This June 6th marks the 80th anniversary of D-Day when the free world again turns its attention to the beaches of Normandy, to honor the sacrifices of so many and to pay tribute to the last of those who liberated Europe in WWII. In this episode, we reflect back on the 60th Anniversary of D-Day when I had the honor to visit the grave of Houston Duhon, a Cajun killed at Omaha Beach on the first wave. His best friend in the service, Carroll Mestayer, jumped off the Higgins boat with him and carried his lifeless body to the shore. Carroll survived the battle and lived to tell the story of that event on Bloody Omaha Beach and about his journey across his ancestral homeland as a French-speaking Cajun.

Contributor: Ricky Dhaliwal, MD Educational Pearls: What are DKA and HHS? DKA (Diabetic Ketoacidosis) and HHS (Hyperosmolar Hyperglycemic State) are both acute hyperglycemic states. DKA More common in type 1 diabetes. Triggered by decreased circulating insulin. The body needs energy but cannot use glucose because it can't get it into the cells. This leads to increased metabolism of free fatty acids and the increased production of ketones. The buildup of ketones causes acidosis. The kidneys attempt to compensate for the acidosis by increasing diuresis. These patients present as dry and altered, with sweet-smelling breath and Kussmaul (fast and deep) respirations. HSS More common in type 2 diabetes. In this condition there is still enough circulating insulin to avoid the breakdown of fats for energy but not enough insulin to prevent hyperglycemia. Serum glucose levels are very high – around 600 to 1200 mg/dl. Also presents similarly to DKA with the patient being dry and altered. Important labs to monitor Serum glucose Potassium Phosphorus Magnesium Anion gap (Na - Cl - HCO3) Renal function (Creatinine and BUN) ABG/VBG for pH Urinalysis and urine ketones by dipstick Treatment Identify the cause, i.e. Has the patient stopped taking their insulin? Aggressive hydration with isotonic fluids. Normal Saline (NS) vs Lactated Ringers (LR)? LR might resolve the DKA/HHS faster with less risk of hypernatremia. Should you bolus with insulin? No, just start a drip. 0.1-0.14 units per kg of insulin. Make sure you have your potassium back before starting insulin as the insulin can shift the potassium into the cells and lead to dangerous hypokalemia. Should you treat hyponatremia? Make sure to correct for hyperglycemia before treating. This artificially depresses the sodium. Should you give bicarb? Replace if the pH < 6.9. Otherwise, it won't do anything to help. Don't intubate, if the patient is breathing fast it is because they are compensating for their acidosis. References Andrade-Castellanos, C. A., Colunga-Lozano, L. Link to bioRxiv paper: http://biorxiv.org/cgi/content/short/2023.07.24.550408v1?rss=1 Authors: Campos-Cardoso, R., Desa, Z., Fitzgerald, B., Moore, A., Duhon, J., Landar, V., Clem, R., Cummings, K. A. Abstract: The rodent medial prefrontal cortex (mPFC) is a locus for both the promotion and suppression (e.g. extinction) of fear and is composed of four anatomically distinct subregions, including anterior cingulate 1 (Cg1), prelimbic (PL), infralimbic (IL), and the dorsal peduncular (DP) cortex. A vast majority of studies have focused on Cg1, PL, and IL. The Cg1 and PL have been implicated in the promotion of fear, while the IL has been linked to a role in the suppression, or extinction, of fear. Due to its anatomical location ventral to IL, the DP has been hypothesized to function as a fear-suppressing brain region however, no studies have explicitly tested its role in this function or in the regulation of memory generally. Here, we provide evidence that the DP paradoxically functions as a cued fear-encoding brain region and plays little to no role in fear memory extinction. By using a combination of cFos immunohistochemistry, whole-cell brain slice electrophysiology, fiber photometry, and activity-dependent neural tagging, we demonstrate that DP neurons exhibit learning-related plasticity, acquire cue-associated activity across learning and memory retrieval, and that DP neurons activated by learning are preferentially reactivated upon fear memory retrieval. Further, optogenetic activation and silencing of fear learning-related DP neural ensembles drives the promotion and suppression of freezing, respectively. Overall, these data suggest that the DP plays a novel and unexpected role in fear memory encoding. More broadly, our results reveal new principles of organization across the dorsoventral axis of the mPFC. Copy rights belong to original authors. Today, I talked with Dr. Christopher Duhon about his passion for working with people on their terrain, the foundation of their health. We had a fascinating discussion on the biofeedback signs our bodies give, why we need to listen to them, and how our habits affect the choices we make in regard to our health. We also delve into the world of telemedicine and hear from him, as a doctor who has worked firsthand with telemedicine, how it is making medical care more accessible to more people around the world. In this episode you will hear: Healing the whole person. How Terrain Ten differs from current Western medicine