Allison is an intuitive medium and owner of Souls of a Feather where she offers readings online. In this episode, she shares her experience with both prenatal and postpartum mood disorders. Allison suffered from PCOS (Polycystic Ovarian Syndrome), and tried for two years to get pregnant. She finally decided to take a break from trying to focus on her health. She asked her doctor for birth control and in the time it took to fill the prescription, she got pregnant. Allison suffered from severe hyperemesis gravidarum which landed her in the hospital for a week. She had gestational diabetes which turned into hypoglycemia. Instead of being excited about her pregnancy, she felt anxious and paranoid. Postpartum only seemed to be worse for Allison. After a traumatic birth, she felt anxious and couldn't sleep. She was in the hospital multiple times for severely high blood pressure, and it was brushed off as anxiety. She still couldn't sleep. She was shuffled back and forth between her OB and her primary care physician, even though she was asking for help. Allison finally ended up in the hospital and spent several weeks recovering at her mother's house, away from her baby. Allison's story is a reminder of just how important it is to support mothers while pregnant and after giving birth. She says there aren't enough resources or education on mental health issues. Episode 150: Peeling Back The Layers of PCOS Polycystic Ovarian Syndrome affects 6-12% of women of childbearing age. According to the revised criteria, PCOS is identified via amenorrhea, polycystic ovaries on an ultrasound, or a combination of hirsutism and hyperandrogenism. Women with PCOS often see elevated LH to FSH, high free Testosterone, low SHBG, high fasted insulin or cortisol dysregulation on serum lab work. Because PCOS is a mix of so many different systemic dysfunctions, it can be hard to treat. Medical solutions include Metformin, birth control, spironolactone, and finasteride. “I ultimately feel like doctors should be judged by how we get people off medications,” explains Jennifer Roelands, MD. Jennifer is the CEO and Founder of Well Woman MD. As an OBGYN with integrative medicine training and over a decade of women's health experience, Jennifer is a catalyst for change within the healthcare community. She believes that women's health requires a team based, integrative approach in order to really help the patient to the fullest extent and she is working to provide this model in her clinic. PCOS is the most common form of infertility in women. What maybe at the root cause of PCOS? And can there be anything that you could do about?
A woman's weight and BMI during and after pregnancy is often discussed and debated. This is especially true in the case of high-weight women, who, as research shows, have a higher risk of complications like cesarean birth. Today's episode is to discuss high weight women in pregnancy and when we need to concerned and when we don't PLUS, we also discuss some very questionable myths and treatments followed for high-weight women that you need to be aware of. But these in no way imply that people with very low body weight have uncomplicated pregnancies. To prove that, we compare pregnancy risks, body weight gain, and recovery in low-weight, normal weight and high-weight pregnant women and see clearly why a lower than ideal body weight could also pose significant birth complications. PS. Excessive weight is sometimes associated with ovulation issues. That, in turn, could be caused due to PCOS (PolyCystic Ovarian Syndrome), which is associated with a hormonal imbalance that messes up your ability to metabolize. Here are a few known complications faced by high-weight women during pregnancy: >> Preeclampsia -- Condition associated with high blood pressure and other complications, including retaining fluid and end-organ dysfunction. In extreme cases, it can lead to eclampsia or seizures, a severe form of preeclampsia. >> Diabetes (pregestational and gestational) -- Diabetes that's detected during the first trimester or the beginning of the second trimester is called pregestational diabetes. These can lead to abnormalities of development and more chances of miscarriages. Gestational diabetes is a form of diabetes that's developed in women during pregnancy. Women who have this condition wouldn't have had diabetes previously. >> Preterm delivery -- A condition that might cause pregnant women to go into labor early. >> Macrosomia -- High weight of the baby >> Higher weight loss -- High-weight women tend to lose more weight during pregnancy, which is unhealthy. It's healthy to end a pregnancy with a greater weight than what they started with. >> High risk of C section -- High-weight women are at a 70 % risk of a C-section -- even after