Today on the podcast we are thrilled to have Justin Larson of JML Woodcraft as our guest. Justin is a one man show and a garage workshop warrior who specializes in highlighting the natural beauty of live-edge slabs. In fact, technically Justin only does woodworking part time about 15 to 20 hours a week, but he says if you ask his wife, she'll say he's out there a lot more than that!  The Creative Focus and Origins of JML Woodcraft Right now the focus of Justin's company is unique serving boards and live edge slab desks and tables, but the history behind the company has been in the making for over ten years now. JML Woodcraft actually all started back in 2011, when Justin and his family bought their house and needed some furniture. As Justin says, he had the thought process, like most woodworkers, that he could just build everything they needed so that they could save some money. First he built a farm table and some shelves, then he started getting more advanced. For the first few years he was just building things at home for his family, but then after a while friends and family started wanting the things that Justin was building for their own houses too.  Justin started building furniture for friends and family, and then some of his business minded friends told him that he should start an LLC and start building a fully realized brand and legitimate business. In 2016 Justin started the LLC and opened up his business to go neighborhood wide. The savvy woodpreneur explains how he used Facebook to get the word out and start attracting customers, and once he did, he did everything they requested, he didn't say no to any projects which really got the ball rolling. Then, once he started the LLC, Justin's wife encouraged him to turn their garage into a woodshop. With the work space complete, JML Woodcraft was then able to take on more complicated custom projects.   The Motivation Behind the Business  Steve and Justin talk a bit about the fact Justin has accomplished so much, and even has a 3-month backlog of customers, all while being part-time. For Justin, he says it comes down to the fact he just loves building. The business growing has been incredible for this exact reason. As Justin sees it, the more projects he can do, and the wider of a variety the better. Justin also talks about how building isn't just a creative process, it's also a learning process, each project he does he feels that he learns valuable lessons for the next one. Another learning experience for Justin has been learning not to get burnt out, and setting rules for himself about what kind of projects he works on. He even let go of some pieces of equipment with the understanding that not having the capability for certain things will allow him more projects he truly is passionate about. 2020 Summit Special: The Rules for Aggressives Sales Growth with Justin Roff-MarshToday's Summit Special is Justin Roff-Marsh. On a wet and windy day in Wiltshire it is great to look at the video of Justin speaking at the Monkhouse & Company Summit September 2020. Justin helps people revolutionise their sales function. If the question you ask yourself is, "if I double the number of sellers, will I double my revenue?". And if you paused, or hesitated, or said no, then implementing Justin's The Machine would give you some clarity. It fixes not just sales; it might even fix many bits of your organisation. But it certainly impacts sales, marketing, and customer service—the whole customer journey. Justin looks at it from the perspective of lean manufacturing. There are a couple of controversial things: no commission, no individual targets. Those are pretty difficult for some people to accept in the first instance. They feel that salespeople are coin-operated. You better be quick because there's only a few left.Enjoy! Born in Cambridge, raised in Australia and resident in Los Angeles, Justin started his fantastic talk at the Summit, defining his accent as a "hybrid". Right after that, he jumps straight into what he calls the three-step programme. Justin and his team are usually introduced to an organisation by the CEO or private equity. "Our mandate is always the same, figure out how to grow this organisation". Then, three ideas are brought to the table. "You could think of these ideas as a three-step programme"The three-step programmeWith a mandate to rapidly grow an organisation, the first thing that Justin and his team do is work to restructure the organisation so that salespeople perform only one activity, which they call selling conversations. "And by only, I mean they do absolutely nothing else", says Justin. That means that if you work for this organisation and you come in with a card that has the word 'Sales' on it, you only have one decision to make: "will I work in this particular instance or not?". If you choose to work, there's no further thinking required. "There's only one activity that you're responsible for, and that is selling conversations". And that will go on, selling conversation after selling conversation on repeat until you go home. Achieving that requires a restructuring of the organisation. So, that's the first idea. If you want to grow your organisation aggressively, Justin's first piece of advice is to "restructure those organisations so that salespeople do nothing other than selling conversations". The second idea is that after restructuring your company, the next thing you want to do is carefully examine the activities performed in the field. "You want to look at each activity and ask yourself one question: does this activity absolutely have to be performed in the field, face to face with customers". If you are honest, you'll conclude that not every activity currently being performed face to face with customers actually needs to be. In fact, virtually none do. Justin reckons that when you identify those activities that don't need to be performed face to face, "the obvious thing you can do is take those activities out of the field and move them inside to a central location". And by that, he means an office in the UK if your market is Europe, not 27 offices spread across Europe. So that's the second piece of advice.The third one is straightforward. Remove your salespeople's autonomy and remove their commissions. At these words came out, you could see the shock on the audience faces. To which Justin admits, "I didn't say salespeople were going to like it. I said it was simple". By doing that, he's not suggesting that you take away their autonomy and invariable pay because salespeople are going to like it or because you're likely to see an increase in performance - "even though you will". He suggests that, in reality, taking away their autonomy and piece-rate pay is a prerequisite for the first two steps. As Justin simply put it: "you cannot restructure your organisation - so that salespeople spend 100% of their time selling - and you cannot move 90% of activities that are currently performed face to face with customers inside." The important reason is that "it's those first two steps that are what's really going to move the needle.""So if you really want to give your organisation a kick in the pants and grow it aggressively", those are the steps you have to take. Assuming that your business has good fundamentals. You have to take away commissions so that you have a "mandate that allows you to implement those first two steps consistently, unflinchingly and exhaustively in your organisations." Selling ConversationsTo achieve aggressive growth following Justin's three-step programme, you need to be clear on what 'selling conversations' mean. We all know what the word 'conversation' means, but what about that other one, 'selling'?If you asked your Director of Sales to define the word selling, "they will have you believe that the word 'selling' refers to all the activities that a salesperson typically performs, which has a degree of circularity that should disqualify for many intelligent explorations into the subject." So then, Justin takes a different approach to the same question: "but what's the one activity that a salesperson performs that genuinely and inarguably generates value?" And what do we mean by closing? - Justin asked the audience - do we mean actually signing the paperwork? PersuasionDrawing from the question what does closing actually means, the author of The Machine, demystify the idea of salespeople sitting with the customer wrestling. He goes back to his past in sales when they used to say, you move the pen, I'll hold the paper. "That doesn't actually happen, and in most of the cases, contracts come in by email or via DocuSign, right?". So when we talk about closing, what we really are talking about is convincing persuasion. Justin then defines a selling conversation as a "conversation where a salesperson is persuading". Now, if you've had a customer coming back for