According to a recent article by Christina Frangou in the Guardian, "doctors are dying by suicide at higher rates than the general population. Somewhere between 300 to 400 physicians a year in the US take their own lives, the equivalent of one medical school graduating class annually." This sobering statistic from the eye-opening Guardian article (aptly titled "US surgeons are killing themselves at an alarming rate. One decided to speak out") resonated with Your Doctor Friends, and we are fortunate enough to have the inspiration of that article, Dr. Carrie Cunningham HERE WITH US! Dr. Cunningham is an endocrine surgeon at Massachusetts General Hospital and holds a professorship at Harvard. She was recently elected President of the Association of Academic Surgery. Her Presidential Address at the AAS 2023 National Meeting, titled “Removing the Mask” created quite a stir, as she shared her personal struggles with depression, anxiety, substance use disorder, and her connection to a friend and colleague (Dr. Christina "Tina" Barkley) who died by suicide. Imagine getting up in front of thousands of your colleagues and baring the most vulnerable parts of yourself. It took tons of courage, and the YouTube video of her address went viral. Dr Thomas Jan is an osteopathic physiatrist practicing in Rockville Center, New York. He is dedicated to PM&R- in managing neuromuscular pain complaints and pain management as well as #MedEd . Nationally, about 12% of children live with a parent who abuses alcohol or other drugs, and about 80% of parents in the child welfare system have substance use disorder. Margaret Lloyd Sieger is an Assistant Professor in the UConn School of Social Work who teaches courses in substance use disorder, research, program evaluation, and social policy. Her professional and practice background includes clinical work with children and adolescents affected by parental addiction at the National Council on Alcoholism and Drug Dependence and as a civil litigation paralegal. She now studies mothers and infants with prenatal substance exposure, child protection policies and systems, and family treatment courts. Professor Lloyd Sieger is leading Connecticut's evaluation of the implementation of the federal Child Abuse Prevention and Treatment Act, known as CAPTA, which requires states to collect notifications when infants are born and found to have been exposed to certain drugs in utero. In this episode, she talks about Connecticut's national-model data gathering system on substance abuse disorder, how to help make social workers' lives easier, and what it's like to be part of drafting a new law. In 2019, the Ministry of Social Justice and Empowerment and The National Drug Dependence Treatment Centre released a report called- National Survey on Extent and Pattern of Substance Use. This week on Wellbeing we continue talking with Grant who recently is recovered from a drug dependence. The first time Grant entered recovery he left with some sense of success with his drug addiction but decided he would be a “controlled drinker” now instead. His drinking become out of control to the point he ended up going to jail for a short time. This cost him the relationship with a long-time partner and impacted other relationship around him. In this episode, Grant tells us of his bumpy journey to recovery. This week on Wellbeing we talk with Grant who has recently recovered from a drug addiction. We spoke with an academic expert last week about drug addiction and now it is time to see what it is like from first-hand experience of having an addiction. Grant started experimenting first with marijuana in his early adolescence before trying other drugs such as speed, alcohol and cocaine later. He went from smoking marijuana every so often then to every weekend then to when he was 15 years old smoking it every day. In this episode, Grant tells us his journey and relationship with drugs in his early life. We are back for season 2 after a long hiatus! Karen and I are joined by Dr. Fabian Pitter Steinmetz, who is both a toxicologist and an advocate for harm reduction and evidence-based drug policy. So cool! Fabian is also the president elect of the European Coalition for Just and Effective Drug Policies (ENCOD), a member of Schildower Kreis, a German expert panel for drug policy reform, and a member Law Enforcement Against Prohibition (LEAP) Germany. He has been involved in important advisory work including giving evidence to the WHO Expert Committee on Drug Dependence, where he recommended not classifying Kratom. In the podcast, we talk all sorts as we all like to go off on tangents. Drug policy reform and harm reduction are the usual suspects, but we also discuss the need for more reform advocates to have a natural science background (we are a heavily social science crowd, and sadly social