She reached 210 pounds at 16 years old, but then switched to a plant-based diet at 17 and began to lose weight and take her health into her own hands. She eventually found strength training and began to learn more about food. And she only went up from there! Many people are told to lose weight to alleviate their PCOS symptoms. But what if you don't have any extra weight to lose? About 20–30% of people with PCOS are living with Lean PCOS, meaning their BMI does not qualify them as overweight or obese. They don't gain extra weight because of PCOS, so they have to address their issues and symptoms differently than the general PCOS population. Often following recommendations that are meant for the more common form of PCOS worsen symptoms for those with lean PCOS.    Meet Martha McKittrick. Martha is a Registered Dietitian and Certified Diabetes Educator with over 25 years of experience. She has been specializing in PCOS for over 15 years. I'm joined by Dr. Felice Gersh, a leading OB-GYN treating patients with PCOS, to talk about the signs, symptoms, and long-term effects of the syndrome. Dr. Felice Gersh is a leading expert on Polycystic Ovarian Syndrome (PCOS). She explains what PCOS is and how common it is among women, affecting between 6% - 12% of Americans. A woman must have two of three specific symptoms of PCOS in order to have a diagnosis. However, there are so many other symptoms related to the syndrome, including excess facial hair, weight gain, and acne outside of puberty. Felice says that one of the leading causes of the rise in PCOS is our reliance on plastics. Plastic, even BPA-free plastic, is toxic. It's an endocrine disrupter that impacts women of all ages. There is no safe amount of plastic at any point and the risks are higher during pregnancy.   Society, Women, and PCOS Felice explains that PCOS existed in women throughout history. The excess testosterone had biological benefits, allowing for extra strength and stamina when needed. As society and culture changed, however, and the introduction of chemicals, this excess testosterone leads to health complications. Society's reliance on pollutants and toxins, from plastic bottles to chemical cleaning products, has a negative effect on women with PCOS. Felice shares why this is and how you can start reducing your exposure to these pollutants. PCOS increases your risk of a number of long-term health issues, such as diabetes, and Felice recommends seeking out treatment. She does not recommend the birth control pill for women with PCOS as it disrupts hormones more than they already are, simply masking a bigger problem. Do you have irregular periods? There's nothing out of plastic that you can put any label on that is going to make it safe.” [11:55] “We now know, unfortunately, that BPA in pregnancy concentrates in the fetus. The amount that's circulating in the maternal blood is significantly less than what's actually in the fetus. It gave people the wrong idea. There's really no safe level of poison. It's just safer. Less is better than more. Not to mention the interactions with all the other endocrine disrupters in the body.” [12:17] “Estrogen is the glue that glues metabolic functions of the body to reproductive functions so it all works together to create one whole, healthy female. So she can safely reproduce and stay healthy.” [16:46] “Women with PCOS, unlike women with menopause, do have estrogen, they just don't have enough. Whereas with menopause, ultimately their ovaries make no estrogen, women with PCOS just don't make enough. But we have poor women with PCOS because they're now producing all this testosterone. Now, why would they make all this testosterone? It's because it's part of the assembly line to make estradiol, the estrogen from the ovaries. If the brain is saying, ‘I don't have enough estrogen,' it puts out the signal, called luteinizing hormone, via the pituitary gland, which triggers the ovary to make the precursor to estradiol: testosterone. Now, you're making all this luteinizing hormone, it goes high, and now you're making all this testosterone, but you have this block so you can't go down the assembly line smoothly to turn it into estradiol. So the brain keeps saying ‘more estrogen' which makes more LH, which makes more testosterone. It's a comedy but without the fun part. You have this assembly line and you can't get from this step to this step properly.” [19:08] “There are innate benefits of being PCOS. If we can take our bodies back to their origins, back to their original healthy state, without all these chemicals or toxic food, we can recreate the health and vibrant life of a woman with PCOS without all the negative stuff.” [25:03] “It doesn't make sense to give birth