his seventh transaction, the amount of persuasion required will depend on your operational performance. "If you screwed up the last six transactions, a whole bunch of persuasion. But if you've done a good job, none." Justin attributes this to "the magical thing called nurture". It's costly for organisations to move their business around the place. You've got all sorts of switching costs, including the risk. If you sell expensive things, the risk can be the highest switching cost. So if you transacted with an organisation more than zero times and sell a product of substance, and you've done a good job at keeping your promise, then there will be no persuasion required. According to Justin, there's only one type of interaction where persuasion is required: "the first. The role of your salespeople should be to convince folks to transact with you for the first time. Because once they transact with you for the first time, they get to experience your operational efficiencies. And on account of switching costs, after they've experienced those operational efficiencies for the first time they're hooked and no further persuasion is required."This will result in what we should call "a relationship". But we don't. We degraded that word, and we use it to refer to the personal relationship between the salesperson and the prospect. But the personal relationship should be overpowered by the commercial relationship. Justin's recommendation is that salespeople should use their superpower exclusively to convince those that have never transacted with you before to do it for the first time. The end result of moving sales activities inside, says Justin, is we get to build what he calls "inside sales team". We don't want telemarketers. We want to take professional salespeople with whom your customers would like to transact with. We want them inside in an environment where they can comfortably have 15 selling conversations a day – many more than what salespeople is currently having."

33. 6 Steps for Successful Influencer Marketing (with Justin Whittaker)

More than a decade ago, I consulted for a company named i.TV where Justin Whittaker served as a top executive. i.TV was the creator of the most downloaded TV guide app in Apple's App Store. i.TV designed and built second screen and social TV experiences for brands like AOL, Huffington Post, Entertainment Weekly, and Nintendo through a software as a service model. Every month, millions of people used i.TV technology to discover, watch, and engage with television. Justin Whittaker used unpaid influencer marketing for i.TV more effectively than almost any other company I have ever seen. I was so impressed with Justin's efforts and success that I've used his model as an example over the years when teaching other people about successful influencer marketing. I've seen some businesses use influencer marketing incredibly effectively by offering high commissions to the influencers. With Justin and i.TV, no financial compensation was offered to the influencers. Yet, Justin was still able to secure huge amounts of exposure for i.TV from the influencers. 6 Steps for Successful Influencer Marketing Here's how Justin secured so much unpaid exposure for i.TV from influencers: (1) Find hundreds of targeted influencers and their contact info. Before the initial release of i.TV, Justin identified hundreds of highly targeted influencers. The list of influencers didn't just include names of media organizations, but also included the list of news writers and bloggers who created content about the types of products and services i.TV created, along with their specific contact information.  Justin researched almost every single reporter or blogger he talked to. He looked at articles they had written on similar topics. i.TV also subscribed to a PR contact database that helped him identify and find contact information for the influencers. Once Justin got in the door with reporters, he asked them for referrals to other reporters who wrote on his topic. Sometimes we don't think of news writers as influencers, but if their publication writes about our niche, and the author writes about our niche, then that author is an influencer for our niche.  (2) Build relationships with the influencers by providing value. Justin was not sending bulk spam messages trying to get influencers and reporters to promote his company. Instead, he worked to build individual relationships with those influencers. And, he built the relationship before he ever asked for anything in return. Then, he nurtured the relationships with the influencers over time. This was another big key to Justin's success is that he didn't approach the influencers and reporters as a one-time transaction. He invested in long-term relationships.  Content creators love it when we are familiar with their work and the things that they care about and write about. So, Justin invested in getting to know the influencers and their work. He provided value to the influencers and reports that helped them be successful in their jobs. As Justin reached out to influencers, he often offered them a chance to interview with the CEO of his company.  One great way to build relationships with influencers is to follow, share and comment on their content and social channels. One of Justin's secrets of success was to treat reporters like influencers (which they are). (3) Build credibility with the influencers. Justin feels that a one-letter domain really helped i.TV's credibility working with influencers. He felt that the influencers took i.TV more seriously. I have seen the same benefit of credibility by owning the authoritative Adoption.com domain name. It can be worth it for young companies without credibility to buy a domain name that can give them credibility.  As an aside, I have used this credible domain name strategy over the years. My businesses or my clients have purchased premium domains such as Adoption.com, Law.com, Retirement.com, Advertising.com, Families.com, StockMarket.com, and many others. These types of domain names gave a lot of credibility and made a big difference.  Also, the enormous number of i.TV users and being the leader in their niche helped grow i.TV's credibility and the willingness of the influencers to write about them. There are many other ways we can grow our credibility with influencers, such as through awards, testimonials, advanced degrees, credible statistics, associating with credible sources, and more. (4) Make it easy for the influencers. As i.TV released new versions of the mobile app with significant new features, Justin worked with the influencers to get articles written about the release.  Justin was very thorough in what he sent the influencers. He didn't just send a press release. He also sent content, images, logos, quotes, story ideas, etc. Justin made it as easy as possible for these influencers to write stories about i.TV. Justin says that he almost wrote their stories for them, and often the influencers and reporters just cut and pasted what he sent them into what he published. This was very valuable because these writers had deadlines to create content, and Justin helped them meet their deadlines by providing them quality content they could publish with reduced effort.  (5) Provide value to the end-user. It is really important that the content we send to influencers provides real value to their customers, instead of being an advertorial for our product or company.  For example, the i.TV content wasn't focused on why i.TV is the best TV Guide app. Instead, the content was focused on the new FREE features and benefits that readers could use to have a better experience with their entertainment. The content has to be focused on providing value to the end-user. (6) Affiliate Marketing - Justin rolled out his i.TV influencer program before influencer marketing became the craze it is today. As such, Justin didn't need to offer compensation to the influencers. Today, so many businesses are reaching out to the influencers, and many of the influencers are requiring compensation. Some influencers may promote our products and services without compensation, but that is becoming rarer, especially for the larger influencers.  Today, many companies are choosing to increase the effectiveness of their affiliate marketing by setting up affiliate programs with tracking links and paying influencers a commission for each person they refer who purchases the company's product or services. I strongly encourage all of my clients to set up paid affiliate programs to facilitate long-term win-win relationships with influencers.  What should we pay influencers? My clients regularly ask me something like “What is the lowest amount I have to pay influencers?” I think that is the wrong question. In today's influencer marketplace, the people who pay influencers the lowest amount possible have a hard time getting the attention of the right influencers, and they leave a lot of money on the table.  