Body of episode: FATHER'S DAY  MEN'S HEALTH  So, well before President Biden's prostate cancer diagnosis, I had planned on discussing men's health for this year's Father's Day episode. In particular, I wanted to discuss causes of male childlessness, such as Azoospermia.  I've decided to put links in the show notes in regards to Azoospermia. Please check out the links for further information.  The basic definition of Azoospermia per The Cleveland Clinic website is:   ‘Azoospermia means there's no sperm in your ejaculate. Its causes include a blockage along the reproductive tract, hormonal problems, ejaculation problems or issues with testicular structure or function. Many causes are treatable. For other causes, it may be possible to retrieve live sperm to be used in assisted reproductive techniques like IVF.‘    As I think back through my episodes, I don't believe I've ever discussed men's health, or medical reasons men may be childless.  We've discussed fibroids, PCOS, endometriosis, adenomyosis, miscarriage, running out of time, aka the biological clock, MRKH, etc.  But I've never gone into much detail regarding issues surrounding the causes of childlessness in men.    I have had the great opportunity to interview several men about their experiences with childlessness, including how they have dealt with it. I will put links in the show notes on interviews I have conducted over the years with these wonderful men. Be sure to take a listen!    I also believe male childlessness is not as openly discussed as female childlessness because men and their spouses may find the issue embarrassing, shocking to their ego in a different way than a woman may feel about her childlessness, or maybe in much the same way.  In either case, there can be emotional and mental pain.    Some of the things that can cause infertility in men include:   AI Overview Male infertility can be caused by a variety of factors, including problems with sperm production or quality, hormonal imbalances, genetic disorders, infections, and environmental factors. Additionally, lifestyle choices like smoking, excessive alcohol consumption, and certain drug use can also contribute to infertility.  Here's a more detailed look at some of the key causes: 1. Sperm Problems: Low sperm count (oligospermia): A low number of sperm in the ejaculate. Absent sperm (azoospermia): No sperm in the ejaculate. Poor sperm motility: Sperm are unable to swim properly. Abnormal sperm morphology: Sperm are abnormally shaped, making it difficult for them to fertilize an egg.  2. Hormonal Imbalances:  Reduced testosterone: Low levels of testosterone can affect sperm production.  Hyperprolactinemia: Elevated levels of prolactin can interfere with sperm production.  Hormonal disorders affecting the pituitary gland or hypothalamus: These glands regulate hormone production.  3. Genetic Disorders: Klinefelter syndrome: A genetic condition affecting males. Cystic fibrosis: A genetic disorder that can cause blocked reproductive ducts. Myotonic dystrophy: A genetic disorder that can affect sperm production.  4. Infections and Inflammation: Epididymitis: Inflammation of the epididymis, the tube that carries sperm from the testicles.  Orchitis: Inflammation of the testicles.  STIs: Sexually transmitted infections like gonorrhea or chlamydia can cause inflammation and damage to the reproductive organs.  5. Environmental and Lifestyle Factors: Smoking: Smoking can reduce sperm count and quality.  Alcohol consumption: Excessive alcohol use can lower testosterone levels and affect sperm production.  Certain drug use: Anabolic steroids and other drugs can negatively impact sperm production.  Exposure to toxins or chemicals: Exposure to certain toxins, like pesticides, can affect fertility.  Obesity: Being overweight or obese can impact sperm quality.  6. Medical Conditions: Diabetes: Diabetes can affect sperm production and quality. Kidney failure: Kidney failure can also impact sperm production. Cystic fibrosis: A genetic disorder that can cause blocked reproductive ducts. Autoimmune diseases: Some autoimmune diseases can attack sperm.  7. Other Factors: Varicocele: Enlarged veins in the scrotum, which can raise testicular temperature and affect sperm production.  Testicular trauma: Injury to the testicles can damage sperm production.  Previous cancer treatment: Chemotherapy and radiation therapy can affect sperm production.  Undescended testicles: Testicles that don't descend into the scrotum.  Blockages or absences of tubes: Blockages in the reproductive tract can prevent sperm from being released.  This is for informational purposes only. Be sure to go to all of your check-ups, especially if there is a family history of health issues. Get checked, don't wait.   