In this podcast episode we want to introduce you to our BCEN Friend, Lionel Lyde. Lionel Lyde is a retired Military Nurse with 35 years of service in the armed forces. He holds his MHA MBA, and CEN. Lionel has taken on clinical, leadership and executive leadership positions during his 31-year nursing career, spending most of his clinical time as a critical care and ER/Trauma Nurse. His last position being an Air Force Critical Care Master Clinician and Special Military Consultant to the Air Force Surgeon General for Critical Care Nursing. He retired from military service 2022 but has certainly not retired from nursing. He's currently the Director of Nursing for a group home and manages a free clinic. His current professional goal is to network primary care clinics that provide care to low income and indigent patient populations. This goal is part of a greater aspiration to facilitate access to the healthcare system and decompress community emergency departments. Come along as Michael Dexter and Hollye Briggs talk with Lionel about his career in military nursing and leadership. On this week's episode of Rural Health Leadership Radio, we kick off a series focused on NRHA's 2023 Rural Health Fellows and their policy papers. The first group we chatted with includes Lisa Rantz, Kevin Lambing, and Dr. Carrie Shaver who had the task of examining Medicare Advantage. In our conversation we discuss the intricacies, impacts, as well as personal experiences that the Fellows uncovered in their research. “Get involved and advocate and educate on these issues. It will require all of us working together to create the change in policy and funding mechanisms that we need to ensure that our rural Healthcare is supported and continues to be strong.” – Dr. Carrie Shaver Kevin Lambing serves as the Senior Program Officer for Health at the T.L.L. Temple Foundation. In this role, he helps ensure East Texas children, adults, and families are healthy and have access to quality healthcare. He organizes and convenes stakeholders and develops community partnerships with potential health related grantees in East Texas, consisting of 22 rural counties and one primarily urban county, Harris County and Miller County, Arkansas (Texarkana area). In addition, he monitors federal and state legislation related to health with intentions for future gaps in funding health resources in rural East Texas. Kevin's 30-plus-year career in the Air Force culminated in his service as the Senior Advisor to the Air Force Surgeon General. In this role he defined, shaped, and executed health care policy in support of 45,000 personnel at 75 medical treatment facilities around the globe, ensuring a cost effective, patient-centered, and prevention-based healthcare continuum for 2.6M beneficiaries. He earned an Associate of Applied Science degree from the Community College of the Air Force, Bachelor of Science degree from Wayland Baptist University, a Master's of Business Administration degree from Webster University and is a 2023 National Rural Health Association Fellow. He is married to his wife Sheliea and they have 2 children, Noah and Lydia. Lisa Rantz is the Executive Director of Hilo Medical Center Foundation which serves as the Hawaii Island Area Health Education Center, focusing on health career recruitment from K-12 to graduate medical education. She is the President of the Hawaii State Rural Health Association and in 2023 joined the National Rural Health Association Board of Trustees. Lisa is a 2023 NRHA Fellow and has over fifteen years of experience in facilitation and focus groups in education and health care, Lisa has developed impactful programs to address rural health challenges and advocate for community needs. Her accolades include the 2020 Small Business Advocate of the Year, the 2021 Peacemaker Award, the 2022Athena Leadership Award, the 2023 Community Star recognition for Hawaii, and the 2024 NRHA Volunteer of the Year award. Dr. Shaver is an Assistant Professor of Health Management and Leadership at Texas Tech University Health Sciences Center and a founding member of the New Mexico Rural Health Association. Dr. Shaver has a strong background in the social service sector and has experience administering disease prevention, treatment, and health and wellness programs. She endeavors to bridge gaps between traditional silos in healthcare and foster collaboration among healthcare professionals through Systems Thinking. By embracing this holistic framework, she aims to contribute to the development of sustainable, inclusive, and equitable healthcare systems. Dr. Shaver's research centers around understanding and addressing the unique healthcare challenges faced by rural and frontier communities along the United States-Mexico border and countries within Latin America and the Caribbean.

