Hinter den Kulissen wird um hohe Millionenbeträge gepokert. Ein Unternehmer aus Hamburg hat jetzt bei Gericht die erneute Zwangsversteigerung der Ihme-Zentrums-Anteile beantragt. In seinem neuen Roman "Die Frau mit den vier Armen" stellt Nolte die tiefgründige Frage, ob man sich am Denken anderer schuldig machen kann. Und er zeigt Hannover vielleicht von einer noch nicht gekannten Seite. Martina Kothe hat Jakob Nolte getroffen und ist mit ihm an die Schauplätze des Romans gegangen. Mikä miehen sadoista miljoonista siittiöistä pääsee hedelmöittämään naisen munasolun? Satunnainen siittiöiden uintikilpailu ei kuitenkaan ratkaise peliä. Hedelmöittyminen tunnetaan jokseenkin huonosti, mutta tutkimus tarjoaa uutta tietoa. Yhdyntään johtava valinta tapahtuu luonnollisesti yksilöiden välillä. Nyt on tietoa, että valintaa tapahtuu myös sukusolutasolla, munasolun ja siittiöiden kesken. Mitä ovat valintaan johtavat periaatteet? Onko immuunijärjestelmän HLA-geeneillä roolia? Ja miten asiaa voidaan tutkia? Itä-Suomen yliopistossa professori Jukka Kekäläisen johdolla on kehitetty malli, jossa voidaan laboratorio-olosuhteissa mitata siittiöiden liikettä ja elinkykyä naisen lisääntymiselimistöä simuloivissa olosuhteissa. Apua näytteiden keräämiseen saatiin hedelmöityshoitoklinikasta. Biologi Annalaura Jokiniemi sai käyttöönsä lapsettomuushoidossa olevien naisten munarakkulanestettä ja kohdunkaulan limaa. Nämä näytteet ja mainosten perusteella mukaan tulleiden miesten siittiöt saatettiin maljalla yhteen. Ohjelmassa mukana Jokiniemen ohella SPR:n veripalvelun tutkimusjohtaja, immunologi Jukka Partanen. Tuloksia kommentoi molekyyligenetiikan professori Juha Kere Karoliinisesta instituutista. Toimittaja on Teija Peltoniemi.

Mitä chileläisten kotoutumisesta Suomeen voitaisiin oppia nykypäivänä? Ohjelmassa tavataan ensimmäisen ja toisen polven chileläissuomalaisia. Lisäksi kuullaan Chilen sotilasjuntan kynsistä pari tuhatta ihmistä pelastaneelta suomalaisdiplomaatilta, voisiko sama temppu onnistua nykyään. This post is a summary of some of my work as a field strategy consultant at Schmidt Futures' Act 2 program, where I spoke with over a hundred experts and did a deep dive into antimicrobial resistance to find impactful investment opportunities within the cause area. The full report can be accessed here.Antimicrobials, the medicines we use to fight infections, have played a foundational role in improving the length and quality of human life since penicillin and other antimicrobials were first developed in the early and mid 20th century.Antimicrobial resistance, or AMR, occurs when bacteria, viruses, fungi, and parasites evolve resistance to antimicrobials. As a result, antimicrobial medicine such as antibiotics and antifungals become ineffective and unable to fight infections in the body.AMR is responsible for millions of deaths each year, more than HIV or malaria (ARC 2022). As a result, antimicrobial medicine such as antibiotics and antifungals become ineffective and unable to fight infections in the body. AMR is responsible for millions of deaths each year, more than HIV or malaria (ARC 2022). For instance, every year, there are 150 million cases of severe fungal infections, which result in 1.7 million deaths annually (Kainz et al 2020). Unlike for bacterial infections, we do not have good estimates of how many of those are associated or attributable to resistance.Concerningly, AMR is  escalating at an increasing rate (USA data, Swiss data, Mhondoro et al 2019, Indian Council of Medical Research 2021). One prominent report estimates that AMR will result in 10 million deaths every year by 2050 (Jim O'Neill report, 2014).Even more concerningly, we may be at a critical juncture, where if we do not drastically change our current trajectory, we could run out of effective antimicrobials. This would mean that our ability to perform surgery, give cancer patients chemotherapy, or manage chronic diseases like cystic fibrosis and asthma, all of which hinge on the effectiveness of antimicrobials, would be significantly impacted. The very foundations of modern medicine could be threatened; the WHO has warned that we could return to a pre-antibiotic age, which would result in the average human life expectancy going down from 70 to 50 (WHO, 2015).Beyond the health effects, there is a profound economic cost to