I think the right question is, “How much can I afford to pay the influencers in my affiliate program?” Remember, that with an affiliate program, we don't pay the influencers anything until they generate a sale. So, we really don't have to pay the influencers anything ourselves. With an affiliate program, payments come from the profit of the business the influencers send us.  Also, remember that with most other advertising sources, we take a big risk by paying for the advertising upfront, because the profits generated from the advertising may not be sufficient to cover the cost of the advertising. With affiliate marketing, that risk is eliminated for the business and transferred to the affiliate partner or influencers, because the affiliate partner only receives a commission after they have generated sales. What size of influencers should we go after? Justin organized his influencers in tiers to prioritize their reach. A lot of the big PR firms focus on the larger influencers because if we get a couple of wins with the bigger influencers it will trickle down. However, Justin's strategy was to go after all of the targeted influencers, in almost a “ground-up” strategy. His philosophy was “Why not get them all?”  However, with this “ground-up” model Justin found that he needed to give some preferential treatment to the top influencers, such as giving exclusive information to the top influencers. He also gave the smaller publishers “embargoes” limiting when they could publish the stories, so the larger influencers could break the story first. Justin's ground-up strategy did not hurt i.TV with the big influencers, and he was able to get covered by the leading influencers. Huge Success from Justin's Efforts Because of Justin's incredible efforts, i.TV received more than 100 mentions from media and influencers each time i.TV put out a substantial press release while I was there. I helped i.TV with their monetization and we were able to sign deals with 5 of the top 7 movie studios in a short period of time, which was only possible because of the tremendous influencer marketing efforts and media exposure Justin secured for i.TV. Then, because of credibility and relationships, i.TV was able to morph into a technology services provider for large entertainment companies, which provided a larger and more stable revenue stream. Justin Schenck is the host of the top rated podcast the Growth Now Movement and founder of Growth Now, LLC which is a full service podcast production and coaching company. He has been named a Top 8 Podcaster to follow by Inc. Magazine and chosen as an ‘Icon of Influence’ in the new media space.   What started out as something fun to do in order to connect with top influencers and help one or two people along their journey, has grown to become a podcast that is currently getting played in over 100 countries every single week and ranked in the top 15% in the world.   Justin now works with some of the worlds elite entrepreneurs and business owners like Fabio Viviani, Sarah Centrella, and Cindy Eckert. He is also the host and creator one of the most exciting live events for entrepreneurs and forward thinkers; Growth Now Movement LIVE!   None of this started from greatness. Justin had the deck stacked against him growing up, with difficulties in his family that should have relegated him to being a statistic rather than a success. The inspiration and purpose he found along his journey fuel the message he shares in his work and in this episode today.   As a note, this episode was recorded before the live, in-person event was cancelled due to the Corona Virus. GNM Live was changed to a virtual event.   Key Points from the Episode with Justin Schenck: Justin started a podcast four years ago with his show “The Growth Now Movement,” not knowing quite what it would be, and it has become a movement. He ended up building a podcasting production and consulting business as a result, too. He also ended up creating a live event out of what was coming from the podcast, called Growth Now LIVE. Justin has had the desire to be an entrepreneur for a long time, which is what his podcast has allowed him to be. He adopted the famous saying, “Live happens for me, not to me.” That didn’t come easily when you look at what Justin’s life was like. His mother was addicted to opiates, his father was in jail, and he had a 1.7 GPA. If you look at the statistics, each of his parent’s situation meant he had a 50% chance of being an addict or going to jail himself. He chose not to live that way. He read the book, Who Moved My Cheese, and realized life is 10% what happens to you and 90% how you react to it. At 19, he was working in a direct sales business, had done well in his role, and started to manage people. This is where the entrepreneurial spark was lit. An employee came to him and said that, while he would have made more money working in fast food, what he learned by working for Justin was worth far more than that. In that moment, Justin realized that he wanted to inspire and guide people full time, and he should pursue making that his life’s work. He put together an event company to do personal development events, and had some success. One over-leveraged event that failed to sell enough tickets to pay for itself ended up killing the business. What he learned in that experience about what he needed to learn still was invaluable to his future success. He also learned that the right response is not to just quit. Justin’s entire life was never just handed to him, so he chose to keep fighting and learn from this failure. The learning was not instant. Right afterward, he called his mother and told her he was quitting. She pointed out that he didn’t fail, but was instead heading in the right direction and needed to keep heading there but with new knowledge. Just because you didn’t get what you wanted out of the current situation does not mean you aren’t headed in the right direction. We talked about his mother’s addiction, and interesting, despite what she was going through, she was still Justin’s rock and there for him through it all, even to her ultimate death at the hands of her addiction. She taught him that our rock bottom moments are the start of so much greatness. It can also be the end for people because you feel like better is impossible. But what actually matters is the choices we make in those moments to make things better for us and the people around us. There is choice and direction all around us. The key is that we have to listen (don’t just hear it, but actually listen and take it in). On the back of his mother’s death, he went on a three month, black-out-drunk bender. He kept going out drinking all night to keep from sitting in the pain, and feeling it. He had a mentor and coach who asked him where he was going one night, and when he said he was going drinking, she told him, “No you’re not. You’re going to sit and you’re going to feel this tonight.” While it was incredibly painful, when he woke up the next day, the weight of the pain was gone. He realized that, when we hit rock bottom, we can go through pain, and we can feel like there’s no way out. But it can end, and it’s up to us to decide when that moment is. He chose growth. He uses the Rule of 1% – he chooses to get 1% better every day, in his relationships, finances, health, etc. He reflects at the end of each day to see where he made progress, and if he didn’t, what he learned from not making it, and what he can do differently. And if he feels that he did nothing in a day, he has compassion for himself and realizes he can still get up the next day and be better. If you focus on 1% each day, you end up with exponential growth in a year. We look at other people, and think, “I want that. I want what they have.” Justin says we need to stop looking at other people because it only makes us feel bad. It’s a waste of time and our potential. What are you cultivating for yourself? Focus on that rather than what the other person is doing. Justin’s events were born of his podcast, and spawned a series of seminars in the early days. As he and his partner were working to launch the podcast, he got the call about his mother, who he dropped everything, drove 7.5 hours, and went to see her before she passed. He promised her he will make her proud and this will not go unnoticed. As the show started, his partner was talking about business, and he was talking about the self-improvement journey and values. The show changed into what it is now, with Justin hosting solo, and went further into this purpose that came from the moment when his mother died. We talked about the pursuit of money or impact, and Justin firmly believes that money is just a reaction to our impact. He said it really well, “I’ll impact a million lives