The hype around Vitamin D supplementation has waxed and waned over the years in the lifting community, but has overall, landed in a place where it's more or less universally seen as a positive supplement. But what is the state of the data? What can you expect as a lifter out of vitamin D supplementation? How do you measure vitamin D status, how reliable are these measures, and should we even be measuring it? The answer probably differs at the population and individual levels. We're joined by Dr. Niamh Aspell of Sigma Nutrition who did her Ph.D. on Vitamin D as well as Austin Baraki, MD from barbell medicine who deals with this on a clinical level on a regular basis to answer these questions.

Between the latest online fads and the crazy media headlines, it's easier more than ever to get confused about your health. There are a lot of arguments as to how Vitamin D can be beneficial to humans, and the recent one is its contribution to fight COVID-19. I stumbled across one of Dr. Deva Boone's articles about people easily getting hypercalcemia or high calcium levels even from mild excesses of Vitamin D. I recommended before to avoid higher levels of Vitamin D as it is less useful and makes you less productive but it turns out it can also be quite dangerous. Joining us is Dr. Deva Boone to understand more of how Vitamin D works in our body and what are the best practices to think about for our general welfare. Dr. Deva Boone is one of the most experienced parathyroid surgeons and has become a renowned expert on calcium and Vitamin D. She attended Cornell University medical school, completed a general surgery residency in New York City, and an endocrine surgery fellowship in Chicago. She spent the past six years as a parathyroid surgeon at the Norman Parathyroid Center, and was the Medical Director there for the past three years. She is currently obtaining an executive MBA at the University of South Florida, and is planning to open a parathyroid surgery center in Arizona in 2021. Anyone following college and professional sports has witnessed the lengths that colleges and professional organizations are talking to keep their respective players and fans safe from the ravages of COVID-19, while attending sporting events, albeit with limited seating capacity.Sporting events have been cancelled or postponed due to a team experiencing multiple player COVID exposures. Arenas have been mandated, at times, to have no fans in attendance, due to the corresponding local population, exceeding a 5% population COVID-19 exposure rate.According to research - The Benefits of Vitamin D Supplementation for Athletes: Better Performance and Reduced Risk of COVID-19 - which appeared in December 2020, in the online, peer-reviewed journal Nutrients, "a hypothesis now under scientific consideration is that taking vitamin supplements to raise serum 25-hydroxyvitamin D [25(OH)D] concentrations could quickly reduce the risk and/or severity of COVID-19.The research investigators from Sunlight, Nutrition, and Health Research Center in San Francisco, Vitamin D Wiki in Washington State, and the Departments of Human Nutrition, Foods, and Exercise and Center for Transformative Research on Health Behaviors at Virginia Tech University, comment that, "clinical trials have confirmed that vitamin D supplementation reduces risk of acute respiratory tract infections, and approximately 30 observational studies have shown that incidence, severity, and death from COVID-19 are inversely correlated with serum 25(OH)D concentrations (how vitamin D is measured in the blood)."These investigators, in a narrative review of the data and literature to date, chose to evaluate, "the use of vitamin D supplementation to raise serum 25-hydroxyvitamin D [25(OH)D] concentrations to optimal values, which may be at least 40 ng/mL for sports. The benefits of vitamin D for athletic performance and general well-being are similarly reviewed."In terms of the Vitamin D lab values - mean 25(OH)D concentrations of less than 15 ng/mL were generally associated with an increased severity and risk of death for COVID-19 patients. Researchers at The University of Glascow just pre-published this conclusion following their analysis of 656 CoVid patient case reports documented the the UK Biobank. At first glance, the unadjusted numbers suggested that Vitamin D deficiency might trigger a greater susceptibility of CoVid. A deeper data dive acquitted Vitamin D while indicting advancing age, male gender, socio-economic disadvantage, obesity, and diabetes. You do want normal Vitamin D levels to maintain strong bones and normal vital organ functioning. Objectives: To explore if short term, high dose vitamin D supplementation is safe and improves balance in persons with Parkinson's disease (PD). Methods: A pilot randomized, double-blind intervention trial to measure the effects of 16 weeks of high dose vitamin D (10,000 IU/day) on balance as well as other motor and non-motor features of PD. We measured balance, gait, strength, falls, cognition, mood, PD severity, and quality of life before and after 16 weeks of high dose vitamin D supplementation or placebo. All participants also received 1000 mg calcium