See all the Healthcasts at https://www.biobalancehealth.com/healthcast-blog In my Anti-Aging-Longevity practice, one of the complaints my new patients tell me about is the fact that their hair is thinning, falling out or changing to a brittle texture, as well as the fact that their hair is turning grey. Hair DOES get thinner, coarser, and greyer as we age!  That is a fact, and one I can't fully explain to you, except that it happens to everyone!  My philosophy is that each of us should work with what we have, and optimize it, as well as treat any condition that makes hair loss work instead of wishing and wishing we had the hair we did when we were 25! Accepting the fact that your hair gets thinner after 40 and working with the hair you have takes knowledge to determine what is normal and what is not. Some of the things that everyone should know before looking for the best path forward to healthy hair. Aging and Slower Hair Growth Low Growth Hormone Lack of Estradiol and T in women and lack of T in men Aging causes Growth Hormone (GH) to decrease, which slows the growth of hair, fingernails, bone.  Your hair falls out at the same rate as it did when you were young, but the growth slows which results in losing more hair than you replace which thins your hair! If you started your young life with fine, thin hair, then this difference between growth and loss of hair can make your hair very thin.  To get to the root of the problem (I apologize) increasing GH will make your hair grow a bit faster. Testosterone replacement and Estradiol replacement both increase the growth of GH. Both hormone replacements increase hair thickness. Testosterone has a second benefit.  Testosterone makes your scalp oiler, which in turn increases the longevity of your hair. At menopause the lack of these two hormones causes a big change in hair thickness. Loss of hair in specific areas—Balding Genetics Scarring alopecia Androgenic alopecia Extensions Dread-locks Balding and alopecia both cause a person to lose hair follicles, not just slow the rate of hair growth.  Balding usually is genetically determined, so look at the older members of your family to determine what is in your future.  This type of hair loss is very difficult to treat. 50% of men have some balding by age 50, and 1/3 of women experience it sometime in their lives.  Until recently there was nothing to stop this process or grow more hair in those areas, however both men and women can resurrect their hair follicles (if they haven't been gone too long) and make them grow with the TED hair restoration painless ultrasound treatment by Alma. Other options are Hans Weiman hair transplants or weaves, both of which are extremely expensive, don't necessarily look natural (President Biden and his son) and must be redone every couple of years. Women can just cover over the problem with a wig, but even that answer has drawbacks—they are hot and itchy until you get used to it. Women can also get extensions to make their hair look thicker, but it ruins the quality of the hair you have which in the long run causes even more hair loss from scarring of the scalp. Women of color have traditionally used tight braids, cornrowing or dreadlocks to control their hair.  This cultural process causes them to break their hair off at the scalp and damage the hair follicle from tension, which results in hair no longer growing in multiple areas of the scalp. Change in Hair Texture and Dryness At menopause for women and when men's testosterone gets very low, we notice a change in texture of our hair. The cuticle area that covers the hair shaft becomes fragile and stops protecting the hair shaft, so the texture becomes frizzy, and hair breaks causing a dull look similar to what my mother used to call a “birds nest”. Our scalps become dry, and the oil glands dry up with age and loss of sex hormones, so hair is dry and frizzy, making us look like we just stuck our fingers in a light switch.  You can read about many “natural remedies” but beyond taking collagen, Biotin and B vitamins most supplements don't work in a dramatic way that would be noticeable. The remedies for hair loss include all the following and you will have to do most of them to improve your aging hair! Hormone replacement of Estradiol and Testosterone Conditioners (which only work a little) Hair color which covers the shaft with pigment and strengthens hair Brazilian treatment that drives straightener into the hair shaft and seals it with heat. Take supplements of Collagen every morning Take methyl B12 and Biotin daily Stop bleaching your hair Eat a diet with healthy fats and protein Wash your hair every 2-3 days Take the fat soluble vitamins A.E.K,D If you are anemic take iron supplements Avoid statins if possible   Other medical causes of frizzy, broken hair can be found in low thyroid hormones which slows hair growth, decreases oil production, and results in brittle hair all over the head. Replacing your thyroid hormones and supplementing your iodine can overcome this obstacle. All medicine changes should be managed by your doctor. Many drugs cause hair loss, and you can't change some of them: Metoprolol or any Beta blocker Blood pressure medication Prednisone and all steroids Cancer treatments Anything that inhibits your B vitamins like some autoimmune diseases What can you do to fix what you can fix! Nutrition: Hair is protein; however hair requires oil (fat) to grow and be beautiful—a diet rich in protein, and healthy fats give you the building blocks to make healthy hair and skin. Add Vitamins of A/E/K/D, vitamin C, Multi Methyl B vitamins with methyl folate and Biotin because our diets aren't perfect!   Hair care: Get hair products without sulfides.  They break hair and make it weak!  Wash your hair as little as possible. Decrease the use of hot hair tools like flat irons.  Color your hair and or Brazilian it to make it stronger. Medications not to take are listed above. But the medications to take to help your hair are your sex hormones, Estradiol, Testosterone, and make sure your DHT doesn't get too high which can cause hair loss in the male pattern. You may need Finasteride or Minoxidil if you have male pattern hair loss. Sun damage is important to maintain your hair in sunny areas.  Wear a hat or scarf when outside and comb some conditioner through your hair at the pool to “cover your hair from sun damage”.   Summary: Now that you know the possible causes and treatments for hair loss, you can do everything possible up to seeking medical care, and then medical care may be necessary. Hair thinning is often familial and also due to our estradiol and testosterone hormones decreasing after 35-40 years of age. You may need a scalp biopsy from a Dermatologist if your hair loss is in patches or severe over a short period of time, which implies an autoimmune disease. When you see a doctor be prepared with a list of your hair products, your diet, a list of medicines and a timeline for your hair loss.  This preparation will get you the best treatment per doctor visit, and your doctor will appreciate not having to ask you all those questions!