Be OK with advocating for yourself. The healthcare industry may say no, you are too young for a particular test. But if you want that test, be insistent. I wanted to get a fresh perspective through a risk management or risk-management-adjacent lens, which is why I reached out to today's guest. [3:49] Kirti Mutatkar is the President and CEO of UnitedAg in California and also the President, serving a two-year term, of the Health Care Administrators Association not for profit. [4:01] We will learn about her fascinating career journey and how leading with empathy has reduced turnover and retained value at UnitedAg. [4:10] Interview! Kirti Mutatkar, welcome to RIMScast!  [4:37] Kirti came to the U.S. to do her Master's when she was 20. She was going to study microbiology and do her PhD in genetics and biotech. Her circumstances changed and she explored getting an MBA. She got a Master's in finance. This was in Connecticut. [5:15] Kirti moved to California and looked for work. She applied at UnitedAg, to work for “a year or two.” Each year since then, UnitedAg has grown like crazy, with many innovations. Kirti has never felt that she has been there so long, she could do it in her sleep. [6:06] For Kirti, it's exciting every day to come to work. She was the CFO and transitioned  10 years ago to become the CEO. [6:25] Kirti was a member of HCAA for several years and is now President for a two-year term. The HCAA is very aligned with Kirti's thinking. She learns something with every conference she attends and every board meeting. [7:36] Kirti says, if everybody in the room feels it's their idea, and it's because of them we have succeeded, and you know you've played a role in that, that's success. [8:02] Kirti says a microbiologist, or any scientist, is constantly running experiments, and learning from experiments. Kirti says UnitedAg is a lab. HCAA is a lab. They're testing out things and trying to see what works and what does not work. [8:36] You understand the risks to the healthcare industry at a deeper level by doing experiments. You watch the results and you learn from them. You create feedback loops and grow. [8:53] Under Kirti's leadership, UnitedAg has tripled in size over the last couple of years, based on experimentation, learning, and adapting. That's what being a science student had taught her. [9:14] From the finance standpoint, in the position of impacting change, just looking at the financial statements and getting a quick picture of what's happening, was one of the best things Kirti could have done for herself in the role of CEO. [9:39] As a CEO, you have a strong CFO behind you. Understanding from a financial standpoint and a risk standpoint what that risk means to the financial, has been a big part of Kirti's success at UnitedAg. Her knowledge of science and finance works well for her organizations. [10:15] UnitedAg has around 110 employees. It has an underwriting department for its health plan. [10:45] Kirti believes her interest in the scientific method of testing hypotheses relating to the human side of risk and finance is what she brings to UnitedAg. [11:25] Kirti explains her upcoming plans for underwriting models for employer group risk and administrative costs. She is testing out opening the books, educating, and working with employer groups. [12:33] With Mental Health Awareness Month in May, Kirti discusses mental health in the agricultural community. She has been thinking of this since COVID-19. Ag workers did not have the choice of working from home. They were out in the fields. [13:15] There was a lot of uncertainty around what COVID-19 meant. In 2020, the need surfaced to provide some form of benefit for mental health. Culturally, in the ag industry, there is a stigma around mental health care. There is a lot of stress in production agriculture. [13:54] Kirti decided to run an experiment. She proposed to the board to use some of the innovation dollars set aside to offer a mental health benefit free of cost to all the members, with no copays and nothing going to the deductible, and doing it for one year. [14:28] It was a huge risk. They didn't look at the financial impact. They said, there's a need for mental health; let's do it for a year. They offered it and it became a huge impact. [14:56] A lot of people did seek out that benefit and used it. UnitedAg used Care Navigators to see what level of care a member's need was, and based on that need, provide that care. They found that some needed much care but more of the risk is in the early stages of need. [15:59] In the earlier stages, individuals may not realize they need care. Kirti stresses identifying the initial need for mental health care. Providing care initially can prevent chronic conditions. Not just for health plan cost, but from a human standpoint, you don't want that to go there. [16:33] UnitedAg spends a lot of time understanding mental health. Are you just having an anxious day today or are you having chronic anxiety? With the Chief Medical Officer, UnitedAg is spending time determining the tell-tale signs of anxiety to identify and help people with it. [16:59] Free