and holistic medicine. The truth of polypharmacy. The social pressures and established habits of our lives. Listening to the biofeedback of your body. The precious nature of your health. Telemedicine versus in-person medicine. Dr. Christopher Duhon grew up in a small town in southern Louisiana near Lafayette. As he matured and experienced life, particularly, medical school, what he discovered to be his passion all along is working with a person's terrain, the foundation of their health, their “soil,” so to speak. Dr. Duhon's gift of listening to his patient's body has allowed him to honor their uniqueness and develop individualized personal care. He has been fortunate to be mentored by some brilliant Swiss medical doctors and local naturopaths who imparted their wisdom and experience to him. It has left him with a wide variety of tools to offer his patients (i.e., IV therapy, Hyperbaric Oxygen, O3 therapy, Swiss-German Biological Medicine, Terrain Ten work, and more). Dr. Duhon works out of his own office in Seattle, WA, both virtually and in person. He is comfortable with all cancer types and applies the Terrain Ten methodology to all cases in order to dismantle the process bit by bit. Why the 2022 Harris County Judge Race will Be Critical for Education Programs in Harris County by Diana Martinez To protect public education, advocates must keep a close eye on local races such as the one for Harris County Judge. In Texas and across the country, education is under attack. Far-right misinformation propagandists are stirring up nonexistent issues to enrage unwitting parents to push bans on books and attacks on teachers. Schools are under constant threat of gun violence thanks to inaction by the governor and outlandish conspiracy policy pushed by Ted Cruz and the NRA. Even a cursory glance at the falsehoods and extremist positions peddled in the wake of Uvalde display the dangers of Greg Abbott's perspective on schools. Rightwing demagogues in league with outwardly white supremacist groups are aggresively protesting public LGBTQ events, a few short months after Indicted Texas Attorney General Ken Paxton decided to leverage the power of his office to attack trans kids through a questionable legal maneuver. ... Voters in Harris County have a choice this November to either embolden MAGA extremists who oppose public education programs that have had a significant positive impact in our community, or support the people who made those programs come to life. Parents, students, and teachers relying on critical services such as literacy programs, special needs education, and early childhood education from the Harris County Department of Education need our support to keep those services going. In particular, we need a County Judge like Lina Hidalgo, who will stand up for our local community against the whims of Greg Abbott, Dan Patrick, and their super donors. By backing pro-ed candidates like Hidalgo and Duhon, we can give every kid the chance to thrive. --- Send in a voice message: https://anchor.fm/politicsdoneright/message Support this podcast: https://anchor.fm/politicsdoneright/support

Terri Duhon is an award winning educator, TEDx speaker, Board Director and speaker, who has over 25 years of experience in financial markets. She wrote the book "How the Trading Floor Really Works", founded her own financial markets firm, and is an Associate Fellow at the SAID Business School in Oxford. She sits on the Board of Morgan Stanley International, which she is Chair of the Risk Committee, and also holds a number of additional independent director roles. She is now based in London but hails from the New Orleans area originally. Our story starts there - with Terri's upbringing in Louisiana, in the South of the US and how this was an additional reason that she felt like an "other" when she came to studying math at MIT and then Wall Street and a career on the trading floor of a major investment bank. This sense of being from a different socio-economic group was more visceral than being one of few women on the floor. She spent close to 10 years in similar roles, and describes herself as mastering the art of reinvention - in that to date she has reinvented herself approximately every 10 years.Terri moved from the trading floor to founding her own firm, and then to her current chapter, which is in a series of board and committee roles. She explains how challenging it was to break into this field initially, and how she almost gave up hope, and then experienced a breakthrough. We spend some time on the question of what makes an effective board member and chairperson, and Terri delivers a mini-masterclass in the art (listen in particular from 6.21 to 9.09). Our conversation then turns to the industry and its "brand" and how it doesn't always appeal to a diverse set of candidates, sometimes because it emphasizes traits that aren't universal. Terri uses her vantage point in a business school to give insights into the current priorities of graduates entering the field. She shares stories of her failures, and her triumphs, but always her humanity, and stresses how it is always our humanity we must stress in order to demystify the world of finance and make it more accessible.