AMR – for patients, healthcare systems and the economy. In the USA, the CDC estimates that the cost of AMR is $55 billion every year (Dadgostar 2019). Studies show that as a result of AMR,  the annual global GDP could decrease by 1% and there would be a 5-7% loss in low and middle income countries by 2050 (Dadgostar 2019). Reducing maternal mortality through informed family planning In June 2021, we founded Lafiya Nigeria, a non-profit organisation that works toward ending maternal mortality in Nigeria by widening the access and information about family planning. TL; DR Introduction to our organisation in a 3-min video This post describes (I) the challenge we aim to solve, (II) our approach, (III), our traction, (IV) our value-add, (V) our plans, (VI) how you can get involved in our initative. (I) The challenge In low and middle-income countries, women are dying from giving life. Nearly 300,000 women and girls are dying from pregnancy-related complications each year, according to the Guttmacher Institute. Other health complications such as obstetric fistula, postpartum anemia, and postnatal depression are also key health burdens borne by pregnant women. Reducing the number of unintended pregnancies is an effective means of reducing health burdens for mothers and newborns. Despite a significant number of women in these countries wanting to avoid pregnancy, many are not using modern contraceptives, resulting in 85 million unintended pregnancies per year. If all women with unmet need were provided access to and used modern contraceptives, the Lancet estimated that maternal deaths globally would drop 44%. An estimated 70,000 maternal deaths could be prevented each year, with 441,000 new-born deaths also averted. Additionally, the Guttmacher Institute estimates that every dollar spent on contraceptive services beyond the current level would reduce the cost of pregnancy-related and newborn care by three dollars. The Copenhagen Consensus also found that every dollar spent on access to modern contraception leads to 120 dollars of social, economic, and environmental benefits. Access to family planning is beyond a health issue: its dividends are seen also in positive effects on education, income generation, and children's welfare. A study in Indonesia found that providing access to family planning was three times more effective than improving school quality in keeping girls in school an extra year. Research in Colombia found that girls with access to family planning clinics were 7% more likely to participate in the formal workforce as adults. Long-term studies have also shown that providing access to family planning programs can lead to improved college completion rates of children and higher family incomes decades later. These spillover effects are difficult to measure and are often neglected in traditional cost-effectiveness analyses. In recent years, there has been an increase in the use of modern contraceptives in countries like Nigeria but there has also been an increase in the unmet need. From 2012 to 2019, the portion of women using contraceptives in Nigeria rose from 11.2% to 14.2%, while the unmet need also rose from 22.4% to 23.7%. These figures are greatly exaggerated Focus: Nigeria Lafiya Nigeria focuses on rural and underserved regions of northern Nigeria Nigeria has >45M women of child-bearing age, and 65% have unmet contraceptive needs (IHME), resulting in around 40,000 maternal deaths a year. In Nigeria, over 83% of women had not used any contraceptive methods for family prevention in 2018. This rate reached 96% among women without any education. This staggering gap in health provision results in maternal and infant deaths. In Nigeria, over 40,000 women die each year from pregnancy-related issues. The loss of life does end with mother, either. Over one million children under the age of five also die as a result of losing their mothers to pregnancy delivery complications. Introduction I previously wrote an entry for the Open Philanthropy Cause Exploration Prize on why preventing violence against women and girls is a global priority. For an introduction to the area, I have written a brief summary below. In this post, I will extend that work, diving deeper into the literature and the landscape of organisations in the field, as well as creating a cost-effectiveness model for