Dr. Michael T. Yen is an oculoplastic surgeon and current Sarah Campbell Blaffer Chair in Ophthalmology at the Cullen Eye Institute at the Baylor College of Medicine in Houston, TX. Dr. Yen attended medical school at the University of Michigan and completed his residency at the Bascom Palmer Eye Institute in Miami, followed by a fellowship in ophthalmic and facial plastic surgery with Dr. Richard Anderson in Salt Lake City. Dr. Yen is a team ophthalmologist for the Houston Rockets in the NBA. Dr. Frank Cotter is a glaucoma specialist in Roanoke, VA. Dr. Cotter completed his Doctorate of Medicine at Georgetown University in Washington, DC. After an internship with Letterman Army Medical Center in San Francisco, he returned to Washington DC to complete his residency at Walter Reed Army Medical Center. He also completed two fellowships with a focus on glaucoma at the University of Virginia and the Medical College of Virginia.

About Dr. Elise KramerDr. Elise Kramer is a residency-trained optometrist in Miami FL who specializes in ocular surface disease and specialty contact lens design and fitting.Her Doctorate degree was awarded in Optometry from the Université de Montréal in 2012. During her fourth year, she completed her internship in ocular disease at the Eye Centers of South Florida and went on to complete her residency at the Miami VA Medical Center. Her time there included training at the Bascom Palmer Eye Institute, the nation's top eye hospital. After her residency, Dr. Kramer became a fellow of the Scleral Lens Education Society (SLS) and now serves as the vice president for the SLS. Dr. Kramer is also vice president of Women in Eyecare (WiE), a member of the American Optometric Association (AOA), the International Association of Contact Lens Educators (IACLE), a Fellow of the American Academy of Optometry (AAO) and of the the British Contact Lens Association (BCLA). She is also the Delegate of International Relations for the Italian Association of Scleral Lenses (AILeS).  Dr. Kramer has published several important articles and reviews and is participates in clinical research trials. She enjoys lecturing all around the world in several different languages about ocular surface disease and specialty lenses.