once daily. Results: Fifty-one randomized participants completed sixteen weeks of high dose vitamin D supplementation or placebo. The intervention resulted in a rise in serum concentrations of vitamin D (25-OH) (30.2 ng/ml to 61.1 ng/ml) and was well tolerated with no serious adverse events. Serum vitamin D (25-OH) levels rose steadily and did not suggest a leveling off at the end of the 16 weeks. There was not an improvement in the primary endpoint, balance as measured by the Sensory Organization Test (p = 0.43). A post hoc analysis examining treatment effects in younger (ages 52–66) versus older (ages 67–86) participants found a significant improvement in the SOT of 10.6 points in the younger half of the cohort (p = 0.012). Conclusions: Short term, high dose vitamin D supplementation appears safe in persons with PD, but did not significantly improve balance as measured with the Sensory Organization Test in this pilot study population. A post hoc analysis suggests that vitamin D may have potential for improving balance in a younger population with PD. High dose vitamin D supplementation in PD needs further study especially in light of new research suggesting that mega doses and even moderate doses (as low as 4000IU a day) may increase falls in an older populations. Is There a Connection Between Vitamin D and Hashimoto's Disease? Does Vitamin D Supplementation Help Heal Hashimoto's Disease? Vitamin D has long been established in literature as a highly essential nutrient with benefit to the musculoskeletal system and bone density. It also functions in the body as an immunomodulator, facilitating normal immune system function and improving resistance against certain diseases. Given this background, one has to wonder if a deficiency in vitamin D would be prevalent among individuals with Hashimoto’s thyroiditis and if so, would supplementation with vitamin D help patients manage the disease or perhaps even prevent it? PART ONE – Vitamin D Deficiency and Hashimoto’s Disease Research goes back to 2009 on the first question: is there a connection between vitamin D deficiency and Hashimoto’s disease? The earlier studies either indicated that indeed there was a connection while other studies concluded that there was none at all. How is one to draw a final answer when the outcomes are 180 degrees apart? Kmiec and Sworczak (2015) reviewed twelve studies published between 2009 and 2014 whereby seven of those studies concluded that there was a connection between lower vitamin D levels and Hashimoto’s thyroiditis while two other studies showed no association and three others were inconclusive. Among the studies that established a connection included the ones by Bozkurt et al. (2013), Camurdan et al. (2012) and Mansournia et al. (2014). Goswami et al. (2009) showed no association between vitamin D levels and thyroid peroxidase antibody (TPO-Ab) positivity. What was the takeaway message from the Kmiec and Sworczak article? The authors concluded that no final word on a correlation could be made. It was neither an absolute ‘yes’ nor a definitive ‘no’. They reported: “…in many points accumulated data are inconclusive, many unresolved questions remain, therefore, it remains necessary to perform further studies that would affect clinical approaches to thyroid disease.” They went on to state that the idea of vitamin D supplementation being able to influence the levels of antibodies in Hashimoto’s was also inconclusive: “Moreover, vitamin D supplementation has not affected disease occurrence in intervention studies, as summarized in 2 recent reviews. The associations between vitamin D deficiency and disease may indicate that 25(OH)D is only a marker of ill health ([Theodoratou et al. 2014]; [Autier et al. 2014]).” Fast forward a few years, and what does the research reveal on whether there is a connection between vitamin D and Hashimoto’s thyroiditis? Anaraki, et al. (2017) found no association between vitamin D levels and Hashimoto's thyroiditis. Boyuk et al. (2016) also reached the same conclusion of no correlation between vitamin D and Hashimoto's thyroiditis. In 2018, Botelho, et al. reviewed several studies conducted around the world between 2012 and 2016 that basically led to the findings that an association between vitamin D and Hashimoto’s Thyroiditis “…remain unresolved in literature”. Some of these included: D' Aurizzio, et al. (2015) No differences in vitamin D deficiency in Hashimoto's thyroiditis patients and healthy controls Yasmeh et al. (2016) No association of vitamin D deficiency and Hashimoto’s relative to controls On the contrary, Sun et al. (2017) revealed Vitamin D levels were inversely correlated with positive TPO-ab and higher D levels were linked to lower TSH in males. Ma et al. (2015) found that there were lower levels of D in Hashimoto's thyroiditis patients relative to controls. Wang et al's. (2015) meta-analysis of twenty studies and the Mazokopakis et al. review (2014) also corroborated with Ma’s conclusion that there is an association. Bozkurt et al. (2013) found a direct relationship between vitamin D and Hashimoto's thyroiditis. He and his colleagues looked at 180 Hashimoto's thyroiditis patients...
