And to prevent them, it's important to understand the possible causes. One of them is an undersized jaw, and Kirk Behrendt brings on Dr. Rebecca Bockow to talk about what influences skeletal growth and development, and the importance of early treatment and intervention. For more on identifying and treating tongue and respiratory dysfunctions, listen to Episode 299 of The Best Practices Show! Main Takeaways: Dentists may be the first providers to identify airway issues in patients. Key things in skeletal growth and development happen as early as during breastfeeding. Breathing and tongue placement habit strongly influences skeletal growth. Undersized jaws can lead to comorbidities. Ask patients open-ended questions to identify possible issues. Look to early treatment and early intervention. Treat the “why”. Look for tonsils, airway issues, and tongue-ties. Understand structure, function, and behavior. Quotes: “When we see malocclusion, we might be, as dentists, the first providers to identify for some of these patients that they do in fact have an airway issue. So, this can present, for example, grinding. Grinding in kids and adults is a sign that airway is not great. We can pick up tongue- ties. Certainly, the easy ones to pick up would be a crossbite, open bite, underbite, overbite. These are all linked to airway dysfunction. So, once you see it, you can't not see it. And you'll start to see how prevalent it is.” (05:11—05:48) “As care providers, we can sometimes ask open-ended questions for the patient, ‘How do you sleep?' And it opens up Pandora's box. ‘Oh, I wake up through the night. My child couldn't breastfeed. She's still wearing Pull-Ups at age eight. We've got a lot of behavioral concerns, not paying attention in school, comes into my bed every night, tired during the day.' And we, as orthodontists, see the crossbite. And so, all of a sudden, we're tying things together for these families that they didn't even know were connected.” (05:49—06:28) “If we think about skeletal growth and development, a lot of the key things that happen happen really early on, going back even as early as breastfeeding.” (07:20—07:28) “Thinking about the tongue, all of those movements [when breastfeeding] require a great deal of coordination, as well as flexibility and strength. And so, if you have a child that has a condition called apraxia, and there's a lot of nuances to this, but globally thinking about a child's ability to move the tongue in a way that's healthy, so getting that tongue up and forward, things like muscle incoordination or things like tongue-ties are going to adversely affect what's going to happen with the tongue.” (08:12—08:49) “The nose is a natural filter. When you have the ability to breathe through your nose, the nose is going to humidify the air. It moistens the air. You filter out all the dust, the allergens, the pollens. Adenoids and tonsils are hypertrophic tissue, so if you have a child that's a mouth-breather, we tend to see bigger tonsils, bigger adenoids. And so, this cycle continues because they're more driven to breathe through their mouth, because now they have nasal respiratory obstruction.” (10:21—10:48) “There's great documentation that links nasal respiratory obstruction with undersized jaws. And then, we start to see other comorbidities that we would notice as dentists, which include grinding, high carious incidents, hypertrophic tissue such as the gum tissue. And the cycle goes on and on. As orthodontists, we see this as crowding because the jaws are small. So, there's insufficient bone to accommodate the eruption of the adult dentition. And so, it's sort of this aha moment when we start to link what goes on early with breathing and tongue function or dysfunction and the way that...

