BUFFALO, NY- October 25, 2023 – A new research paper was published in Aging (listed by MEDLINE/PubMed as "Aging (Albany NY)" and "Aging-US" by Web of Science) Volume 15, Issue 19, entitled, “Live while the DNA lasts. The role of autophagy in DNA loss and survival of diploid yeast cells during chronological aging.” Aging is inevitable and affects all cell types. Thus, yeast cells are often used as a model in aging studies. There are two approaches to studying aging in yeast: replicative aging, which describes the proliferative potential of cells, and chronological aging, which is used for studying post-mitotic cells. In this new study, while analyzing the chronological lifespan (CLS) of diploid Saccharomyces cerevisiae cells, researchers Tuguldur Enkhbaatar, Marek Skoneczny, Karolina Stępień, Mateusz Mołoń, and Adrianna Skoneczna from the Polish Academy of Sciences and Rzeszów University discovered a remarkable phenomenon: ploidy reduction during aging progression. “To uncover the mechanism behind this unusual process we used yeast strains undergoing a CLS assay, looking for various aging parameters.” Cell mortality, regrowth ability, autophagy induction and cellular DNA content measurements indicated that during the CLS assay, dying cells lost their DNA, and only diploids survived. The researchers demonstrated that autophagy was responsible for the gradual loss of DNA. The nucleophagy marker activation at the start of the CLS experiment correlated with the significant drop in cell viability. The activation of piecemeal microautophagy of nucleus (PMN) markers appeared to accompany the chronological aging process until the end. “Our findings emphasize the significance of maintaining at least one intact copy of the genome for the survival of post-mitotic diploid cells.” During chronological aging, cellular components, including DNA, are exposed to increasing stress, leading to DNA damage and fragmentation in aging cells. The researchers propose that PMN-dependent clearance of damaged DNA from the nucleus helps prevent genome rearrangements. Episode 123: Spontaneous Bacterial Peritonitis. Kaitlen defines spontaneous bacterial peritonitis (SBP) and also explains the diagnosis and management. Written by Kaitlen Roy-Ross, MS4, Ross University School of Medicine. Moderated by Hector Arreaza, MD. Definition:An ascitic fluid infection with no obvious surgically treatable intra-abdominal source (bowel perforation, abscess, perforated ulcer). Commonly seen in patients with cirrhosis and ascites. Patients may have symptoms of fever, abdominal pain, abdominal tenderness, altered mental status, and hypotension.Etiology: The most common pathogens (75%) are gram-negative aerobic organisms. Klebsiellapneumoniae accounts for 50% of the cases. Gram-positive aerobic bacteria (Streptococcus pneumoniae or viridans group streptococcus) account for the remaining cases. Some report E. coli as the most common cause of SBP. Random information: in Korea, Aeromonas hydrophila is an important pathogen of SBP during the summer. Diagnosis: To diagnose SBP, a paracentesis should be performed to analyze the ascitic fluid prior to treating the patient with antibiotics.The ascitic fluid should be analyzed for the following: PMN (Polymorphonuclear cell) count: > or = to 250 cells/mm3 Aerobic and anaerobic culturesSerum ascites albumin gradient (serum albumin-ascitic albumin): this measures portal pressure.If the gradient is > 1.1 = portal HTN is present (cirrhosis, heart failure, large liver malignancy, alcoholic hepatitis, portal vein thrombosis) – SBP is likely.If the gradient is 6 mg/ suggests a gallbladder perforation. No SBP.Treatment:The treatment for spontaneous bacterial peritonitis is broad-spectrum antibiotics. Empiric treatment is indicated if a patient with ascites has any of the following:Temperature > 100 FAbdominal pain or tendernessAltered mental statusPMN in ascitic fluid > 250 (but if there is bacteria in ascitic fluid, start antibiotics stat)Alcohol-induced hepatitis*Important note: Patients on beta blockers should have them permanently discontinued prior to treatment for SBP as beta blockers are associated with worse outcomes. In one study, patients on beta blockers had a 58% increase in mortality risk compared to patients not treated with beta-blockers. Beta-blockers were also associated with higher rates of hepatorenal syndrome and longer lengths of hospital stay.1st line treatment- 3rd generation Cephalosporin Cefotaxime 2g IV Q8H (preferred) or Ceftriaxone 2 g per day2nd line treatment- Carbapenems. Usually reserved for patients with severe disease/critical illness.3rd line- Fluoroquinolones- Cipro 400 mg IV BID to patients with normal renal function. (Patients should not get this if they already receiving it prophylactically.)Duration of treatment:5 days, then re-assess the patient's PMN count:PMN 250 or greater than pre-treatment PMN count > look for a surgical source of infection.If PMN is > 250 but less than pre-treatment value, continue ABX for 48 more hours and then