complications like hypertension and diabetes have been neutralized. >> Less sensitivity to oxytocin -- Hence an increased need for oxytocin augmentation. >> The need to use IPCs (Intrauterine Pressure Catheter) -- a device placed inside the pregnant woman's uterus to monitor uterine contractions during labor. >> Pelvic outlet is narrower -- This is alleviated by making positional changes. >> Uterine tilt if a woman previously had high weight and lost it abruptly -- This is alleviated by making positional changes. >> Remodelling of bones -- As the bones become more narrow, changes are caused in getting the baby. >> Shoulder dystocia and postpartum hemorrhage -- Massage from postpartum hemorrhage is also harder. >> Difficult labor -- Caused due to positional issues of internal organs and the baby A common pregnancy complication seen (especially) in low-weight women: >> Hyphemeresis -- A condition that causes severe metabolic disturbances, including abnormal amounts of nausea and vomiting. In this podcast, I want to cover what PCOS is and what can cause PCOS. Symptoms of PCOS: • Infrequent or prolonged menstrual cycle • Excess male hormones (androgens) • Infertility • Excess hair growth on the body • Thinning hair on the head • Acne • Insulin resistance Most websites will tell you that the potential causes of PCOS are genetic or maybe environmental. But, I think the missing piece that you need to know is that high androgens can be caused by high insulin. And high androgens may cause many of the symptoms of PCOS. Unfortunately, insulin levels are rarely checked. You can't really determine what's going on with insulin by checking your blood sugar levels. You could actually have normal blood sugars and still have high levels of insulin. If you're not sure what causes high insulin, I have a lot of other videos on that topic. The Case: Laura was diagnosed with PCOS (Polycystic Ovarian Syndrome) in her 20s She’s experiencing acne, weight gain, hair loss, and irregular cycles. She took birth control as recommended by her doctor and her symptoms subsided, but within a month of getting off of it, her acne, weight, sleep issues returned Laura’s period didn’t return after 3 months later of getting off the pill. Frustrated by not getting anywhere with her primary doctor, Laura was ready to just start taking the pill again to make the symptoms go away, but she knew this would only be a band-aid solution, and that’s when she decided to see if I could help. The Investigation Difficult to manage menstrual cycles is an issue I often see, and there are a number of underlying factors that could be at play. With Laura, we did more bloodwork and saw that her DHEA sulfate was elevated and her testosterone was also on the higher side. I wanted to make sure this did not become just another health mystery. Dr. Erin Kinney is a Naturopathic Doctor. In her private practice as well as her workshops and retreats, she helps women rebalance their bodies so they can achieve optimal health and become their best selves. So, I knew she was the perfect person to share Laura’s case with. Getting Your Period Back When the Menstrual Cycle Stops In a case like Laura’s, the first priority is to have her cycles return because bleeding on a monthly basis is very important so that the lining is able to build up and then properly shed. Conventionally, using 2 weeks of progesterone may do the trick but it does not get to the root of the issue. To help things work from the inside out, it is important to check estrogen and progesterone levels. If progesterone is not rising, its possible there may be too much estrogen and if so, speeding up the estrogen detox is very helpful to bring the body into balance. If estrogen levels are fairly normal but progesterone is low, then Dr. Kinney does not typically recommend too much liver detoxing so that we don’t push out too much of the needed progesterone and instead use things to help naturally boost progesterone. 2 Lab Markers to Explore When Dealing With Irregular Periods Follicle Stimulating Hormone (FSH): FSH helps control the menstrual cycle and stimulates the growth of eggs in the ovaries. Ideal levels are 5-65 Luteal Stimulating Hormone (LH): Luteinising hormone is produced by the pituitary gland and is one of the main hormones that control the reproductive system. Ideal levels are under 12. Hormones, PCOS, and Stress Hormones are produced on a daily basis and without a way out, they can build up in the body. When we are under stress, it is harder for the body to detox and so high stress levels (which often translates to high cortisol levels) will impact detox and in turn create more hormone imbalances that can lead to PCOS. Natural Ways to Support Your Liver with PCOS There are a number of natural and healthy ways to help