In this episode of HFA Ian Maslin joins us to discuss the importance of Critical Incident Stress Management (CISM) for SAR responders. Ian is a Certified Traumatologist from the Traumatology Institute and Certified from UMBC in Critical Incident Stress Management. He is an Advanced Approved Instructor with The International Critical Incident Stress Foundation. A Peer CISM responder with British Columbia Search and Rescue Association, and Emergency Support Services. He is also a Community Care Nurse in the Lower Mainland of BC. Additionally he serves as the Training and Competency coordinator for British Columbia Search and Rescue association Critical Incident Stress Management program. During this episode we discuss the essentials of why SAR teams should implement a CISM program, and the positive benefits a well coordinated CISM program can have for team cohesiveness. We also breakdown the essentials of what's in a CISM program, and how each element is utilized after a stressful situation. We concluded the episode by providing resources that SAR teams can utilize to implement a CISM program for their local team. https://www.chaplaincyinnovation.org How can chaplains help healthcare institutions respond best in moments of crisis? One chaplain proposes Go Teams for healthcare. Following a series of traumatic incidents and losses, including on-site suicide, colleague deaths, and assaultive patients that deeply shook the staff community, Chaplain Alyssa Adreani created and launched a crisis response program called the Newton-Wellesley Hospital GO Team. Inspired by Critical Incident Stress Management, the GO Team is comprised of professional colleagues from Spiritual Care, EAP, Behavioral Health, Peer Support and other trained and experienced professionals. The team are activated and deployed by executive leadership following crisis events at the hospital. Supportive responders debrief after every incident. This webinar will discuss the history of the GO Team, the path taken to create and launch it, and how other chaplains can create similar resources in their own healthcare institutions. We're joined by Alyssa Adreani, Director, Spiritual Care Department, Newton-Wellesley Hospital.

Victor previously served as Director of Acute Trauma Services for the Sheppard Pratt Health System and as a consultant to the State of Maryland, Department of Health and Mental Hygiene in the area of disaster behavioral health. Victor maintains a private consulting and clinical practice in Towson MD, where he specializes in the treatment of Trauma and Stress related conditions, organizational training, and program development. Victor is the Clinical Director for the Anne Arundel Fire Dept. Critical Incident Stress Management team in MD, as well as a supervisor for the Sheppard Pratt Health Care CISM team development. He is a consultant to the Harford County Maryland Sheriff's Department's Crisis Negotiation, and Peer Support teams and serves on the adjunct faculty of Towson University in Psychology, Nursing, and Homeland Security. Victor currently serves on the editorial board of the International Journal of Emergency Mental Health. The Context of White Supremacy welcomes Dr Heather Brown. The Director and President of Behind The Line, Inc., Dr. Brown holds a Bachelor of Social Work from Salisbury University, her Master of Science from Capella University, and completed her Ph.D. in Forensic Psychology at Walden University. She is certified to teach both Critical Incident Stress Management and Mental Health First Aid, and she helps instruct cadets in the mental health unit at the Eastern Shore Criminal Justice Academy. We'll discuss her 2022 dissertation: The Role of Child and Adult Sexual Fantasies and Criminal Paraphilia Involving Serial Sexual Murderers. Gus found her report after speaking with Bishop Tony Caldwell about the missing black females in Missouri. Bishop Caldwell referenced previous serial murders in the area, including Robert Berdella. Dr. Brown's report features Berdella, Seattle's Gary Ridgeway and a number of other White rapists and serial killers. She makes a point of the fact that most of the serial sexual killers are White Men. We discuss what it is about White Culture that produces this pattern of White Male behavior. We re-visit the concept of necrophilia (including the Joliet, Illinois case from 2013, where White teens had sex on the corpses of two black males.) She is the founder of Crisis Response Consulting and The Coach Alliance. Denise received a Bachelor of Arts in Social Work from Wartburg College, Waverly, IA, a Master of Social Work from Florida State University, Tallahassee, FL and is a trained professional Coach through the Institute of Professional Excellence in Coaching. Denise is a Veteran of the US Air Force, was called back to active duty on 11 Sep 2001 and spent 8 years on active duty, 3 years as Chief, Sexual Assault Prevention and Response Deployment Operations at the Air Force Personnel Center, Randolph AFB, TX, and 5 years as Chief, Behavioral Health for the Air Force Reserve Command (AFRC), Robins AFB, GA. In her position with AFRC, Denise was responsible for the implementation and oversight of the following