He is now 2 and his grommets have come out and I see the symptoms of the constant runny nose and cough come back. I have heard that food sensitivities can sometimes cause these symptoms and kids are getting these procedures done when it could have possible been avoided. What are your thoughts on this? I have ordered the equilibrium food sensitivity test for him and also had him dairy and gluten free for 6 months! Thanks for bringing your amazing knowledge to us listeners every week. Mel: Hi Dr Cabral! May I get some feedback on Lugol’s Iodine please. My thyroid levels are in the ‘optimal’ range but when I paint a hand size amount of Lugols Iodine on my skin it fades generally within an hour or two. Do I need to keep doing this or not? (Thyroid levels were taken before I started painting the LI) I asked on your FaceBook page but no one was familiar with it and it was suggested I ask here instead.Many thanks Mel Cristian: Hey Dr Cabral. Great show. I put everyone I know onto it. My question is, is there anything from your practice whether it be advice or supplements a man in his early 30's can do to aid his sex life in his marriage? Otherwise healthy but struggle to maintain erections for long periods like I used to when I just got married in mid 20's. I've been listening everyday for a couple years now. You are an incredible human. I'd love to hear a Friday Review of the benefits of EDTA... and any other specific herbs/supplements used as a part of the heavy metal detox. Thank you! Krista: Hi Dr. Cabral, I have two pre-teen/teen sons with multiple tics (technically they would be diagnosed as having Tourettes Syndrome). They have some other issues as well including some digestion stuff (including food sensitivities), anxiety, OCD, and occasional eczema. Several years ago we stumbled on a woman, by the name of Dr. Reid, and her TED talk which basically explains her discovery of the effect of free-glutamates on the brain. By altering her daughters diet to eliminate nearly all glutamates (including those from supplements, citric acid, protein shakes and other sources), and including a good amount of veggies and cleansing herbs, her daughters Autistic 'symptoms' completely disappeared! As soon as she was exposed to glutamates again, like when eating out, her symptoms returned. We have since joined a fb support group and have met several other families with the same results - not only for those dealing with autistic symptoms, but other neurological issues as well including what my boys have. We have followed this diet (about 90%) for the last several years and have had both of our sons symptoms decrease DRAMATICALLY (one of them no longer has tics at all). That said, we still see a need for improvement which is why I am currently having them take your OAT and HTMA tests. Here are my questions : Are you familiar with Dr. Reid and the success' she is finding with this protocol? And also, if my kids are responding positively to a decrease in glutamates, should I be concerned with ADDING sources of glutamates (such as protein shakes, supplements, and most notable L-glutamine) in an effort to heal their gut? My concern here is that in my effort to help them, I will be adding to their glutamate load and they will regress from the progress we have made so far. I appreciate your work and especially your desire to continue to learn (a sentiment I too share). Air and breathing is essential! So why do we take it for granted and not seek out solutions to the root cause of the problem. On this Episode 15 he will share his expertise on looking at an airway space through the eyes of a specialist. Airway issues must be evaluated for what aspect of the pathway is obstructing the flow or movement of the air. Snoring can be a significant sign of an obstruction. Weight, size of neck and mid-section plays a role in breathing efficiency. Noses can be straight on the outside and crooked on the inside but just because you have a crooked nose on the outside doesn’t mean it is on the inside. Turbinates heat humidify and filter the nose of allergens but can swell and take up too much room inside the nose. Add that with a deviated septum and you really feel the effects of difficult breathing. Adenoids are a small part of the immune system that sits behind the nose and above the tonsils. They can get too large and stop the flow of air. Tonsils and the back of the throat can collapse more when you lay down and get relaxed. Simple at home evaluations can be performed. Place a finger gently on the side of your nose and listen to the airway flow. Then change sides. Is one side more open than the other? Does it sound like it is struggling or flowing gently in and out. Open your mouth and look in the mirror. What do you see? Can you see the back of your throat and tonsils at all? Is your tongue blocking all visibility of your throat? Parents you must listen to Dr. Mehendale’s take on children’s airway issues that can affect their sleep, focus, their attention, and energy levels. Then transfer that to you or a loved one as an adult. A lifetime of poor sleep will no doubt take years off of your life. An open airway will support a better deeper more restful sleep. I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started! Megan: Hello! absolutely love your podcast and have the highest respect for you and what you do! That being said, I need your help! I am having the hardest time losing weight! I have a functional medicine doctor that has run many blood labs along with a Nureval test and long story short, the highlights of all my labs were... *a normal functioning thyroid *a healthy gut with no markers for yeast or bacteria, *low progesterone (2.8 on day 19 of my cycle), *high need for vitamins B6, 7, 9 and 12, *high need for vitamin C *high need for alpha lipoic acid, *high need for vitamin D *low glutathione Is all of this what is keeping me from losing weight? I can't understand how I can have a healthy thyroid, eat a healthy diet high in vegetables and yet I am still not able to lose weight. How do you naturally boost progesterone and