Our exploration takes a detour to the US Institute of Surgical Research Burn Center in San Antonio, where Dr. Renz shares his valuable insights on complex burns and wound care management. The discussion reveals intriguing practices such as avoiding prophylaxis antibiotics for isolated burn injuries and the strategic use of negative pressure wound treatment. Learn more about the fine balance of initiating and adjusting fluid for patients, the crucial decision-making process involved in combat wound closure, and the pressing need to pass on knowledge and training in the advances of technology. We guarantee that this episode will leave you with a newfound respect for Military Medicine and those who dedicate their lives to its advancement. So, join us for an eye-opening journey into the world of military medicine with Dr. Renz! Chapters: (0:00:00) - Military Wound Care Evolution (0:13:59) - Wound Management in Damage Control (0:28:44) - Burn Center and Wound Care Management (0:46:13) - Decisions in Combat Wound Closure Chapter Summaries: (0:00:00) - Military Wound Care Evolution (14 Minutes) Retired Army Colonel Dr Evan Renz, a General/Trauma Surgeon with extensive experience in training and working at the US Institute of Surgical Research's Burn Unit in San Antonio, provides insights into the evaluation and treatment of complex wounds. He emphasizes the critical importance of documenting and learning from our past to help prepare us for future conflicts and to make sure that these lessons are included in military medical training opportunities. We discuss the changes in wound management from World War II to today, from the importance of hemorrhage control and debridement to advancements in medications and technologies improving wound care. (0:13:59) - Wound Management in Damage Control (15 Minutes) Dr. Evan Renz discusses the importance of wound management in damage control resuscitation. He shares his experience with his first use of a Wound Vac in 1997 and how the use of negative pressure wound therapy evolved over time. (0:28:44) - Burn Center and Wound Care Management (17 Minutes) Dr. Renz explains that isolated burn injuries do not require prophylactic antibiotics; however, traumatic combat wounds should be treated with an initial dose of antibiotics in addition to a Tetanus booster. Dr. Renz shares his experience with the use of the Wound Vac in the Burn Center and the key questions that he would ask when taking calls for burn management care. He also explains the importance of avoiding over-resuscitation and how to initiate and adjust fluid for the patient. Finally, he outlines the care that the patient would receive in regard to their wounds once they reach Role 4 or 5 facilities. (0:46:13) - Decisions in Combat Wound Closure (7 Minutes) Dr. Evan Renz shares his experience with the Emergency War Surgery manuals and the use of Wound Vacs in the field. We also discuss the importance of repeating a primary and secondary survey with each transfer of care and how to identify when a wound can or should be closed primarily or covered by a flap or graft. Finally, Dr. Renz speaks to the importance of passing on knowledge and training in the advances of technology so that it is not forgotten. Take Home Messages: Military medicine has evolved significantly from the World War II era to the present day, highlighting the importance of learning from the past to enhance future medical training. Advances in medications and wound management techniques, such as leaving more wounds open and exteriorizing the bowel for abdominal injuries, have marked significant milestones in military medicine. Negative pressure wound therapy has seen remarkable evolution over the years and plays a crucial role in wound management in damage control resuscitation. Surgeons need to make critical considerations when evaluating a wound downstream from a tourniquet in order to preserve the patient's hemodynamic stability. Burn centers play a crucial role in wound care management, with innovative practices like avoiding prophylaxis antibiotics for isolated burn injuries and the strategic use of wound vac. Adjusting and initiating fluid for patients and making critical decisions on combat wound closure are part of the complex process in wound care management. Knowledge and training in technological advances in wound care need to be passed on to future generations to ensure continuous improvement. Initial wound care at the point of injury prioritizes hemorrhage control, as highlighted by the TCCC guidelines. Wound management in damage control resuscitation involves the careful evaluation and treatment of wounds and extensive documentation of each treatment process to guide subsequent care. Wound care management, especially in a resource-limited setting, requires a practical approach, including bedside care, ensuring proper lighting and pain control. On December 5, 2005, Air Force Special Ops Paratrooper Israel Del Toro's life changed completely when, returning to his base, the Humvee he was in ran over an IED. The resulting explosion and fire burned Del Toro over 80% of his body. He survived, somehow, and was transported back to a U.S. hospital. Against enormous odds, his trials and tribulations to live were just beginning. For the next four months, Del Toro lay in a coma. On three separate occasions his devoted wife, Carmen, was called to his side as doctors feared he would not make it overnight. His story is not only one of courage and overcoming horrific obstacles from war, but also conquering myriad roadblocks throughout his life. His amazing story began inspiring other wounded troops, and soon thereafter Del Toro became an advocate for others in the Burn Unit and those badly injured. He continues to inspire today, speaking at numerous organizations and foundations, and numbers among his strongest friends President George W. Bush, Prince Harry, Gary Sinise, and Jon Stewart. Del Toro has gone on to set records and win medals at The Warrior Games and The Invictus Games. He'as the recipient of a Purple Heart and the Bronze Star. He won the Pat Tillman Courage Award at the ESPYs. Perhaps his most amazing accomplishment, though, occurred in February of 2010 when he became the first completely disabled airman ever to re-enlist in the Air Force and become an instructor. Today Del Toro is officially retired from the Air Force, but still encouraging troops and others dealing with major setbacks in their lives. For the first time, Del Toro tells the full story of his ordeal in his truly inspiring memoir, A PATRIOT'S PROMISE: Protecting My Brothers, Fighting for My Life, and Keeping My Word (St. Martin's Press, on sale July 4, 2023, $28.99). At age 12, he lost his father and became the man of the house. Having been told by his high school guidance counselor he should just concentrate on going to a local junior college, the determined teen ended up with a scholarship to the University of Illinois. But just as things started rounding into shape in his young life, his mother unexpectedly died. Now Del Toro had to fully embrace the promise he gave his father to take care of his younger siblings. He walked away from his college dream and returned home to care for his family. With his siblings growing up, and family members close by, Del Toro made the tough decision to enlist in the Air Force. His skills were a natural fit and his superior officers could see his natural leadership qualities. Though recently married, with a baby boy to care for as well, he was deployed to Afghanistan. His strength and perseverance made his fellow cadets gravitate towards him. And then.December 5, 2005. Still, after being told he'd spend 18 months in the hospital, Del Toro was so determined to return home that he was released only two months into his stay. He still needed daily care from his wife, Carmen, and his greatest fear remained - would his young three-year-old son be frightened of his physical appearance? When father and son met again after months of separation, the young boy ran into his father's arms. All he wanted was his Dad back. Perhaps his most amazing accomplishment, though, occurred in February of 2010 when he became the first completely disabled airman ever to re-enlist in the Air Force and become an instructor. Though he couldn't deploy again he was at least back in the game.