control pills to women with PCOS. Originally from Fargo, North Dakota, Dr. Barnard received his M.D. degree at the George Washington University School of Medicine and completed his residency at the same institution. Dr. Barnard has led numerous research studies investigating the effects of diet on diabetes, body weight, and chronic pain, including a groundbreaking study of dietary interventions in type 2 diabetes, funded by the National Institutes of Health, that paved the way for viewing type 2 diabetes as a potentially reversible condition for many patients. Dr. Barnard has authored more than 90 scientific publications and 20 books for medical and lay readers and is the editor in chief of the Nutrition Guide for Clinicians, a textbook made available to all U.S. medical students. As president of the Physicians Committee, Dr. Barnard leads programs advocating for preventive medicine, good nutrition, and higher ethical standards in research. His research contributed to the acceptance of plant-based diets in the Dietary Guidelines for Americans. In 2016, he founded the Barnard Medical Center in Washington, DC, as a model for making nutrition a routine part of all medical care.     Working with the Medical Society of the District of Columbia and the American Medical Association, Dr. Barnard has authored key resolutions, now part of AMA policy, calling for a new focus on prevention and nutrition in federal policies and in medical practice.  You can consider this episode your 101 course in hormonal health. Carol Lourie is a Naturopath, Licensed Acupuncturist, Functional Medicine expert and Homeopath, with over 30 years of experience in providing integrative health care. Carol joins me today to talk about women’s health and polycystic ovarian syndrome or PCOS.In this episode of the Clean Eating for Women podcast, we talk about:   What PCOS is and Carol’s full spectrum approach to treatment. How PCOS symptoms are different for everyone and it is difficult to get a diagnosis. Increased environmental toxins can increase PCOS symptoms. Some highlights of this episode on how to make veggies taste delicious: I am a survivor of PCOS. This is not a condition that is easily resolved or easily tolerated. It impacted my life for two decades, and I've only had it under control for three years. [01:11] Carol always wanted to be a medical doctor. In the 70s, She lived in New York in a spiritual community. She knew a chiropractor who did detox diets. Carol ended up working in his office and saw all the people that he has helped. Carol ran across a flyer for a College of Naturopathic Medicine, and she decided to go, and she graduated in the 80s. [01:48] The more Carol has practiced, the more she has noticed the connection between the mind and the body. She has also noticed that all chronic disease has a foundation of a history of trauma. There are multiple factors and multiple symptoms in PCOS. Everyone's symptoms are a little different. This can be confusing for medical doctors, and it makes getting a diagnosis difficult. [03:38] Carol has been treating PCOS for the last 25 years. She has seen symptoms become more severe and more hormonal disruption as toxins in our environment increase. [07:36] People with PCOS have elevated sugar levels and increased cholesterol and cysts on their ovaries. Carol includes an underlying inflammatory process and the emotional component that she feels is very important too. As well as a hormonal imbalance. [08:24] Metabolic syndrome or blood sugar disproportion is one of the spokes. Having extra sugar swimming around in your bloodstream has a direct impact on the formation of cysts in your ovaries. Targeted nutritional changes and supplements can make a big difference in blood sugar. Carol begins by getting a proper breakfast in place like a healing protein smoothie with milk thistle, probiotics, and vitamin C. [11:47] The basics of Carol's approach is to work nutritionally to lay the foundation for cells and decrease inflammation. Stabilize your blood sugar and support your liver and your triglycerides will go down. Your body likes to be healthy and will respond in a positive way if you give it what it needs. [17:23] The Hypothalamic-Pituitary-Adrenal Axis. The hypothalamus and pituitary are connected. There is a clear system of hormonal feedback loops. When one hormone is released it goes to the thyroid gland and then the thyroid gland releases thyroid hormone. When we are stressed, the body releases cortisol and this clogs up the messaging system. Everything we do relates to our health. [23:30] Environmental chemicals that get in our body can be estrogen disruptors. This can also lead to excessive estrogen and other hormonal imbalances.