some of the most promising preventative interventions. Based on this, I will offer some concrete recommendations that different stakeholders should take - from individuals looking to donate, to funders, to charity evaluators and incubators. The key recommendations I make, in order of importance, are: Support community-based interventions that seek to shift harmful gender norms and reduce violence- they have a high quality of evidence, and cost $180/DALY (disability adjusted life years) or $150 for a woman to live a year free from violence. Two particularly promising organisations are CEDOVIP and Raising Voices. Fund and conduct a well-designed randomised control trial of radio or television dramas to shift harmful gender norms and reduce violence- they have a startling cost-effectiveness of $13/DALY or $11 for a woman to live a year free from violence, but currently lacks a well-established evidence base. Fund organisations undertaking economic programs supporting women (e.g. microfinancing, cash transfers, village savings and loans association) to add on social empowerment programs focused on reducing violence; they have a cost-effectiveness of $180/DALY, or $145 for a woman to live a year free from violence. Found new charities focused on community-based interventions that seek to shift harmful gender norms and reduce violence, particularly in neglected geographies and populations Consider supporting self-defence training programs - although analysis suggests that self-defence training may not be as cost-effective as other interventions for VAWG, it is nevertheless a relatively cost-effective (at $260/DALY or $215 for a woman to live a year free from violence) and potentially scalable intervention. No Means No Worldwide and Ujamaa appear to be two organisations scaling this intervention well. Fund academic research to understand what types of culture change programs targeting boys and men are most effective at reducing violence against women and girls You can find a 2 page summary of my initial post and this post here Why VAWG is an important cause area Nearly one third of women and girls aged 15 years of age or older have experienced either physical or sexual intimate partner violence (IPV) or non-partner sexual violence globally, with 13% (10–16%) experiencing it in 2018 alone (Sardinha et al 2022). It is one of the leading burdens of disease globally, responsible for 8.5 million DALYs and 68 500 deaths annually. In several countries, violence against women is in the top 3-5 leading causes of death for young women aged between 15 and 29 (Mendoza et al 2018). VAWG has wide-ranging effects on women's physical, sexual and mental health- in fact, it is responsible for 11% of the DALY burden of depressive disorders and 14% of the DALY burden of HIV in women (IHME 2019) Globally, the rates of VAWG are both alarmingly high and have increased over the last 30 years, despite gains in other areas of women's health, such as maternal care (Think Global Health). In 2016, the global economic cost of violence against women was estimated by the UN to be US$1.5 trillion, equivalent to approximately 2% of the global GDP (UN Women 2016). In this podcast, Dr. Eeks chats with Dr. Christopher Murray about his recent article published in The Lancet (2022) titled The Global Burden of Cancer Attributable to Risk Factors, 2010-2019, a Systematic Analysis for the Global Burden of Disease Study 2019. This is the largest effort to date to determine the gobal burden of cancer due to risk factors. He will discuss what the global burden of disease study is, how he and his team estimated the burden of cancer, the risk factors and the assessment framework used to link specific cancers to specific risks, what percentage of cancer is attributed to risk factors, what the greatest risks are, differences in high income vs low income countries and public health implications. Dr. Murray is Chair of Health Metrics Sciences at the University of Washington and Director of the Institute for Health Metrics and Evaluation (IHME). He also leads the Global Burden of Disease collaboration ( a systemic effort to quantify the comparitive magnitude of health loss due to diseases, injuries and risk factors.)He is both a physician and a health economist and his work has led to innovative methods for strengthening health measurements, analyzing the performance of health systems and producing forecasts for the future state of health. Lt Col Ihme is the Commander of th 232nd Intel Squadron ,132nd  Wing, Iowa National Guard.   