BEERS: Revival is both one of Rhode Island's staples and up-and-coming locations. Now, with new beers and a new buzz, Will tries the brand-new Extra Thirsty IPA. At 10.5%, where does this stack up among the region's triple IPA's? Everyone loves Trader Joe's, and we're no exception, so Jake takes a crack at one of their thirteen beers you can find in a state where alcohol in grocery stores is legal. Josephsbrau PLZNR is today's choice, and while it's certainly not the best pilsner on the market, Jake's glad he tried it. BUSINESS: It's been a long while since we've had any significant Amazon news, but the recent vote by its front-line workers might have just made up for it. Amazon's Staten Island warehouse will be the first in company history to unionize, and it's sending ripples across the world. Amazon has fought labor unions in its workforce, but now it faces a critical dilemma and still remains at the forefront of the labor crisis - will this throw lead times, staff, and same-day-deliveries into a spiral? Also, we crown our first Entrepreneur of the Week to a German man that was vaccinated 90 times so he could sell the vaccination cards. Kudos… BALLS (27:31): Former NBA player and Duke National Champion Chris Duhon joins the show. We discuss his journey to getting a full scholarship at Duke, being recruited by Coach K, and winning the national championship with Carlos Boozer & Jay Williams in 2001. Chris tells us some of the lessons he learned from Coach K, his philosophy on how the Duke teams should operate, and some of the things that truly make him the best coach of all time. We also discuss his 9-year NBA career as a reliable point guard, including on the 2012-13 “Superteam” LA Lakers with Kobe Bryant. Now an assistant coach at Bryant, Chris tells us about the Bulldogs' first ever conference championship in March, and his first time back at March Madness since 2004. This particular Covid testing is not sensitive orspecific. Therefore what you can get are high rates of false positives and negatives.• Thus following if the numbers are inflated and there aren't truly that many cases, then themortality rate is actually higher. But the reported deaths in the hospitals are very well inflated as well. Ultimately itprobably evens out to be somewhere around 1.5% or lower.• Also consider the attack on PCP not being allowed to treat outpatients• It's assumed 85% of people who died could have been saved if doctors wouldhave been allowed to actually practice medicine. Their patients died because they were denied treatment or forced to fit the patient into a failing protocol• Can cause chronic issues Typical testing of vaccines (length and methods) vs Covid and number ofvaccine injuries before a product was pulled from the market vs covid• Used to take roughly 7 years to actually manufacture a vaccine• Typically there is a minimum 2 year period for trials to determine sub-acute and chronicadverse effects like geno-toxicity, cancer, birth defects, etc• Start with animal studies to rule out alarming short term complications• Stopped working on RSV vaccine when 22 infant deaths• Stopped working on H1N1 after 53 deaths and Guillan-barre, roughly 550 cases• This vaccine is producing nearly 53 presumed deaths a day, that haven't been fullyassessed for actual cause• In such a short turn around time, no studies could be done to assesschronic adverse events, nor can effectiveness be determined in such shortperiod• In the studies that were done, recruited patients that weren't beingexposed to covid were younger and healthy. And much like other vaccine trials theydidn't have a true placebo for control group What is different about this vaccine?• Very obviously an agenda• December 10th: “Trusted news initiative,” via BBC announcement• We are only going to publish info that promotes vaccines and stopanything that creates vaccine hesitancy, ie; no talk of vaccine safety or effective early/ outpatient treatment• YouTube, FB, local and national and global news censorship• Informed consent actuality vs, what should we be informed of with integrity• Declaration of health safety in US and UK: abide by Law of autonomy: NOONE will have anything forced in to their body based on coercion or peer-pressure, fear of reprisal• True informed consent is talking about the risks and benefits so you canmake an educated decision• Vaccine that now has 15% effectiveness with these new variants• And high risks such as embolism, myocarditis, cancer, infertility, etc What's the difference between vaccines and what does FDA approval actual mean?