Prostate artery embolization may be performed by interventional radiologists, but its indications are rooted in urologic evaluation. In the second installment of our 2026 PAE University Series, Dr. Chris Beck is joined by Dr. Art Rastinehad of Northwell Health, a urologist with formal interventional radiology training, to share how his dual background informs both when to offer PAE and how to execute it thoughtfully. --- This podcast is supported by an educational grant from Guerbert. --- SYNPOSIS Dr. Rastinehad discusses his path from urology into IR and how that combined training shapes his current hybrid practice. He outlines a practical BPH consult framework grounded in urologic evaluation, emphasizing appropriate imaging, careful patient selection, and the importance of ruling out malignancy before proceeding with embolization. From his perspective, durable outcomes begin with disciplined workup and clear counseling around expectations, including sexual side effects and alternative treatment options. The conversation then turns to procedural strategy. Dr. Rastinehad reviews anatomic considerations, large-gland and technically challenging cases, and his experience incorporating liquid embolics into PAE. He compares glue and particles, detailing workflow decisions, medication strategy, and post-procedure management. Throughout, he highlights scenarios where PAE may not be the most appropriate intervention and how other BPH tools may better serve the patient. The episode concludes with a discussion of the future of PAE, including questions of training, collaboration between specialties, and reimbursement; underscoring the value of cross-specialty insight in contemporary BPH care. --- TIMESTAMPS 00:00 - Introduction01:26 - Interventional Urologist with IR Roots04:13 - Leaving Urology for IR: Fellowship Life, Case Volume & Mentors08:45 - Building a Hybrid Urology/IR Practice14:32 - PAE Benefits, Sexual Side Effects & Why MRI Matters17:39 - BPH Consult Playbook22:17 - Anatomy Deep Dive24:27 - Edge Cases & Big Glands28:24 - Why Glue?35:39 - Glue vs Particles39:40 - Post-PAE Follow-Up41:28 - Antibiotics and Medications46:18 - Tough Cases50:53 - The Future of PAE --- RESOURCES Early Outcomes of Prostatic Artery Embolization using n-Butyl Cyanoacrylate Liquid Embolic Agent: A Safety and Feasibility Studyhttps://pubmed.ncbi.nlm.nih.gov/39074551/ Dr. Rastinehad's Websitehttps://drrastinehad.com/

How Aurelius Systems proved Viability through nationwide field demonstrations in extreme conditions | Michael LaFramboise

Category Visionaries

Play Episode Listen Later Dec 18, 2025 24:29

Aurelius Systems is tackling one of defense's most critical challenges: cost-effective counter-drone warfare. The company builds lightweight, edge-deployed laser weapon systems with 10-million-x marginal cost advantages over traditional interceptors—shooting down drones for approximately 10 cents versus $2 million per Sea Sparrow missile. With systems priced in hundreds of thousands rather than tens of millions of dollars, Aurelius is proving that commercial manufacturing principles can revolutionize defense technology. In this episode of BUILDERS, I sat down with Michael LaFramboise, CEO and Co-Founder of Aurelius Systems, to unpack how his background spanning automotive manufacturing at Chrysler, R&D at Coherent (the largest U.S. laser manufacturer), and defense sales positioned him to build what he calls "the F150 of directed energy systems." Topics Discussed: Why Michael's unusual combination of heavy industrial manufacturing, high-power laser R&D, and directed energy sales made him one of "probably like five people under 70 in the country" positioned to build this company Aurelius's contrarian R&D thesis: build everything from commercial off-the-shelf components first, only upgrading to bespoke when field tests fail The tactical fundraising progression: first prototype to pre-seed, DIU grant in February 2025, Singapore Defense Force joint challenge, Army X-Tech competition wins Government relations as infrastructure: why Aurelius retained a lobbyist six months post-pre-seed and how Congressional support addresses 1-3 year sales cycles Navigating the DOD acquisitions reorg: 100+ technology acceleration organizations consolidating to 10-20 under new PAE structure, with goals of 90-day turnarounds replacing multi-year cycles The demonstration strategy that changed everything: earning signed memorandums from high-ranking officers after shooting down drones in Hawaii and Austin under adversarial conditions (heavy rain, 99% humidity, heat warping, night operations) Founder-led marketing ROI: why acquisitions officers, funders, and engineering talent all follow different channels (LinkedIn vs. X) and require different voices The three-stakeholder sales complexity: when your end user (warfighter), purchaser (acquisitions), and budget authorizer (Congress) are separate entities who don't communicate GTM Lessons For B2B Founders: Follow proven playbooks in specialized markets, then execute obsessively: Michael explicitly followed Anduril's early-stage defense playbook, particularly around government relations: "I think it's like following the Anduril playbook for how you do an early stage defense company is probably a very appropriate thing to do." In highly specialized B2B markets (defense, healthcare, financial services), pattern-match to companies that have successfully navigated regulatory and procurement complexity rather than inventing process from scratch. The differentiation comes from execution and technology, not from reinventing go-to-market structure. Treat specialized expertise as infrastructure, not overhead: Aurelius