I was interested in her journey with being a parent of two autistic children and how she navigated the entire process of getting resources before and after a PCS and the  issues with getting children the help they needed and the energy required to advocate for them while being an active duty commander.    Her story is compelling but as I am learning not unique.  I hope by doing this segment as with the other segments on parents with special needs children, I have opened the door to more discussion on how we can navigate this journey together. Support the show

Tracking Covid is increasingly difficult, a function of both underreporting of cases and overreporting of incidental hospital admissions. Studies are emerging which suggest that protection against severe ill and death may be waning after 20 weeks. Without far better data on hospital admissions, however, we are “flying blind.” Essential “big” investments in next-generation vaccines that block infections and address multiple variants are expensive. Welcome to The Nonlinear Library, where we use Text-to-Speech software to convert the best writing from the Rationalist and EA communities into audio. This is: The IHME Report, published by Zvi on January 10, 2022 on LessWrong. An IHME report came out over the weekend. I was pleasantly surprised by it contents. Its conclusions and projections broadly match what I said in the last Omicron post, with only relatively minor obvious nonsense, and seemed worth their own post to break down. Headline Takeaways Our models for the United States suggest that infections may be currently peaking at 6 million a day, with varied peaks by state. We expect more than 50% of the US population will be infected with Omicron in the next 6 weeks. The infection-detection rate (IDR) is declining, although shortages of testing may lead to more rapid decreases in the IDR than we currently model. Nevertheless, we expect reported cases to exceed 1 million a day, peak by the third week of January, and then decline sharply. I too expect reported cases to peak during the third week of January (to be clear, when I say we will peak on 19 January, I mean cases detected on 19 January, not infections that happen on 19 January. If counting from initial infections, I'd have said 16 January, but it's hard to ever know what the actual lag was there.) Once the peak arrives, a sharp decline is to be expected shortly thereafter, although I still expect cases to remain elevated (above pre-Omicron levels) for several months. Note that ‘more than 50%' sounds like the kind of thing one says when one does not want to make a precise prediction, but there are graphs later on. Hospitalizations will increase to a peak that may be twice as high as last winter. These figures, however, include incidental admissions. Because the prevalence of Omicron infection is so high, many individuals hospitalized for other conditions will have asymptomatic infections. Incidental admissions may exceed 50% of total COVID-19 admissions in some states. Daily deaths will increase but remain below 2,000 a day, peaking by the end of the month. This means the peak of deaths will remain below the summer Delta surge and well below the peak last winter. That sounds like a best case scenario. If hospital admissions peak at twice last winter, but half of them are incidental, then things won't be substantially worse than last winter. We survived last winter. Deaths peaking below 2,000 per day means they won't even double once, and the peak will come within a few weeks. Expecting them to peak by month's end seems wrong given the historical lag times, but it's only a difference of a few weeks. Words of high wisdom follow, noting that all of this is baked in. Our alternative policy scenarios, including more rapid scale-up of boosters to all who have been previously vaccinated, increasing mask use to 80%, and vaccinating the partially hesitant, have only a small impact on the trajectory over the next 4 months. The speed of the epidemic is so fast that policy interventions will have little impact. In previous waves, the control strategy has been to control infection and