We had a really great conversation about the nuances of the processes we have within our practices, how to manage these processes to make the every day easier, and how some of the processes can bog us down if we don't manage them well.  About our guest Dr. Brianna Rhue: Brianna Rhue, OD, FAAO, a board-certified optometric physician, brings her expertise in diagnosing and managing ocular diseases to West Broward Eyecare Associates in Tamarac, Florida. Among her specialties are myopia control (orthokeratology), management of keratoconus, diabetic eye care, and treatments for glaucoma and diseases of the cornea. She is a nationally recognized leader in Myopia Management, lecturing extensively throughout the year, and is currently a key principal investigator in the study of myopia management. Dr. Rhue earned her undergraduate degree from the University of Arizona in Tucson before earning her Doctorate of Optometry at Nova Southeastern University in Broward County, Florida. She completed residency training at Bascom Palmer Eye Institute in Miami, Florida. During her optometry education, Dr. Rhue also donated her time and services providing eye care to under-served populations in Peru, Trinidad, and Guatemala. In addition to treating patients at West Broward Eyecare Associates, Dr. Rhue is the immediate past president of the Broward County Optometric Association and was awarded the Florida Optometric Association Local Society Member of the year in 2019. She is an active member of the Broward County Optometric Association, Florida Optometric Association and American Optometric Association. After 14 years of living in rural Alaska, Dr. Hartl has seen firsthand the limitations geography can have on access to medical care. She is passionate about creating a new gold standard for telemedicine in underserved America via their groundbreaking hybrid model.   Dr. Hartl was inspired to start Troy Medical after witnessing one of her own patients become overwhelmed by a debilitating medical condition. Unable to find a medical specialist in his region, his condition had progressed to the point where even walking was a challenge. Dr. Hartl recognized the potential of telemedicine to bring life-changing care to remote areas, so she reached out to specialists across the country to organize a team of doctors who can provide world-class, personalized medical care to patients anywhere. According to the American Academy of Ophthalmology, over 150 million Americans wear glasses and about 37 million wear contact lenses to correct daily eyesight issues. In hopes of long-term treatment of refractive errors and achieving 20/20 vision, laser eye surgeries, such as Laser-Assisted In Situ Keratomileusis (LASIK) and Photorefractive keratectomy (PRK), are performed.We are joined today by Dr. Dagny Zhu, a board-certified ophthalmologist and fellowship-trained specialist in cornea, cataract, and laser refractive surgery. According to American Library Association, around 22,000 children's books were published in 2009 alone. A myriad of papers and storylines arrive in people's homes every day, ranging from magical worlds to scientific pamphlets. However, only a small fraction of these books are targeted at kids who have developed an interest in the sciences of the human body. Due to an extensive list of medical terms, organ systems, and anatomical parts, the pool of children's books that satiates the interest in learning about the inner workings of people's bodies is very poor and sometimes inaccessible. The newest Amazon book series, "The Medical School for Kids," seeks to change all of this.We are joined today by physician couple authors, Dr. Betty Nguyen and Dr. Brandon Pham. Betty is a new-graduate physician, receiving her BS in Biology from UCLA in 2017 and MD from UC Riverside School of Medicine in 2023. She received a mentorship grant from the American Hair Research Society in 2021 that guided her to be a research fellow at the University of Miami's Department of Dermatology and Cutaneous Surgery in 2022 and was rated by Doximity in the Top 10% Most Published Clinician of 2022. Brandon is an Ophthalmology resident physician at Bascom Palmer Eye Institute, the #1 ranked eye hospital in the nation. He received his BS in Microbiology, Immunology, and Molecular Genetics from UCLA in 2017 as Summa Cum Laude and MD from Stanford Medical School in 2021. This podcast is about big ideas on how technology is making life better for people with vision loss. In 1997, Gary Kasparov lost an epic chess rematch to IBM's supercomputer Deep Blue, but since then, artificial intelligence has become humanity's life-saving collaborator. This episode explores how AI will revolutionize vision technology and, beyond that, all of medicine. Karthik Kannan, co-founder of AI vision-tech company Envision, explains the difference between natural intelligence and artificial intelligence by imagining a restaurant recognizer. He describes how he would design the model and train it with positive or negative feedback through multiple “epochs” — the same process he used to build Envision. Envision uses AI to identify the world for a blind or visually-impaired user using only smartphones and smart glasses. Beyond vision tech, AI enables faster and more effective ophthalmic diagnosis and treatment. Dr. Ranya Habash, CEO of Lifelong Vision and a world-renowned eye surgeon, and her former colleagues at Bascom Palmer, together with Microsoft, built the Multi-Disease Retinal Algorithm, which uses AI to diagnose glaucoma and diabetic retinopathy from just a photograph. She acquired for Bascom Palmer a prototype of the new Kernal device, a wearable headset that records brain wave activity. Doctors use the device to apply algorithms to brainwave activity, in order to stage glaucoma, for example, or identify the most effective treatments for pain. Finally, AI revolutionizes drug discovery. Christina Cheddar Berk of CNBC reports that thanks to AI, Pfizer developed its COVID-19 treatment, Paxlovid, in just four months. Precision medicine, targeted to a patient's genetic information, is one more way AI will make drugs more effective. These AI-reliant innovations will certainly lower drug costs, but the value to patients of having additional, targeted, and effective therapies will be priceless.   The Big Takeaways: Natural vs. artificial intelligence, and the “restaurant recognizer.” Karthik Kannan, CEO and co-founder of Envision explains the difference between natural and artificial intelligence by describing how humans recognize restaurants in a foreign city and comparing that to how he'd train a “restaurant recognizing algorithm.” Here's a hint: the algorithm needs a lot more data. Sensor fusion AI. AI developers are interested in using different types of sensors together to give the algorithms a sense of the world closer to human intelligence. One example is the use of LiDAR in the Envision app, in addition to the phone camera. Transhumanism. Humans don't have LiDAR. Does that mean AI will surpass human capability? Karthik offers that some radiology AI have higher accuracy than human radiologists, but he thinks it will be much more of a partnership between the human and the machine. Multi-Disease Retinal Algorithm. Dr. Ranya Habash and her colleagues at Bascom Palmer Eye Institute worked with Microsoft on an AI diagnostic tool. They fed the algorithm 86,000 images of eyes, labeled with relevant diseases, and taught the machine to diagnose eye disease with just a photograph, making remote diagnosis not just possible but inexpensive. The Brain-Machine Interface. Dr. Habash wrote a grant that earned Bascom Palmer a prototype of the Kernal device, a helmet-like device that measures brainwave activity. Doctors used this device to create a “brain-machine interface” which advances brain research on glaucoma, diabetic retinopathy, Alzheimer's, and pain management. Bias in AI. Karthik Kannan reminds us that the biggest threat that humanity faces from AI is bias encoded in the algorithms. This is a real harm that humans have already experienced, and AI engineers need to be extremely sensitive to ensure they are not encoding their own biases. AI for Drug Discovery. Christina Cheddar Berk, a reporter for CNBC, shares how the pace of drug discovery is set to speed up, thanks to AI algorithms and supercomputing power that can cycle through millions of possible chemical compounds per second to I.D. effective options. Dr. Amy Schefler graduated from Yale University and received her medical degree from Cornell University, followed by an ocular oncology fellowship at Memorial Sloan-Kettering Cancer Center, and residency, chief residency, vitreoretinal surgery training, and ocular oncology fellowship training at Bascom Palmer Eye Institute. After two years on the faculty at Bascom Palmer, she moved to Houston, Texas where she has been for eight years. In Houston, she runs the adult ocular oncology service at Houston Methodist Hospital and the pediatric ocular oncology service at Children's Memorial Hermann Hospital. These ocular oncology services are among the busiest in the United States, consistently enrolling among the most patients in multiple large multicenter clinical trials. Dr. Schefler is a renowned researcher in eye cancer, and has published over 80 peer-reviewed publications, 20 book chapters, and given over 90 national and international meeting presentations. In addition, she serves as a reviewer for 20 major ophthalmic journals. TAG #35 GEDDE Steve - Glaucoma surgical trials: lessons learnedIntro:There have been many trials proving surgical intervention does indeed reduce the risk of progressive glaucoma damage for our patients. These include Treatment vs No Treatment studies as well as studies that compare different types of treatment. In this episode, I talk with Steve Gedde, from the University of Miami's Bascom Palmer Eye Institute, about lessons learned from these clinical trials.We begin by discussing the importance of Randomized Clinical Trials as the highest evidence-based medicine to compare treatments.Studies that we discuss include: the Fluorouracil Filtering Study Group; Tube vs Trab (TVT); Primary Tube vs Trab (PTVT); Ahmed Baervedlt Comparison (ABC); Ahmed vs Baerveldt. Studies that we discuss include: the Fluorouracil Filtering Study Group; Tube vs Trab (TVT); Primary Tube vs Trab (PTVT); Ahmed Baervedlt Comparison (ABC); Ahmed vs Baerveldt. Some of the concepts and lessons learned include: reducing IOP prior to surgery to lessen the risk of choroidal hemorrhage from a sudden IOP drop; trabeculectomy offering titratable control that MIGS procedures do not; that it's harder to get a 20% reduction in IOP to count as a success if the IOP is lower than 25 pre-op; that Ahmed Valves have a better safety record but Baerveldt better pressure-lowering; and that antimetabolites have been shown to not be effective for tube-shunt surgery.