hired a lobbyist six months after their pre-seed—before significant revenue—because defense sales involve three disconnected stakeholders. Michael explained: "your purchaser, your end user, and your authorizer for funds are all separate people that don't know each other... whenever you have these different points, it doesn't expand linearly the difficulty or the complexity of the sales cycle. It expands exponentially." B2B founders should map stakeholder complexity early and staff accordingly. If your buyer doesn't control budget, your user doesn't make purchase decisions, or your champion needs internal air cover, these aren't edge cases—they're your sales model. Demonstration beats documentation when overcoming category skepticism: After decades of directed energy failures, Aurelius spent 2024 conducting nationwide field demonstrations, culminating in adversarial drone shoot-downs in heavy rain, 99% humidity, and night conditions. Michael noted they needed to "clean up the mess that a lot of these other companies have created" with signed memorandums from high-ranking officers. When your category has a failure history, customer education isn't about better pitch decks—it's about systematic proof that eliminates objections through witnessed performance. Plan for demonstration costs and timeline in your first-year budget. Build your R&D thesis around manufacturing reality, not engineering perfection: Aurelius's core principle: build everything from commercial off-the-shelf components, upgrading only when field tests fail. Michael's insight from automotive and laser manufacturing: "you can get 80-90% physics perfection on a system for 2% of the cost" versus traditional directed energy's approach of "400 ARL and AFRL PhDs all coming together to make the most super bespoke, hyper perfect thing ever." They use material processing lasers (identical output at 1/10th the cost of directed energy lasers) and commercial components from automotive supply chains. B2B founders should define their "good enough" threshold explicitly and build cost structure around it—perfection is often the enemy of scalability and margin. Attack market dislocations where wrong-fit solutions reveal unmet needs: Aurelius doesn't compete with Sea Sparrow missiles for shooting down aircraft at 9 miles—they target the dislocation where $2M missiles designed for large ordinance are being misused against $500 drones with 30% effectiveness. Michael identified that "there isn't anything in the market that's been developed for counter drone at any significant distance." The opportunity isn't better missiles; it's purpose-built solutions for Group 1 and Group 2 drones (FPV quadcopters and small planes) where no appropriate system exists. Map where customers are forced to use expensive, inappropriate solutions—that's where new categories emerge. //  Sponsors: Front Lines — We help B2B tech companies launch, manage, and grow podcasts that drive demand, awareness, and thought leadership. www.FrontLines.io The Global Talent Co. — We help tech startups find, vet, hire, pay, and retain amazing marketing talent that costs 50-70% less than the US & Europe. www.GlobalTalent.co // Don't Miss: New Podcast Series — How I Hire Senior GTM leaders share the tactical hiring frameworks they use to build winning revenue teams. Joe is joined by Quin Cho, Author and Fellow, Pacific Atrocities Education to discuss his books and expertise on World War 2. Quin Cho is an accomplished historian and author specializing in the history of global conflict and geopolitics in the Asia-Pacific region. A 2023 summa cum laude graduate of the University of Southern California, Quin earned a 4.0 GPA as a double major in History and International Relations. His academic excellence and rigorous training in archival research and historiography earned him the prestigious Outstanding Senior Award from USC's Phi Beta Kappa society, an honor bestowed upon only three students in his graduating class.Quin's debut book, Competing Empires in Burma: A Chronicle of the China Burma India Theater of World War II, achieved critical acclaim, ranking in the top 10 of Amazon's military regiments category and earning an average rating of 4.91 stars on Goodreads.Currently, he is authoring a multi-volume work on the Kwantung Army, exploring its role in the false flag attack on the South Manchuria Railway that precipitated Japan's conquest of Manchuria and set the stage for World War II in the Asia-Pacific.As a fellow at Pacific Atrocities Education since January 2024, Quin has made significant contributions to the organization's mission. Despite a three-month sabbatical to study Korean through the State Department's Critical Language Scholarship, he wrote 176 pages of his forthcoming book, which now totals approximately 427 pages. In March 2024, Quin conducted extensive archival research at the National Archives in College Park, Maryland, scanning thousands of pages of primary source documents. These materials enriched his book and expanded PAE's archival collection. Additionally, he completed a 40-page mini-book on the Battle of Wuhan, slated for release in early 2025.Looking ahead, Quin's first volume on the Kwantung Army (150–200 pages) is scheduled for release in September 2025, coinciding with a commemorative event at the Officers Club in San Francisco marking the 80th anniversary of World War II's end. This strategic launch aims to amplify the book's reach, with subsequent volumes planned for future years. To continue this ambitious project, Quin seeks to extend his fellowship with PAE through 2025 and 2026, ensuring further contributions to the organization's educational and archival efforts.