before I have a million dollars.” With that intention, it’s not surprising to see that he’s clearly somewhere around that level of impact given that he’s close to his 300th episode, and gets thousands of listeners to each show he does, meaning there have already been over a million times his show has been taken into someone’s life. We got into the details about the upcoming Growth Now Movement Live, which is happening on September 25-27th. He named a number of seriously impactful people, some of which are doing keynotes, and some are part of panels. This includes people like Nick Santonastasso, Anthony Trucks, Justin Wren, Natalie Jill, Adam Schaeuble and others. None of them is paid to do it, which means they’re all there because of the purpose of it, which is part of why it makes sense to call it a Movement. The big difference with GNM Live is that the speakers are all intertwined in the event and the people, and you see it in the posts people share after the event. They name all the great people they connected with, and that includes the speakers, who they are doing more than just tagging in a post. These people have genuine connections on a personal level. As Justin says, it’s what happens in between the margins, where we get to make the direct relationships we thrive on in life. **Almost all leaders (clergy) in the Church of Jesus Christ of Latter-day Saints are unpaid and work normal jobs.** This episode is TOUGH. First and foremost, we want to emphasize that we abhor inappropriate conduct from ANY leader in the Church, regardless of position in the Church. As Justin says in this episode, if you’ve ever been sexually or physically abused by a leader (or anyone, for that matter), don’t be afraid to speak up! Such leaders should not be in positions of leadership. While there are extreme cases of misconduct, there are also cases of leaders making comments that are offensive or inappropriate. Such comments may draw us away from the Church and the feelings are REAL! Justin and Taylor share their experiences in being hurt by different comments from Church leaders and what they felt, how they acted, and what it means for them moving forward. We are so sad when anyone is offended by church leaders, by us, or by anyone in the Church. We hope that such things can be discussed and resolved! Last week New Jersey, Montana, Arizona and New Jersey voted to legalize recreational marijuana, while South Dakota approved legalization for both recreational and medical. Only 6 states in the country do not permit any use of marijuana with a handful of states permitting only limited medical use. While the majority of states will be benefitting from additional revenue through licensing and taxes, many of these now-legal businesses will still not have sufficient access to financial services and basic banking opportunities. Justin Fischer, CEO and Co-Founder of RiskScout, stops by the next episode of Clark Hill’s Credit Eco To Go to discuss the challenges small and community banks continue to have attempting to serve these legal businesses. RiskScout is a platform that streamlines the compliance process in an attempt to level the playing field for community banks. As Justin tells us, challenges also exist in the hemp space, which was legalized at the federal level under the 2018 Farm Bill. Many farmers are turning to hemp as an alternative crop only to be shut out by their local banks, especially community banks, because they do not have the infrastructure to manage the compliance risks. RiskScout provide these smaller entities a chance to compete. Unless the basic function of banks fully utilized, states seeking to expand their cannabis and hemp footprint will only see their efforts go to waste. Justin Christianson is a self-proclaimed number junky and a digital marketing veteran. Test the biggest leverage points, and don’t overcomplicate it.” Justin Christianson   Worst investment ever Helping a client out At the end of last year, Justin got a call from an e-commerce business owner who was freaking out because his business was falling apart. Justin and his partner had a meeting with him, and they soon realized that they could help him out. I want a piece of the pie The business was something that Justin could relate to, and so he got quite excited about it. He wanted a piece of it, and he proposed to the owner to help him grow his business, and in return, Justin would buy a 30% stake in the company. They shook on it. Justin and his partner invested a bit of money into this business. What mess did I get myself into? As Justin was doing a background check on the company, he found out that the books were a mess and even had receivable loans. Though this was a red flag, he dismissed it. He figured his accountant would sort it out. What attracted Justin to this partnership was the fact that there was a huge fanbase, and he knew the business had the potential to make huge profits. It’s a deal The trio signed the deal, created a new LLC, and pulled over the assets making the partnership official. They set up new bank accounts and tried to do everything the right way. Justin went all in and started humming along and focused on sales. He spent a bunch of money on advertising and dialing things in. He increased the average order value by about 40% in a short amount of time. Deal goes sour After some time, the partner went back to his old ways and started spending company money on personal stuff. At first, $2,000 went missing from the business account, then $2,500, and then $4,000. To make matters worse, all of a sudden, two more receivables loans popped up. So now the company was triple-dipping before they even got to make any profits. Every sale they made had to be channeled to repay the loans. Soon enough, Justin realized that this partnership would not be beneficial to him. His partner’s spending and the loans would cripple the business. Justin tried to have a conversation with him about his spending, but he just scoffed at him. Calling it quits One day while at his son’s football game, Justin got a notification on his phone that he had a change in his access to the bank account. He tried logging in but had no access to anything, the bank account, the PayPal account, the website, nothing. He has been locked out of everything. Justin sent a group text to the business partner, and he made up some big story about how he didn’t want to burden him with his debt, and because he started the company, he wanted to take care of it alone. Justin decided not to fight him or even take him to court as it would not be worth it, and he might just end up losing more money than he had already invested. He decided to write the investment off as a bad debt. Lessons learned Do not get emotions involved when entering a partnership When you see something exciting that you can relate to, and you want in, be careful not to let your feelings guide your decisions. Do your due diligence Do your due diligence before entering into a partnership, look out for red flags such as commingling of funds, lack of books, lack of true expenses, and P&L balance sheet. Do not rush Do not be in a rush to enter into a business partnership. The timing will come when the right time comes. Andrew’s takeaways Think the red flags through When you see a red flag, stop and step back. Think things through and see how to deal with the red flags first. Also, you do not have to stop the deal, but you must slow down your emotions. Understand the difference between emotion and intuition Intuition is an instantaneous feeling that will go away quickly, and then your emotions and your mind will override it. In many cases, it will be the right thing. Make sure you’re open and aware to the intuition message that is coming to you, and so you can receive it. Know who can bind your company to any agreement Before buying into a company or getting into a partnership, know who has the power to sign the checks and the power to bind the company. Understand how far that power goes before you commit to any agreement. Avoid confrontations You do not have to have a confrontation over everything that happens in life. Sometimes avoiding confrontations is the best way to deal with conflicts. Actionable advice Just be patient. Take it all in and trust your intuition. No. 1 goal for the next 12 months Justin’s number one goal for the next 12 months is to double his current business. Episode 082 - Justin Wineburgh, President and CEO of Alkemy X This week's guest is the President and CEO of Alkemy X, a production company that brings "big ideas to life by working together in an end-to-end process that allows (them) to ideate, produce, and deliver faster (and better) than you ever thought possible."  