Overactive bladder is a prime candidate for bioelectric solutions — Forty-five million Americans have overactive bladder, with nine million preferring adult diapers to traditional treatments.Nitty GrittyAlthough not fully understood, overstimulation of nerves in the bladder wall produces spasms, creating an urge to urinate as often as thirty times a day in extreme cases.Few sufferers choose sacral nerve stimulation, the gold standard treatment, which requires a device implanted near the spine to stimulate nerves regulating bladder behavior.“The moment you say surgery, it complicates things.” Schiaparelli explains. “You need a physician who knows how to do it, you need a patient who's willing to have what could be a very extensive surgery, and you need a payer who's willing to pay for the surgery.”Drugs for overactive bladder have unwelcome side effects that cause most patients to drop out of the care pathway. “When you look at those dynamics,” Schiaparelli says, “it screams a need. Patients want something that takes surgery out of the equation, doesn't have the side effects of drugs, and is convenient.”Avation Medical's Vivally System is an ankle-worn device that indirectly stimulates the sacral nerve through the tibial nerve without surgery. The device measures responses to adjust its stimulation automatically in real-time.“This physiologic closed loop allows the patient to have personalized, effective therapy in just thirty minutes once a week,” Schiaparelli says.Data that made a differenceAs a serial entrepreneur, Schiaparelli has learned that success requires understanding and meeting the needs of three key stakeholders: the patient, the physician, and the payer:Overactive bladder patients dissatisfied with traditional treatments are an enormous market. Most physicians can only offer prescriptions for imperfect drug therapies that do not generate revenue for their practices. Payers don't like either option since surgery costs reach $40,000 while drugs require lifetime prescriptions.“Every area we checked into, it made sense. This technology in this market checked all those boxes to say there's a need. We thought this was a real opportunity to disrupt the market.”But success requires addressing the needs of other stakeholders, including regulators and investors.Regulators' expectations, for example, drove Avation Medical's decision to implement quality control processes while starting its first clinical trial. “We knew that was going to be very important because we planned to use the clinical trial with our FDA submission.”Aligning Avation Medical's investors' expectations was just as important, with each investment round supporting the next stage in development and commercialization.Schiaparelli takes a holistic perspective on a Med Tech startup's data strategy.“It's speed, absolutely,” Schiaparelli says, “but it's also intelligent data that speaks to the needs of all the people that you'll need to demonstrate to down the line.”

This is: Reformative Hypocrisy, and Paying Close Enough Attention to Selectively Reward It., published by Andrew Critch on September 11, 2024 on LessWrong. People often attack frontier AI labs for "hypocrisy" when the labs admit publicly that AI is an extinction threat to humanity. Often these attacks ignore the difference between various kinds of hypocrisy, some of which are good, including what I'll call "reformative hypocrisy". Attacking good kinds of hypocrisy can be actively harmful for humanity's ability to survive, and as far as I can tell we (humans) usually shouldn't do that when our survival is on the line. Arguably, reformative hypocrisy shouldn't even be called hypocrisy, due to the negative connotations of "hypocrisy". That said, bad forms of hypocrisy can be disguised as the reformative kind for long periods, so it's important to pay enough attention to hypocrisy to actually figure out what kind it is. Here's what I mean, by way of examples: *** 0. No Hypocrisy Lab: "Building AGI without regulation shouldn't be allowed. Since there's no AGI regulation, I'm not going to build AGI." Meanwhile, the lab doesn't build AGI. This is a case of honest behavior, and what many would consider very high integrity. However, it's not obviously better, and arguably sometimes worse, than... 1. Reformative Hypocrisy: Lab: "Absent adequate regulation for it, building AGI shouldn't be allowed at all, and right now there is no adequate regulation for it. Anyway, I'm building AGI, and calling for regulation, and making lots of money as I go, which helps me prove the point that AGI is powerful and needs to be regulated." Meanwhile, the lab builds AGI and calls for regulation. So, this is a case of honest hypocrisy. I think this is straightforwardly better than... 2. Erosive Hypocrisy: Lab: "Building AGI without regulation shouldn't be allowed, but it is, so I'm going to build it anyway and see how that goes; the regulatory approach to safety is hopeless." Meanwhile, the lab builds AGI and doesn't otherwise put efforts into supporting regulation. This could also be a case of honest hypocrisy, but it erodes the norm that AGI should regulated rather than supporting it. Some even worse forms of hypocrisy include... 