This week on HairPod, I sat down with an expert in all things hair loss, Dr. Angie Phipps. She is a hair transplant surgeon with over two decades of experience in the industry, and she was the star of the TLC reality series, Bad Hair Day. She joined us to shed some light on an important issue that affects over 100 million Americans: male and female pattern hair loss. Female vs. Male Pattern Hair Loss Female pattern hair loss is different from male pattern baldness in a few key ways. They are both caused by the impacts of hormones on different receptors throughout the body, and these hormones cause hair loss when those hormones affect our hair follicles in certain ways. In men, this type of hair loss centralizes on certain areas, like the forehead and top of the crown. Women can experience thinning in any zone, and it typically happens in a more diffuse way. For both men and women, the amount of hair loss you'll experience due to this condition is genetic, and science doesn't have a consistent way to shut down the gene or the receptors. How Effective is Finasteride? Male pattern hair loss happens due to one specific hormone, and science has found a way to block that hormone without any negative side effects. That treatment is called finasteride, and it helps many men stop their hair loss. Women, on the other hand, experience hair loss due to a combination of hormones, and there are negative effects to blocking them. This means that hormonal treatments are only really helpful for men at this time. For men who don't experience great results with finasteride, Dr. Phipps typically prescribes dutasteride. In either case, the medications must be continued to block the hormone. If the medication is stopped, hair loss will resume. Finasteride as a Preventative Measure Genetic hair loss can skip generations, but if it's somewhere in your family history, you may very well experience it. Dr. Phipps recommends starting the medication early so that you don't experience extensive hair loss before stopping it. While there are always risks for medication to cause side effects, finasteride has been studied long-term and it has not had major long-term side effects on any internal processes or organs.  Is Finasteride Right for Me? Regular hair shedding is normal, so how do you know if you're experiencing hair loss? It's normal to lose 100-150 hairs per day, so if you see a bit of hair in your comb every day, there's no need to panic. But if you start to see an increasing amount of hair coming out in your brush or in the shower, there's no harm in consulting with a professional. They have the tools and knowledge to determine whether you're experiencing hair loss. If you think you're experiencing hair loss and hair loss runs in your family, Dr. Phipps suggests starting preventative care early. But hair loss can be caused by other factors, and a hair loss specialist can give you an idea of what is causing the hair loss. Rather than trying just one tactic, the greatest chance you have for success in reducing your hair loss is to take a multifaceted approach. Oral proteins are ineffective because they are broken down in the digestive system and cannot be absorbed properly. Topical proteins are similarly ineffective because their large molecules cannot penetrate the skin.Darshan mentions stem cell treatments but notes that the evidence for their effectiveness is still limited. Experiencing hair loss at a young age, or any age, can be challenging; there are a lot of options out there, and finding the right solution often requires time and guidance. In this episode of HairClub, our host, Kevin Rolston, shares his experience with hair loss solutions.  The Shock of Balding and the Hunt for the Best Hair Regrowth Treatment Like many of us, Kevin's hair loss was not just a physical change but also an emotional journey. It was a part of his family history, and he had always dreaded the day his hair would start to thin. So when he began balding at 26, it was not a shock, but it was still a challenge. Kevin took immediate action, trying over-the-counter topical solutions without much luck. In the early years of his hair loss, Kevin received some guidance and began taking Finasteride. The medication successfully slowed his hair loss for a time. As the years passed, Kevin noticed increased hair loss and began seeking new methods. It is important to keep in mind, that just because something works for a time, this does not mean it will work forever. The good news is there are many different options for all stages of hair loss.  