   In this episode, you will hear about how Col. Don Taylor began his Air Force career as an architect and then progressed into a career in hospital administration. On 9/11, he worked for General Carlton in an office overlooking the Pentagon, and he describes watching the attack on the Pentagon. As a military healthcare administrator, he explains how he interacted as an advocate for the delivery of healthcare and balanced the need for resource utilization and delivering high-quality healthcare. Hear about the insights into hospital administration and the Tricare contract standards and development of standardization in healthcare and development of a healthcare plan and his briefings from the Pentagon to Congress in the Tricare for Life Program. He explains the role of the Medical Service Officer in supporting the various missions of Military Medicine and his leadership positions from Air Force hospital to combat theater trauma hospital.  He shares his insights and lessons learned as a Hospital Commander in the United States and Commander of a large, deployed Air Force Theatre Combat Hospital and medical evacuation hub located in Iraq.  He also describes how he prioritized patient needs and helped promote primary care patient teams and the development of the Primary Care Medical Home.  He details how he as a nonphysician, led physicians and helped synergize their efforts for optimal healthcare delivery. You will hear him describe how he navigated difficult circumstances with physicians and the situations that arose in the war zone, such as how to deliver care best when there are blood shortages. He also discusses how he handled these difficult situations and helped build rapport with those healthcare teammates. He also describes the training he encountered that most closely reflected combat casualty care.  As an advocate for physical, mental, and spiritual renewal, he describes the importance of personal care.  Interestingly, he explains how he had a high-profile casualty and managed the flow of communication to ensure respect for the casualty's privacy and family.  He describes how he positively engaged with the media to respect patient confidentiality and ensure family members received information before the media broadcast.  We also discussed how he thinks the military should focus on clinical training to optimize platforms for providing ready providers for casualty care. Hear about the challenges in military healthcare and recapturing patients due to the fierce competition in the healthcare industry.  He discusses the integration of the healthcare models and his thoughts on the balance between active duty and reserves healthcare providers and the limitations of pulling from the private sector for military care.  He is an expert in these areas as he received his Bachelor of Science in Architecture from the University of Texas at Arlington, TX. He earned his Master of Healthcare Administration from the Pennsylvania State University in State College, PA.  Don had a distinguished career in the Air Force and served as a primary advisor and consultant to the Air Force Surgeon General immediately before and following the attacks on 9/11.  He served as Commander of the Air Force Hospital at Luke AFB and was a pioneer in developing the concept of the primary care medical home.  He also was the first non-physician to Command a deployed Air Force Theater (Trauma) Hospital (Balad AB, Iraq). He received his D.D.S. in 1983 from UT Health San Antonio, his M.S. degree from UT Health San Antonio in 1990, and his certificate in Periodontics in 1990 from Wilford Hall Air Force Medical Center at Lackland Air Force Base in San Antonio, Texas. Dr. Mealey served more than 21 years on active duty in the United States Air Force, including positions as the chairman of the Department of Periodontics at Wilford Hall Medical Center, the program director of the U.S. Air Force Periodontics Residency, and the Chief Military Consultant in Periodontics to the Air Force Surgeon General. Dr. Mealey has published extensively in the scientific literature, with an emphasis on interrelationships between oral and systemic health. He is co-editor and author of the textbooks Periodontics: Medicine, Surgery, and Implants (2004, Elsevier Publishing), and Periodontal Medicine (2000, B.C. Decker Publishing). Dr. Mealey is a Diplomat of the American Board of Periodontology and is a consultant to the American Dental Association Council on Scientific Affairs. He currently serves as a Director of the American Board of Periodontology. The print version will be mailed in late July, along with the regular June issue. The result of this landmark gathering is a redesigned disease classification framework that guides comprehensive treatment planning and allows for a personalized approach to patient care. Highlights from the updated classification include a multi-dimensional staging and grading system for periodontitis classification, a recategorization of various forms of periodontitis, and the inaugural classification for peri-implant diseases and conditions. Consequences of Periodontitis:  Untreated gingivitis can advance to periodontitis. With time, plaque can spread and grow below the gum line. Toxins produced by the bacteria in plaque irritate the gums and stimulate a chronic inflammatory response in which the body essentially turns on itself, and the tissues and bone that support the teeth are broken down and destroyed. Gums separate from the teeth, forming pockets between the teeth and gums that can become infected. As the disease progresses, the pockets deepen and more gum tissue and bone are destroyed. Eventually, teeth can become loose and may fall out or need to be removed. Periodontitis diagnosis is typically classified by stage and grade. After your periodontist conducts a thorough assessment of your unique case, he or she may assign a stage ranging from initial to severe that describes the acuteness of disease. Additionally, your periodontist may assign a grade which communicates the rate of disease progression and anticipated response to treatment.