• Old vaccines: dead/live-attenuated virus or protein• New technology• mRNA technology• Discovered in the 80's, Dr. Robert Malone on team• Patented for 20 years and failed every time at creating anything effective• J&J: adenovirus vector (triple ) code for original spike protein• Tricks body into making spike protein; basically dumbs down your own immunesystem• Moderna 100 mcg mRNA, Pfizer 30mcg mRNA, J&J and Astrazeneka =adenovirus particles (not mRNA)• They made it to last (with nucleoside caps) and loading it on a charged lipid nanoparticle.• They said it would stay (localized) for only a few days (short term).• Japan denied use of Pfizer vaccine when they couldn't prove it waslocalized/ stayed in arm.• Therefore it is distributed to the entire body- lipid particles found as well asfree spike protein.• Body will attack the cells that have the spike protein.• Now research shows the spike protein existing 9 months later and in blood/body fluids• Very high affinity for ovaries and adrenal glands and bone marrow.• Long term effects: infertility, cancer• FDA/ Dr. Malone: will see in 6 months, 3 years, 6 years if actually causescancer• Signs of potential carcinogenic, thrombocytopenia, reactivation of latentvirus (shingles, retroviruses)• Now we are starting to see increase incidences of tumors• FDA approval of the Pfizer vaccine• Mainstream media promoting what the government wants them to say which makespeople feel safe.• 2 approval letters• Authorization to market in response to biologic application• Extension for emergency use authorization• Pfizer has legally created two vaccines, the one we know of that has beenapproved for emergency use, and the other that is being made which the FDAbasically gave authorization to market as it becomes available.• (German product distributed by Pfizer) Commernity is licensed withconditions (not currently available). They have just approved the label on itand documentation as possible adverse events. This is available in othercountries.• Full indemnification for emergency use vaccines• But for FDA approved, means there isn't a full indemnification- it's possiblesee some financial support if you have damage from vaccine• Currently they are not mandating this particular vaccine- out of the 3.• Which ones are safest for seniors, women of childbearing years, or pregnantwomen?• Moderna vaccine study in animals- lowered fertility. The FDA would notinclude women of childbearing years or in pregnant women in the study due to possible negative side effects• Graphene oxide is not listed as ingredients- very important to note • Shedding and why• FDA has said that it is possible for those who have a weaker immune system tocontinue to produce the spike proteins and constantly shed it• Breakdown of the risks associated and adverse event reporting:• Who is monitoring our government?• No independent safety committees• Even with huge increase in deaths, this is nothing we have ever seen in the field of medicine• Nothing was being done about it by FDA or a committee• Early this year a reported possible 1,600 deaths• FDA/CDC appointed doctors (not board certified and not out-sourced). These doctors declared that ZERO deaths were from the vaccine• In normal circumstances, it would take 18 months minimum to review• Study McLachlan and Scott showed 86%+ had no other possiblecauses than the vaccine• J&J claimed 3 deaths from blood clots. VAERS reports 47- so how many is it really?• Multiple patients feeling fatigued after vaccine (adrenal glands)• D-dimer tests after the vaccine - 63% had clotting• His theory is that in the future they will get right sided heart failurewith pulmonary HTN.• This could be potentially 60% of the population in the next fewyears with congestive heart failure.• Myocarditis has a 60% 5 year mortality rate• Infertility not being reported What are the current numbers according to different entities, ie cdc, vaers, etc?• https://www.cdc.gov/coronavirus/2019-ncov/vaccines/safety/adverseevents.html• On VAERS reporting, with all vaccines combined, about 158 deaths totalreported in a year• January: 182 deaths, 455 hospitalizations, 1,700 emergency room visits• As of September, 386 million doses• Myocarditis, 1,491 (in people