Alkemy X has offices in offices in Philly, LA, NY, and Amsterdam, and their team has a proven track record of bringing a unique approach to every production or project they are involved in. Justin is not your average lawyer. In fact, as Justin points out during this episode, he considers himself a "reformed lawyer." He built the entertainment practice at Cozen, by maintaining strong friendships that eventually led to a big opportunity.  In the course of representing one of Justin's best friends, in a lawsuit over the film The Ring, Justin found himself quickly making a name for himself as an attorney that could help film and entertainment producers get things done. This opportunity begot other opportunities, which eventually led to Alkemy X (check them out here: https://www.alkemy-x.com/), a production company based in Philadelphia, reaching out to Justin because the company was failing. Alkemy was hit hard by market factors (the commoditization of post-production equipment, consolidation of the market for post-production services, etc.), and found itself spread thin, having difficulty in a tough film/media production market. Justin, who at the time had served as outside counsel for Alkemy, came up short when he tried to help by recruiting a new CEO to rescue Alkemy. That’s when Alkemy's Chairman of the Board asked Justin if he would be willing to take over the company. This was in November 2015, and Justin had built a large media-and-entertainment law practice at Philly-based Cozen O’Connor, and was enjoying life as a partner. Justin, knowing the media and entertainment business to be a fickle one, actually said "no." But, ultimately that did not end the discussion. Justin knew his life would be pretty solid if he continued on at the firm, but wanted a challenge.  Something inside of him told him this new opportunity could provide a big upside as well as add some excitement. As Justin mentions in this episode, he just thought to himself "the worst thing that can happen is I have to go back to being a lawyer."  So Justin finally decided to resign from Cozen and take on a new challenge at Alkemy. He started on February 1, 2016 and has since transformed Alkemy into a top production house, including receiving recognition by winning six 2019 Addy Awards from the Philly Ad Club, including "Best of Show" for The Plastic Ocean. When it comes to following God's call on our lives, it's easy to get so wrapped up in serving God that we forget to be with Him. We get so concerned with doing the next thing, and the next thing that we begin to define ourselves by what we do, not who we are in Christ. Justin calls this, the pursuit of the “Resume Virtues.” But we need to stop and ask, “What will they say about us at our funerals?" Will they be talking about the items on our resumes, or will they be talking about the kind of person we were. How we were kind, loving, and patient. Or how treated the people around us. That's why we need to pursue God's calling on our lives with these "Eulogy Virtues" in mind. As Justin talks about in this chapter, it's good that we should pursue our callings, but we must not seek achievements and accomplishments if we lose sight of who we are. Whenever there’s a big event, there’s bound to be news crews. That was no exception when police descended on the accused Claremont Serial Killer’s house on December 22, 2016. But unlike now, those news crews had no idea just how big that raid would be. With special guests, veteran 7NEWS cameramen Ray Raab and The West Australian photographer Justin Benson-Cooper joining Natalie Bonjolo and Tim Clarke, they discuss, even when the opposition got a shot of ‘a man’ being taken away by police from his house, they had no idea what they were looking at. As the day progressed, rumours started to swirl. Rumours turned into questions and then scrambling by news houses across the state. It was the biggest story WA had ever seen. Did police catch the Claremont Serial Killer? We still won’t find out for another few months as the trial continues. Reporters, camera operators and photographers had one chance at getting a shot of the man arrested for one of the most infamous crimes in WA history. As Justin and Ray relive the day they stayed out until 4am to get “that” shot of Bradley Edwards being driven to police lockup after a six-hour interview with police. The preparation, the nerves, the 10 seconds of action. Sexual Fantasies and Eroticism I know I say it often, but this topic is one of my favorites. In this episode, I talk with Dr. Justin Lehmiller about the all-important topic of sexual fantasies. Justin is a celebrated speaker, researcher, author, and a very effective educator on the psychology of sexuality. His blog Sex and Psychology (https://www.lehmiller.com/) gets millions of visitors every year, and he regularly contributes his writing to major publications. This talk about his research is guided by his expertise and experience in the field. The Most Common Sexual Fantasies Justin says that when he surveyed almost 4,200 Americans from 2014 to 2016, the most common fantasies encompassed 7 different themes. Multi-partner sex BDSM Novelty, Adventure & Variety Taboo activities Emotional connection and fulfillment Homoeroticism and gender-bending Non-monogamyJustin describes these as the building blocks of fantasies, meaning that they are not mutually exclusive and many overlap. For example, you can very well dip your toes into multiple categories in your own personal fantasy life. Are people ashamed of their fantasies? As Justin states, he found that men reported more shame about their fantasies than women. Overall, the majority of study participants reported that they held a positive relationship with their fantasies, but there were still some who felt negative emotions towards their fantasy. Another important thing he found during his research is that just sharing sexual fantasies can open up eroticism and alleviate feelings of embarrassment or shame for having certain fantasies. The Differences between Men and Women regarding fantasyAlthough the data showed that both sexes share a lot of commonalities, there were still some marked differences. Men had more multi-partner fantasies than women did. And women had more fantasies about an emotional connection with a partner. Women also had way more BDSM fantasies than men by a large margin. In addition, men usually had a specific person in mind during their fantasies, and the women focused more on the setting and environment overall. Justin also found that the LGBTQ community had more sexually adventurous fantasies, as well as taboo fantasies. Justin provides some insight into why women might like BDSM more than men, as well as the LGBTQ community and their sexual fantasy preferences. Listen in for that. Sexual Fantasy by Personality Type Justin shares some interesting insight into the correlation between personality type and sexual fantasy. For example, those who are more extroverted by nature will be more outgoing the bedroom. And for those who are ‘agreeable’ personality types, there will be a higher incidence of focusing on their partner’s sexual satisfaction in the bedroom. He also talks about what conscientiousness has to do with fantasies, as well as self-esteem. “Sometimes a Cigar is Just a Cigar” Justin says that sexual fantasies don’t really have to mean all that much. They can offer a glimpse into something deeper, but for the most part, they are just a product of your environment and genetic makeup and can be left out of the examination room. Fantasies can be a good evaluative road map to follow for your own unique sexual satisfaction, though. But when talking about sexual traumas, there were small connections between sexual victimization and types of fantasies. But there was a lot of inconsistency in the data. Hear Justin explain the data on this subject. How to Share Your Fantasies with Your Partner Justin says that before you share with your partner, you first have to feel good about yourself. You aren’t alone in your fantasies, so there’s a normalization that needs to first occur. He says to lay low and start slow. A gradual buildup for disclosing your fantasies to your partner is much more powerful than an overwhelming information dump! He also goes into detail on how important sharing is for increasing overall sexual desire and satisfaction within the relationship. Justin Ross was a local high school student, a son, and a brother who joined the Army Reserves in 2006, before he even graduated from Bay Port High School. Corporal Ross died serving our country in in Afghanistan in 2011, a story covered extensively by Action 2 News in the days and months that followed.In this Get 2 the Point episode, we talked with Justin’s parents, Ron and Debbie Ross.  The Rosses tell us how Justin always loved playing cowboy as a little boy.  “He was always the sheriff,” says Ron.  As Justin grew up, he admired those in civil or military service.  “It really wasn’t a surprise when he came home from high school his junior year and started talking about the (Army) recruiter being there. He came home saying that’s what he was going to do.”

