Dean's Chat hosts,  Dr. Jeffrey Jensen  and Dr. Johanna Richey, are  joined by Dr. Wayne Bakotic and Dr. Warren Joseph on today's show discussing their careers and their specialties that overlap in dematopathology and infectious disease.   Discussions range from PCR testing for both fungus and bacteria, to their careers, their influence in the podiatric community, the advancement of AI in medicine, and relationship among specialists. A wonderful discussion, you'll wonder where 45 minutes went! Dr. Wayne L. Bakotic began his medical training with early acceptance into Barry University School of Podiatric Medicine where he completed his biomedical sciences degree through his podiatric medical studies. Prior to graduation, Dr. Bakotic left his podiatry training to initiate studies in Osteopathic medicine at Nova Southeastern University College of Osteopathic Medicine. It was, however, his time at Barry University which set the stage for his interest in podiatric pathology. After graduating with honors and completing his Doctor of Osteopathy in 1998, Dr. Bakotic continued his journey at Emory University School of Medicine where he gained training in anatomic and clinical pathology. He remained at Emory University to complete fellowship training in surgical pathology, Cytopathology and Dermatopathology, gaining board certification in anatomic pathology, clinical pathology, and Dermatopathology. In 2008 Dr. Bakotic co-founded Bako Diagnostics in Alpharetta, GA with the goal of serving the podiatric medical community and where he continues in the role of Chief Medical Officer.  Dr. Joseph a leader in our profession and a member of the Podiatric Hall of Fame. Dr. Joseph is a specialist in the treatment of lower extremity infectious diseases. He received his podiatric degree from the Wm. Scholl College of Podiatric Medicine in Chicago. Dr Joseph completed his postgraduate training in podiatric medicine/surgery at St. Joseph's Hospital, and a fellowship in infectious diseases at Hahnemann University School of Medicine, both in Philadelphia. He is currently an Adjunct Clinical Professor at the Arizona College of Podiatric Medicine at Midwestern University in Glendale, AZ. Dr. Joseph is a fellow of the Infectious Diseases Society of America and is an author of the IDSA Diabetic Foot Infection Guidelines. He is also certified by the American Board of Podiatric Medicine and currently serves as Editor of the Journal of the American Podiatric Medical Association. Dr Joseph lectures extensively and his presentations have focused on the treatment of bacterial and fungal infections of the lower extremity. The goal of this national observance is to help increase awareness among parents and educators on the prevalence of undiagnosed or misdiagnosed vision problems. Diabetes is the leading cause of new cases of blindness in adults. This is a growing problem as the number of people living with diabetes increases, so does the number of people with impaired vision. Diabetes can cause a disease of the eye called diabetic retinopathy (DR). In its early stages, you may not notice any symptoms or changes to your eyesight, and you cannot tell that this condition is damaging your eyes. If it is not detected and treated in a timely manner, your vision can be damaged permanently. Among adults aged 45 and over with diagnosed diabetes, 9.2% had vision loss due to cataracts, 4.1% had vision loss due to diabetic retinopathy, 2.2% had vision loss due to macular degeneration, and 2.1% had vision loss due to glaucoma. She has been training and mentoring students and residents for almost two decades. Be prepared for the unexpected. Today, Dr. Christine Bryson reminds us that it's okay for our priorities to evolve as time goes on—and to look at everything with an open mind. Although some people seem to have it all figured out from day one, it's okay for us to go with the flow, figure out what we want as we gain more experience and knowledge. The most important thing for us to keep in mind, though, is the search for finding our passion: We need to ask ourselves what makes us excited to get up in the morning and go to work each day. Pearls of Wisdom: 1. Leading a team involves humility on the part of the leader, and transparency with the team. Not everyone will agree with every decision that is made, but keeping the process transparent helps keep the team together. 2. Give space for the patient to tell you what is important to them, and leave your own preconceived notions at the door.  The best patient care comes with listening. 3. Be confident in your training, but know that this is a lifelong learning process. It's okay to know everything—because we are here to learn. Ask questions, admit what we don't know, and take on any opportunity for growth with an open mind. 4. Do not bring your life outside the hospital inside the patient's room. Pause to reflect on what is affecting us in our day to day lives, and know it's okay to take a day off, and talk to our peers about our experience. In order to give quality care to patients, we have to take care of ourselves.