support the liver with its natural detoxification process. For those with PCOS, it’s critical to consider the role the liver plays in wellbeing and recovery. Diindolylmethane (DIM): used for improving estrogen levels, DIM is a compound derived from the digestion of indole-3-carbinol, found in cruciferous vegetables. Milk Thistle: one of the best liver cleansing herbs. Since excess estrogen gets filtered out of the body through the liver, this is very important. Cruciferous Vegetables: eat things like broccoli, brussel sprouts, cabbage, and kale to assist in the liver’s natural detoxification process. N-acetylcysteine (NAC): Helps with Phase 2 liver detox and glutathione production. Inositol: May help with controlling blood sugar by improving insulin sensitivity, and may also improve fertility in PCOS cases. What About Elevated DHEA? When DHEA (Dehydroepiandrosterone, also known as androstenolone, an endogenous steroid hormone precursor) is high, cortisol may often be off as well. There is a relationship between high DHEA levels and other hormones like testosterone and it is very helpful to test the full panel to get a clear picture. Listen to Your Body’s Natural Energy Cycles Overall, it is important to pay attention to what Dr. Kinney calls the ‘body benchmarks’. Even though numbers are useful, what’s going on in the body and the physical symptoms are much more important. Breathing, yoga, meditation, and doing less is the best way to reduce stress and lower cortisol. One other way to lower cortisol is to build in rest on Days 18-25 when your body is asking for extra rest. Doing this will help your adrenals build more cortisol in the second half of your cycle. In Laura’s case, she was eating well. But, if blood sugar is a concern then you may want to look into a Comprehensive Metabolic Panel, checking Hemoglobin A1C (average fasting blood sugar), or Fasting Insulin levels. Keep in mind that diet changes are best done gradually. If you are stressed out by a new diet or if it feels too drastic, that in itself can raise cortisol so starting slow and going at your own pace is very helpful. Mystery Solved It was clear in Laura’s case, that there were many factors causing her issues. Stress isn’t only emotional but can be physical too. In this case we looked at food sensitivities and she had issues with dairy, corn, and oranges and eating those foods was creating a physical stress on her body. We took out those foods and worked on balancing her blood sugar (which is important, due to the insulin/PCOS connection). We also added PS 150 for Cortisol, and mind-body exercises to help manage stress even further. After that, our focus went to the liver to make sure old hormones were getting processed. To support the liver, I added Livton Complex (which helps with Phase 1 & 2 liver conjugation), and Calcium D -Glucarate to help lower the beta-glucuronidase enzyme because when this enzyme is high, it slows excretion of hormones and other carcinogens. We then added OmegAvail Ultra, a concentrated fish oil and chaste tree supplement which helps to balance progesterone. Happy Ending The treatment outlined above may look like a lot of work, but truly it only took 3 months for Laura to get her period again! Her skin cleared up, and she was finally losing weight. At this point, I knew we could stop the liver support. We kept going with fish oil and chaste tree to maintain everything. Laura was consistent, so when we retested her labs, we were pleased to see that her DHEA-S and testosterone levels are now completely normal. Another mystery solved! Eliminating Health Mysteries For Laura, we were able to find that missing piece of the health puzzle and help her regain her health. Could this be the missing clue for you or someone in your life? Good day Anele and the Team I write this mail after hearing you spread joy on 947 and helping someone out that just needs some happiness. And the only person I could think of was my wife, Tammy. This year has been, to say the least, terrible on everyone. Especially my wife. December 2019 - My wife experienced a miscarriage, we never knew how hard that was going to hit us, but it did... We were so excited to tell our daughter (3) that she was going to be a big sister but never had the chance. It was early stages and we didn't even get the chance to find out if we were having a boy or a girl. Then early 2020 - My wife and I separated (a "TIME-OUT" so to speak, and tried to reset our lives) before "lockdown" became a catchphrase. Once lockdown hit, we decided that I would come back so that it wasn't too difficult for our child. During this time, we hashed things out and regained our love for one another and I am happy to say - stronger than EVER before and very much in love.X But that is where the