We had the chance to talk to guitar legend Steve Vai about his latest album. "Vai/Gash" and the story behind it is better told by the man himself: “This record was written and recorded in somewhat of a stream of consciousness in 1991 within perhaps a 2-week period as an answer to my desire to have a particular kind of music to listen to when I was riding my Harley with my friends. It's reminiscent of a certain type of rock music I enjoyed as a teenager in the 1970's. These recordings sat on the shelf for over 30 years and are being released now in 2023. Here's the story…Around 1990, I was overcome with a desire to rip out what I thought would be a straight-ahead type of rockrecord that contained the kind of music I wish I had to listen to when I was that teenager ensconced in the biker culture. And then I met Johnny “Gash” Sombrotto.”John was born in Queens, New York and as a youth was an avid motorcycle enthusiast. In 1977 at the age of 21, he endured aserious accident that caused his body to catch fire while plunging 30 feet onto a barbed wire fence. Vai wrote in the liner notes, “The doctors told his family that he had third degree burns over 60% of his body, and that if he even survived, there was a strong possibility that he would lose his right arm and left leg. While in the Burn Unit, he endured excruciating pain, especially when given the intense daily cycle of hot baths for his burns. He soldiered on, and after a month he was finallydischarged from the hospital. Luckily, no limbs were amputated. He was, however, left with a partial left ear and layers of skin grafts over his neck, arms, legs, and entire chest. The stories he told about his recovery and the excruciating pain involved were crushingly gut wrenching. But he survived, and eventually thrived again. He made his way out to Los Angeles in 1982and got right back up on his motorcycle.”Vai met John through a mutual friend, Mark Cimino. They bonded over biker culture, and he chose the name “Gash” as a nickname."He continues, “…something in me wanted to get him in the studio and see how he would belt over these biker type songs I had demoed, but nothing could have prepared me for the voice that came out of his mouth. I had to think, of course he sounds like that because that's him, confident, authentic, fearless but with a light-hearted intention. This was the voice I wanted to hear wailing over these slamming rock tracks. I was stunned. Around that time frame I was also starting to launch work on ‘Sex and Religion' and was hoping to eventually record more than the 8 songs I had laid down with John for an alternative project, but that was proven impossible. On September 7th, 1998, two days after my father had passed away, the phone rang, and it was John's beloved girlfriend Nancy. Her voice was quaking, and I somehow knew what she was going to tell me. ‘John was in a bike accident and was killed'.”After three decades, Steve Vai has finally shared this master with The World. In his opinion, “He would have absolutely been the greatest rock lead singer you would ever want to know. All the elements were in place, but he was gone. Disheartened, I put the entire project on the shelf, and would listen to it at least once a year for the past 30 years, especially around the anniversary of his passing. Then recently something compelled me to want to put it out now. I wish you would have had a chance to get to know John. I believe you would have loved him just as we all did. For now, he is ‘In the Wind'.”

While the Vancouver Canucks early season difficulties continue (they lost again while this episode was being recorded), the troubles the team face pale to those former Canuck Aaron Volpatti encountered on his journey to the NHL. Aaron joins hosts Anna & Chris to talk about the launch of his new book, FIGHTER: Defying The NHL Odds, and the story he tells about overcoming adversity and defining his own comeback story to simply play hockey. Using a unique visualization process, Aaron took back control of his life at a time he was told his journey was to go in a different direction, not only getting back to the game he loved yet demonstrating a story that took him to the NHL. And after speaking to Aaron, Anna & Chris take stock of what's happened (or hasn't) in the past week as the Canucks continue to lose - from some hot quotes to jerseys hitting the ice, to home-team players getting bood and a team potentially in disarray. FIGHTER: Defying The NHL Odds For 54 days following the book's release on October 25, 40% of the profits will go directly to the Burn Fund Centre here in Vancouver – a tribute to his Vancouver Canucks jersey number (#54) and 40% second and third-degree burns. This episode features Jonathan Bayuo (Presbyterian University College, Abetifi, Eastern, Ghana and The Hong Kong Polytechnic University, Kowloon, Hong Kong). The immediacy of death in the burn unit is usually in the order of hours or days requiring the timely application of palliative care. Although the culture of burns/ critical care units is evolving the support the integration of palliative care, several barriers such as prognostication and lack of guidelines exist. Clinicians working in burn/ critical care units may experience emotional exhaustion The findings suggest a complex decision-making process which may delay the initiation of comfort care. Collaboration with palliative care practitioners and training are required to support the integration of palliative care in the burn unit. Guidelines to support the delivery of palliative care may focus on communication, symptom management, and post-bereavement support for families and staff. A collaborative model of care is required to support palliative care integration in the burn unit. Palliative care for severely burned patients may need to commence alongside active burns management.