access to mental health care, using Teladoc, Spring Health, other vendors, and the Member Advocacy Department at UnitedAg can be as simple as getting on a call and talking through anxiety, whether it's caused by environmental issues like flooding, or other stresses. [18:14] Kirti says UnitedAg is seeing anxiety around ICE deportations as labor is a big part of agriculture. There are a lot of people who come to work in the ag industry and then go back home. [18:39] Employees have anxiety over whether they will be there tomorrow. Employers have anxiety over whether they'll have access to labor tomorrow. [19:12] Uncertainty creates anxiety. For more information, and to register, please NewJersey.RIMS.org. [21:59] Let's Return to My Interview with Kirti Mutatkar! [22:07] Justin remarks on Kirti leading with compassion and empathy. Both at HCAA and as CEO of UnitedAg, she puts humans at the center. People remind her it's a business, but she has found that using compassion and empathy has an impact on real numbers. [23:26] Kirti reminds people she has a degree in finance and is a businessperson. UnitedAg has tripled in size. It is a quarter-billion-dollar trust. Its admin costs are very low and its loss ratio is very low. It's done very well financially. [23:53] Kirti makes a decision based on the financial aspect of it and then fits in the compassion and empathy. Eventually, compassion has a big impact on the financial. Now at HCAA, people are looking at how compassion translates into numbers. UnitedAg is a perfect example of it. [24:52] Some people may want to look at it purely from a business standpoint. Kirti feels that adding compassion and empathy manages the risk factor and has a financial impact. It's better for your business than you think it is. It's not a fluff word, you can show it in numbers. [25:52] Kirti is part of the Claims Committee at UnitedAg. Appeals come in and they make decisions on what can or cannot be approved. There is legal representation, client representation, and financial representation. Kirti shares an example of a mental health appeal. [26:41] The appeal was denied and Kirti listened to the compliance and financial positions for why it was denied. She didn't feel right about it, and after the meeting, she reached out to the employer. Then she came back to the committee and asked them to rethink it. [27:34] The committee found a solution within the compliance needs, to approve the claim. In the future, that employer will be apt to renew with UnitedAg because trust was created and help was provided to the employee. It was a financial decision that started with compassion. [29:16] UnitedAG's customer retention is over 90%. That speaks for itself. [29:26] RIMS Plug! [30:46] Kirti says that bringing mental health into the Enterprise Risk Management strategy starts with the HR department. UnitedAg does a lot of training around mental health. There are good products available, but just saying we have this product, as a check mark, is not enough. [31:16] Go deeper, listen to your members, and train your supervisors and managers to identify some of the tell-tale signs of needs and bring them forward. Kirti's Chief of Staff remarked to her that physical scars can be seen, but mental scars cannot be shown. [31:49] You have to go deeper to understand mental scars. Spread the word. Talk to your employees and HR teams. Be OK with someone getting emotional at work. Give them a safe space and a trusting environment. Everybody goes through anxiety and stress. [32:20] Providing a safe space does more than providing a program. Engaging, building relationships, and having a connection would be helpful. [32:35] As a CEO, Kirti's biggest takeaway is to maintain a connection with everybody. She tries to do that as much as possible. She goes to CEO lunches and roundtables and is constantly talking to people. You never know what someone will say or do that is a tell-tale sign of need. [33:06] Kirti's advice for risk management professionals is to go a little deeper. Don't take things at face value. Take a step back and ask what is the true problem. Go one or two layers deeper looking for the real issues. Understanding the issue is good from a risk standpoint. [33:50] Are there not enough mental health professionals or should we look at the model for how we provide care for people? [34:10] Kirti also has a podcast, This Is Ag!, available on Spotify and other podcast platforms. She loves podcasting! The reason behind it is relationship and connection. It's to tell people what ag is all about. This year's focus will be on the ground workers and telling their stories. [34:58] There is so much to learn from agriculture! A lot of things still happen with a handshake in agriculture. There's a lot of trust and a lot of relationship and connection. Kirti loves working in the industry. She's very grateful for that. [35:14] Kirti tells what she likes about leading a