Searching for Hair Loss Solutions - Laser Caps and Hair Transplants Determined to have a full head of hair, Kevin tried laser hair therapy. He found it to be convenient and accessible. However, Kevin's hair loss became more significant as he aged; he wanted a permanent solution. At this point, he decided to undergo a hair transplant procedure. He shares what this experience was like and some of the key things to look into when considering a transplant. Kevin was very satisfied with the results of his procedure for many years, but his journey wasn't over yet. It occurs whether or not a patient is taking any sort of medication or remedy for hair loss. But when transitioning from Finasteride to Minoxidil or vice versa, it certainly is possible to experience some degree of shedding. This is because they work differently through different mechanisms. Finasteride primarily inhibits the hormone responsible for hair loss, miniaturization, thinness, brittleness, and all the negative things that go along with male pattern hair loss and also in females as well, to some degree, and that is DHT. Minoxidil works through a different mechanism relative to Finasteride. Although there is some overlap and evidence in smaller studies about the hormonal impacts of Minoxidil, it is not a potent DHT blocker. Minoxidil, which is an old blood pressure medication, works by stimulating hair growth. The theory is that it increases blood flow through the follicle, which increases nutrient delivery to the follicles themselves and can enhance hair growth. The mechanism is more complex, but I'll save that for another video. Obviously, hair loss is very individual, depending on genetics. Lifestyle, hormones, stress, illness, a variety of other things. Stopping Finasteride can lead to a temporary increase in hair shedding as the body adjusts to the absence of the DHT-blocking effects at the levels of hair follicles. Next is the time it takes to regrow hair, which is really variable and depends on genetic factors, as I alluded to before, and a variety of other things, including environment, health, and lifestyle. One of the things that people fail to understand or recognize is that there are a variety of environmental toxins that can impact not only general health but also hair loss and hair regrowth. It takes a few months for hair to go through its normal life cycles, and therefore, it will take a few months in general for most to see any particular change, whether or not they're using Minoxidil, Finasteride, topical, or a combination of any of the previous. During this transition period, the hair that's lost from stopping Finasteride might not be immediately replaced by new growth from that triggered by Minoxidil. It's important to have some realistic expectations to understand that this process is gradual. It's going to take time. I get many concerned messages that "my hair is not all back after two weeks." Being patient and understanding time are critical factors. Oral Minoxidil obviously is more systemic and, based on absorption and the mechanism of action, should work on viable follicles wherever they are, which is in part by the explanation of side effects by patients who are taking Minoxidil may note hair growth in other places that maybe they don't prefer like the back, sides of the face, the temples, areas like that. So, it's crucial to understand that the changes and the process of hair loss are individual and depend on many factors. This video is just a mechanism and a method to share information and education. Interpreting Your Own Labs For Health Optimization With Dr. Yablonsky “Normal” lab results doesn’t mean you are optimized. Become the CEO of your own health! On today’s podcast I have returning guest, Dr. David Yablonsky on to help you learn how to take control of your health and be able to interrupt and understand your lab results. We believe in bringing your labs into optimal range, not just out of a disease state. This is going to be a multipart series as there are a lot of different panels to cover. Lab panels discussed: Hemoglobin A1C A hemoglobin A1C test is a blood test that shows what your average blood sugar (glucose) level was over the past three months. Optimal: < 5.2 Pre-diabetic: 5.7 Diabetes > 6.4 AST & ALT Rising levels of ALT are often the first clue that something is wrong in the liver. Women ALT: < 25 Men ALT: < 33 PSA Age specific Watch for velocity changes Know that certain medications such as Finasteride falsely lower a PSA by 50% These are rough averages: 20-30 years of age < 1 30-40 years of age < 1 40-50 years of age < 2.0 50-60 years of age < 3.0 * Velocity changes is one of the most important indicators which is why establishing a baseline PSA is so important. Never a PSA > 4 unless you are 80+ years of age. Iron Iron is commonly miss understood. Checking your Ferritin and Iron % Saturation are important to understand your iron levels. Ferritin levels are a measure of your iron stores, which should remain < 50 ng/mL. Although, elevated ferritin levels > 300 ng/mL can be an indicator for inflammation in the body. Hereditary hemochromatosis is a disorder that causes the body to absorb too much iron from the diet. The excess iron is stored in the body's tissues and organs, particularly the skin, heart, liver, pancreas, and joints. Because humans cannot increase the excretion of iron, excess iron can overload and eventually damage tissues and organs. For this reason, hereditary hemochromatosis is also called an iron overload disorder.