A lot of companies use positive-sounding cliches like “people over profit” and “work-life balance”. This sounds good, until somebody pulls the sales reports and says “we need more revenue and profits”. Then, the bottom line becomes the bottom line. This is sometimes served with a side of “you come to work to do work, change the world on your own time”.Now, picture a model where putting people over profit actually increases profits. Where work-life balance is not measured using number of hours worked or percentage of time in the office vs. working remotely. Being involved with a company that breaks the rules and norms for its industry, where coming to work to do work means doing your part to change the world.Christian Agulles has created all of these things in an industry known for squeezing its engineers like getting that last drop out of the toothpaste tube.Born and raised in College Point, a little town in Queens, Christian was a devout Catholic who went to church, went to Catholic school, and had the whole Catholic experience that, as he describes it, formed him and scarred him at the same time. His father, a police officer, was shot in the line of duty when Christian was seven. Even being able to retire on three-quarters pension, that money didn't go far supporting a family including Christian and his two younger brothers. His mother went to work in the school cafeteria so she could align her schedule with her sons, and cleaned houses on weekends. By example, she taught Christian to not be afraid to do hard things and to do them with grace. While starting his family, Christian spent the first 20 years of his career with an engineering firm that moved him around the country to open new offices. But over those 20 years, his employer was repeatedly acquired and evolved into company that focused more on acquisition, revenue, and EBIDA. It was right around this time he was recruited to join PAE. Everything worked out fine, yes? Well, Fine is a 4-Letter Word. Did his moment of realization come from a coaching session at PAE with Sandy Lindstrom, known for the “polishing the diamond” principle, where he found out they did not feel he was very good at delivering feedback? Was it discovering that his foundational values around doing hard work with grace could be merged with volunteerism, altruism, and contribution? In a moment, when I bring Christian on, he's going to guide us through his experience leading a Certified B-Corporation and how he's taken one of their metrics – utilization – and redefined it to build PAE into a company that has a net positive impact not only on its employees, but on society as a whole. If, up until now, you've seen your best-laid plans get trumped by “the bottom line” when bills are due, this could be what you need to hear. Christian's hype songs are "Bring It On Home To Me" by Sam Cooke and "Right to Be Wrong" by Joss Stone.Resources:Christian Agulles' website: https://www.pae-engineers.com/LinkedIn: https://www.linkedin.com/in/christian-j-agulles/Invitation from Lori:This episode is sponsored by Zen Rabbit. June 6, 2025 In this episode, Ray, Mark, and Scott welcome back Dr. John Lin to discuss topics brought up on the Thriving Urology Practice Facebook Group.1) Just got a call from ER. 19 year-old cannot get an erection...2) This notice came from the hospital last week regarding change in financial protocol about scheduled / elective surgical procedures.Increased Patient Responsibility:As is customary in the Phoenix market and in-line with other providers, we will move the down payment requirement to 50%. We will be increasing the required patient payment for deductibles and co-pays prior to the day of surgery from 20% to 50% of the estimated patient liability.3) Our office will be beginning PAE {prostatic artery embolization) for BPH in the near future. This will be performed in the office, POS 11 by an interventional radiologist, who will manage the sedation and the performance of the procedure. We have not had to bill for conscious sedation in the office before. I was hoping to get some guidance on what codes should be investigated for the anesthesia portion (administration, meds, etc.) so we can better prepare ourselves for what is to come.Bonus TopicI received an email from a surgery center recently which stated the following: Recently, we have been using a larger number of disposable ureteroscopes which are very costly to the Center. In order to capture the code C9761 for reimbursement of the disposable flexible ureteroscopes, we need your dictation to reflect the usage of that scope and that suction/aspiration was used.  