repeat paracentesis. Note: In general, ascitic fluid PMN count should be reduced by at least 25% after 48 hours of antibiotic therapy.Renal failure is the major cause of death in patients with SBP and develops in 30-40 % of the patients. We can decrease this risk by administering IV albumin. IV albumin should be given when the creatinine is > 1 mg/dl, the blood urea nitrogen is > 30 mg/dl, or the total bilirubin is > 4 mg/dl. Treatment with octreotide or midodrine is helpful if renal failure develops.Prevention:Antibiotic prophylaxis can be given to patients with risk factors for SBP. Abstract: Background: Receptor-interacting serine/threonine-protein kinase 1 and 3 (RIPK1 and RIPK3) dependent cell death has been identified as a crucial mediator of abdominal aortic aneurysm (AAA) development. RIPK3 mediates phosphorylation of Mixed lineage kinase domain like pseudokinase (MLKL) thereby inducing its oligomerization and translocation to the cell membrane. Given the dual role of RIPKs being involved in necroptosis as well as in apoptosis induction, the specific role of MLKL-induced necroptotic cell death in AAA remains unclear. Methods: We monitored elastase-perfusion (PPE) induced progression of AAA in C57BL/6N (WT), RIPK1 kinase deficient (Ripk1D138N/D138N), MLKL knockout (Mlkl-/-) and MLKL phosphodeficient (MlklAA) mice by ultrasound measurements, histological analyses and bulk mRNAseq techniques to assess structural and molecular aortic changes. Bone marrow transplantation studies in WT and MlklAA mice were utilized to dissect the role of MLKL in smooth muscle cells (SMCs) and myeloid cells in AAA development. MLKL expressing human SMCs were generated to investigate necroptosis-induced proinflammatory cytokine secretion and subsequent polymorphonuclear neutrophil (PMN) migration and activation in vitro. Results: Ultrasound analysis showed that ~70% of the WT animals developed PPE induced-AAA with significant aortic structural alterations and enhanced myeloid cell infiltration. In contrast, Ripk1D138N/D138N, MlklAA, and Mlkl-/- mice were protected from AAA. This protection was associated with reduced adverse extracellular matrix (ECM) remodeling and leukocyte infiltration. MLKL deficiency was associated with a significant downregulation of genes involved in fibrinolysis, anti-inflammatory response, immune response and complement activation in aortic tissue in AAA. Bone marrow transplantation studies showed the lack of MLKL in SMCs to be the main driver of AAA protection. Proinflammatory cytokine secretion was elevated in necroptosis induced SMCs and resulted in a significant accumulation and activation of PMN. Conclusions: Overall, these findings indicate that MLKL-induced necroptotic SMC death and subsequent proinflammatory leukocyte activation plays a causative role in AAA development and suggests that pharmacological inhibition of MLKL may represent a promising treatment strategy for AAA disease. Abstract: According to most memory theories, encoding involves continuous communication between the hippocampus and neocortex leaving the temporal dynamics of hippocampal-neocortical interactions often overlooked. Recent work has shown that we perceive complex events in our lives as dynamic, with relatively distinct starting and stopping points known as event boundaries. Event boundaries may be important for memory, as they are associated with increased activity in the hippocampus, and extended neocortical regions (the posterior cingulate cortex, lateral parietal cortex, and parahippocampal cortex). Our objective was to determine how functional connectivity between the hippocampus and neocortical regions during the encoding of naturalistic events (movies) related to subsequent retrieval and retention of those events. Participants encoded two 16-minute cartoon movies during fMRI scanning. After encoding, participants freely recalled one of the movies immediately, and the other after a 2-day delay. We quantified hippocampal-neocortical functional connectivity (FC) at time windows around each event onset, middle, and offset, and compared these FC measures with subsequent recall. These analyses revealed that higher FC between the hippocampus and the posterior medial network (PMN) at an event's offset related to whether that event was subsequently recalled. In contrast, mid-event connectivity between the hippocampus and PMN was associated with poorer memory. Furthermore, hippocampal-PMN offset connectivity predicted not only whether events were retained in memory, but also the degree to which these events could be recalled in detail after a 2-day delay. These data demonstrate that the relationship between memory encoding and hippocampal-neocortical interaction is more dynamic than suggested by most memory theories, and they converge with recent modeling work suggesting that event offset is an optimal time for encoding. The study included 1,188 women over the age of 75 years enrolled in the Calcium Intake Fracture Outcome Study, which evaluated the effect of calcium