Dr. Green served as the 20th Air Force Surgeon General from August 12, 2009, to July 19, 2012. Hear in this episode his training to become an aerospace medicine physician. He led through multiple crises in the Pacific, commanded multiple military hospitals, including Wilford Hall Medical Center in San Antonio, helped lead Air Force medicine following 9/11, and oversaw aeromedical evacuation for operations Enduring Freedom and Iraqi freedom. He also provides a unique look into the role of a Command Surgeon from his experiences with US Transportation Command and the North American Aerospace Defense Command. An expert in disaster relief operations, he planned and led humanitarian relief efforts in the Philippines after the Baguio earthquake in 1990 and supported Operation Fiery Vigil following the 1991 eruption of Mount Pinatubo. enise J. Thompson is the Director, Resiliency & Peer Support and Director, Critical Incident Stress Management (CISM) for Bexar County Emergency Services District 7, San Antonio, TX. She is the founder of Crisis Response Consulting and The Coach Alliance. Denise received a Bachelor of Arts in Social Work from Wartburg College, Waverly, IA, a Master of Social Work from Florida State University, Tallahassee, FL and is a trained professional Coach through the Institute of Professional Excellence in Coaching. Denise is a Veteran of the US Air Force, was called back to active duty on 11 Sep 2001 and spent 8 years on active duty, 3 years as Chief, Sexual Assault Prevention and Response Deployment Operations at the Air Force Personnel Center, Randolph AFB, TX, and 5 years as Chief, Behavioral Health for the Air Force Reserve Command (AFRC), Robins AFB, GA. In her position with AFRC, Denise was responsible for the implementation and oversight of the following programs: Suicide and Violence Prevention, Post-Suicide Review, Critical Incident Stress Management, and Operational and Post-deployment Stress. For 7 years she provided oversight and training for over 200 AFRC personnel, military and civilian, who were part of the AFRC CISM team. Denise deployed to Iraq in 2008 and in 2009 to Kuwait. Denise finished her career as the Mental Health consultant to the 25th Air Force Surgeon General providing resiliency and operational stress management support to Intelligence, Surveillance and Reconnaissance personnel. Prior to September 11, 2001, Denise, while an Air Force Reserve Officer, was in private practice working with adults and children affected by traumatic events; a contract provider for the Veterans' Administration and an Employee Assistance Program provider for the US Postal Service. Denise currently provides coaching to civilians and first responders regarding life and occupational concerns. Denise presents on a variety of topics for military and civilian personnel regarding suicide prevention, intervention and postvention, workplace violence, Critical Incident Stress Management, combat/operational stress management, post-traumatic stress disorder and sexual assault prevention and response. She teaches several International Critical Incident Stress Foundation (ICISF) courses, is on the ICISF faculty and co-authored the ICISF Suicide Prevention, Intervention, and Postvention course. Denise is a core faculty member of the Resiliency Sciences Institute, International at UMBC Training Centers, instructing the certification courses, Resilient Leadership and The Resilient Child. Denise has provided crisis intervention and organizational consultation following airplane and helicopter crashes, homicides, suicides, line-of-duty deaths, manmade and natural disasters, robberies and combat. She is the founder and volunteer clinical director of the Alamo Regional Response Team, which covers a 13-county area surrounding San Antonio and provides mutual aid throughout Texas.      In this episode you will hear Dr. Ediger talk about what led him from an early career as a Rural Family Medicine Doc to join the Air Force to become a Flight Surgeon.  He provides a behind the scenes look at what it is like to train to become a Flight Surgeon in the Air Force and support a fighter aircraft squadron.  He relates his experiences while serving as Command Surgeon for the Air Force Special Operations Command during the attack on 9/11.   He deployed to Iraq as Commander of the 363rd Expeditionary Medical Group and shares some of the unique challenges of preparing for and then supporting Forces invading Iraq.  He describes how the Air Force was able to adeptly handle a significant amount of en route medical care from the battlefield all the way back to the United States.   Leveraging lessons learned from his experiences in military aviation, he was able to improve communication and safety within Air Force Medicine modeling processes after high reliable organization principles. Elizabeth Wells-Beede, PhD, RN, C-EFM, CHSE-A, CNE is currenlty a Clinical Assistant Professor at Texas A&M University. She has been a nurse educator and simulationist for 12-years teaching in associate, bachelor, and master's programs. Before teaching in nursing, she was an educator for healthcare professions in the military and hospital setting. Dr. Wells-Beede has been incorporating simulation since 2011 when she led her educational department in the implementation of simulation through an initiative by the Air Force Surgeon General. In 2013, she furthered her education and sought the Leadership in Simulation Instruction & Management Certificate from Robert Morris University. Dr. Wells-Beede has conducted research in the use of the flipped classroom in nursing education and integration of virtual reality simulation in nursing. Since receiving her certificate, she has gone on to implement and help guide simulation in each program she teaches. She is actively involved in the College of Nursing Simulation committee, serves on an advisory board for the Health Sciences Clinical Learning Simulation Center, and was co-lead for writing and submitting for provisional accreditation with the Society for Simulation in Healthcare, which was recognized at IMSH 2022. Since receiving her CHSE-A, she has actively served the International Nursing Association for Clinical Simulation Learning by collaborating with fellow simulationist in the revised 2021 Healthcare Simulation Standards of Best Practice ™: Objectives & Outcomes and Professional Development. She has also been a presenter with the Best Practices workshop with SSH & ASPE, and is currently a member of the Board of Directors for INACSL.