Running with Cerebral Palsy Justin Gallegos has always had a different life than most. By the time he was three he still had difficulty learning to walk and was diagnosed with Cerebral Palsy (CP). Physical activity and speaking were and have been two of the more challenging aspects of Justin’s life. Justin wasn’t able to participate in P.E. classes throughout elementary school, and has had to work on reducing his speech impediment throughout his life. But CP isn’t the only thing that sets Justin’s life apart from most.  Over the past few years Justin has been busy building quite an impressive resumé for himself. He is studying journalism at the University of Oregon while simultaneously running for their track team and working with Nike to design a special pair of shoes. There seems to be no obstacle too large for Justin. He is now a motivational speaker and professional athlete. It was official when he became the first athlete with CP to sign with Nike, a dream come true for Justin.  And he is not done yet. What is Cerebral Palsy? Brain damage, especially while the brain is developing and the child is in the womb, is the cause of Cerebral Palsy. The symptoms include a wide variety of muscle movement challenges. Involuntary movement, abnormal posture, and speech impediments are some of the most common.  As Justin explains it, “It is very a diverse disability.” For some people this means they are paralyzed, for others it means they struggle with speaking and walking. Justin considers himself somewhere in the middle, closer to the high-functioning side. Cerebral Palsy is challenging, but as Justin shows, it is not defining.  You Are Not Your Disability Justin is a wonderful ambassador for being much more than his disability, and running has been a huge factor for helping him come to that realization. When Justin first started running, he was unable to pick up his feet and knees properly, causing his feet to drag. He would fall during his workouts frequently. But slowly, his body grew stronger, and as it did, so did his mind. “I never set out to be a motivational speaker or inspiring to be a pro athlete,” says Justin, “But running really did that for me, in the sense that it changed my mental game and showed me that I’m no different than anyone else.” Running has shown Justin that he can overcome. It has given him the confidence to tackle other impossibilities. This is one of the most beautiful things about running. It is a sport that allows you to improve every day, to work hard on something every day, and it rewards you with determination to live life fearlessly. For Justin and for anyone, running can change your life.  Appreciating the Push When Justin began running cross country and track for his high school, he experienced something new and refreshing. Instead of being told to be careful, he was asked to work harder. He relates that his coach often pushed him to work just as harder or harder than the other athletes. Justin thrived in this environment. Having the trust of a coach to encourage Justin to run faster instead of to slow down, gave him a drive and confidence. Sometimes it is easy to overlook our opportunities because of their difficulty. Justin’s mindset is a wonderful example. Next time you feel overwhelmed with the tasks before you, take a step back and be grateful that you are allowed to take on those tasks. An attitude of gratitude will change your world. Your Only Limit is Your Mind Each of us has challenges that are unique to us. Some may appear harder than others, but Justin’s message is that all of us have limitless potential. “Never forget you are limitless,” he says. You will be happily surprised when you put your heart into your work with gratitude and fearlessness. Like Justin, you too can change the world. This week the boys sat down for a chat about an opinion piece titled “CrossFit Still Needs Its Heroes”, written by the Editor-in-Chief of The Morning Chalk Up, a daily CrossFit focused email dedicated to keeping it’s readers up to date with the CrossFit community. This article was the perfect follow up to our conversation last week in episode 34 about the changes to the CrossFit.com main site and the overall change in focus of CrossFit HQ because it raised the questions of “who will we all look to as inspiration to help us dig a little deeper at the end of a workout?” Which is where we all know the most progress is made physically and mentally. And, “who will be the face of the sport that our kids look up to and pretend to be when they’re lifting weights, running laps and doing pull-ups? Transformation became a theme because it’s what Justin and Janell become known for As Justin and Janell began to experience the incredible benefits of their own life transformations they wanted to help others. From fitness training to diet and weight loss coaching, Justin and Janell dove into helping people. Their fitness business grew quickly and they became known for helping people with various transformations they needed to experience. That’s when the two of them decided to re-brand the club, “The Transformation Club.” Fast forward to today and you’ll find Justin and Janell have come a long way. Dr Carolyn Lam:                Welcome to Circulation on the Run, your weekly podcast summary and backstage pass to the journal and its editors. I'm Dr Carolyn Lam, associate editor from the National Heart Center and Duke National University of Singapore. Sacubitril-valsartan reduces the risk of cardiovascular mortality among patients with heart failure with reduced ejection fraction. However, what are its effects on kidney function and cardiac biomarkers in people with moderate-to-severe chronic kidney disease? Well, stay tuned to find out, as we will be discussing the results of the UK Harp III Trial, right after these summaries.                                                 The first original paper this week reveals that inhibition of a long non-coding RNA may serve as a novel molecular therapy for aortic aneurysms. First author, Dr Li, corresponding author, Dr Maegdefessel from Technical University Munich, and colleagues, identified the long non-coding RNA H-19 with functional relevance in experimental aortic aneurysm progression in two mirroring models, a novel genetically mutated mini-pig model, as well as end-stage human disease. They found that H-19 mediated expression levels of the transcription factor hypoxia inducible factor 1-Alpha. Which, in the chronic hypoxic environment of an aneurysm, triggers apoptosis in aortic smooth muscle cells. This study, therefore, introduces inhibition of H-19 as a novel molecular therapy to limit smooth muscle cell death in progressing aortic aneurysms.                                                 The next study provides insights into molecular mechanisms underlying heart failure progression in chronic pressure overload. Co-first author, Dr Chiang and Alsina, co-corresponding authors, Dr Heck, from Utrecht University, and Dr Wehrens, from Baylor College of Medicine, and their colleagues developed a novel and unbiased way to comprehensively study protein phosphatase 1 or PP1 interactors in a mouse model of progressive heart failure induced by elevated afterload. This so-called PP1 interaction enabled simultaneous interrogation of multiple pathways relevant to heart failure pathogenesis. They found nine specific PP1 interactors that were strongly associated with heart failure progression. Among these, the PP1 regulatory subunit 7 was shown to play a central role by regulating the PP1 interaction, and by acting as a competitive molecular sponge of PP1.                                                 In clinical trials of direct oral anticoagulants for atrial fibrillation, patients with end stage kidney disease on dialysis were excluded. Today's study answers the question, "What are the outcomes with Apixaban in dialysis dependent end stage kidney disease patients with atrial fibrillation?"                                                 Co-corresponding authors Dr Siontis and Dr Saran from University of Michigan and their colleagues performed a retrospective cohort study of Medicare beneficiaries included in the United States Renal Data System from 2010-2015. All eligible patients were those with end stage kidney disease and atrial fibrillation undergoing dialysis who had initiated treatment with an oral anticoagulant.                                                 