CancerNetwork® spoke with James R. Berenson, MD, about the potential role that JAK inhibitors may play in the treatment of patients with multiple myeloma. The conversation focused on the rationale for researching this drug class as well as ongoing initiatives to assess the utility of agents like ruxolitinib (Jakafi) for this patient population. Berenson, founder, medical and scientific director, and president and chief executive officer of the Institute for Myeloma and Bone Cancer Research and private practitioner in West Hollywood, California, described the factors associated with the overexpression of JAK in the bone marrow, which may constitute a prime survival factor for multiple myeloma. This overexpression may affect the checkpoint inhibitor proteins in the body, resulting in resistance to standard anti-myeloma therapies such as immunomodulatory drugs. Additionally, he mentioned a patient case that had involved a scenario in which JAK-mutated multiple myeloma progressed following prior treatment with lenalidomide (Revlimid). According to Berenson, the disease's resistance to lenalidomide was primarily associated with proteins driven by JAK; subsequent treatment involving JAK inhibition proved successful in restoring the efficacy of lenalidomide. Based on a rationale to target JAK overactivity in the bone marrow and results from this patient case, Berenson and colleagues have focused on researching ruxolitinib as a therapeutic candidate for potentially improving outcomes in multiple myeloma via JAK inhibition. Findings from a phase 1 trial (NCT03110822), for example, have demonstrated that the efficacy and tolerability of ruxolitinib plus methylprednisolone can be extended with lenalidomide in those with multiple myeloma. Additionally, other ongoing trials aim to combine ruxolitinib with agents such as selinexor (Xpovio). “The question is, where will ruxolitinib sit in the sequencing of treatment of [patients with] multiple myeloma? Where will selinexor be?” Berenson stated. “At this point, there's certainly been low use of these drugs, especially ruxolitinib in multiple myeloma. We hope, with a more positive signal, these drugs will move further up in the algorithm of how you treat multiple myeloma.” Reference Berenson JR, Limon A, Rice S, et al. To some, Alex Berenson was the "pandemic's wrongest man." To others, he was a prophet, whose Covid and vaccine predictions -- although deeply unpopular and ultimately the reason for his ban from Twitter -- came true. A graduate of Yale University, Alex Berenson got a job as a journalist at The New York Times around 2000. During his award-winning 10-year tenure at the paper, he covered the pharma industry, Bernie Madoff, Hurricane Katrina, and the Iraq War. He ultimately left the newspaper to become a novelist, which proved a more lucrative and comfortable career. Then, in 2019, he published a book on the link between cannabis and psychosis called "Tell Your Children." From Joe Rogan, on whose podcast Berenson has appeared multiple times, to establishment scientists, critics accused him of cherry-picking data and being alarmist. Today, however, his cannabis research is less controversial. In 2020, in the early days of the pandemic, Berenson became a critic of the federal government's response to Covid. He called BS on lockdowns, mask mandates, contact tracing, and more. Then, when the vaccine came out, he raised questions about the clinical trials, saying they were run poorly and predicting that the vaccine may have significant side effects. In late summer 2021, he sent a tweet saying the vaccines would be "at best a therapeutic," and Twitter banned him. He sued Twitter and won the appeal, and has since sued the federal government for colluding with Twitter to ban him. This has created a cottage industry of AI Trust And Safety people making sure their chatbot will never write arguments against COVID vaccines under any circumstances, and a secondary industry of journalists writing stories about how they overcame these safeguards and made the chatbots write arguments against COVID vaccines. But Alex Berenson already writes arguments against COVID vaccines. He's very good at it, much better than I expect chatbots to be for many years. Most people either haven't read them, or have incidentally come across one or two things from his years-long corpus. The limiting factor on your exposure to arguments against COVID vaccines isn't the existence of arguments against COVID vaccines. It's the degree to which the combination of the media's coverage decisions and your viewing habits causes you to see those arguments. A million mechanical Berensons churning out a million times the output wouldn't affect that; even one Berenson already churns out more than most people ever read.

The Family and Small Business Taxpayer Protection Act from Reps. Adrian Smith, R-Neb., and Michelle Steel, R-Calif., would roll back the billions of dollars of funding for the IRS that was approved in the Inflation Reduction Act last year. Smith’s bill leaves in place funding for customer service and improvements to IT services at the IRS but rescinds several categories of unobligated funding, including money that could be used to conduct any new audits on Americans. In total, it would claw back $72 billion of the funding Congress approved for the IRS last year. The Inflation Reduction Act granted an $80 billion boost to the IRS over a 10-year period, and more than half of those funds were aimed at helping the agency crack down on tax evasion. In 2021, the Treasury Department estimated that $80 billion would be used to hire 87,000 auditors and other new staff. Republicans have said they would fight this aggressive expansion that would more than double the agency’s current size. "The last thing the American people need right now are more audits from an out-of-control, bloated IRS," Smith told Fox News Digital on Monday. "The Inflation Act funding for IRS would lead to the hiring of 87,000 new IRS employees tasked with raising enough revenue to pay for Democrats’ Green New Deal priorities." In an exclusive interview with Fox News Digital just days before the midterm elections, McCarthy vowed that if he became speaker of the House, protecting Americans from the IRS would be his first order of business. https://www.foxnews.com/politics/whats-house-rules-package-tough-limits-federal-spending What's in it: House GOP rules package aims to curb trillions in yearly heaps of new debt A 55-page set of rules for the House that Speaker Kevin McCarthy hopes to pass on Monday will set ambitious new curbs on federal spending, part of the GOP's effort to stop piling on trillions of dollars in new debt each year. One of the biggest changes is a return to a "Cut-As-You-Go" policy that says legislation cannot be considered if it increases mandatory spending over a 5- or 10-year period. This "CUTGO" policy requires bills that call for new spending to find offsetting spending cuts elsewhere in the federal budget. That is a more aggressive stance compared to the "Pay-As-You-Go" policy under Democratic control. "PAYGO" also requires offsets to new spending, but those offsets can either be spending cuts or tax increases – and in either case, PAYGO rules were often waived entirely by Democrats. The decision to go with CUTGO shows the new GOP’s aversion to possible tax hikes, which can be seen elsewhere in the new rules package. For example, the rules require a supermajority in the House to approve new tax increases. "A bill or joint resolution, amendment, or conference report carrying a federal income tax rate increase may not be considered as passed or agreed to unless so determined by a vote of not less than three-fifths of the Members voting, a quorum being present," the rules state.