3. Dishonest Hypocrisy, which comes in at least two importantly distinct flavors: a) feigning abstinence: Lab: "AGI shouldn't be allowed." Meanwhile, the lab secretly builds AGI, contrary to what one might otherwise guess according to their stance that building AGI is maybe a bad thing, from a should-it-be-allowed perspective. b) feigning opposition: Lab: "AGI should be regulated." Meanwhile, the lab overtly builds AGI, while covertly trying to confuse and subvert regulatory efforts wherever possible. *** It's important to remain aware that reformative hypocrisy can be on net a better thing to do for the world than avoiding hypocrisy completely. It allows you to divert resources from the thing you think should be stopped, and to use those resources to help stop the thing. For mathy people, I'd say this is a way of diagonalizing against a potentially harmful thing, by turning the thing against itself, or against the harmful aspects of itself. For life sciencey people, I'd say this is how homeostasis is preserved, through negative feedback loops whereby bad stuff feeds mechanisms that reduce the bad stuff. Of course, a strategy of feigning opposition (3a) can disguise itself as reformative hypocrisy, so it can be hard to distinguish the two. For example, if a lab says for long time that they're going to admit their hypocritical stance, and then never actually does, then it turns out to be dishonest hypocrisy. On the other hand, if the dishonesty ever does finally end in a way that honestly calls for reform, it's good to reward the honest and reformative aspects of their behavior....

In this episode, I tackle a nuanced situation shared by Nell from Maryland, a play therapist working with an 11-year-old client who does not speak during therapy sessions. Despite not being diagnosed as selectively mute, the child exhibits behaviors akin to selective mutism, engaging in play but not verbal communication. While the child's parents are fixated on verbalization as the primary goal, I offer a crucial reframing. I also discuss the child's interaction with toys, particularly stuffed animals, to communicate and express themselves, highlighting the significant progress this represents. My advice to Nell and all child-centered play therapists is twofold: first, we must clearly set expectations with parents from the start, helping them understand the stages, timelines, and principles of this therapeutic approach. Second, we must remain unwavering in our commitment to those principles, respectfully yet firmly guiding parents to trust the process and our clinical expertise. Forcing verbalization would undermine the child-led nature that allows for organic healing. This year I decided to follow my own advice and commit to one social media platform. Sure I still post content in lots of places, but I am spending time every day on only one platform - LinkedIn. And, I am not just showing up, posting an update, liking a few things and leaving. I did some research to find out what the LinkedIn algorithm likes and adjusted my approach accordingly. Comments matter. So I am spending time writing meaningful comments on multiple posts each day. I engage with people who engage with me as well as influential people I follow. Video works. I am incorporating lots of video clips. It means I have fewer people listening to the full episodes, but I am reaching more people with my content, so I will take that as a win. Strategic connection requests. Selectively sending connection requests to people who engage with the same content I am engaging with has helped me to build a more interesting and engaged community. Working Together to Expand Reach Let's face it, one of the most frustrating things about sharing content on any social platform, is not getting engagement. While the algorithm certainly controls some of that, you can influence the algorithm. On LinkedIn, the more engagement your post gets in the first few hours, the more visibility it will have. So, you need a community that responds when you post. One way to increase engagement is to start talking to other individuals so you see their content and they see yours. Tag a few select people in the comments, ask a question that encourages their response. Then respond to people who comment. No simple likes or thumbs up. You need to keep the conversation going. Does this extra attention pay off? It as only be about six weeks, but so far I am satisfied with the results. I have doubled the amount of traffic to my website from LinkedIn. Given the push to a zero click environment, this growth is pretty impressive. I have added about 400 connections. Not just