**Dr. Constance Okeke received her undergraduate and medical degrees from Yale and trained in ophthalmology at the Wilmer Eye Institute of Johns Hopkins. She completed her glaucoma fellowship training as a Heed Research Fellow at the prestigious Bascom Palmer Eye Institute of University of Miami. She became a clinician scientist under a National Institute of Health K12 grant at the Scheie Eye Institute of University of Pennsylvania, where she also earned a Master of Science degree in clinical epidemiology. She has practiced ophthalmology with a specialty in glaucoma and cataract surgery since 2001. Dr. Okeke single-handedly pioneered micro-invasive glaucoma surgery (MIGS) at Virginia Eye Consultants in the southern Virginia Hampton Roads region. In 2009, she was the first in the area to perform Trabectome surgery and was the first in the country to combine this advanced technology in glaucoma with the femtosecond laser, allowing the surgery to be performed without a blade. **Opthamologist Dr. Leslie Jones-Dr. Jones is an associate professor in the College of Medicine and serves as the residency program director and director of Glaucoma Services in the department. Dr. Jones completed her medical degree in 1996 and ophthalmology residency in 2000 at Howard University and Howard University Hospital, followed by a fellowship in glaucoma at Wills Eye Hospital, the country's oldest eye hospital located in Philadelphia, Pennsylvania.    Dr. Jones is the recipient of the American Glaucoma Society Clinician Scientist Award 2005. Her research focuses on the genetic epidemiology of glaucoma, the evaluation of current and innovative glaucoma medical and surgical therapies, and use of on-line simulation as an adjunct to traditional ophthalmic clinical skills training.