Joshua Lenchus, DO, RPh, FACP, SFHMJoshua Lenchus holds a pharmacy degree from the University of Florida, and graduated from Nova Southeastern University College of Osteopathic Medicine in 2000. He currently works as a hospitalist at Jackson Memorial Hospital for the University of Miami Miller School of Medicine.In 2012, was elected as the youngest and the first osteopathic President of the Jackson Health System medical staff and chairman of its medical executive committee. He was subsequently re-elected in 2014 and 2016. In 2007 he developed a novel curriculum centered about simulation-based invasive bedside procedural instruction. Saving Lives of Addicts Through The Marchman Act and Mark AstorMARK G. ASTOR BIO Born and raised in the United Kingdom until the age of twenty-one, Mark Astor has been an attorney since 1994. Before entering private practice, Mr. Astor began his legal career as a prosecutor with the Palm Beach County State Attorney’s Office. Mr. Astor served the citizens of Palm Beach County as an Assistant State Attorney from 1994-1999. During his career as a prosecutor, he served as Chief of two different County Court Divisions and, thereafter, was promoted to a felony trial division. He has handled thousands of cases ranging from first-degree misdemeanors to Capital Murder. Mr. Astor was admitted to the Florida Bar in 1994 and, in 1995, was admitted to practice before the U. S. District Court for the Southern District of Florida. In 2005, he was admitted to the District of Columbia Bar. He received his Bachelor of Arts Degree from the University of Michigan in 1990, his Juris Doctorate from Nova Southeastern University College of Law in 1994, and his Master of Laws Degree (L.L.M.) from American University, Washington College of Law in 2005. Mr. Astor’s law practice, Drug and Alcohol Attorneys (http://drugandalcoholattorneys.com) focuses on representing and empowering individuals who are in crisis because of drug, alcohol and mental heath disorders. Many times, individuals or families come to Mr. Astor because they have a friend or loved one who either refuses to go into treatment, or will not stay in treatment. When this is the case, Mr. Astor gives counsel to help navigate through the Marchman Act process, Florida’s involuntary commitment law for drug, alcohol and co-occurring mental heath disorders. Additionally, Mr. Astor provides counsel when there is a need to file an Emergency Guardianship Petition (ETG) so that an individual who is no longer competent to make medical decisions for themselves can have that burden taken off their shoulders. On other occasions, individuals find themselves in the criminal justice system and need assistance navigating it’s complexities, having been arrested for an offense that stems from drug addiction or mental health disorders. When he’s not working in his law practice, Mr. Astor empowers members of the community by teaching Krav Maga (the Israeli system of hand to hand taught to the military, special forces and anti-terror units). Dr. Kimberly Sackheim is an Assistant Professor in the Department of Physical Medicine & Rehabilitation at New York University Langone Medical Center. As a post-traumatic headache specialist, she has a focus on interventional pain management of chronic headache. She is board certified by the American Board of Physical Medicine & Rehabilitation with sub-specialties in both pain management and brain injury medicine. Her medical degree is from the Nova Southeastern University College of Osteopathic Medicine. She completed her residency in physical medicine and rehabilitation at the Broward General Medical Center in Fort Lauderdale, FL.  Post-doctoral training took place at the Beth Israel Medical Center in New York City. Dr. Brent Schillinger is a board certified dermatologist who has been practicing in Palm Beach County, Florida since 1983. He received his MD degree at SUNY Downstate Medical Center in Brooklyn, New York.   Dr. Schillinger offers a full complement of medical, and surgical options for diseases of the skin, hair and nails.  He also handles an array of cosmetic concerns.  Dr. Schillinger emphasizes an approach he refers to as KEEP IT SIMPLE Dermatology.   In today’s high tech world often the basic simple approach will work best so our patients are presented all the options, but with an emphasis on keeping things smooth and easy and economical. Dr. Schillinger is past president of the Palm Beach County Medical Society, a founding member and past president of the Palm Beach County Medical Society Services Foundation.  He has served on the Board of Governors of the Florida Medical Association, where he chaired the Council of Ethical and Judicial Affairs. Dr. Schillinger is a clinical assistant professor at Nova Southeastern University College of Medicine and the Palm Beach Consortium for Graduate Medical Education at JFK North Hospital in West Palm Beach.  He represents the medical community serving on the Ethics Partnership Council of he Center For Applied Ethics at Palm Beach State College in Lake Worth where he is also involved in medical ethics education. Dr. Schillinger organizes and participates in various international volunteer medical outreach programs.  He has spent time working in many developing nations including Colombia, Peru, Cuba, Jamaica, the Dominican Republic, the Bahamas, and India.  He is the recipient of the 2013 Physician Hero in Medicine Award, and the 2016 Excellence in Medicine Award, both from the Palm Beach County Medical Society.

Janet Lynn Roseman, PhD, is an assistant professor in medical education at Nova Southeastern University College of Osteopathic Medicine in Fort Lauderdale, Florida, and director of the Physician Fellowship Program in Integrative Medicine. She specializes in spirituality and medicine, and created the Sidney Project in Spirituality and Medicine and Compassionate Care™, a unique model in medical education that reminds physician residents of the sacredness of their profession and the importance of creating caring environments for both patients and physicians. She was the David Larsen Fellow in Spirituality and Medicine at the Kluge Center for Scholars at the Library of Congress.She leads workshops for people with cancer and offers the “Cultivating Courage with Joan of Arc” training program for healthcare professionals who work with oncology patients. She lectures on the intersection of compassion and medicine, and is dedicated to changing the medical culture from what it is to what it can be. Dr. Roseman is also a Reiki master, dance therapist, and intuitive healer. Her column on healing with Joan of Arc appears in Sedona Journal of Emergence.
