Captain Joe Woyjeck, a Los Angeles firefighter, lost his son, Kevin, in the 2013 Yarnell Hill Fire. Kevin was one of the 19 members of the Granite Mountain Hotshots, a group of wildland firefighters within the Prescott, Arizona Fire Department killed in the Yarnell Hill Fire. Monte Fronk, another emergency responder, lost his daughter, Nada, to a domestic violence murder in St. Paul, Minnesota, in 2021. A new wildfire quickly spread through the Hollywood Hills early Thursday morning as Los Angeles and the surrounding region continues to fight the most destructive fires in the city's history. There are no signs of the fires stopping. Meteorologists are forecasting high winds and no rain through the weekend. Five people have been confirmed dead from the fires but LA County says the death toll is likely higher since the majority of burned neighborhoods have not yet been searched. The situation has emergency responders rushing in from all over the nation, including those from the Minnesota and Dakotas region of the American Red Cross. One year after benefitting from life-saving efforts by emergency responders here, Walsh is honoring police, firefighters and EMTs on National First Responders Day (Oct. 28). He also is planning to take the all-volunteer New Canaan Emergency Medical Services training (it's being offered in January, details here) to pay it back.
Some ambulance workers feel the Government's funding boost is too little, too late. The Government's increasing ambulance service funding by 6%. Hato Hone St John says the extra money has helped it settle pay disputes with unions, ending weeks of industrial action. First Union National Ambulance Coordinator Faye McCann told Mike Hosking that it hasn't really fixed any of the problems they're concerned about, but it has alleviated some of the financial burden people are facing. An ambulance union says the Government's made the right call in increasing funding by 6% for Hato Hone St John. It's putting an additional $21 million into the ambulance service, increasing the overall funding to nearly $357 million. Ambulance Association Secretary Mark Quin told Ryan Bridge that it hasn't resolved the issues of pay parity, but it's more than they were offered previously. Payne a veteran who served the Victorian community within Corrections, Government Investigations, and as a Volunteer Firefighter. During a career spanning nearly three decades, he worked on many incidents, including the state's worst ever fires since colonisation, known as Black Saturday, which started on February 7 2009. 173 people died in these devastating fires, more than 400 were injured and 450,000 hectares of land was burned.John detailed his experiences in a memoir 'What My Eyes Have Seen', to share his lived experiences with Posttraumatic Stress Disorder, a condition that he was diagnosed with a few years after Black Saturday.Talking about the beginnings of what he now knows was PTSD and his body's signs (including several migraines within one week whereas he'd only experienced these a handful of times) John said: “In those early stages I was more concerned…with the welfare of everybody else and I wasn't listening to myself, to my own body's reactions…”.John explains his PTSD developed over several years and caused panic attacks, impacts on his short term memory, his moods, ability to make decisions and he lost interest in things he used to enjoy.“I'd gone from being the life of the party to the person who sits in the corner, if you can get me to go. My wife would ask me what I wanted for tea and it was too hard a question to answer…everything just eventually became too difficult.”It was a conversation with his daughter, who said, “Dad, there's something wrong” that propelled John to reach out for help. John says she was the first person who had what he now calls a “courageous conversation” with him about the changes in his personality.John's journey with PTSD has seen him become a passionate advocate for mental health and suicide awareness to encourage people to get help and know the warning signs of this condition earlier. More than just complaining when he saw areas of frustration in the emergency services field, Darren Dake did something about it. His dream is to educate and encourage those who serve and protect, those who help and heal. Darren is a dream-tender, a tend-talker, and a dream-doer, and he lives by two principles. Find out where it’s raining and lay down a tarp. Take all of your passions and giftings, and develop them into something that helps others—this often reveals a motivating business or other life-long endeavor. As a long term dream-tender, Darren understands that what you set your mind to will determine what you choose, which in turn, will alter your reality. He learned a long time ago to be careful about what he sets his sights on. As he shares what his eighteen-year-old self dreamed about and tried, we see how his quest ultimately led him to discover his true interests. He didn’t allow disappointment to take root, but instead, learned lessons from setbacks and failures, and allowed his talents to guide him to his true calling. Darren works with departments and agencies of all sizes, training and speaking often to large groups and conferences. But he has a special heart for the over 21,000 police departments with only ten officers or less, EMT’s and paramedics with tiny budgets, the volunteer firefighters, 911 operators in cramped quarters, and the coroner’s office operating with only one or two people. He helps leaders and their teams, no matter how large or small, determine what their strengths are, and educates them on how to further develop them to do a more effective job. As Darren Dake well knows, none of us gets to do what we like or are the best at, all of the time, but sacrifices are necessary, in order to tend your dreams. We can learn disciplines and methods, from doing what challenges us, that propel us even further toward where we ultimately want to go. Other people can also help us move faster and more efficiently. We