thus reduce hospitalization and death. Given that there is little prospect of controlling infection, strategies need to focus on reducing harm in the vulnerable and minimizing disruption. It is essentially too late to do anything but mitigation. If the baseline scenario is as they describe (and roughly as I predict as well) then only those with a strong desire to avoid infection should do much. So this, very much this: Given the massive numbers of infections in the community, testing and quarantining asymptomatic individuals may not be helpful. There appears to be no prospect for controlling transmission and considerable prospect for disruption of schools and essential services due to screening. On June 10th, Greg Glassman stepped down from his position as CEO of CrossFit, and on this segment of Business Casual, Tyler Kern, Taylor Bagley, and Daniel Litwin opine on whether or not this move will be enough to ensure the future of the company as well as many of its affiliate gyms. Established in 2000 and characterized by high-intensity functional interval training and sound nutrition practices, CrossFit has been a highly popular health and fitness lifestyle solution that enables members to accomplish any goal—from improved health to weight loss to better performance. Boasting a network of more than 13,000 affiliates around the world with over 7,000 gyms offering the program in the US alone, it's estimated that there are roughly 4 million CrossFitters so devoted to this competitive approach to fitness that the community has often been described as cult-like. Well, Mr. Glassman's recent insensitive remarks regarding George Floyd as well as the COVID-19 pandemic on social media and via email may have driven a wedge in this close-‘fit' community, alienating members and gyms alike while outraging socially-oriented organizations like Black Lives Matter, medical-focused groups like the Institute for Health Metrics and Evaluation (IHME), and even the NFL. Litwin, Kern and Bagley chat about the many companies and brands that have crafted statements in support of the BLM movement and the fact that CrossFit remained silent during these weeks of nationwide protests and civil unrest, the racially and socially tactless tweets and emails between Glassman and the IHME and several of his affiliate gyms that sparked this national scandal, the proposed withdrawal of many gym affiliates from their contracts with the company, name brands like Reebok, FitAid and Rogue severing their partnerships with CrossFit, and the financial bind gyms are already facing due to pandemic social distancing and how Glassman's callous statements will further tip the fiscal scale for these workout locations as we move into the future. 猜猜台灣最嚴重的疾病是什麼?揭曉答案之前,讓我們從一則國際新聞談起。2017年1月,梅琳達和比爾·蓋茲基金會向華盛頓大學的一個計畫捐贈2.79億美元,以追蹤世界各地的健康和死亡數據。當時是蓋茲基金會歷來規模最大的一次捐贈,捐贈的對象是一個叫做IHME的機構。這個機構是甚麼?為什麼它得到比爾蓋茲這麼大的捐助?為什麼過去的調查,會讓全世界的人都「死三遍」?人不是只能死一遍嗎?為什麼IHME的調查可以避免這種錯誤?為何它可以揭開全世界70億人受傷、生病與死亡的真相,甚至有助於拯救生命?原來IHME大約九成的力量,都投入GBD(全球疾病負擔)計畫,不但可以告訴我們,全世界各地區造成死亡最多的疾病是什麼,做出排名,更可以反映全人類與各國的健康狀況,例如「失能」而生活無法自理的狀況如何。它到底是怎麼做到的?台大公衛學院的林先和教授,是台灣少數實際參與GBD計畫的學者,本集特別與聽眾分享這項計畫的重點。今天,GBD的調查在全球已經贏得權威地位,我國的衛福部也提供完整的資料,讓GBD報告中顯示的台灣現況更貼近真實。於是,林教授更可以有把握地回答:台灣最嚴重的疾病是什麼?為什麼他說所謂「十大死因」,其實不是「因」,而是「果」?哪些事情,是威脅台灣健康最大的「危險因子」?又給我們哪些啟示呢?健康是最大的財富,而要保障健康,就要知道破壞健康的殺手是什麼,進而防範。這集分享的全球與台灣生死大數據,正由統計出發,給我們重要的參考!

Tyrone Grandison joins us and shares that you need to focus on the customer and that your business is an IT business. First, "You need to be focused acutely on your customer, your client. So, I run the technology department, so my customer is everyone that is at IHME, and the 3,000 collaborators that depend upon us, right? Plus everyone that actually uses our results, etc, etc, etc. My thing is, how do I actually make them efficient, make them happy, and make sure that whatever I do fits within their workflow? Creating a separate entity to actually do that." And second, "IT is a business driver, IT should be at the table when you actually make decisions. All decisions should be about what the core mission is, which is completely ... Well, not completely different, but a different way of thinking about it. But, if you don't believe that IT is going to actually help you be more efficient, is going to support your task, is going to drive you to actually more profit, more growth, more impact."
