All too often when talking about infertility, we only tell one side of the story. Traditionally in the public discourse, it has very much fallen into the category of a ‘women's issue'. However, as our guest this week Reece Conca can all too painfully attest, 40% of infertility issues are actually male factor related. Reece Conca is a former Australian Rules Footballer, who played over 150 games for Richmond and Freemantle. When Reece and his partner Bell were finding it difficult to conceive, it took them far too long to discover that Reece has a condition called Azoospermia, which effects 1% of men - a condition where in Reece had no sperm in his semen. Abstract: Non-obstructive azoospermia is the most serious cause of male infertility. The testis has a special immunological environment, but the relationship between immune cells in the testicular microenvironment is still unclear. Therefore, it is urgent to identify the interaction mechanism and molecular determinants of immune cells in the testicular microenvironment. To further elucidate the etiology of azoospermia and provide a reference for the treatment of azoospermia. The GSE145467 and GSE9210 datasets were analyzed by Limma package, and then the differential genes were analyzed by enrichment analysis and protein-protein interaction analysis. In addition, we combined single-cell analysis(scRNA) to identify immune cell types and verified the expression of Hub genes in these immune cells. Finally, CellChat was used for cell-to-cell communication analysis. We found the distribution of immune cells in the microenvironment of Y chromosome AZF region microdeletions (AZFa_Del), idiopathic NOA (iNOA), and Klinefelter syndrome (KS) was significantly different from that of normal adults, especially monocytes/macrophages. In normal subjects, monocytes/macrophages mainly played the role of the signal source, while in patients with azoospermia, monocytes/macrophages mainly received signals from other immune cells. Monocytes/macrophages in AZFa_Del, iNOA, and KS communicated with other immune cells mainly through MDK-LRP1, PTN-NCL, and MDK-NCL ligand-receptor pairs respectively. Jazman knew she was called to birthwork from the age of 5 years old but it wasn't until after moving halfway across the country and finishing her doctorate when she heard the Lord clearly hit her with the call to doula work. She was called out of the work she had trained 8 years for in mental health with moms and their children and into birthwork as a doula, fertility supporter, and lactation consultant. When she decided to take that step of faith in the direction God was leading her, He has been working ever since! But, can you imagine being called to this area of work and watching family after family grow while you haven't received the blessing of a child yourself? Jazman was diagnosed with PCOS and then received supernatural healing and got her cycle back immediately while praying and fasting. She has been regular ever since. So why were they still not getting pregnant? On a whim, she suggested her husband get tested. To their devastation he was diagnosed with azoospermia. Azoospermia is the absence of motile (and hence viable) sperm in the semen. In other words, he had zero sperm count. Despite the diagnosis, that wasn't the hardest part. It was the lack of talking about this issue that was internally agonizing. It wasn't until her husband finally broke down and became vulnerable that they were able to truly heal and see the beauty from the ashes. Through ups and downs, trials and valleys, she has allowed God to heal her heart and no longer struggles with grief when she sees another baby being born, another shower invitation in the mail, or a baby announcement on social media. It is only through God's strength that she can do this work without breaking down. Jazman's story is encouraging to anyone who is called to something they don't feel qualified for. An extra X Chromosome and male fertilityI can't actually believe I made it to 100. Well, I can as there's so much that needs to be discussed when it comes to fertility. When I started on this podcasting journey, I had no idea there were so many different factors affecting our success in getting pregnant. First of all, I want to say thank you for being willing to share your stories with me as I feel I'm in an extremely privileged position to be able to talk to you, and please know how vital it is that we keep this conversation going. In this landmark episode, I'm putting the spotlight back on male infertility by chatting with Dr. Kevin McEleny about the work he does to train medical professionals to understand more about dealing with men and the issues that could be affecting their fertility. We discussed  Azoospermia and how 10% of men who have production type of Azoospermia have Kleinfelter Syndrome. This can result in men having issues with their education, weight gain,  breast enlargement, a lack of confidence, communication issues, dyslexia, dyspraxia. Sadly a lot of men aren't diagnosed early and Kevin explains how there may be opportunities to have put intervention into education which could have affected their life. And botox in your penis and bladder?   Today, we talk with Dr. Trenton Barrett. Dr. Barrett is a urologist, with specialty on male genito-urinary complaints as well as sexual health. And his subspecialty interest is in female urology, andrology, and male infertility. Dr Barrett talks about Azoospermia, which is a medical term used when there are no sperm in the ejaculate, as well as its causes, what can be done for this type of cases, procedures, and more. He also talks about Oligospermia, which is a term for low sperm count and how it's different from Azoospermia. Dr Barrett also answers the question of whether procedures relating to the scrotum/testes (ie vasectomy) can cause erectile dysfunction or not, and whether varicoceles are the possible reason for pain during sex/orgasm (for those experiencing it). He also brushes upon testicle pains, diagnosis, and its possible treatments (ie botox). Yes, you heard it right, botox can also be injected to the bladder (has been done since the 80's) for overactive bladders! Further, Dr Barrett and Melissa discuss vasectomies (and even reversals), what's true and what's not. Spoiler alert - there shouldn't be a crippling pain. A few more topics discussed are penile implants and injections. This is a fully-loaded episode with tons of golden nuggets that not only talk about the penis but also the bells around! Picture for a moment, being young, vibrant, and in a healthy place in your life, only to be told that your husband has a condition called Azoospermia. How would you react, would it be a deal-breaker in your relationship, do you believe your spouse would be willing to use donor sperm? Men with Azoospermia have a complete absence of sperm from the fluid ejaculated during orgasm (semen). Azoospermia may occur because of an obstruction in the reproductive tract or inadequate sperm production. This condition can cause male infertility. Sperm may be surgically extracted or provided by a donor for artificial insemination. Today's guest, Jessica Thornton, knows personally about the effects of male factor infertility. Jessica and her husband knew from the very beginning that they wanted to be parents, but it never occurred to them that fertility struggles were on the horizon. When the first results of Jessica's husbands' sperm revealed low sperm count, it became apparent that their hopes of parenthood wouldn't come to fruition without a fight. Jessica describes their experience searching for a sperm donor surreal and weird wrapped together. A few things you will learn from Jessica: how to be patient with your infertile spouse, remembering that you aren't the only one suffering, using personal strengths such as, comedy and playfulness to stay connected with your spouse, being patient with yourself, and how to include close family and friends along your journey.