programs: Suicide and Violence Prevention, Post-Suicide Review, Critical Incident Stress Management, and Operational and Post-deployment Stress. For 7 years she provided oversight and training for over 200 AFRC personnel, military and civilian, who were part of the AFRC CISM team. Denise deployed to Iraq in 2008 and in 2009 to Kuwait. Denise finished her career as the Mental Health consultant to the 25th Air Force Surgeon General providing resiliency and operational stress management support to Intelligence, Surveillance and Reconnaissance personnel. Prior to September 11, 2001, Denise, while an Air Force Reserve Officer, was in private practice working with adults and children affected by traumatic events; a contract provider for the Veterans' Administration and an Employee Assistance Program provider for the US Postal Service. Denise currently provides coaching to civilians and first responders regarding life and occupational concerns. Denise presents on a variety of topics for military and civilian personnel regarding suicide prevention, intervention and postvention, workplace violence, Critical Incident Stress Management, combat/operational stress management, post-traumatic stress disorder and sexual assault prevention and response. She teaches several International Critical Incident Stress Foundation (ICISF) courses, is on the ICISF faculty and co-authored the ICISF Suicide Prevention, Intervention, and Postvention course. Denise is a core faculty member of the Resiliency Sciences Institute, International at UMBC Training Centers, instructing the certification courses, Resilient Leadership and The Resilient Child. Denise has provided crisis intervention and organizational consultation following airplane and helicopter crashes, homicides, suicides, line-of-duty deaths, manmade and natural disasters, robberies and combat. She is the founder and volunteer clinical director of the Alamo Regional Response Team, which covers a 13-county area surrounding San Antonio and provides mutual aid throughout Texas. Christy Warren  is a retired Fire Captain from the Berkeley Fire Department in California. She has twenty-five years of service as a professional paramedic, with seventeen years as a professional firefighter. She was diagnosed with PTSD in 2014 and spent several years recovering. Since retiring from the fire service, she has completed a triathlon, the Escape From Alcatraz swim, and a bachelor's degree in business from Washington State University. WHO WE ARE As a group of Rural Volunteer Firefighters, our Fire Department has built our firefighter training from both old and new tactics in the fire service and adapted them to fit the realities of equipment, manpower, and costs around what our community requires. WHAT WE DO We create video, audio and written word content to share these training and tactics methods with our Brothers and Sisters in our extended fire families, and to showcase to those that may be interested in becoming firefighters the fun you can have and the amazing skills and abilities you get to learn, whilst keeping the lives of the people around you safe! WHY WE SHARE Firefighters are problem solvers whose role requires them to be on their best day whilst others are having their worst day, and while we do not claim to be perfect we are willing to put our lives on the line to try. Today's episode is an invitation to be a pattern-breaker.When we experience “negative” emotions like fear, shame, anger and grief...we isolate that emotion, put it in a box and hide that emotion so it never sees the light of day. However, this strategy doesn't magically *poof* make the difficult emotions disappear. Rather, they show up through overblown reactions and physical signs and symptoms. Some of us have closets full of unexamined emotions. And it is completely understandable that we do this! First of all, it's uncomfortable to sit with difficult emotions. And societally, we're validated for holding our feelings in. We tell our children to “be strong” when they are on the verge of tears. We tell ourselves that we have to “keep it together”.This is why it is a revolutionary act to lean into our emotional pain. After ten years on the job he began having issues with anxiety, stress, and depression and was later diagnosed with Post Traumatic Stress Disorder (PTSD). After 13 years of marriage and three children, he experienced divorce. At the time, he did not know that it would help drive his passion to help the families of first responders.Today, Matthew is a sober mentor to men and women in the First Responder community and a volunteer at the Save A Warrior program. As an outreach specialist at First Responder Wellness by Simple Recovery, he proactively reaches out to brothers and sisters who may be struggling. Recently, he also became an instructor with Santa Ana College to facilitate mental health blocks to new recruits so they have healthier coping skills. His passion is to provide other first responders with education, resources, hope, and a path towards healing. Kristen Roman was appointed Associate Vice Chancellor and Chief of Police of the University of Wisconsin-Madison Police Department in January of 2017. She holds a