not-for-profit organization. She runs it as a business but the core of it is the mission it serves. HCAA serves the underserved and she is testing how compassion and empathy play a role in the process. The charges include one felony count of cruelty to children and one felony count of aggravated assault strangulation. The alleged abuse against a 2-year-old in her care includes holding a blanket over the child's nose and mouth, dropping him on his head, dragging him by his neck, shaking his head forcefully, and slapping him. The incidents, which occurred on March 13, were captured on security cameras. The daycare immediately terminated O'Connor's employment and notified authorities and the child's parents. O'Connor was released on a $15,000 bond after being booked into the Cherokee County jail. STORY 2: Caretaker Charged with Stealing from Elderly Cherokee County Resident Sharon Aline Young, a caretaker from Jasper, has been arrested and charged with multiple offenses, including theft of prescription drugs and personal items from an elderly disabled woman she was caring for. The charges include theft of jewelry, prescription medications (Oxycodone and Xanax), cash, bed sheets, and unauthorized use of the woman's credit or debit card for grocery purchases. The alleged incidents occurred between April 13 and May 8, 2023, while Young was employed as a caretaker for Comfort Keepers. She was released on bond after being booked into the Cherokee County jail. Comfort Keepers, the employer, emphasized its commitment to client safety and stated it is cooperating with authorities in the investigation. STORY 3: Trump, Co-Defendants Lose Bid to Dismiss Georgia Election Interference Case A Fulton County Superior Court judge rejected a bid to dismiss election interference charges against former President Donald Trump and 14 co-defendants, ruling that the indictments brought by Fulton District Attorney Fani Willis last summer do not violate First Amendment protections of political speech. Judge Scott McAfee stated that the defendants' expressions and speech were allegedly made in furtherance of criminal activity, including false statements and attempts to overturn the 2020 presidential election results in Georgia. McAfee dismissed six of the indictment's 41 counts but upheld the majority of the charges. Trump maintains his innocence and contends that the case is politically motivated. Panelists, including Georgia Department of Behavioral Health and Developmental Disabilities Commissioner Kevin Tanner, discussed the importance of breaking down barriers to seeking help and promoting awareness of available resources. They also addressed the growing mental health challenges faced by children and youth, particularly exacerbated by the COVID-19 pandemic. Strategies to enhance mental health support in the workplace were also emphasized, such as creating a supportive culture and providing access to resources like telehealth services. The event underscored the importance of community collaboration and access to care in addressing mental health needs. STORY 5: Reinhardt University Hosting Solar Eclipse Viewing Event April 8 Reinhardt University is hosting a solar eclipse viewing event on April 8, even though Georgia is not in the direct path of the total solar eclipse. From 1:47 to 4:21 p.m., a partial eclipse will be visible on campus, with the maximum coverage expected at 3:05 p.m., obscuring 81% of the sun. The event will take place on the lawn of the Bannister Glasshouse from 1:30 to 4:30 p.m. Free eclipse glasses will be provided on a first-come, first-served basis, and attendees are urged not to look at the sun without proper eye protection. Reinhardt advises against using phones to record or photograph the eclipse to fully enjoy the moment. STORY 6: Chattahoochee Tech Earns Top 10 Military Spouse Friendly School Designation Chattahoochee Technical College has been recognized as one of the top Military Spouse Friendly Schools for the 2024-2025 academic year, ranking eighth in the Large Community College category. With a commitment to supporting military families, the college offers resources such as the Veterans Education Career Training Resource Center at its Marietta Campus. STORY 7: Cherokee County Opens New Tag Renewal Kiosk in Macedonia A new tag renewal kiosk has been installed in Cherokee County's Macedonia community, located at the Kroger in Woodmont Village. This initiative, led by Tax Commissioner Denise Mastroserio, aims to provide convenience for residents in the area, including those in Macedonia, Free Home, Lathemtown, and Buffington. The kiosk operates during Kroger's business hours, allowing users to renew vehicle registration anytime between 6 a.m. and 11 p.m., seven days a week. Users can scan their renewal letter, driver's license, or enter their tag number along with the last four digits of their VIN number. Credit and debit cards are accepted, with a $4.95 fee per