happiness ended... On 1st of August 2020 after a braai with friends, her mom (a paraplegic), and a saint amongst humanity, became non-responsive and suddenly succumbed to what we believe was a heart attack. And having to share the news with our daughter was even more heart wrenching - Granny's little angel... Suffice to say my wife still has not been able to deal with her passing as she is also a self-employed super mom. Then, late October 2020 - My wife was diagnosed with early signs of PCOS (Polycystic Ovarian Syndrome) - taking into account all that has happened the last 11 months and she did not have her mom around anymore for advice, it didn't go down too well. Yet we are taking each day as it comes and are thankful for what we have. With all of this going on, she has never taken a break from work, sacrificing family time with our daughter and friends and just downright hustling she even started another company! She blows me away!... She keeps herself so busy that she doesn't have time to actually deal with life. Now with December coming, it's our first Christmas without her mom - funds ridiculously low, I don't know what to expect... I don't even know where to begin. What I do know though, is that my wife needs a holiday or a girls weekend, ANYTHING really that can help her release emotion - de-stress, debrief herself and come back to grips with life and look to the future with new purpose... So that she can be the best version of her I know that she can be! Anything that can bring her some happiness after this atrocious year... About a year ago, after experiencing some increasingly bothersome hormonal symptoms, I received a diagnosis of PCOS - Polycystic Ovarian Syndrome. My doctor advised me that if I wanted to improve the condition on my own without birth control or other medications, I might want to consider losing weight.That statement is loaded for anyone, but especially for people recovering from disordered eating, as I have been for 20 years.I felt into her recommendation, and was surprised by what I discovered. As young as age 8-9, she realized that she wasn’t the same as her friends. She was “bigger.” She was pressured to do things to get “healthy” (aka: lose weight), but Nicola accepted her size, and came to her “fat identity” at this age even with external pressure to see the number on the scale get smaller. At age 16 she was diagnosed with PCOS (Polycystic Ovarian Syndrome), and told she would never be able to have children. Trauma and How It Led to Becoming A Fat-Positive Fertility Coach When Nicola was just out of university, she was living in London. She was soundly asleep when a man was shot on her doorstep, and bullets flew just inches above her window. This led to PTSD, and, in her search for healing, she discovered acupuncture. The acupuncture was a game changer, helping to relieve her PTSD, and spurring her interest to help others with this tool. Her natural interest in PCOS and hormones led her to become a trained fertility coach. Embracing Her Own Fat Identity and Helping Other Bigger Bodied Women Get Pregnant Even though Nicola did not struggle with becoming pregnant, it was after delivering her own child that she not only embraced her fat identity, but she wanted to help other bigger bodied women to achieve their dream of parenthood as well. She focuses on finding supportive healthcare professionals, mental and physical aspects of how being fat affects the goal of becoming pregnant, and creating a positive mindset to set your body up for success. Laurence gives us insight her experience with PCOS, why she got into this field and how she helps women with PCOS now. PCOS (Polycystic Ovarian Syndrome) sounds like it's an issue with the ovaries only but really it's a metabolic disorder that affects 1/5 (possibly more) women of reproductive age. Many women with PCOS may not even know they have it, especially since it is a disorder of exclusion and there is a spectrum you can fall along. Not to mention there are many symptoms associated with PCOS, but not every women has every symptom. Some examples are: cystic acne, irregular periods, weight gain (especially around the belly), sugar cravings, abnormal hair growth, fertility issues, hair thinning on the head, mood swings, fatigue and more. The human body has a set of intrinsic signals that send us messages about the current state of our bodies. This is particularly true when it comes to our hormonal levels and other hormone-related concerns. In this episode, we are joined by Dr Sharzad Green, a practising pharmacist who specializes in menopause. For over 20 years, she has been providing private hormone consultations for patients concerned with various reproductive health problems like hormonal imbalance and perimenopause, menopause, andropause, and