supplementation in the prevention of osteoporosis. Tea intake was assessed at the beginning of the study and at two and five years. The subjects were followed for ten years, during which 288 women developed an osteoporotic fracture, including 212 major fractures and 129 hip fractures. Among women whose intake of tea was three cups or higher per day, there was a 30% decrease in the risk of any osteoporotic fracture in comparison with those whose intake was a cup or less per week. Subjects whose flavonoid intake from tea and foods was among the highest one-third of subjects had risks of osteoporotic fracture, major osteoporotic fracture and hip fracture that were 35%, 34% and 42% lower than those whose intake was among the lowest third. When individual flavonoids were analyzed, higher consumption of flavonols, flavan-3-ols and flavones was significantly associated with a protective effect against osteoporotic fracture risk. “The current study found that flavonoid intake was associated with a reduced risk of hip, major, and all osteoporotic fractures in elderly women,” write authors Gael Myers and colleagues. “The major flavonoids found in tea, flavan-3-ols, and flavonols were also associated with a reduced fracture risk, providing evidence for the role of tea flavonoids in promoting bone health.” Skipping breakfast may increase chance of kids and teens developing psychosocial health problems University of Castilla-La Mancha (Spain), August 23, 2022 Young people who eat healthy breakfasts at home have better psychosocial health, shows a recent study in Frontiers in Nutrition. While previous research has reported the important role of a nutritious breakfast, this is the first study to look at the reported effects of whether kids eat breakfast, as well as where and what they eat. These results provide valuable insights and recommendations for parents and their children. “Our results suggest that it is not only important to eat breakfast, but it's also important where young people eat breakfast and what they eat,” said first author Dr. José Francisco López-Gil of the University of Castilla-La Mancha in Cuenca, Spain. “Skipping breakfast or eating breakfast away from home is associated with increased likelihood of psychosocial behavioral problems in children and adolescents. Similarly, consumption of certain foods/drinks are associated with higher (eg, processed meat) or lower (eg, dairies, cereals) odds of psychosocial behavioral problems.” Among the most important results, López-Gil and the team found that eating breakfast away from home was nearly as detrimental as skipping the meal entirely. The authors suggest that this may be because meals away from home are frequently less nutritious than those prepared at home. The results also showed that coffee, milk, tea, chocolate, cocoa, yogurt, bread, toast, cereals, and pastries were all associated with lower chances of behavioral problems. Surprisingly, eggs, cheese, and ham were linked with higher risks of such issues. Amla Tea in Hospitalized Patients With COVID-19 Ahvaz Jundishapur University of Medical Sciences (Iran), August 14, 2022 Randomized, double-blind, placebo-controlled trial to evaluate whether the addition of Phyllanthus emblica (amla) tea to standard protocols affects lung function, symptomology, and length of hospital stay in a population of hospitalized patients with Covid-19 While amla tea did not reduce the severity of lung involvement nor significantly affect the reverse transcription–polymerase chain reaction (RT-PCR) results, it did lessen severity of symptomology and C-reactive protein (CRP) levels. Duration of hospital stay was significantly shorterin those taking amla versus placebo. First-line therapy for all patients consisted of hydroxychloroquine tablets (200 mg) and lopinavir/ritonavir (Kaletra) every 12 hours after meals for 7 to 14 days. Phyllanthus emblica (Linn), Euphorbiaceae, known as Indian gooseberry or amla or Emblica officinalis, 2 grams of powder in a sachet for 10 days was the intervention, and the placebo was starch, 2 grams of powder daily for 10 days. Both treatments were taken as 100 mL tea every 12 hours. Hospital nurses administered the treatments every 12 hours for 10 days, and the study team tracked the patients with daily phone calls. Key Findings Lymphocytes decreased significantly in the intervention group but increased significantly in the control group (P=0.001). There was no significant difference in hemoglobin, polymorphonuclear (PMN) count, platelets, or erythrocyte sedimentation rate (ESR) between the intervention versus placebo groups. CRP was significantly less in the intervention group versus the control (P=0.004). Fever decreased in both groups, with a significantly greater reduction in the intervention group (P