transaction allowing for up to 10 renewals. The individuals with whom you choose to surround yourself hold significant importance throughout your entire life, and this influence becomes even more pronounced during times of grief. I count myself exceptionally fortunate to have received incredible support from my family and a diverse group of friends who have offered unwavering assistance in numerous ways. Among these friends are those who simply provide feedback on the episodes of this podcast. As a podcast listener, you may not be aware that responses and feedback on our work are surprisingly limited. When my friend Amanda, the creator and host of the travel podcast "Me & the Magic," shared her thoughts on Episode 116 - "It's OK Not to Be OK," she not only affirmed the episode but also introduced a second viewpoint, which became the foundation for this episode, "The Guilt of Grief." Amanda reflected on how the episode made her ponder the flip side of grief, suggesting that moments of being okay during grief could be accompanied by feelings of guilt. There is no doubt that her insight deserves further discussion and clarification, especially for those of us who struggle to grant ourselves grace when experiencing the guilt of grief.

Dear Songwriter,In honor of recently turning 35, I'll be sharing the 35 of the most important lessons I learned throughout my career, which have paved the way for touring, international recognition, features in major publications, music video production in iconic cities, and the establishment of my dream songwriting mentoring business.This is a 2-parter, finishing out on 19-35, so be sure to listen to last week's episode to catch the rest!19 - If someone is doing something for you, for free, don't be surprised if it takes longer than if you were to pay someone to do the same thing20 - Guitar Center and Walmart have very flexible Return policies21 - Don't be afraid to ask your friends for help. They may actually be stoked to get involved22 - You can absolutely write an entire song on just four chords23 - It may be neccessary for songwriting to feel like work if you're trying to consistently write and release your music. And that's OK24 - If you don't make mistakes publicly, you may never learn from them25 - No one else needs to understand why you want to do this, except for you26 - A Chorus will only feel like a Chorus if it contrasts with the verse27 - Bring your best to every show on tour, regardless of crowd size. It's someone's first time seeing you play28 - If your lyrics are good, people will listen to them29 - You have as much of a right as anyone to express yourself through your songs30 - Songs are meant to be shared. Have you ever noticed that when you clear a space in your home, you instinctively want to fill it with something else? We call this the vacuum effect. A space will draw things into it unless you set an intention for the space you've created.Today I chat with Bonnie from Little Miss Organised about the Vacuum Effect as well as other areas of decluttering we can't help but discuss!Think of your kitchen table... you clear it off and then 24 hours later a new pile of random clutter has accumulated. Or you empty out a cupboard and it just doesn't stay empty, random things are magnetised into the space, almost like it's a crime to have empty space in your home.The vacuum effect is strong - and it's only nemesis is INTENTION. If you create intention of what the empty space is for (say actually leaving empty) then the vacuum effect is neutralised. The vacuum effect also happens when you declutter and a room/ cupboard/ wardrobe/ pantry etc. When you've reduced the quantity, you might instantly have a compulsion to go and purchase things to 'fill the space'.You declutter 40% of your clothes, because honestly you just don't wear them... but even though you know you don't need it, you start searching online for new clothes. Instead... make the decision and intention that you're going to experiment living with less clothes for the next month and see how that goes. Be OK with making mistakes. As the product of a highly musical family—his father Jody has played in rock bands for over three decades, his brother Billy is singer/songwriter signed to Interscope—Raffoul grew up immersed in the timeless records of the Beatles and Led Zeppelin and began exploring his own musicality by taking up piano at 17. “The music I'm making today really comes from all that time I spent in the shed over the years,” notes Raffoul, now 27. “It was always a world away from everything, where I could mess around with songs on this little keyboard and figure out how to put chords and melodies to all the lyrics I had in my head.” Before long, he'd added guitar to his repertoire and created a song called “Maybe I Do,” a bittersweet but radiant track slated for his forthcoming debut EP. “I was trying to paint a picture of the start of a relationship where the other