PCOS (Polycystic Ovarian Syndrome). (20:22) There is no easy answer to the weight-gain or weight-loss difficulties that you face during menopause. (23:20) It's ok to order supplements from Amazon since it's an easily accessible avenue, but make sure to enter transactions only with reputable supplement companies. (27:20) Adaptogens are herbs that help your body cope with stress. (28:40) Referring patients to Fullscript, an Amazon-like website for supplements, helps them acquire supplements from secure sources without the added hassle of scouring through thousands of available choices. (29:24) Glandular supplements are supplements made out of dried and powdered adrenal glands of pigs or cows. Dr Green doesn't recommend this type because of autoimmune disorder risks. (34:10) Despite what vegans say, meat is still an essential aspect of human nutrition. (37:39) Lifestyle management to avoid hormone supplements: ingesting the lowest dose or doing it for the shortest period brings a quality of life to women. (40:31) In the age of increasing research results with opposing results, it's crucial to remain analytical of what you read and what you consume. (44:10) Is there a difference between the bio-identical and pharmaceutically-manufactured hormones? I’m that big-hearted empath that wears my heart on my sleeve. My Love language is any one of the five on a given day but my biggest is quality time. As a Muslim, Charity is one of the five pillars of our religion of Islam. I talk more about that in a previous episode (Episode 05 Islam A Way of Life). Charity becomes more prominent during the holy month of Ramadan. It is more so common for women, especially if they are on their menstrual cycle or pregnant because if you are unable to fast or sustain from eating and drinking from sun up to sun down, then you should be charitable. That can be monetary, or a matter of giving your time and serving or making food for those who are fasting or less fortunate. Also because of my history of PCOS Polycystic Ovarian Syndrome which I talked about in a previous episode (Episode 02 PCOS Polycystic Ovarian Syndrome), I end up doing more charity than fasting. It’s like taking a breath for me. My first real paid job was as a summer camp counselor to deaf-blind in middle school. No I was not 16 yet, but it was a part of vocational and orientation/life skills. They paid us bi-weekly. It was such an exciting job because I was able to be of service. I was able to be a listener. I wanted to be a psychologist. At my first high school, I became a member of Key Club, high school portion of Kiwanis International, during my freshman year. Even though I ran for secretary and did not win the election, my club advisor asked me to be the Class President. I was the president of all the Sophomores. I started a Key Club chapter at my second high school in Fort Lauderdale. Key Club has nothing to do with the making of door keys, but I do believe that service is the key to life as well as kindness. As a young adult, I became a member of Kiwanis, the Young Adult Club in Washington D.C. where I led a chapter version of the fundraising program called Eliminate (maternal and neonatal tetanus; a bacterial disease can affect joints in third world countries). The year I graduated high school, I was looking up ways to be of service with my photography skills. I learned about Help-Portrait and photographer Jeremy Cowart, who is based in the Nashville area. He founded it in 2008. It’s an annual event. Photographers, volunteers, makeup artists come together to take a professional picture for free. It’s an exhilarating experience. I became one of those who would always give the homeless or less fortunate money whenever I could. My favorite and most exciting service projects are the ones where I can interact with people. There was a period where I would question their motive, but then it took me some growing to realize that my intentions are pure and it’s between them and God what they do with the money. My next real job (A monthly paycheck) was Operation Crossroads Africa. I also talked about them in a previous episode. (Episode 03 Operation Crossroads Africa) I wore many hats: Photographer for orientation, communications associate for DMV Alumni Association, videographer for their promo videos, social media marketer (Created their Twitter, Instagram, and Pinterest), email manager, productivity consultant, advisor, gala steering committee member, and volunteer. That opened a lot of doors for me. They were my first real client. I’m grateful for the opportunities. I look forward to being a group leader either to my husband’s country of The Gambia or to a new country as a part of my goal to visit 30 African countries by age 30. Crossroads Meant so Much to me as a paid