On Sunday President Biden held a meeting with Turkish President Erdogan on the margins of the G20 Summit. Abstract: Brain regions within a posterior medial network (PMN) are characterized by sensitivity to episodic tasks, and they also demonstrate strong functional connectivity as part of the default network. Despite its cohesive structure, delineating the intranetwork organization and functional diversity of the PMN is crucial for understanding its contributions to multidimensional event cognition. Here, we probed functional connectivity of the PMN during movie watching to identify its pattern of connections and subnetwork functions in a split-sample replication of 136 participants. Consistent with prior findings of default network fractionation, we identified distinct PMN subsystems: a Ventral PM subsystem (retrosplenial cortex, parahippocampal cortex, posterior angular gyrus) and a Dorsal PM subsystem (medial prefrontal cortex, hippocampus, precuneus, posterior cingulate cortex, anterior angular gyrus). These subsystems were anchored by two complementary regions: Retrosplenial cortex mediated communication between parahippocampal cortex and the Dorsal PM system, and posterior cingulate cortex mediated communication among Dorsal PM regions. Finally, the distinction between PMN subsystems is functionally relevant: whereas both Dorsal and Ventral PM connectivity tracked the movie content, only Ventral PM connections increased in strength at event transitions and appeared sensitive to episodic memory. REALTORS® are being targeted more and more over the past ten years. With September being REALTOR® Safety Month, Donny Allen joins Monica on the show today to talk about safety on the job —best practices to keep yourself and your clients safe. Monica and Donny discuss effective ways to help REALTORS® live in confidence, not in fear. Donny grew up on large farms and ranches and had to learn to defend himself from the different animals and also gained a heightened mindset of awareness. When he got started in real estate, he found he had a passion for teaching REALTOR® safety. When you think about safety, it’s not just physical safety; it’s our reputation’s safety and financial safety. Situational awareness is key to the real estate business now more than ever. Some vulnerable situations are meeting clients at unfamiliar properties, sitting in an open house alone, and even issues like cybersecurity. Donny shares safety tips for meeting strangers or new clients. If you don’t know the client, try to meet them at the office first, rather than meeting them at the property. If you’re able, take someone to go along with you. Also, be conscious of what time you are meeting new clients, and arrive early to get a lay of the property. Monica talks about the protocol she uses for herself when meeting clients. It is important to have some sort of consistent system or policy so all potential clients are treated fairly. If you run an office or a brokerage, it is important to have an office policy as well. There also needs to be training around agent safety so everyone is aware and follows the same policy. Donny and Monica talk about the public reaction to some of these safety precautions. For the majority, these safety measures are met with positive response because it protects not only the REALTOR® but also the client. In any situation, it’s important to approach your decision based on your training and experience. Donny shares some statistics on the self-defense weapons REALTORS® carry. If you don’t think you can use a weapon for defense, don’t carry it. Regardless of what philosophy you adopt in your own business, make sure you have a plan for getting out of potentially dangerous situations, and practice that plan so you can execute it should you ever need to. Donny shares some of the statistics on how many agents have experienced safety concerns, whether a physical attack or cyber attack. Thirty-three percent of all REALTORS® have felt uncomfortable in a situation. Monica shares her own experience with this. There are some safety apps you can download to your phone to help keep you safe. It will come down to what is best for you, but some apps Donny recommends are CentraLock, SupraBox. Many smartphones also have an emergency SOS feature. A battery-operated noisemaker that you keep on your person can also be very effective. While these apps and devices are valuable, don’t put all of your comfort in an app. Make sure you have some sort of backup. Donny’s parting thought for agents is to have a plan and practice different scenarios. Take a self-defense class, and always be willing to ask for help or take a buddy. The most important thing is to trust your instincts and increase your situational awareness. While patients may have the triad of fever, abdominal pain and increasing ascites, they are far more likely to only have 1 or 2 of these symptoms In patients admitted to the hospital with ascites, consider performing a diagnostic paracentesis on all patients as limited literature shows an association with decreased mortality and, the procedure is simple and low risk Once you get the fluid, focus on the cell count: WBC > 500 or PMN > 250 should prompt treatment with a 3rd generation cephalosporin and albumin infusion Gaetano et al.