In prognostic score-matched analysis, Apixaban was associated with lower rates of major bleeding compared with Warfarin, whereas there was no difference in stroke or systemic embolism. Patients on standard dose of Apixaban of 5 mg had a lower rate of stroke and death compared to those on reduced dose Apixaban of 2.5mg. Thus, Apixaban may be associated with superior safety and comparable effectiveness outcomes as Warfarin in dialysis patients with atrial fibrillation. However, these findings require confirmation in a randomized trial setting.                                                 Does Canagliflozin have benefits in people with chronic kidney disease, including those with an Estimated Glomerular Filtration Rate, or EGFR, between 30 and 45, in whom the drug is currently not approved? First author Dr Neuen, corresponding author Dr Perkovic from the George Institute of Global Health, and their colleagues performed a secondary analysis of the CANVAS Program to describe outcomes in participants with and without chronic kidney disease, as well as according to baseline kidney function as measure by EGFR.                                                 They found that the effect of Canagliflozin on HbA1c was progressively attenuated at lower EGFR levels, but blood pressure and body weight reductions were comparable. The reduction in risk of major adverse cardiovascular events, hospitalization for heart failure and progression of kidney disease appeared similar across different levels of kidney function, down to an EGFR of 30. Safety outcomes were also mostly consistent, but the risk of hypoglycemia may increase as EGFR declines.                                                 That wraps it up for our summaries, now for our feature discussion.                                                 Cubitalis-valsartan improves outcomes in patients with heart failure with reduced ejection fraction, and we know that from the Paradigm trial, but what about its effects on kidney function and cardiac biomarkers in people with chronic kidney disease?                                                 Well, this week's feature paper provides important randomized trial data addressing this question. To discuss it, we have none other than the first and corresponding author, Dr Richard Haynes from University of Oxford, as well as our editorialist for the paper, Braden Manns and Matthew James, both from University of Calgary and in addition, we have Dr Justin Ezekowitz, associate editor who manages paper, and Justin is from University of Alberta.                                                 Welcome gentlemen, we have a full house. Richard, could you start by sharing about your trial and your findings? Dr Richard Haynes:          So, the trial was called UK Harp-III, and it was really a pilot trial, just to work to investigate the effects of Cubitalis-valsartan on patients with chronic kidney disease, and in particular to see what it did for their kidney function in the short term, and also what it did to other measures of interest like their blood pressure and cardiac biomarkers.                                                 It was a randomized control trial double blind, among just over 400 people with chronic kidney disease, and we compared Cubitalis-valsartan with Irbesartan, which is standard of care for most of these patients. Our primary outcome was really to look at the effects of these drugs on kidney function when it was being precisely measured in hospitals. We found, actually, that Cubitalis-valsartan had very similar effects to Irbesartan on kidney function. So, there was no real difference in kidney function at any point in the trial between patients who were allocated the Cubitalis-valsartan or those allocated Irbesartan. Dr Carolyn Lam:                Richard, the way you described it I'm sure you're prepared for this question so why Irbesartan as the control versus Valsartan? Dr Richard Haynes:          That's a very good question and a question asked quite often. There were six of one and half a dozen of the other. We could have chosen Valsartan. The difficulty with that is that Valsartan doesn't have a license indication for the treatment of chronic kidney disease so if we found a difference people might have said we just chosen an inferior comparator, so we chose Irbesartan because that does have an indication for the treatment of proteinuria kidney disease and obviously that leaves us open for the question about how different Valsartan and Irbesartan are. My opinion is they might be subtly different, but I don't think the difference is big enough to really impact these results in any meaningful way. Dr Carolyn Lam:                Indeed, and I know Braden and Matthew you have thought about it a lot. Congratulations on the beautiful editorial. I love the way you set the context in the heart failure world where perhaps we have noted something different with regards to kidney function. Would either of you like to start the ball rolling with discussing that? Matthew James:              Sure, this is Matthew James. So really the Paradigm Heart Failure Trial is a very important place to start in thinking about the effect of these medications on kidney function. That was a very large trial that did report changes in estimated Glomerular Filtration Rate and did show a small but statistically significant change in kidney function between the Sacubitril-valsartan arm and the control arm. There are many potential mechanisms for that, but it is important to realize that there were limitations in the population specifically around chronic kidney disease due to the level of kidney function that the patients were enrolled in to the study. So, some of the patients with more advanced chronic kidney disease wouldn't have been included in the Paradigm Heart Failure Trial so this trial is actually giving us more information about patients with kidney disease who we would expect to be at higher risk of seeing progressive loss of kidney function or progression of their kidney disease. Dr Carolyn Lam:                Thanks for setting that up and just to clarify for the audience here so in Paradigm EGFR went down to 30 right, and here in UK Harp we are talking about measured GFR down to 20. Am I right? Dr Richard Haynes:          Eligibility was actually determined by the EGFR, the estimated GFR.                                                 Yeah it went down to 20, up to 60. We also had a much more proteinuria in the patients in Paradigm. Dr Carolyn Lam:                Right, and do you have a take Richard on why the results seem different from at least the secondary analysis that Milton Packer wrote about on its effects on kidney function in Paradigm? Dr Richard Haynes:          I do have a take. I'm really interested to hear what Braden and Matthew thought. My take was that probably when you've got heart failure one of the major determinants of how well your kidneys work is actually how well your heart is working. That is probably one of the major determinants in that setting and because we know Sacubitril-valsartan has such beneficial effects on cardiac function in people with heart failure perhaps it's not surprising that it then is protected by kidney function a little bit better than people given Enalapril in Paradigm. However, in UK Harp III, we had a group of patients whose kidney had very definite kidney disease and probably the determinants of kidney progression quite different and having any impact on their heart function probably wouldn't really be noticed because the effect of their kidney disease would outweigh that. Perhaps, Sacubitril-valsartan doesn't have any beneficial effects on the kidney itself. As far as we can tell, from what is a relatively small and a relatively short trial. Dr Carolyn Lam:                Justin, I mean you come from the heart failure world too just like me. What was your take? Dr Justin Ezekowitz:        I think there are a number of features here we should take a step back and think about. Number one is as Richard outlined there is a lot more proteinuria here than would typically be seen in a heart failure related population. So, the comparator between the two groups, while similar in overlap while co-manage these patients is somewhat different in terms of what the result we are looking for. So, you know, it brings to mind that what we look at in the secondary analysis in for example Paradigm, is simple EGFR creatinine changes versus here we are looking at a much more sophisticated measure of GFR plus also looking at a comparator that is known to reduce proteinuria and I would say stabilize or not change or prevent their progression of renal disease in the larger trials in the renal population. So, it's a slightly different population, a slightly different comparator as well. The importance in the choice of comparators becomes really important when we are looking for this specific effect.                                                 