master's degree in Criminal Justice from Boston University, and a bachelor's degree in English from the University of Wisconsin-Madison. While a student at UW-Madison, she was a member of the UW Women's Volleyball team. Prior to her appointment, Chief Roman served for 26 years with the City of Madison Police Department. During her tenure there, she held a variety of positions including Police Officer, Recruiting Officer, Community Relations Sergeant, Professional Standards & Internal Affairs Lieutenant, Detective Lieutenant, Patrol Operations Lieutenant, Special Events Team Commander, Crisis Negotiation Team Commander, and Community Outreach Captain. In addition, she coordinated department Critical Incident Stress Management efforts and developed and supervised the department's Peer Support Program. A defining feature of Chief Roman's career is her extensive work in the area of improving police services to people with mental illness. A past board member of NAMI Dane County and Journey Mental Health Center, she has conducted multiple local, statewide, and national trainings/presentations on police-mental health response and collaboration. Most recently, she developed and instructed a course for IACLEA's Universal Issues Series entitled, Mental Health Concerns on Campus. In addition to her work as a trainer/presenter for IACLEA, and Distance Learning Committee member, Chief Roman has presented on various topics throughout her career including Leadership, Police Officer Wellness and Resiliency, and Campus Response to Protests and Demonstrations.   Chaplain Dave Williams Demystifies  His Role With Law Enforcement and First Responders Interview with Chaplain Dave Williams Guest Bio Being invited to serve alongside first responders in a pastoral role is not for every pastor, but it has been a part of David Williams’ ministry since 1987. Public safety chaplains are uniquely suited to serve the public in times of trauma and they are a unique level of support to first responders processing difficult calls, administrative stresses and public perceptions.   David has served as a senior chaplain in the Central Valley since 1997. He has been a trainer in Critical Incident Stress Management since 2005, a certified trainer with the International Conference of Police Chaplains to help others in providing peer support and chaplain services. He has been a Certified Crisis Chaplain with the American Academy of Experts in Traumatic Stress for many years.   After just a year as a chaplain, David found himself with the wife of a police officer, who had been shot in the line of duty. Sadly, the police officer died that evening. A year later, he was sitting alone with an officer who was just involved in an officer involved shooting; with pastoral training and intuition, David supported that officer through the months following. He later learned additional tools of support. So David’s work with people suffering from critical incident stress, a mental health crisis, chemical dependency, relationship struggles and administrative difficulties is a combination of hands on and academic study. His desire is to help as many public responders to retire healthily as possible.                  In this episode we talk about: Chaplain Dave Williams has listeners on the edge of their seats with this engaging, informative, and educational interview. Chaplain Dave describes how he fell into his pastoral role with first responders and law enforcement in 1987 and to this day, continues to support and promote emotional health among first responders, law enforcement, and their families. Welcome to Episode 5 of Assuming Command with Angela Leath. Bob had the privilege of hiring Angela to be the Crisis Intervention Administrator for Las Vegas Fire and Rescue in 2015. Angela now works for the City of Henderson as their Public Safety Wellness Program Manager and offers training in Mental Fitness and Critical Incident Stress Management Program Building through her business, Leathalminds, LLC. Angela has had a long career in mental health, or mental fitness as she prefers to call it, and she provided a wealth of information during this episode on building a peer support program, utilizing the program, and training both peer support teams and the overall organization. Here are some of our key takeaways from the conversation. When building a program, it is important to consider the size of your organization, including all divisions or sections, including everyone from maintenance, training, call-takers/dispatch, to first responders. Peers from and for all areas need to be included. There needs to be enough members on a Peer Support Team to be able to provide support without members of the team burning out. If your organization isn't large enough to support its own Peer Support Team, consider a regional approach by partnering with other agencies. Training to support mental fitness will help employees build resiliency before they need it. Training should start in recruit academy's, a Peer Support Mentor should be assigned to rookies, mental fitness and resiliency should be regularly incorporated into trainings, and retirees should be both prepared for transition into retirement and