A vendor's representative has provided a few examples from other physicians that have resulted in successful reimbursement of these scopes. Below are the examples. The op report documents a steerable and suction/aspiration was used. It says "A disposable single use flexible scope with steerable and suction/aspiration was used to improve stone-free rate."The op report documents only a scope using a suction feature. It says "A disposable flexible ureteroscope utilizing a suction feature was utilized for the procedure."   If you are not already, please have your dictation reflect that you are using a disposable flexible ureteroscope with suction so that we can bill for reimbursement. As always, we appreciate your help with cost containment and appropriate reimbursement for the Center. This week, Libby Pritchard welcomes industry veteran Rob Lanham back to the podcast to share his personal health journey following over 42 years in Virginia's transportation construction sector. Rob recounts how a routine medical check-up uncovered a life-threatening condition, highlighting the importance of early detection and regular screenings. After recovering from a heart attack in 2021, a prostate issue led to the discovery of a massive tumor, underscoring the unpredictability of health challenges. Rob reflects on his emotional journey through a cancer diagnosis and recovery, emphasizing the life-saving impact of routine medical care and early intervention. His story serves as a powerful reminder to prioritize personal health, even when there are no apparent symptoms. Main Themes: The significance of routine health check-ups and early detection Rob's journey from a heart attack to discovering a cancerous tumor The emotional impact of waiting for a diagnosis and navigating the healthcare system The life-saving benefits of minimally invasive procedures like prostate arterial embolism (PAE) The importance of listening to your body and taking action when something feels off Rob's surgery to remove a 14-pound liposarcoma tumor and his fortunate recovery The ongoing process of regular scans and follow-up appointments to monitor his health Toolbox Talk Discussion Questions: Rob talks about how seeking treatment for a normal medical concern revealed a tumor he didn't know he had. What does this episode make you think about regular doctor's appointments?  Speakers on this podcast have previously talked about listening to their bodies, but even when your body isn't telling you something is wrong, there are things you can do to support your health. What are some examples?  Does anyone have a story they'd like to share about a relatively routine medical procedure that turned into a more serious health situation, for themselves or for others? Key Takeaways: "Life was pretty good last fall. Retirement was on the horizon, and then everything changed." "Choosing the PAE procedure probably saved my life because it required a CAT scan that uncovered a much bigger issue." "The scan read prostate enlarged... and additional findings, a large liposarcoma tumor." "That's probably the scariest words I've ever heard: 'We need to refer you to an oncologist.'" "The tumor weighed 14.92 pounds... I had no idea it was growing inside my body." "I went through weeks of waiting, scans, and appointments—it felt like time was moving so slowly." "Luckily, the cancer had not spread. The tumor was confined, and the surgery was successful." "It's a reminder to never skip a routine check-up, even when you feel fine." "Early detection saved my life—don't ignore routine health checks." Links: National Stone, Sand & Gravel Association website

Last month, a 5-story commercial building in Portland’s Old Town historic district was awarded one of the most rigorous sustainability certifications in the world, given by the Portland-based International Living Futures Institute. There are only three dozen certified Living Buildings in the world, and the Portland building is the first commercial one to achieve it. Living Buildings must meet a range of strict requirements, from the design to operation. That includes using renewables like solar power to generate at least 5% more energy than is needed and harvesting rainwater to supply all the water for drinking and other uses.  The PAE Living Building at SW Pine and 1st Ave. not only met these standards but went one step further by being engineered to last at least 500 years to withstand a massive Cascadia Subduction Zone earthquake. Kathy Berg is an architect and partner at ZGF Architects and the project lead. Paul Schwer is the president emeritus of PAE, an engineering firm that co-owns, along with ZGF Architects, the PAE Living Building and is headquartered inside it.