This week we welcome Dr. Jennifer Sahmel and Kirk Phillips to explore the concepts of Total Exposure Health (TEH) and Total Worker Health (TWH). TWH's primary goal is to improve the well-being of the U.S. workforce by protecting their safety and enhancing their health and productivity. Total Exposure Health is a strategy to evaluate individuals' exposures to hazards at work, from the environment, and lifestyle choices, integrating these evaluations with health promotions initiatives to better ensure the long-term health of the individual. TEH and TWH take a more holistic approach to health, safety and well being. The concepts are gaining traction and this week we have two of the leading proponents joining us.   Dr. Jennifer Sahmel is a Managing Principal Scientist with Insight Exposure and Risk Sciences in Boulder, Colorado. She is a Certified Industrial Hygienist and a Certified Safety Professional with 25 years of experience in exposure assessment science and workplace health and safety. She has worked in a variety of public and private sector positions including the U.S. EPA's Office of Pollution Prevention and Toxics the National Park Service and Comprehensive Health Services at NASA's Goddard Space Flight Center. She earned her MPH degree in Environmental Health and Industrial Hygiene from the University of California at Berkeley and her PhD in Environmental Health at the University of Minnesota. She is a Research Fellow with the University of Minnesota's Exposure Science and Sustainability Institute. She is also active in the American Industrial Hygiene Association and is a past member of their national board of directors.   Colonel (ret) Kirk A. Phillips currently holds the position of Director, Air Force Office of Energy Assurance (OEA), Washington DC. Prior to his current position Kirk was the Health Safety and Environmental Practice Leader and Vice President at LJB Inc. In 2018, he retired as the BSC Associate Chief for Bioenvironmental Engineering (BE) in the Office of the Air Force Surgeon General. In 2014 he developed Total Exposure Health as a strategic initiative to institutionalize primary prevention in work, environmental and lifestyle exposures. Prior to his current assignment, Col Phillips was the Director of Policy and Programs for the Deputy Assistant Secretary of the Air Force for Environment, Safety and Occupational Health, Pentagon, Washington DC. Mr. Phillips entered the Air Force from a four-year AF ROTC scholarship in 1985. He obtained his BS in Aerospace Engineering and his MS in Engineering and Environmental Management. Colonel Phillips has held a broad range of leadership positions throughout the Air Force.

Bionca is a consultant for Nutrition Care Professionals and a counselor with Montgomery Community Corrections for domestic violence. She also serves as the President for Alabama's Association of Nutrition and Foodservice Professionals while also serving as Vice President for two associations: Autauga County Master Gardener Association and Alabama Herb Society. She is a retired Chief Master Sergeant, who served 30 years in our United States Air Force. She is also the only person to date in the history of the Air Force who has risen from the Dietary Therapy Career Field to Air Force Medical Service Senior Executive Management Level. Before assuming her last position in the Air Force, she held the highest position in the Diet Therapy career field as senior enlisted consultant to the Air Force Surgeon General on Air Force Dietetics governing 55 nutrition sites world-wide. The Niswonger College of Optometry at one of the nation’s oldest universities, Tusculum University, is totally focused on vision care in rural America “That is robbing these people of their vision faster than should ever be conceivable in America.” ~Dr. Andrew Buzzelli Dr. Andrew Buzzelli is the Executive Vice President for Tusculum University, College of Health Sciences, and the Founding Dean for Niswonger College of Optometry. Dr. Buzzelli received his doctorate in optometry from the Illinois College of Optometry in Chicago. He holds a Master of Science degree in child development and visual perception from the State University of New York. He has practiced in the private health care sector as a specialist in dysfunctions of binocular vision and visual information processing disorders. A noted international lecturer, Dr. Buzzelli served as a consultant to the National Aeronautics and Space Administration for the medical protocols currently utilized on the International Space Station. The author of more than 25 articles in both optometric and military publications and a recognized expert in the field of chemical and biological weapons, Dr. Buzzelli authored the first-ever ophthalmic textbook series for the treatment and management of injuries resultant from terrorist attack. A retired colonel in the United States Air Force, Buzzelli has held command positions as the chief of optometry for the 105th Military Airlift Wing and commander of the 105th Medical Group. He served as the assistant to the command surgeon for Air Force Material Command and was selected as the Chief Advisor to the Air Force Surgeon General of the Air National Guard. Recognized as an authority in the field of binocular vision and visual perception, Dr. Buzzelli has taught programs in the diagnosis and treatment of acquired brain injury, pediatric optometry, and binocular vision, as well as child abuse and intimate partner violence. He is one of 36 optometrists in the world recognized as a diplomat in binocular vision and perception for the American Academy of Optometry. Dr. Buzzelli has already led the efforts to begin several new programs including the Niswonger College of Optometry, He has and will continue to administer the preparation of materials and documentation required for accreditation and will ensure that the academic and patient care missions of the College are accomplished. He does this all while providing leadership for recruiting and retaining highly qualified faculty, staff, and students.

