of Structural Integration, LMT, and creator and founder of imaging software Body Align Pro and inventor of the Sqr Shot device holder about his practice and his recent experience as an attendee and exhibitor at the 4th Fascial Research Congress in Reston Virginia. A brief bio: Isaac has 17 years in the chronic pain industry. He began his bodywork training while living on Kauai learning Hawaiian Lomi Lomi, Swedish Massage, Deep Tissue Massage, Shiatsu, Lymph Drainage Massage, American and Russian Sports Massage, Craino-sacral Therapy, Thai Yoga Therapy, and Myofascial Release. For 2 years Isaac was the lead medical massage therapist in a Physical Therapy Clinic where he focused on pre and post surgical care, work comp injuries and auto injuries. Realizing that the techniques he was using could be improved to help his clients (and himself because he was suffering from his own chronic low back and neck pain for over 12 years). He sought out the Rolf Method of Structural Integration to further enhance his training and address his pain. Isaac became certified in the method and incorporated the technique into his practice at his own clinic that he started after leaving the physical therapy clinic. After a couple of years of practicing the Rolf Method and getting his advanced certification Isaac was invited to teach with his mentor Emmett Hutchins at the Guild for Structural Integration. Isaac has taught with them since 2005 with beginning, advanced classes, and workshops and also taught a class in Japan. The next progression in his training occurred when Isaac discovered the Egoscue Method of Posture Alignment Therapy. Learning the Egoscue Method felt like the missing puzzle piece that allowed him total freedom from his remaining pain of over 16 years. An Accredited Exercise Therapist from Egoscue University and former Instructor for Egoscue University, Isaac combines his extensive experience and passion for sharing his knowledge to help people free themselves of chronic pain, address their postural compensations, and improve athletic performance. Isaac Osborne managed the imaging software for the Guild for Structural Integration for several years and has used imaging software in his clinic for over 10 years.