random spammers, but people I am actually talking to. which make the time I spend on this platform more interesting and productive. (Adam@TheWorldview.com) By Jonathan Clark Western countries guilty of violating religious liberty 168 times The Family Research Council reports that Christians in Western countries faced 168 incidents of religious freedom violations between 2020 and 2023. Examples of persecution include arrests and fines for public preaching and praying, punishments for publicly expressing Biblical beliefs, and fines or imprisonment for pastors that did not comply with COVID-19 restrictions. Tony Perkins with the Family Research Council said, “It is shocking to see Western countries -- the same ones we think of as free and open societies -- take authoritarian measures against Christians simply trying to live out their faith. Hostility toward Bible-believing Christians is clearly and steadily rising in the West.” Pro-lifer Nayib Bukele expected to win in El Salvador Nayib Bukele is expecting a landslide victory in his re-election bid for President of El Salvador. Preliminary results from Sunday's election show Bukele winning with 83% of the vote. His nearest rival got 7% of the vote. He faces criticism for his consecutive second term, which is banned in the country's constitution. However, favorable judges, voted in under his administration, approved the second term. Bukele is known for cracking down on gang members in El Salvador, dropping the homicide rate, and making the cryptocurrency Bitcoin legal tender. He also opposes the murder of unborn babies and faux homosexual marriage. Biden selectively punishes church vandalism The U.S. Department of Justice announced sentencing last week for a man who attempted to burn down the building of Community Church of Chesterland, Ohio. The arsonist was angered by the church's plans to host two drag events. It's a rare case of the Biden administration prosecuting vandalism of religious buildings. Most other cases involve attacks on the buildings of pro-life churches and organizations. There have been nearly 400 cases since the leak of the U.S. Supreme Court's decision to overturn Roe v. Wade in 2022. Tom McClusky with CatholicVote said, “The response from the Biden administration has been the equivalent of a shrug and a yawn.” Ecclesiastes 8:11 says, “Because the sentence against an evil deed is not executed speedily, the heart of the children of man is fully set to do evil.” Murders decreased by 12% in America Vox reports that violent crime and homicide dropped significantly last year from a recent peak in 2020. Murders decreased by 12% in cities in 2023, according to the FBI. Some major cities were exceptions with none greater than Washington, D.C. The nation's capital had its deadliest year in over two decades in 2023. Violent crime rose by 40%. D.C. also saw its greatest shortage of police in five decades. However, Jeff Asher, a crime data analyst, noted that 2023 overall saw “one of the lowest rates of violent crime in the United States in more than 50 years.” Country musician Toby Keith died of cancer Country musician Toby Keith Covel died on February 5th at the age of 62 after losing his battle with stomach cancer, reports NBC News. The entertainer revealed his diagnosis in a June 2022 Instagram post, noting he "spent the last 6 months receiving chemo, radiation and surgery." He became known for his 2011 hit song, “Red Solo Cup,” which advocated drunkenness and debauchery. KEITH: “Red Solo cup, I fill you up/Let's have a party, let's have a party/I love you, red Solo cup/Proceed to party, proceed to party” In the video, Focus on the Family notes that Keith wanders around a house party in which everyone's drinking from red Solo cups. Virtually every woman in the video, it seems, is wearing a bikini or short shorts. By song's end, Keith has not only enumerated the joys of drinking from said cups, but also how women holding them are easier to hit on.” Proverbs 20:1 says, “Wine is a mocker, strong drink a brawler, and whoever is led astray by it is not wise.” Toby Keith charted 61 singles on the Billboard Hot Country Songs charts, including 20 number one hits and 22 additional top 10 hits like “As Good As I Once Was” in 2011 and “Should've Been A Cowboy” in 1993. All told, he released 19 studio albums, 2 Christmas albums, and 5 compilation albums, totaling worldwide sales of over 40 million albums. On March 24, 2001, Keith's father, H.K. Covel, was killed in a car accident. That event and the September 11 attacks in 2001 prompted Keith to write the song "Courtesy of the Red, White, & Blue," a song about his father's patriotism and faith in the United States, originally intended only for live shows. Plants can talk with each other And finally, scientists believe that plants can actually communicate with each other. Scientists have caught this phenomenon of God's creation on camera for the first time.