science does not have the same clout). It's good to hear about how much Fabian has learnt from social scientists though! Makes me proud. We also talk about the need to have people who use drugs as role models (a stoner kid who is into skating can become a scientist after all!) We discuss the work of debunking myths about certain heavily stigmatised substances, our use of language in relation to all things drug-related (we could not stop using loaded terms like “hard” drugs or “hard” science and then we tried to correct ourselves – these expressions are so ingrained!) I think we even talked about basic income?! Addiction is a treatable disease, but how can people differential between quality care and fraud? Dr. Rosenthal dispels the truth on quality treatment. Mitchell S. Rosenthal, MD Mitchell S. Rosenthal, M.D., the founder of Phoenix House, one of the nation's leading non-profit substance abuse treatment organizations, is president of the Rosenthal Center for Addiction Studies. The Center plays an increasingly significant role in the treatment field, providing a valuable platform for advocacy and the formulation of public policy. Its surveys, investigations, publications, and substantial presence on the Internet provides critical information on substance abuse issues to policymakers, healthcare professionals as well as families and individuals coping with addiction. Dr. Rosenthal has served as a White House advisor on drug abuse, a special consultant to the Office of National Drug Control Policy, chairman of the New York State Advisory Council on Drug Abuse, and president of the American Association of Psychoanalytic Physicians. He is a lecturer in psychiatry at Columbia University's College of Physicians and Surgeons. Dr. Rosenthal is a psychiatrist whose research, clinical skills, and advocacy have produced model treatment protocols, widely replicated throughout the country and abroad. His pioneering efforts in substance abuse treatment date from his work at the U.S. Naval Hospital in Oakland, California (1965-1967), where he formed the first service-sponsored therapeutic community for the treatment of addiction. As deputy commissioner of New York City's Addiction Services Agency, he created the Phoenix House citywide network of treatment facilities in 1967. In 1972, he resigned his city post to become president of the now independent Phoenix House Foundation and oversee the national expansion of its treatment and prevention services over the following three and a half decades. Dr. Rosenthal earned his B.A. from Lafayette College and his M.D. from the State University of New York's Downstate Medical Center. He is a member of the Council on Foreign Relations, deputy chair of the National Council on Alcoholism and Drug Dependence, and serves on the board of the Partnership for Drug Free Kids and Delancey Street Foundation.

Dr. Yasmin Hurd is Professor of Psychiatry, Neuroscience and Pharmacology and Systems Therapeutics as well as the Ward-Coleman Chair in Translational Neuroscience at the Icahn School of Medicine in New York. She is also Director of the Center for Addictive Disorders in the Mount Sinai Behavioral Health System. Yasmin studies the brain and addiction. She wants to know how drugs impact the brain, as well as how genetics and other characteristics shape disease vulnerability. Her work also has applications for developing new therapeutic treatments for substance abuse. Outside of work, Yasmin loves cooking, including the challenge of assembling meals from the random ingredients in her pantry and hosting elaborate dinners for her friends. She has also developed a passion for painting. She received her PhD in Medical Science from the Karolinska Institute in Sweden and spent time as a Pharmacology Research Associate Fellow with the NIH and Staff Fellow at the National Institute of Mental Health. Afterward, Yasmin returned to the Karolinska Institute where she remained as a faculty member for 13 years before coming to Mount Sinai. She is also a member of the American Society for Neuroscience, New York Academy of Sciences, and the College on Problems of Drug Dependence. Yasmin shares more about her journey through life and science in this interview.

This being a holiday week the guys chose to air a special episode on substance abuse. This was originally part of the Mental Health episode but BJ went "Full Med Student" on the notes and produced a few shows. As many of us gather with our families this week we can see issues with substance abuse come out more than at other times. Part of this is from not having the responsibility of getting up and going to work and part from the stress that usually comes with travel and large gatherings. Next they take a look at specific workplace behaviors and how drug abuse can effect the workplace, coworkers, and job retention.