offered the opportunity to stay engaged in their organization after they retire. Families should also be integrated into the process; it's important for families to understand to some degree the stress first responders are dealing with and how it may impact them at home. Angela also discussed the important of Critical Incident Stress Management, which is trained in intervention techniques for larger scale or more traumatic incidents, whereas the Peer Support Team is used for more day-to-day type stressors. She described critical incidents as a bullseye, with the incident in the center and the rings being made up of people and their interaction with the incident. Training on how to deploy the team during a critical incident, all the way through the de-brief process. Identification and utilization of outside resources and having a plan with your EAP are also integral to the preparation process. Angela mentioned a variety of training and support programs. Welcome to Episode 3 of Assuming Command. This episode is modified from a Peer Support class Bob gave with Jeff Buchanan from Las Vegas Fire and Rescue for the International Associations of Fire Chiefs Conference. In this episode, Bob and Jeff discuss peer support programs and the importance of mental health in the fire service. The class and the importance of the topic inspired Bob to do a series on mental health and wellness, so this episode will be part of a three-part series on the topic. During the class, a game board for developing a peer support program is used as a reference point; that “game board” is available at https://drive.google.com/file/d/1S3VvIeKlu0LAUw1iRA_EW7CYGOiklPV5/view?usp=sharing. The path to building a program starts at denial; moves to recognizing the problem of life imitating art, especially in the case of larger scale events; requires the labor and management buy-in; should be built into the organizations mission, vision, and values; needs initial and ongoing training of team, supervisors, and staff; an SOP for deployment and utilization of the team should be developed; “messengers” should be identified, that is, who within the organization can serve as respected and trusted advocates; different technologies for connection between the peer support team and staff should be considered; recruits and staff should have access to regular meetings and communication with the peer support team; training on and knowledge of the SOPs should be built into the promotional process; the path does not end with development of the team and plan; and program success requires maintaining a performance and growth mindset. Bob and Jeff also discussed the importance of understanding everyone will react to situations differently. Some people will come out of a larger critical event OK, but be impacted by a smaller event, and vice versa. It is important to have peer support available to the organization anytime they may be feeling stress or need someone to speak with. Peer support needs to be integrated into the culture of the organization from recruitment through retirement and beyond. Jeff spoke about using retirees as part of the peer support team to allow them to keep engaged with the organization and to take advantage of their wealth of experience. When building the team, management needs to consider the needs of the organization and how to tailor the model and branding to their team. It is vital to the success of the program that the people who will be utilizing it feel comfortable reaching out to the peer support team. One popular model for developing and program is the Jeffrey Mitchell model. Tim Kaplan is a 20 year veteran of the Berkeley, (CA) police Department. We talk about his chronic anxiety and how he dealt with it on the job. He spent the majority of his police career in the patrol division while holding a number of collateral assignments including Field Training Officer, Crisis Negotiator Team Leader, and Crowd Management Team member. Tim was assigned to the Sexual Assault Investigations Division for 3 years, and spent 2 1/2 years on a high visibility street level narcotics and violence suppression team. He was trained in Critical Incident Stress Management and assisted on the Peer Support Team. Tim retired at the age of 43 years. He was ready to move on to the next chapter in his life and felt leaving the job would help decrease a constant anxiety that plagued him. His current passion is helping others reach their fitness, health, and wellness goals. Critical Incident Stress Management. Unless you’re in the military or a first responder, you’re likely not familiar with this terminology. It is a protocol developed specifically for dealing with traumatic events. I’ve shared many times that I was a cop before becoming the Toothcop. But what many don’t know is that I was also a chaplain. I was trained to help fellow first-responders through physical and psychological trauma.  