balance out hormones? Thank you so much for sharing your wealth of knowledge with people like me on a daily basis!! Megan Judy: Dear Dr. Cabral, I am so grateful to have found your podcast. I am a 34 year old woman battling acne since my teenage years with the exception of when I took Yaz birth control for about 2 years. I later tried a generic brand to help with acne but it never worked. It has been at least 5 years since I was last on birth control. My acne continues to get worse mostly on my chin and cheeks. I have been on a vegan, plant based diet for almost a year. Several of my digestive issues improved once I gave up animal products such as I can now process beans and most cruciferous foods that for years I just thought I was allergic to. However, my acne continues to get worse. Starting a month ago, I am now taking about 5mg Maca, and 2tsp of flax oil and a vegan epa/dha supplement and eliminating all other oils and my beloved nuts to reduce inflammation and boost my omega 3s which I heard could help acne. However, I am constantly craving sweets especially before my cycle and I often binge on sugar on the weekends. Also, my eyelashes fall out causing bald spots so I started using a lash enhancer to keep the balding at bay although I can clearly see the new growth that is replacing the hairs that fell out. I am also concerned that I have lost my half moons on all my nails except my thumbs which are starting to go away too. Do you have any suggestions on what may be causing my acne, binging, hair loss and nail issues? Is there a vegan solution for me? I look forward to you reply. Anonymous: I’m not sure which tests to order and I’m hoping you can give me some direction. I am Hypo/Hashi x 10 years and still don’t have my health anywhere close to where I want to be in my overall health and life. I am a 52 yr old female. I’m still experiencing some fatigue, some muscle/joint pain especially with a change in routine, exercise intolerance (can walk 4 times a week for 30 minutes but can’t do an hour long gentle yoga, Pilates, or any gentle weight lifting for strength and muscle tone). I am COMT homo, MTHFR Heter, CBS. I KNOW I do not tolerate crucerious veggies in large amounts, sulfur anything gives me a migraine that can last weeks - no exaggeration. I have seen a couple of different ND’s, thyroid nurse expert over the last 7 years and although I am at about 50-60% in my overall health I am not happy with that. I feel like I am running on half my pistons. I wouldn’t doubt mitochondrial issues. I have psoriasis which is 90% improved but still have a few small patches. I am taking a ton of pharmaceutical targeted supplements after doing the AIP diet for a year and half. I have a sensitive constitution (Kapha/Vata about 50/50) and have worked extremely hard on lowering the ‘bucket affect’. I eat a modified Paleo diet, avoid most nightshades most of the time. Gluten and dairy free for 7 years. Very little sugar in the form of honey/maple syrup or low glycemic fruit. Meditate daily for years and walk, do yoga stretches. I take T3, NP Thyroid and LDN 2 mg. I am happily married and have a lot of great support. Stress in managed well. So other than what labs might you suggest I would also like to know if Dr. Cabral would think outside the box with me. If he recommends a ‘protocol’ to treat something, say candidiasis for instance, will he be able to recommend a different care plan if I am intolerant to an ingredient in one of his protocols? I absolutely cannot take Molybdenum and refuse to suffer any more migraines when I know I have tried that, several times in the past, even a tiny doses with dire consequences. I think this is what makes me a more difficult case but I would love to work with him if he thinks he could truly help me. Do you think he could? I am and have been for 7 plus years 100% committed to my health and wellness. I have done a ton of work with old past trauma, addressed environmental chemicals/personal care/diet/lifestyle/addressing hypoadrenia, phase 1/neurotransmitter imbalance/oxidative stress/micronutritional deficiency/genetic deficit, enzymatic, methylation. I am a changed woman in many ways due to his podcast and Motivation Mondays! Thank you for all that you do for all of us. Dominic: Hi Dr. Cabral, I enjoy your podcasts. After consulting ear specialist, doctor for my 2 year old recommends adenoids removal. He has fluid on one ear and the other ear drum is inverted. Don't like the idea of my boy being cut open and body parts removed. Is there an alternative? Thanks you, Dominic Michelle: Hello, I have been prescribed and have been taking Alprazolam for several years for anxiety, insomnia, and to counteract the side affects of Vyvance. This past year I have started experiencing several terrible side affects and when I questioned my doctor she told me long term use is fine. I do not feel well and as I age my symptoms get worse and I want off of this medication!! As my doctor does not want me to go off of the Alprazolam I decided I would take myself off of it but I would like to ask for your recommendation on how best to taper off. Also, I have Gilbert’s which I suspect is affecting my tolerance and overall liver function, but all of the doctors I have spoke to about this have very limited knowledge surrounding Gilbert’s. As this may affect the way my body processes Alprazolam any insight you may have in regards to tapering off and/or any information in general you could share would be grately appreciated!!! Thank you, MIchelle Lindsey: Two years ago, I had a Bartholin cyst surgically removed. I’ve had the cyst come back a few times since then but opted out on having surgery again. I’ve read about the causes of Bartholin cysts but I have yet to find any information on how to rectify whatever is going on inside my body to prevent them from developing. Is the cause diet based or is there something else going on inside me that is causing them to develop? I know there’s an underlying cause and I would much rather correct my body from the inside out as opposed to having multiple surgeries.