person seems so perfect, you're almost waiting for something to go wrong,” says Raffoul. “It felt melancholy and hopeful at the same time, and once I found that contrast I knew I wanted to keep writing more songs in that vein.”All throughout his debut, Raffoul builds a hypnotic tension between that raw emotionality and a wildly unpredictable sound informed by everything from late-'80s college rock to left-of-center hip-hop to the Beatles' excursions into dreamy psychedelia. Among the EP's standout tracks are his debut single “Fucked Up Together”: a gorgeously stripped-back piece centered on his delicate piano work, soulful vocals, and lyrics that speak to a deep longing to be understood. On “Cigarette Holes,” he spins a piercingly honest portrait of emotional chaos, upending the song's heavy-hearted mood with brightly uptempo rhythms. Meanwhile, “Bad for Me” brings potent beats and prismatic textures to an unflinching meditation on cycles of self-destruction. “When you're going through a breakup or any other hard time in life, sometimes it feels easier to do things that are bad for you, as a way to numb yourself,” says Raffoul, who co-wrote “Bad for Me” with Connor Thuotte and Kate York (Lady A, Caitlyn Smith). “We started riffing on that idea and Connor played a very simple chord progression on this junky little guitar he'd gotten for $5 at a yard sale, which we used on the final version—it was this magical moment where everything just came together so naturally.”True to the playful spontaneity and DIY spirit of its creation, Raffoul's debut EP offers up plenty of idiosyncratic moments that intensify the unbridled energy of each track. To that end, the chorus to the gloriously frenetic “I Just Want to Be OK” takes the form of a cathartic outburst, a component he improvised in co-writing the song with his brother. And on “Call Me,” Raffoul shares an epic heart-on-sleeve ballad threaded with hauntingly stark and spellbinding piano melodies. “I wrote that song late at night in a hotel room and made a demo where the piano sounded super-thin, almost like it was playing through a truck driver's transistor radio,” he recalls. “When we worked on it in the studio, we ended up manipulating the piano sound so that you can still really feel that loneliness of the original, which is one of the things that makes that song so special to me.”With the release of his first batch of material, Raffoul hopes that others might find solace in his unfiltered yet finely crafted outpouring. “Most of these songs are about admitting that things are far from perfect, but to me there's hopefulness in that,” he says. “I think it's possible to find comfort in being uncomfortable, as long as you know you're not the only one who doesn't have it all figured it out. I want to let people that it's okay to be happy and sad at the same time—and that instead of trying to ignore those uncomfortable feelings, we should try to make the most of them.”We want to hear from you! Procrastination is not just the act of delaying an action, it's unnecessarily postponing things in a way that doesn't make sense, and may even cause you harm.  Sound familiar? Of course it does! Turns out that  95% of us admit to procrastinating (and the other 5% are probably lying). SHOW NOTES Procrastination has negative effects to all aspects of our well-being – from financial to physical health.  But like all things, there is a reason we do something that is not so good for us.  After a study, Drs. Pchyl and Sirois concluded procrastination is really about avoiding negative moods and emotions associated with a task.  And the task may be negative because it is hard, boring, frustrating, meaningless, ambiguous etc.  We just do not want to do it. Sooo, we delay because we want to avoid those negative feelings. The delay creates something identified by many experts as the procrastination cycle.  We have an expectation of ourselves that we are supposed to do something, we have some discomfort about what we are supposed to do, which in the moment is rewarded by avoiding the discomfort associated with the task, but then we have not done what we expected of ourselves – and we feel bad.  When we are in this cycle, we have a hard time imagining what it will be like when we do not meet the expectation we have set for ourselves – or let someone else set for us.  Our present self feels great avoiding the discomfort – and is not considering that our future self will be stressed, lose opportunities, get disciplined or fired by our boss, feel awful we let down a co-worker  . . .. What does the research say about procrastination: Get in touch with how your future self will feel if your present self procrastinates As always, be gentle with yourself.  Students who forgave themselves for not studying for a test ended up procrastinating far less on the next test. Solving the Procrastination Puzzle Get curious about why – what is the negative feeling?