In this episode of Talking with the Toothcop, Andrea and I talk about grief and the grieving process. We talk about critical incidents and how to recognize the signs and symptoms that you’re dealing with psychological trauma. We also share some stress-management strategies and resources for yourself and your team to work through critical incident stress. The psychological trauma we’re all facing  We’re all grieving right now. Some of us have physically lost someone to COVID-19. Many have lost their jobs, their businesses, and their independence. We are mourning our loss of connectedness and dealing with severe loneliness. Whether you recognize it or not, we’re dealing with unprecedented levels of stress. Even though we can’t SEE the damage, the damage is just as pervasive, certain, and severe as physical trauma.  Andrea points out that losing a job is a normal occurrence in society—but we can’t cope with the repercussions the way we normally would. It isn’t as simple as going out and getting another job. Everyone is being affected emotionally. In most instances I’ve witnessed, it’s harder to come out of emotional trauma than physical trauma. I want to emphasize that it’s okay to feel how you’re feeling—but you need to recognize that you’re grieving to move through the 5 stages of grief (denial, anger, bargaining, depression, and acceptance).  The critical incident stress management system Critical incident stress management (CISM) is “an adaptive, short-term psychological helping-process that focuses solely on an immediate and identifiable problem”. The way people respond to emergencies and disasters strains their ability to function. Their future depends on learning how to manage the psychological stress and impact of those incidents. The system is geared towards: Normalizing your instinctive reactions Lessening the impact of the incident Encourages the natural recovery process Restores adaptive functioning skills And helps determine the need for further support No one signed up for the trauma we’re all facing. We are grieving our old way of life—because when things return to “normal” they’ll never be the same. We may not all be dealing with death, but we are still dealing with loss. We must seek to understand it to lessen the impact.  Signs and symptoms of psychological trauma Stress is a mental phenomenon that can be manifested physically. Some of the symptoms include: Exhaustion, nausea, vomiting, weakness, chest pain, rapid heart rate, headache, excessive thirst, fainting, elevated blood pressure, exacerbate allergies, symptoms of shock, and more.  Our bodies are trying to tell us there’s something wrong. We can also experience cognitive, emotional, and behavioral reactions.  Cognitive reactions: blaming others, confusion, reduced attention span, poor concentration, troubled thoughts, nightmares, and so forth.  Emotional reactions: frustration, anxiety, guilt, sense of loss, anger, denial, fear of loss of control, feelings of isolation, and more.  Behavioral reactions: Emotional Outbursts, changes in activity level, disturbed sleep, increase in smoking or drug use, easily startled, anti-social, withdrawn, change in eating habits, fidgety and restless, change in sex-drive.  The odds are everyone is dealing with some of these symptoms in one form or another. You have to recognize that this is normal right now, but YOU have the power to change how you react and move forward. How to reduce the effects of trauma—for yourself and your team There are a few simple recommendations for dealing with the effects on an individual level:  Limit exposure to noise and odors Take 15-minute rest breaks Drink non-caffeinated fluids (water) Don’t eat excessive sugar or fat Don’t rush back to work Talk through your feelings Whenever we eventually return to our offices, owners and managers need to keep these things in mind. Many people may be hesitant to return to work and it’s your job to help them feel safe. You must also convey to patients that your office is safe.  One way you can help your team transition back to work is to facilitate a structured critical incident debriefing with your team. It’s a 7-step phase that helps you work through the psychological trauma together—because none of us are in this alone.  Listen to the whole episode for an in-depth discussion on each of these topics. As Certified Athletic trainers, we are often the “go to”, the “handle everything” and the “be everywhere” person. We typically put everyone else first. During a time of crisis, this is likely even more the case.  As we find ways to navigate this COVID-19 pandemic, we also have to remember to take care of ourselves. In this episode I am sharing the audio from a presentation that Sandy Lynch Massi, MA, LAT, CCISM gave last summer at the annual RWJBarnabas Health "We Matter Too" Athletic Trainer Summit.  