Eye treatment he says is not the primary issue anymore; we can treat the eye. It's the rest of the body, particularly spread to the liver or lungs or other areas of the body that are the most concerning. A study found that ultraviolet C (UVC) germicidal lamps used by people to try to kill Covid-19 may damage the outer layer of the eye. Researchers from Florida reported at least seven cases of patients who have suffered eye injuries characterized by burning sensations and sensitivity to light after using UVC lamps.  According to Dr. Jesse Sengillo, an ophthalmologist from the Bascom Palmer Eye Institute, the cornea, the transparent part of the eye's surface, is susceptible to light emitted by UVC lamps. Ophthalmologist Dr. Deepinder Dhaliwal did not find the news of the eye injuries surprising since UVC light looks harmless, and people may not be aware that they should not look at the light directly. Sengillo likened the eye injury to a sunburn because it is painful and takes a few days to heal. The injury causes red, itchy eyes that are difficult to open because of light sensitivity. The burning sensation does not occur immediately, so some patients did not discover their injuries until hours after they had already damaged their eyes with the UVC lamps. According to the Food and Drug Administration, UVC light may kill the virus that causes Covid-19, but improper installation of UVC devices in rooms may cause skin and eye burns. Ophthalmologists advise people who use UVC lamps to leave the room once they turn the devices on and come back only when it is time to turn them off. Another option, they say, is to wear eye protection when using UVC lamps. For people who experience eye pain after using UVC devices, doctors advise immediate consultations to get prescriptions for ointments to alleviate the burning sensation and antibiotics to prevent infection.

In this episode, I get to hang with Angie Choudhury and her sister, Eileen Choudhury Bowden. Angie is a rising fourth-year medical student at the University of Texas-Rio Grande applying for residency in ophthalmology this fall, while Eileen is finishing up her fellowship in Glaucoma at the world-renowned Bascom Palmer Eye Institute at the University of Miami. These two share both a sisterly bond and love for ophthalmology, but also support each other for who they are as individuals. I had a blast laughing with them and getting to know them as people. Angie's persistence to seek opportunities in ophthalmology stem from her hunger to make it in the field without a home ophthalmology program, something we related with each other on. With the guidance of an older sibling who is a polished soon to be Glaucoma specialist, along with Angie's grit and determination, I have a feeling she will be making her move this year! Go get them, Angie, you have a friend in me rooting for you.

Dr. Anne Fung is the Global Development Lead for the Port Delivery System with Ranibizumab and Lucentis at Genentech. She is also a practicing retina specialist and researcher at Pacific Eye Associates and California Pacific Medical Center in San Francisco. Macular degeneration is a degenerative condition of a portion of the retina in the eye. This disease generally affects older adults. While there are some amazing medicines that can help people with macular degeneration, these medicines must be injected into the eye every 4-6 weeks. Anne is working on a tiny implant that serves as a reservoir for medicine so it can be slowly released over 6 months or more. They are currently investigating how long this implant can effectively treat the disease. Outside of science, Anne loves practicing yoga, as well as reading, listening to podcasts, and listening to audiobooks on a variety of topics including business, organizations, and psychology. Anne received her undergraduate degree from Wellesley College and her Medical Degree from Cornell University. She completed her residency in ophthalmology at Stanford University School of Medicine and then pursued a Medical Retina Fellowship at the Bascom Palmer Eye Institute and the University of Miami Miller School of Medicine. Anne worked in clinical practice for ten years before joining the team at Genentech in 2014. Anne is a Board Certified Ophthalmologist and is a Fellow of the American Academy of Ophthalmology. In our interview she shares more about her life and science.

In 2015, three patients who had a stem cell mixture injected in their eyes were severely injured. To try to prevent this from happening again, their treating physicians reached out to the FDA and clinical societies, and ultimately they wrote a paper for the New England Journal of Medicine to warn others about the dangers of these so-called treatments. In this podcast, one of the co-authors, Thomas Albini, MD, of the Bascom Palmer Eye Institute, talks about the cases and the surgeons’ decision to try to prevent other dangerous treatments.

In this episode, I talk with Tracy Wright, now based at Kaiser Permanente in Washington DC. He was at the Bascom Palmer Eye Institute at the University of Miami when we recorded this conversation. The implications immediately effected how I look at visual fields in my patients but I have yet to see further studies that support what we discussed, namely that visual fields continue to progress for a long time even once the stimulus that led to the initial glaucoma damage has been removed, likely due to apoptosis. The findings you are about to here may be shocking.