If you genuinely care for your fellow soldier, sailor, airman, marine or coastie, then you will gain the trust and respect of others and your unit will benefit from everyone's success. When you are willing to sacrifice for others, then we will know that you are attempting to make the best decision for your organization. In the end, you will walk away from your military service with very few regrets.

Executive Director, Jeffrey “Jeff” Sventek Jeff Sventek was born March 31, 1954 in Corry, PA and moved to Sherman, NY in 1961. He graduated from Sherman Central School in 1972. Following graduation, Jeff enlisted in the United States Air Force and spent the next 34 years in the Air Force, retiring in 2006 at the rank of Colonel. He holds a BS in Biology (magna cum laude) from the University of Nebraska at Omaha and an MS in Physiology from Rutgers University. Jeff is board-certified in Aerospace Physiology and is a Fellow of the Aerospace Medical Association. During his Air Force career, Jeff served as an AF Aerospace Physiologist and was selected by the Air Force Surgeon General as the first-ever Lieutenant Colonel to serve as the Chief of AF Aerospace Physiology. Jeff held a number of command and leadership positions from 1999 until he retired from the Air Force in 2006 as the Deputy Command Surgeon for the Air Force Materiel Command at Wright-Patterson AFB, OH and as 14th Chief of the AF Biomedical Sciences Corps. Following his retirement, Jeff served in various government-contractor positions providing human performance expertise for the Department of Defense. In January 2010, Jeff assumed his role as the eighth Executive Director of the Aerospace Medical Association in Alexandria, VA, the first non-physician to hold this prestigious position. Jeff is the son of Joseph and Shirley Sventek of Sherman, NY and is married to Debra S. Jostand of Taylor Ridge, IL. Lieutenant General (Dr.) Mark A. Ediger retired last year after serving as the Surgeon General of the Air Force, Headquarters U.S. Air Force, Arlington, Va. General Ediger served as functional manager of the U.S. Air Force Medical Service. In this capacity, he advised the Secretary of the Air Force and Air Force Chief of Staff, as well as the Assistant Secretary of Defense for Health Affairs on matters pertaining to the medical aspects of the air expeditionary force and the health of Air Force people. General Ediger had authority to commit resources worldwide for the Air Force Medical Service, to make decisions affecting the delivery of medical services, and to develop plans, programs and procedures to support worldwide medical service missions. He exercised direction, guidance, and technical management of a $6.1 billion, 44,000-person integrated health care delivery and readiness system serving 2.6 million beneficiaries at 76 military treatment facilities worldwide. Prior to that assignment, General Ediger served as Deputy Surgeon General, Headquarters U.S. Air Force, Arlington, Va. General Ediger is from Springfield, Missouri. He entered the Air Force in 1985 and has served as the Aerospace Medicine Consultant to the Air Force Surgeon General, commanded two medical groups, and served as command surgeon for three major commands. He deployed in support of Operations Iraqi Freedom, Enduring Freedom, and Southern Watch.  Full bio. I was so thankful that General Ediger, who retired just last year after 32 years in the military, agreed to be interviewed for the podcast to discuss a distinguished career spanning six administrations. We had a great conversation about everything from combat medicine advancements to tips on leadership, and even some quick advice on staying healthy and fit. His rise to the top of medical leadership the United States Air Force, from joining as a reservist to eventually becoming the Air Force Surgeon General, was (as he explains) the result of a quest for life-long learning, staying open to new experiences and opportunities, and building strong relationships with solid mentors.