Sandy is the NATA "ATs Care" District 2 Representative. In this talk she presented ways to help Athletic Trainers recognize when you may have neglected yourself for too long and how to create a plan to get you back to your best. She discusses mental health self-assessment, recognizing when you need to make your mental health a priority, strategies for creating your own mental health Emergency Action Plan, and ways to advocate for yourself as part of the plan. Sandy has been a certified athletic trainer and career firefighter/EMT for over 18 years and is an instructor with the International Critical Incident Stress Foundation. She is certified in Critical Incident Stress Management and has participated on the Mercury Critical Incident Response Team, a peer support team for first responders since 2002, currently serving as director. Sandy, along with another peer from NJ, helped organize one of the first peer support teams for athletic trainers in America. Serving as the District 2 representative to the NATA ATs Care Committee,  this position tasks her with coordinating team development, training and requests for service in NJ, PA, DE and NY. Additionally, Sandy is an instructor at the Emergency Services Training Center in Westhampton, NJ. where she teaches EMT and Firefighter courses. Sandy is a Mom to 9 year old Jackson, two pit bulls and a cat. She is married and lives in Delran. He connects first responders and veterans to mental health and/or substance abuse programs.Scallon was the founder and first director of the Norfolk Police Department’s Critical Incident Stress Management (CISM) & Peer Support Unit and Crisis Intervention Team Co-Coordinator for the Norfolk Police Department. He is a peer and current board member of Virginia Law Enforcement Assistance Program , a certified peer with the West Coast Post Trauma Retreat/First Responder Support Network. Having been involved in multiple shootings, he is intimately familiar with trauma and the consequences of exposure. This episode is a wrap up covering the highlights from the Tactical Trauma international conference on pre-hospital critical care and trauma. This conference emphasizes tactical medicine, with a panel of experts speaking throughout the 2 days. 0:10 – Introduction to day 2 wrap up 0:40 – Introduction of the panel 1:15 – Mike Abernethy wraps up his session as a moderator on Day 2. Takeaways include Michael Lauria’s discussion on the preoccupation with protocols and guidelines. 2:45 – Three basic concepts include speed, simplicity, and coordination of care. Tactical medicine boils down to how efficiently one can perform these three tasks using evidence based medicine. Take the lessons learned from the military medicine, and a lot of them can be applied to civilian EMS and in-hospital care. 5:30 – One thing to add, is being able to do the basics very well. These basic skills will lay the foundation for new advanced technologies and interventions. 7:00 – Discussion on Mike Klumpner’s talk on medical best practices at MCI’s. The phrase “Just because you can, doesn’t mean you should” is discussed among the panel members. Being able to look at these mass casualty events, their injuries, and intervention with simplistically is the key. An example here includes an anecdote regarding a vascular neck injury, and the ability to ask “Am I making a difference, or am I delaying definitive care?” 9:30 – The panel discusses the criticism of triage in an MCI setting during day 2. One example given is that during most MCI’s, the triage tags were not used including the Boston Marathon bombing, where triage had to ‘go out the door’. Another example is the way the walking wounded are huddled into a corner and sometimes forgotten, while they may be gravely injured as evidenced in the Manchester bombing. 11:40 – FDNY’s new triage protocols include any penetrating injuries between the clavicle and the pelvis are immediate red tags. 12:15 – Breakout sessions with LEO’s who discussed the medical care of the K9’s. 13:30 – The point on situational awareness with the K9 colleagues is discussed. This includes muzzling them early if gravely injured and in danger. 13:45 – Anesthesiology talks about how dogs have a fenestrated chest cavity, and its importance with a tension pneumothorax. The end result is that the resulting obstructive shock may be worse in dogs. 15:00 – Ketamine takes a hit when it comes to pain control with K9’s for multiple physiologic reasons. Morphine IM 30-50mg was preferred for K9 pain control. 15:35 – Currently, it is a felony in the USA to provide ALS to animals if you are not a veterinarian. Propositions for exclusions for EMS workers trained in animal care are in the works right now. One anecdote is during a NC MCI, kid pools were filled with ice for the explosives K9’s, drawing a parallel to firefighter rehabilitation. 18:05 – Psychosocial aspects when providing medical care is discussed along with PTSD learning points. While feelings of anger and hostility towards your patient may be natural, providers must be able to accept that and continue to give medical care. 19:35 – Learning points in PTSD. One interesting finding was that those with minor injuries who received early intervention developed worse PTSD when compared to those with severe or no injuries. This raises questions on mandatory Critical Incident Stress Management, and how it should always be voluntary. 21:05 – Best practices after tough calls in EMS. Debriefing, assessing for safety, and assuring readiness for the next job are the top priorities. Being able to spend time with your colleagues, who have been through similar experiences versus mandatory CISM is discussed as well. 23:15 – The longer people stay in lockdown during MCI’s, the more likely they are to develop PTSD. Data coming out is showing that school lockdowns are causing PTSD in pediatrics. 