Now, to Richard's point, which he opened with, which is talking about this as a pilot project to a larger outcome trial, it is hard to know whether or not the effects that Richard and his team on the NT-proBNP, troponin, and other effects would play out in the larger cardiovascular outcomes trial that would be potentially different results than simply a GFR change or proteinuria change. I would be interested in Richard's thoughts on that and Matt and Braden's as well. Matthew James:              Maybe we can also get add another question to Richard which this was a really well-done study and you talked about it being relatively small and certainly by heart standards this was a relatively small pilot study with a limited duration of follow up. By kidney standards, this is a fairly this would be a usual sized clinical trial and so getting all these patients in the trial was a wonderful result to start with and while the study wasn't directly looking at safety of these medications, there is some I think assurance we have some tolerability data at least with this medication and the challenge as Richard would well know in managing patients with chronic kidney disease once they developed more advanced chronic kidney disease GFR is less than 30 is often difficult to use medications because of side effects, high potassium, and things. The most challenging types of patients we see are patients with lower levels of kidney function and with low ejection fractions. So at least this paper provides some hope that we've got a medication that is reasonably well tolerated in that population.                                                 I think that when Richard talks about this being a pilot study where a lot of patients, in fact patients with chronic kidney disease are much more likely to die from heart disease than they are to develop end stage renal disease. For many types of patients that is true at least. So, we are often thinking about what medications could be used to improve cardiovascular outcomes. So, in that sense, again given that the majority of the structural heart disease is not necessarily reduced heart function but is left ventricular hypertrophy I'm sure, and perhaps Richard has some comments as to the next study that might be considered given this medication seemed tolerable. It didn't have the effects that were perhaps hoped on progression although in the Paradigm sub study there was only a difference of 0.5 ml per minute and they were powered to detect 3 ml per minute in this study but actually the immediate hemodynamic drop was about 3 ml per minute and then kidney function was relatively stable thereafter. So hard to imagine this study would have showed a difference in kidney function now in retrospect but potentially this opens up some additional studies to look at cardiovascular outcomes in patients with chronic kidney disease who don't have reduced ejection fraction. Dr Richard Haynes:          I think that's a really good point. I think it would be fascinating to see the results of the Paradigm Trial with Sacubitril-valsartan in patients with heart failure and preserved ejection fraction. Nevertheless, I think this trial does raise the hypothesis that this might be a drug that could improve regardless of whether it has any effect on the kidney or not. It could be possibly be used for improving cardiac outcomes but I just don't think the trial that we've done is enough to justify that at the moment. I think it's a good indicator that it may well work, but I think before anybody could recommend that with much enthusiasm I think it would require a large outcomes trial but focusing quite rightly on cardiovascular outcomes in people with chronic kidney disease which as Matthew said is actually the major burden of disease in those patients. Dr Justin Ezekowitz         I think the question remains though is if as a pilot trial at that time as a longer-term trial would there be any difference because the mechanism of action of Sacubitril is different from that of Irbesartan and that was also shown in the nice table you have in the supplemental file which talks about the Sacubrital lapse concentration going up with the lower GFR's. So, there is the potential for those small subgroups where the GFR is lower they may have a substantial benefit over a longer period of time, not measured necessarily by GFR but measured by clinical outcomes. I think that is where the balance of getting the pilot trial versus a longer follow-up clinical outcomes trial is really important to get.                                                 I may actually just state one other thing or two. First, it's really important to investigate or initiate a trial and this is one of critical parts of why we do clinical trials. Medicine tests the effects initially a pilot and then hopefully a larger trial.                                                 The second is the importance of randomization here. We all think that the shiny new medications are important but getting randomization in trials like this done are really advanced knowledge, so we know what to do with the medication if we are faced with it or if we want to make an important choice for a patient that we can really make a point for the patient that we will base it on the best scientific knowledge.                                                 The third point that I would just come back to something else that we have not talked about yet is this overall is a neutral trial. There are no major effects that were seen but the importance of getting a neutral trial done and published is really critical as this advances the field potentially, so others can now decide what to do and perhaps launch larger trials with cardiovascular outcomes or decide to do a different comparator or different other tasks forward. So, this one we emphasize it is critically important to get these types of trials done and then published. Dr Carolyn Lam:                You know Justin, I couldn't have said it better and completely echo your words. We are so proud to be publishing your paper Richard and that beautiful editorial in circulation. So, I'm just going to wrap up then because in the absence of better data at the moment what is the main take home message of this trial for patients with CKD right now and their care providers. I would love to start with Braden because you wrote about it in the editorial as well. What do you think of the take home messages? Braden Manns:                 Well again I think that we often struggle when peoples GFRs are in the 20 to 30 range with identifying a medication that's tolerable particularly in the context of people with reduced ejection fraction. I must say personally I would now be comfortable using this medication in patients with reduced ejection fraction who remain symptomatic who have GFRs in the 20 to 30 range. Those patients aren't that common but feel comfortable now using that type of medication there despite the fact that most patients weren't necessarily enrolled in the Paradigm study. A much larger population though of patients with structural heart disease but not reduced ejection fraction who have chronic kidney disease. It is not clear to me where this medication fits in the armamentarium. As Justin says it certainly wouldn't use this in preference to an ace inhibitor or an angiotensin receptor blocker at this point. So, it's hard to know where it fits without some larger studies looking at cardiac outcomes. Matthew James:              I agree with Braden. I think we are already seeing this medication now enter practice here in Canada. There is this overlap in population between the patients with kidney disease and impaired left ventricular ejection fraction, so this is actually very helpful for us when we see these patients in practice around the appropriateness of continuing these medications in this patient population. Dr Justin Ezekowitz:        So, I think it's critically important to remember the take home message here is to do proper clinical trials and then do again the large trial because without that would not really advance in knowledge. There could be a huge value to a newer medication or potentially the old ones are still just as good as we if we continue them safely. Dr Richard Haynes:          I'd like to echo what everybody said already really. I mean I think what Justin just said trial is the key. We can't get away from the need for randomized control trials. I'm pleased that we've managed to deliver this one. In terms of a clinical take home message I think if I was a patient with kidney disease and heart failure, especially with reduced ejection fraction, I hope that I would feel a bit more comfortable to take this drug now knowing is it going to benefit me from a cardiovascular point of view it doesn't seem it is going to do my kidneys any harm either. So, hopefully it will reassure more patients that they can yield the benefits of a trial this drug has. Dr Carolyn Lam:                Great stuff! Thank you so much gentlemen.