job. My love for Africa grew working with Crossroads. I will give my old clothing items and electronics to Goodwill, Salvation Army or companies like those before I sell it. If it’s not in horrible condition, I will give to someone who I know will be more grateful for it. That mentality stayed with me when it came to recycling. My favorite quote by Muhammad Ali: “Service is the rent you pay for your room here on Earth.” Takeaways: Love Always is the Answer Love Always Wins Be Charitable Be selfless You can never truly make it entirely on your own. It does not have to be lonely climbing the ladder to success My first few jobs involved interacting with people/being of service. One of the 9 Guiding rules of my part-time job is service. Pre-diabetes means you have blood glucose levels that are higher than normal but not high enough to be called diabetes. If you have prediabetes, you are more likely to develop type 2 diabetes, heart disease, and stroke. PCOS (Polycystic Ovarian Syndrome) is a condition that causes an imbalance of female sex hormones. As a result, women with PCOS often don’t have menstrual periods or only have periods on occasion. It is the number-one cause of female infertility. PCOS causes insulin resistance, the hallmark of type 2 diabetes. Singer Angie Stone is a Grammy-nominated singer, songwriter, producer, actress and mother. She was diagnosed with Type 2 diabetes in 1999. “I was always on the go, and thought I was too busy to develop something like this,” Stone said. “I thought at the time that diabetes went along with bad habits, but I was the last one in my family to eat junk food.” What she didn’t realize was that she was a perfect candidate for diabetes: She had a family history of diabetes and was fighting weight problems. “I came to accept my diabetes when I realized just how many people around me, even in my own family, were living with diabetes," she says. "It gave me back a lot of courage to see all these people just like me, going places, involved in normal things, and I became determined to learn what I needed to better manage my diabetes." Hello everyone, Welcome to episode 58! Thank you for visiting another episode of 21st Century Hannah! I appreciate your taking the time to listen in once again. Thank you to my repeat listeners. About PCOS I knew nothing about PCOS—polycystic ovarian syndrome, until I started researching the illness. It saddened and surprised me as I read the symptoms and the manifestations of the disease. Many of you are boldly living your lives as you battle the disease, and I am proud of you for not giving into it! So one of my first questions as I researched PCOS, was just how many women have this disease? And I found that according to a 2015 article that was updated in 2016, Huffington Post article written by Heather Huhman, ‘Between five to 10 percent of women of childbearing age in the United States, or roughly 5 million, have PCOS. That percentage makes it one of the most common hormonal endocrine disorders in women. What's more, it also makes it the most common cause of female infertility.’ According to a Science Weekly article, 1 in 15 women worldwide suffer from PCOS. And what’s more, apparently the medical community still does not know much about the disease! What are the symptoms? Acne Weight gain/trouble losing weight Extra body hair Irregular periods And of course, trouble getting pregnant Those are just some of the symptoms. As with any of the other issues that cause childlessness, or trouble having children, all you can really do is what is in your power to do. That means eating the right foods and watching your weight as much as possible. How does nutrition play a part? So of course because nutrition is on the forefront for the podcast this year, I had to check out what the best foods are for women suffering from PCOS. I wasn’t surprised, and you probably won’t be either: High fiber foods, anti-inflammatory foods, and lean protein. Limit the processed carbohydrates. No surprise, right? I know it’s probably easier said than done especially if you live in a place where it’s hard to get to these types of foods, but please try your best. I know you will feel better. As usual, you know your body best. If you are just not feeling right regardless of what your physician says or prescribes, it is OK to do your own research, to exercise proper nutrition, and remain positive. I know, we have our up days and our down days in this world called childless not by choice, but overall, we have got to encourage ourselves. And for those of you who are still holding out hope. It’s OK to do so. There is a percentage of women who suffer from PCOS, who ended up having children. If you suffer from PCOS and you and your doctor are still hopeful, I will hope along with you!
