Sodium (and salt for that matter) has been made out to be the villain when it comes to nutrition. In fact, without sodium, you wouldn't be alive. While high amounts of salt in the diet are related to high blood pressure, the evidence to support a link with heart and other diseases isn't clear. In this episode we discuss why you need sodium, how much is too much, is sodium as harmful as we're told and how getting more potassium can help.   References cited in this podcast: Urinary Sodium and Potassium Excretion, Mortality, and Cardiovascular Events: https://www.nejm.org/doi/full/10.1056/NEJMoa1311889 Sources of potassium in food: https://www.dietaryguidelines.gov/food-sources-potassium

The average person consumes 39 000 to 52 000 microplastic particles per year through food and drink. This number doubles when taking into account how many microplastics we might inhale. Microplastics have been found in blood samples and almost every organ in the human body. What does the term "microplastics" mean to you? Why is seemingly everyone talking about them lately?  A new study published on March 6th in the New England Journal of Medicine titled, “Microplastics and Nanoplastics in Atheromas and Cardiovascular Events" has been circulating around the internet, and raises concerns about the risk of exposure to microplastics and long-term adverse health outcomes. What does this study suggest? What does the other data about microplastics show? Our exposure to plastics is ubiquitous; its in our packaging, our clothing, in our air and water. The chemical additives used in plastics manufacturing are also worrisome, as they have demonstrated adverse health effects in animal models. As we have hammered home in many prior episodes, DOSE and EXPOSURE matter when it comes to risk of harm from a particular substance. The same rules seem to apply for microplastics. In today's episode, Jeremy explains what we do and don't know about the risks of long term exposure to microplastics. He ends with some actionable items on how you can choose to limit your exposure with some simple daily behavioral changes. Resources for this episode include: The very recent NEJM article titled "Microplastics and Nanoplastics in Atheromas and Cardiovascular Events". A 2022 Frontiers in Endocrinology study regarding endocrine changes in mammals related to exposure to micro/nano plastics. A May 2023 article from National Geographic titled "Microplastics are in our bodies. How much do they harm us?" A March 19th, 2024 Everyday Health article on microplastics. It aimed to investigate the impact of dietary interventions, specifically the reduction of saturated fat intake and an increase in polyunsaturated fat intake, on cardiovascular health. The Oslo Diet-Heart Study involved 412 men who had already suffered a myocardial infarction 1-2 years before the start of the intervention. Despite some known limitations, the Oslo Diet-Heart Study played a role in shaping early understanding on the relationship between dietary fat, cholesterol levels, and heart disease. Subsequent research and larger studies have contributed to a more nuanced understanding of the complex factors influencing cardiovascular health. In this episode we discuss why this trial is important in the history of diet-heart research and how it connects to other seminal work in the field.

Bempedoic acid is an effective option for some statin-intolerant patients with an elevated predicted risk of cardiovascular events. However, as JAMA Executive Editor Gregory Curfman, MD, discusses with editorialist Dhruv S. Kazi, MD, MSc, MS, bempedoic acid should not be considered a substitute for statins, which remain the first-line therapy for primary prevention.