all need influencers, encouragers, and inspirers to guide us toward our dreams. There will always be people who are two steps ahead of you, who can answer questions, show you the way, and lead you even further than you could go alone. But conversely, there will always be people two steps behind, who you can teach, reach, and show. Pouring into others makes your dreams worth tending. Filling Needs: Are you so narrowed in on how you think your dream should look, that you’re missing deeper layers or inflections or details you are drawn to, that might allow you to fill a niche? Who have you sought out who can teach you more about tending your dreams? Who are you mentoring and supporting? Remember the two-step rule: there are people ahead of you who can help, and people coming behind that you can encourage. This episode’s Dream Planting Tips: Look for a lack and listen for complaints—what can YOU do something about? When you feel frustrated by what others overlook, you may be the one called to enact a change or give voice to the issue. Who are the people you can relate to naturally, without much effort? This may be evidence pointing to your dream-tending destiny. What can you help empower and heal? Is this kind of difference a dream you can pursue? Be open to a dream off-shoot or branch, as you follow your passion. Life can surprise you and reveal opportunities you never imagined, so don’t narrowly define your own options. Don’t lose yourself as you navigate expectations, emotions, and necessary demands in order to tend your dreams. Unify your efforts with others who share your passions. Engage with people who feel strongly about similar or same dreams. Monitor your motives daily: Am I considering other people’s needs and how they feel? Are my drives and ambitions other’s focused, or are they solely about me? Remember, if you don’t chase your dream, no one’s going to give it to you! Narcan and Synthetic Opioids: vive la résistance? Probably not. Read this absolutely brilliant piece from The Tox & The Hound here. (They did all the hard work and we stole their sources.) Opioid "resistance" to naloxone is most likely not a thing, per se. The reported effect from synthetic and novel opioids are unlikely to be due to the agent's binding affinity for receptors within the brain, but rather from an ability to rapidly permeate the blood brain barrier much faster than "traditional" opioids such as heroin. Even in cases where a synthetic opioid agent was identified, the vast majority of cases did not need more than 4mg of naloxone to achieve reversal. Synthetic opioids don't bind any more "tightly" to receptors than naloxone. Synthetic opioids will usually cross the blood brain barrier faster than traditionally encountered agents. Most available evidence shows that synthetic opioid toxicity does not require significantly more naloxone to achieve clinically significant effect. Ergo, the traditional serial naloxone dosing algorithm does not need much modification. 0.04mg -> 0.4mg -> 2mg -> 4mg -> 8mg -> 10mg Not all that "overdoses" is an opioid. Consider all other causes of altered mental status or coma. Namely: hypothermia, hypoxia, and hypercarbia. Acidosis may potentiate the effect of opioids, highlighting the demand for timely and effective ventilation. Polypharmacy or adulteration is increasingly common. Consider intoxication by additional agents. Anchoring bias is a dangerous phenomenon: don't get burned! The Nose Knows. Or does it? Intranasal (IN) naloxone is popular among many EMS agencies as well as law enforcement, fire departments, and bystanders. IN naloxone has been shown to be effective in several randomized controlled trials for successful reversal of opioid intoxication. However... There are important pitfalls to be cognizant of when choosing this option for delivering naloxone. Intranasal naloxone has poor bioavailability when compared to IV or IM dosing, so higher doses may be required to achieve clinical effect. This is further potentiated by the maximum volume able to be absorbed by the nasal mucosa (around 0.5mL). Patients administered intranasal naloxone may have a variable or delayed response in achieving reversal. Protect Ya Neck Standard isolation precautions are adequate protection against the overwhelming majority of overdose scenes. In the rare instance where respiratory or splash exposure is a concern, a properly fitted N95 mask and goggles will be sufficient. To date, there has yet to be a laboratory confirmed case where a first responder or emergency healthcare provider has suffered a clinically significant opioid intoxication (bradypnea, hypoxia ) as the result of an occupational exposure to fentanyl or its analogues. Tonight's special guest isDr Anna Baranowsky from Toronto, Executive Director of the Traumatology Institute (Canada) and co-author of the new book "What is PTSD?: 3 Steps for Healing Trauma." Anna will be joined by co-author Teresa Lauer of Redmond, Washington. In addition to the book we'll explore their unique on-line training courses, offered through the Online Traumatology Institute's Clinical Traumatologist training program, where mental health professionals get comprehensive trauma therapy training in an on-line platform (anytime ... anyplace). Designed to help both those who work with trauma survivors and anyone who wants to learn about PTSD, courses are offered over SKYPE. Students quickly and easily develop skills as a trauma responder and counselor. They offer on-line training for: 1) Mental health professionals and counselors with a trauma/PTSD specialization to be completed at your own time and place, 2) Emergency Responders looking for skills in post-trauma care, 3) Teachers or School Administrators finding approaches to handle the aftermath of trauma, 4) Justice System Workers who want to use Trauma-Informed interventions with incarcerated populations, 5) Anyone who encounters trauma survivors in the course of their work or daily lives, and 6) Trauma Survivors, their family, friends, colleagues, co-workers and other connections.