24:50 – ‘Just culture’ is discussed, as is the importance of making system level changes to prevent errors. Most of the time, it is organizational culture that leads to mistakes, and not just individual mistakes. 26:10 – No non-discoverable mistakes exist in EMS, as opposed to hospital-based medicine. 26:55 – Takeaways from afternoon lectures including penetrating trauma with Dr. Tom Koenig, tactical medicine in mass casualty events with Dr. Matthew Lengua, OB trauma, and blast injuries. 27:30 – Resuscitative hysterotomies in Finland, and other advanced procedures done quickly and in austere environments. Discussing the decision gap, which the is the time from when the decision to perform a critical procedure is made until when that procedure is performed. 33:30 – Advances in resuscitative hysterotomies and thoracotomies, and there are now clear indications for both. However, this does not mean that Top Cover should be eliminated. 34:00 – Takeaways from blast injuries and penetrating trauma, specifically to the head and neck region. Major points include how EMS Physicians can treat some of these patients in the warm zone with critical interventions. 36:00 – Learning points from the lecture on burn care, and the unpredictability of the burn patient. One takeaway is that due to the current school of thought, providers are over-intubating patients with harm. Studies have also shown that escharotomies performed outside of burn centers are often performed incorrectly and incompletely. 39:30 – Use of vehicles as a weapon of mass destruction has become more common recently. A takeaway is that the extent of injury tends to be worse when the attack is intentional, whether using vehicles or other weapons. Logistically, the scene tends to be complex as it generally encompasses are large area. The discussion is brought up again about how as medical personnel, we can empower and train the general public to help. 48:55 – Next steps include teaching our communities the basic skills that have been proven to save lives, and working together to minimize these threats in the future. 49:15 - Conclusion

Chaplin Kevin Johnstone’s career path has included being a Chicago Suburban fireman, EMT, paramedic and a Navy Corpsman. Johnstone began working for the L.A. Open Golf Tournament in 1977 and orchestrated every possible aspect of this nationally recognized major public event until attaining the position of General Chairman of the 1984 L.A. Open. He took a position at United Expositions and became vice president of sales and provided services to many of the largest trade shows and conventions in the U.S. In 1991, Johnstone joined Advanstar Expositions. In 1994 he became director of the massive NAMM Show. He also produced the Summer NAMM Show a well as events in Germany, China and Russia. After 18 years he left NAMM. Kevin joined Staff Pro Inc., an affiliate of U.S. Security Associates, and managed many events for Staff Pro including the U.S. Open Tennis event, the GRAMMY’s, the Emmy’s, the Coachella and Stagecoach Music and Arts Festivals, to name just a few. Now, as Safety Operations Manager at the Rock Church & Academy he oversees safety operations, event operations, Safety Team Training, CPS Investigations and helps produce of the Annual Church Safety + Security Conference. He is also a commissioned minister. Dr. Jeff Roth joined the Brandywine School District in Delaware where he initiated Operation Cooperation — a school-wide approach to conflict resolution, helped initiate and lead the district Crisis Response Team, and was named School Psychologist of the Year by the Delaware Association of School Psychologists. He developed Instructional Support and Positive Behavior Support teams, a social skills curriculum, and a program of self-directed professional development for school psychologists. He was trained in the PREPaRE Model, Critical Incident Stress Management, Crisis Prevention Institute (CPI), and Red Cross Disaster Mental Health. He is an American Red Cross Disaster Mental Health volunteer. Dr. Roth received his undergraduate degree in psychology at Temple University. After serving in the Air Force, he practiced social work and returned to graduate school at Temple where he earned his doctorate. During graduate school, Dr. Roth worked as a recreational therapist at St. Christopher's Hospital for Children and taught graduate courses at Arcadia University. He is a licensed psychologist in Delaware. Dr. Roth is an adjunct professor at Wilmington University, Delaware. He co-developed a mentoring program for new school psychologists for the state Department of Education. Good afternoon everyone! Whitney and I are back with episode 11 of Within the Trenches. This week we spoke with Cheryl, a dispatcher and Critical Incident Stress Management, (CISM) team member with SunComm in Yakima, WA. This was an interesting episode because we were battling with the Internet, our friend and foe. We dealt with some issues over Skype and the audio was cutting in and out but after some good editing, you can't even tell that anything messed up. Despite the technical difficulties this episode was excellent and Cheryl shared some great stories with us.
